Abstract

AbstrakVitamin C dan derivatnya banyak digunakan dalam sediaan kosmetika sebagai pencerah kulit. Pada penelitian ini akan dikembangkan gel rejuvenasi yang mengandung derivat vitamin C ascorbyl glucoside (AA2G) dalam sistem penghantaran macrobead®. Selain sebagai pencerah kulit, AA2G juga dapat dipakai untuk pengelupasan dan peremajaan kulit (rejuvenasi). Persen jeratan ditetapkan terlebih dahulu dengan perbandingan (AA2G: macrobead®) 1:2, 2:2 dan 3:2, basis yang digunakan adalah acrylates copolymer (5%, 10%, 15%) dan alkyl acrylate crosspolymer (0,5%, 1%, 1,5%). Formulasi gel rejuvenasi dibuat dalam variasi konsentrasi AA2G yaitu 1%, 2% dan 3%. Uji efektivitas dilakukan terhadap 30 orang relawan dengan menggunakan punggung tangan bagian kanan. Uji difusi dilakukan selama 360 menit menggunakan sel difusi franz. Hasil penetapan persen jeratan menunjukan perbandingan 1:2 memiliki jeratan terbesar yaitu  88,60%. Basis alkyl acrylate crosspolymer dipilih sebagai basis paling stabil dengan konsentrasi 1%. Hasil penelitian menunjukan tangan yang diolesi formula gel terpilih (1:2) menunjukan perubahan terhadap kulit yaitu kulit lebih cerah, lebih kencang dan lebih halus dibandingkan tangan kiri sebagai kontrol.  Hasil uji difusi menunjukan bahwa formulasi gel rejuvenasi dapat melintasi membran sebesar 0,12% dalam waktu 360 menit.Kata kunci: ascorbyl glucoside, acrylates copolymer, alkyl acrylate crosspolymer, macrobead®, rejuvenasiAbstractVitamin C and its derivatives are widely used in cosmetics as a skin lightening preparations. This research will be developed rejuvenation gel containing vitamin C derivative ascorbyl glucoside (AA2G) in macrobead® delivery system. Beside as a skin lightening, AA2G can also be used for exfoliation and skin rejuvenation. Percent entrapment set beforehand by comparison (AA2G : macrobead®) 1:2, 2:2 and 3:2, the base used is acrylates copolymer (5%, 10%, 15%) and alkyl acrylate crosspolymer (0.5%, 1.0%, 1.5%). Rejuvenation gel formulation is made in various concentration of AA2G is 1%, 2% and 3% . Effectiveness test is carried out on 30 volunteers by using the back of the right hand. Diffusion test is performed for 360 minutes using a Franz diffusion cell. Determining percent entrapment results showed 1:2 ratio has the biggest trap that is 88.60%. Base alkyl acrylate crosspolymer selected as the most stable base with a concentration of 1%. The results showed that smeared hand gel formula selected (1:2) showed changes in the skin is skin brighter, firmer and smoother than the left hand as a control. The test results showed that the diffusion gel formulation of rejuvenation can traverse the membrane of 0.12% within 360 minutes.Key word : ascorbyl glucoside, acrylates copolymer, alkyl acrylate crosspolymer, macrobead®, rejuvenasi

