Abstract

Chronic kidney disease is a global health issue affecting more than 10% of the world's population and poses a significant disease burden, especially in developing countries. In Indonesia, the National Health Insurance program (JKN) has enabled access to dialysis care for patients with chronic kidney failure, but it also presents challenges in sustaining financing. This study aims to analyze the trends in dialysis service claims in Pematangsiantar City during the period from 2017 to 2022 and project the service needs until 2025. The research design is descriptive-analytic, using claim data from BPJS Health related to dialysis procedures. Analysis is conducted using polynomial trends to identify historical patterns and future projections. The results show that the average annual cost of dialysis service claims in Pematangsiantar City is 85 billion rupiah. There was an annual increase in visits and claims costs by 25.56% and 23.52%, respectively. In 2021, dialysis visits surged by 91.6%, with claim costs reaching Rp128.2 billion. Projections up to 2025 indicate a significant upward trend, with an estimated number of visits approaching 350,000 in that year. The study's conclusions emphasize the importance of maintaining sustainable health financing through the JKN program and adjusting dialysis service capacity according to demand to prevent over- or undersupply. Additionally, primary prevention efforts through health education and early detection of kidney disease should be prioritized to reduce the number of new patients requiring dialysis.

Abstract

Penelitian ini bertujuan untuk mengeksplorasi perbedaan gaya kerja, stres kerja, dan loyalitas kerja antar Generasi X, Y, dan Z di Rumah Sakit Pusat Pertamina. Data dikumpulkan melalui kuesioner terstruktur yang telah diuji validitas dan reliabilitasnya. Hasil penelitian menunjukkan bahwa generasi secara signifikan memengaruhi aspek Work Centrality (p<0,001; aOR=0,253), Work-Life Balance (p<0,001; aOR=3,455), Leadership (p<0,001; aOR=2,365), dan Recognition (p<0,001; aOR=4,261). Generasi Z mencatat skor tertinggi pada Work Centrality dan Recognition, sementara Generasi Y unggul dalam Work-Life Balance. Loyalitas kerja tertinggi ditemukan pada Generasi X (p=0,017; aOR=2,909). Generasi Z menunjukkan kemungkinan tertinggi untuk mengalami stres kerja sedang (p<0,001; 6,9 kali lebih tinggi dibandingkan Generasi Y). Hasil analisis multivariat menunjukkan bahwa berbagai faktor demografis, termasuk jenis kelamin, pendidikan, status kepegawaian, jabatan, dan lama kerja, juga memiliki pengaruh signifikan terhadap dimensi-dimensi ini. Penelitian ini menyoroti pentingnya organisasi untuk mengadopsi pendekatan manajemen yang komprehensif dan berbasis kebutuhan untuk memaksimalkan potensi karyawan dari semua generasi.

Abstract

Pictorial health warnings already implemented for more than two years since June 24, 2014, however, there are no changes made following the Minister of Health Regulation No. 28 of 2013 until now. According to the results of the National Survey of Assessment of the Implementation of Pictorial Health Warning (PHW) in Indonesia in 2015 towards the compliance of the tobacco industry, it is known that the cigarette industry has not yet adhered to pictorial health warnings. The Government hopesthat the existence of the Minister of Health Regulation No. 28 can reduce the prevalence of smokers, but data from 2016 SIRKERNAS shows that the prevalence of smoking is increasing. The purpose is to dig deeper towards the implementation of the inclusion of health warnings and health information policy on cigarette packaging, henceforth it can be input into the formulation of tobacco control strategies. This study uses descriptive studies with qualitative analysis through in-depth interviews and document review. Aspects of the process (communication, bureaucratic structure and disposition), aspects of the actor (commitments and relationships) aspects of the content (level, benefits and policy objectives) as well as aspects of the context (political culture and socio-economic) are interrelated and influence each other in the implementation of the inclusion of warnings and health information on the cigarette packs. The implementation of the inclusion of warnings and health information on cigarette packaging has been carried out, and the level of industry compliance in including the PHW at the retail level has reached 99.91%. However, the cigarette industry is not compliant in fulfilling the provisions because the size is not appropriate, PHW images covered by excise tapes, and some terms indicate quality, superiority, security, and imaging.