Abstract

Proses penuaan kulit terjadi karena kombinasi penurunan kapasitas proliferasi sel-sel kulit, berkurangnya sintesis matriks dermis, dan peningkatan ekspresi enzim yang mendegradasi matriks kolagen. Penelitian ini merupakan eksperimental laboratorik, menggunakan 24 ekor tikus jantan galur Sprague-Dawley sebagai objek penelitian. Pada kelompok perakuan, kulit dorsal kedua kelompok dicukur, kemudian diberikan paparan sinar UVB seminggu tiga kali selama 4 minggu dengan dosis total penyinaran 840 mJ/ cm2. Kulit kelompok perlakuan diolesi krim Lactobacillus plantarum FNCC 0020 yang tidak bereplikasi, sehari 2 kali dengan komposisi aplikasi topikal koloni 247,27x107, sedangkan kelompok kontrol tidak diolesi apapun. Hasil penelitian diperoleh, baik intensitas, distribusi, maupun histoskor MMP-13 antara kelompok perlakuan dan kelompok kontrol tidak menunjukkan ada perbedaan yang bermakna (p>0,05). Intensitas kolagen III baik pada kelompok perlakuan maupun kontrol semua mempunyai derajat kuat, sedangkan distribusi kepadatan kolagennya paling rendah 20–50% dan tertingginya di atas 80%. Perbandingan distribusi kepadatan kolagen III secara statistik sangat bermakna (p<0,01). Pemberian krim topikal L. plantarum FNCC 0020 meningkatkan ekspresi distribusi kepadatan kolagen III namun tidak menurunkan ekspresi MMP-13. Kata kunci: Lactobacillus plantarum, kolagen, (matrix metalloproteinases) MMP13 Effect of Topical Probiotic on MMP-13 and Collagen III Expression in The Dermis Layer of Male Rats Irradiated with Ultraviolet-B Abstract Nowadays, there is a big interest in the use of topical probiotic preparations for skin health. One of the probiotics therurapeutic benefits is used as anti-aging. During aging, there is stimulation of activator protein-1 (AP-1) which is a transcription factor that inhibits the production of collagen and AP-1 supports the breakdown of collagen by enzymes called matrix metalloproteinases (MMPs). As administration of oral Lactobacillus plantarum could inhibit skin aging by lowering the activity of MMP, so the collagen degradation can be derived so probably topical use of Lactobacillus plantarum may give more prominent effects. We used 24 male rats Sprague-Dawley strain as research objects. This study was divided into two groups, the treatment and control groups. The shaved dorsal skin of rats were irradiated with UVB three times a week for 4 weeks with total irradiation dose of 840 mJ/cm2. Skin cream, containing 247.27x107 CFU non-replicating Lactobacillus plantarum FNCC 0020, was smeared on the treatment group, two times daily, whereas the control group did not receive any treatment. Skin biopsies were done at the end of the study for examination of MMP-13 and collagen III expressions. Intensity, distribution, and histoscore of MMP-13 between the treatment and the control group showed no significant difference (p>0.05). The treatment group showed a significant different in the intensity of collagen III with the density distribution of 20–50% and the highest density was 80% (p<0.01). Administration of topical cream L. plantarum FNCC 0020 increased the expression of collagen III density distribution, but not the MMP-13 expression.Keywords: Collagen, Lactobacillus plantarum, (matrix metalloproteinases) MMP13

Abstract

BACKGROUND: Systemic lupus erythematosus (SLE) patients living in Indonesia are prone to tuberculosis (TB) infection, since this country ranks second globally for TB prevalence. Isoniazid, an anti-tuberculosis (TB) drug, is metabolized by enzyme N-acetyltransferase 2 (NAT2) that is encoded by NAT2 gene. NAT2 haplotype, referring as acetylator status, may predispose as genetic factor in SLE development or complicate SLE therapy. This study explored the NAT2 haplotypes and acetylator status among SLE patients living in a TB endemic area.METHODS: Genomic DNA of 260 registered SLE patients at The Rheumatology Clinic of Dr. Hasan Sadikin General Hospital, Bandung, Indonesia were isolated. NAT2 gene was amplified and sequenced, then NAT2 haplotypes and the acetylator status among SLE patients with or without TB history were determined and presented.RESULTS: Most of SLE patients registered were female (n=250; 96.2%). The median age of patients when SLE was diagnosed for the first time was 27 years old (8-69 years), with organ involvement predominantly in musculoskeletal (80.8%) and mucocutaneous (73.1%). TB history, mostly pulmonary TB, was present in 23.1% of SLE patients of whom TB was diagnosed before SLE (10.4%) or after SLE (10.7%) or both before and after SLE (2%). The acetylator status was mostly intermediate (61.5%) with the NAT2*4/*6B was the most prevalent haplotype (25.8%).CONCLUSION: There is a high number of intermediate and low acetylator status among SLE patients. Since these SLE patients live in TB endemic area, the NAT2 acetylator status determination among SLE patients before starting TB therapy may have clinical benefit to decrease a possible drug induced liver injury, and this warrants further study.KEYWORDS: NAT2, acetylator, systemic lupus erythematosus, tuberculosis

Abstract

Background: Atopic dermatitis (AD) is a skin disease which cause stress to the patients. The chronic process of AD can cause physical, social, and psychological impairments. The severity of AD can also be affected by allergen exposures, which in turn will affect the quality of life of the patient. Skin prick test (SPT) can be used to evaluate allergen sensitization. Purpose: To evaluate correlation between SPT positivity to severity and quality of life of AD patients. Methods: The study was an observational cross-sectional study. Twenty five AD patients were recruited based on inclusion and exclusion criteria. Questionnaire was used to measure the quality of life of patients, and Scoring of Atopic Dermatitis (SCORAD) to measure the disease severity. Most of participants were women (80%), with median of age 26.84±13.71. Results: Positive SPT was obtained in 68% of patients, most of them were caused by house dust mites (55.2%). Seventy six percent of patients experienced mild AD, 12% moderate AD, and also 12% patients experienced severe AD. Significant correlation was observed between the severity and quality of life (p=0.001;r=0.617), while the SPT positivity has no correlation with the severity (p=0.912;r=-0.023) and quality of life (p=0.959;r=0.011). Conclusion: This study revealed that the severity of the disease has a correlation with quality of life, but SPT positivity has no significant correlation with severity and quality of life.

Abstract

Background: Methotrexate (MTX) and cyclosporine have been used as effective systemic mono-therapy for psoriasis. Several factors are considered to switch monotherapy to combination therapy because monotherapy is no longer effective and has higher side effects. Hence,clinicians have avoided systemic therapy combinations due to its toxicity. However, some studies showed that this combination therapy could be usedeffectively for psoriasis patients. Purpose: This study aimed to analyze the efficacy and adverse effects of systemic MTX and cyclosporine combination therapy in Indonesian psoriasis vulgaris patients. Methods: The retrospective study assessed the effectiveness of 3 monthsmono-therapyand combination therapy of systemic MTX and cyclosporine in psoriasisvulgaris patients from 2016–2017 in Dermatology Clinic, Dr. Hasan Sadikin Hospital, Bandung, West Java, Indonesia. Result: Psoriasis area and severity index (PASI) score 90 were achieved in the group MTX (50%) and cyclosporine group (50%), while none in the combination group.However, eight patients (50%) in group MTX and cyclosporine reached the primary endpoint of PASI 50. One patient in cyclosporine group had adverse effects on kidney profiles. Nonetheless, other patients had no biochemical changes. But, there was no significant difference in the change of PASI between each group (p=0.102). Conclusion: We propose that combination therapy of MTX and cyclosporine is relatively safe and efficacious in treating Indonesian psoriasis vulgaris patients. This combination treatment isas effective as MTX or cyclosporinemono-therapy.

Abstract

Latar belakang: Cakupan ASI eksklusif di Kota Jogja rendah 40,24%, sementara target nasional 80%. Banyak faktor yang mempengaruhi keberhasilan ASI eksklusif, antara lain pengetahuan ibu, dukungan suami dan keter- ampilan teknik menyusui. Upaya meningkatkan cakupannya adalah dengan memberikan konseling menyusui pada pasutri. Dukungan suami berpengaruh terhadap keberhasilan teknik menyusui.Tujuan: untuk mengetahui pengaruh konseling menyusui pada pasutri terhadap pengetahuan, dukungan, dan ke- terampilan teknik menyusui serta korelasi pengetahuan suami dengan istri di kelompok pasutri.Metode: Penelitian ini merupakan penelitian Quasi Eksperiment yaitu menilai “Pengaruh Konseling Menyusui Pada Pasutri Terhadap Pengetahuan, Dukungan dan Keterampilan Teknik Menyusui Di Puskesmas Kota Yogyakarta”, dengan rancangan pretest-posttest control group design. Pengambilan Sampel dengan teknik consecutive sam- pling di puskesmas Jetis,Tegal Rejo, dan Mergangsan, sampel adalah suami dan ibu nifas primipara yang me- nyusui bayinya sebesar 60, teknik randomisasi blok digunakan untuk membagi ke dalam kelompok perlakuan dan control masing masing kelompok 30 sampel. Variabel pengetahuan diukur dengan kuesioner sebelum dan sesudah konseling. Variabel dukungan suami dengan kuesioner sesudah konseling, sedangkan variabel keterampilan me- nyusui diamati dengan daftar tilik menyusui sebelum dan sesudah konseling. Analisa data menggunakan Wilcoxon,Man Whitney dan Rank Spearman.Hasil: penelitian menunjukkan skor pengetahuan dan keterampilan pada kelompok pasutri dan kelompok sendiri.ABSTRACTBackground: Percentage of exclusive breastfeeding in Jogja is low at 40.24 % , while the national target is 80 % . Many factors influence the success of exclusive breastfeeding, such as knowlwdge skills and techniques in breas- feeding and husband support. Efforts to increase the percentage is to provide breastfeeding counseling to couples. Husband’s support leads to successful breastfeeding techniques.Objective: The purpose of this study was to determine the effect of breastfeeding on couples counseling to the knowledge, support, and technical skills as well as the correlation of the couples knowledge of breastfeeding in a group of couples.Method: This research is a quasi experiment that assesses “ Effect of Breastfeeding Counseling In A married couple on Knowledge, skills technical Support to Breastfeeding In Public Health Centers of Yogyakarta “, by pretest-posttest control group. Sampling with consecutive sampling techniques in health centers Jetis, Tegalrejo, and Mergangsan, samples are husband and postpartum primiparous mothers who breastfeed their babies which is 60, randomization techniques are used to divide the block into a treatment group and a control group of 30 samples each. Knowledge variables were measured with questionnaires before and after counseling. The variable of husband support in the questionnaire after counseling, while the variable of breastfeeding skills is observed with breasfeeding the checklist before and after counseling. Analysis of the data uses the Wilcoxon, Man Whitney and Spearman Rank.Result: The results showed the score of knowledge and skills in a group of couples and groups prior to the interven- tion itself not significant (p> 0.05). The median value of knowledge for couples groups is 56.2, skills is 27, 27. The median value of the group itself to the knowledge is 55,0 27.27. The results of the knowledge, and skills after the in- tervention showed significant differences (p< 0.05). Couples groups is knowledge median value is 92.5, the value of the skills is 100, while the value of the group’s own knowledge 88,7 and skills is 72.72. Median value of knowledge on husband support to couples is (88,8), higher than own group median value (72.2), there is a significant difference(p < 0.05) . There is a husband and wife correlation knowledge which is significant (p <0.05)Conclusion: There is an effect of breastfeeding on couples counseling to the increase of knowledge, support, and technical skills of breastfeeding in Yogyakarta city health centers. Knowledge, support, and technical skills about breastfeeding in a group of couples is higher than the group itself. Husband’s good knowledge can increase wife’s knowledge about proper breastfeeding techniques.

Abstract

The order (sequencing) determinationof DeoxyribonucleicAcid (DNA) bases is the gene’s most basic information, using the methodof Polymerase Chain Reaction (PCR) as its stage. A key factor of successful detection by PCR is specific PCR primer design choice. Thedetection of diversity of Mycobacterium Mannose Receptor (MMR) gene, responsible for recognizing mannosylated antigen structureof Mycobacterium tuberculosis (M.tb) by DNA sequencing of exon 7 chromosome 10p12, related to susceptiblity for PulmonaryTuberculosis(TB), was first performed in China in 2012. The purpose of this study was to find specific primerfromboth design originatedfrom the research in China/primer I and my own design/primer IIby using Primer3 software. This study was based on 10 healthy subjectsand was a preliminary study of a research titled. The Relationship of Single Nucleotide Polymorphisms (SNPs) of Macrophage MannoseReceptor Gene to Pulmonary Tuberculosis Cases. The examination materials consist of 3 mL of EDTA blood and DNA extraction from itsbuffy coat. The resulting DNA was processed by PCR to amplify MMR gene with primer I and II. The primer I successfully amplified DNAfragments up to 780bp while primer II only 329 bp. The MMR gene DNA sequencing analysis was performed on the amplification resultof both kinds primers by using DNA Baser and Ensembl−BLAST software. The results were different, DNA sequencing result by using theprimer I was found in several chromosomes and also in several loci. Whereas, by using the primer II, it was only found in chromosome10 and in the same locus. Based on this study, it can be concluded that the specific primer design is one of the most important factorsin the success of DNA sequencing.

Abstract

Background: The role of the clinical instructor is very important in clinical learning process that preparing students to become health workers who are competent in the practical environment. Effective clinical instructor is instructor who has profesional competence, interpersonal relationships, personality characteristics, and teaching skills in the process of providing guidance. The aim of the study was to analyze the relationship between clinical instructor characteristics and the score of pregnancy care clinical skills and analyze the most influenced characteristics of clinical instructor in the score of pregnancy care clinical skills in students of the Akademi Kebidanan Sari Mulia in Banjarmasin City.Methods: This study used a cross-sectional design with 11 clinical instructors and 25 Sari Mulia Midwifery Academy students using total sampling. This study used a Nursing Effectiveness Clinical Teacher Inventory questionnaire (NCTEI) questionnaire as instrument.Results: There is a relationship between clinical instructor characteristics and the score of pregnancy care clinical skills in students, namely interpersonal relationships (p = 0.043) and personality characteristics (p = 0.024). The dominant factor associated was personality characteristics (POR = 4.4; IK 95%: 1.02-19.08) with a value of p = 0.048.Conclusion: From this study, it can be concluded that the interpersonal relationships and personality characteristics can enhance the role of clinical instructor in the clinical learning process and personality characteristics represent the clinical instructor characteristics that are most in line with the score of pregnancy care clinical skills.