Abstract

Insan Permata Woman and Child Hospital was facing Patient Safety Incidence especially for periode of mid to late 2016 there were 12 incidents and 11 phlebitis cases. This indicated a gap between the conditions in the field and the ideal situation that should be achieved. This study aimed to see how the behavior of employees in implementing patient safety program and the factors that influence it. The sample of this study were 151 respondents as sample, consist of health and non health workers. The patient safety program focused on the accuracy of patient identification, reduction of risk of infection and reduction of risk of falling patients. This research was a quantitative research followed by qualitative method. The results showed that most employees already had good patient safety behavior, age and attitude were the most affecting factors in patient safety behavior. The lack of socialization of accreditation and patient safety, poor monitoring and some incomplete facilities should became the concern for the hospital as it was considered to have great influences in the daily implementation of patient safety program.

Abstract

Many cases of medical and dental malpractice in Indonesia are still related to patient safety and healthcare quality. This narrative review aims to identify solutions that can be implemented. The main focus is to compare the incidence of malpractice in clinical medical and dental practice, emphasizing legal implications, coverage, and internal organizational issues. A systematic literature search was conducted through databases such as Pubmed, ScienceDirect, Google Scholar, Springer, Garuda, and other online databases. Articles published no later than 2024. Eleven studies were selected that discussed the problem of malpractice in Indonesia. Independent screening and critical appraisal were carried out to minimize bias, and data were retrieved and assessed through narrative synthesis. The results showed limited or lack of informed consent, non-compliance, administrative negligence, poor communication, and laws on medical malpractice. Causal factors, such as negligence in the operating room, misdiagnosis, and lack of consent forms, were common and most often resulted in patient injury or death. This review also explains a lack of studies related to this due to the poorly organized data collection system on complaints of medical malpractice in the country and the lack of adequate legal governance regarding dental malpractice. Regulations related to legal practices in Indonesia are still poorly organized, and even in similar cases, consistent enforcement is not guaranteed. These factors indicate the need for an evaluation of the healthcare system in Indonesia. Suggestions that can be given are to involve the community in the informed consent process, improve medical training, and improve the legal system so that these incidents can be reduced in number and patients feel protected.

Abstract

This study aimed to analyze the role of artificial intelligence in enhancing the quality of healthcare services and patient safety. The research followed the methodological framework proposed by Arksey and Malley, and a scoping review was conducted using the PRISMA guideline method. The authors searched several electronic databases, including Pubmed, NCBI, Elsevier, Proquest, EBSCO, Scopus, and Google Scholar, between 2017-2022 using specific keywords related to artificial intelligence, quality of health care, and patient safety. Out of 550 articles obtained, 61 were included in the review. The analysis of the selected studies indicated that artificial intelligence can significantly improve the quality of healthcare services and patient safety, especially in hospitals. However, further research is needed to develop AI systems that can be tailored to the specific needs of healthcare facilities, particularly hospitals and health centers in Indonesia. This study provides evidence for healthcare policymakers and practitioners to consider incorporating AI-based technologies to enhance healthcare quality and safety.

Abstract

Elders need friendly, competent, and responsive staff and an organized, clean and welcoming environment. However, preliminary interviews with multidisciplinary teams in nursing homes showed they sometimes felt bored. This study aimed to understand the multidisciplinary teams’ experiences of caring for elders in nursing homes. This is descriptive qualitative research used semi-structured interviews, forum group discussions (FGDs) with 64 participants in Jakarta provincial government-owned nursing homes, document analysis, and observations. The data were collected from September 2022 to July 2023 and analyzed using Colaizzi's measures. The results showed the elders at the nursing homes experienced physical, psychological, behavioral and social changes, where some of them came from poverty or neglect background. Their needs included biological, psychological, social, and spiritual aspects. The multidisciplinary teams felt a variety of emotions when serving the elders, such as happiness, sadness, and sometimes annoyance. Some also felt “reframing,” a feeling of being forced due to no other choice. Therefore, cultivating characters such as empathy, compassion, patience, and sincerity in providing the care is important. Changes in elders with different backgrounds require integrated support. Nursing home management should provide periodic training to the multidisciplinary teams; thus, the service quality can be improved.

Abstract

Dalam rangka mendukung peningkatan kapasitas aparat pemerintah daerah dalam melaksanakan peran dan fungsi setelah penerapan kebijakan desentralisasi, Unit Desentralisasi Departemen Kesehatan telah menyusun beberapa rencana strategis yang bertujuan mengantisipasi transisi desentralisasi, diantaranya adalah membangun organisasi pembelajar. Melihat bagaimana peranan pelatihan berpengaruh pada perubahan individu dan institusi pada lingkungan birokrasi yang lebih ketat pada tingkat pemerintah daerah. Penelitian ini adalah sebuah penelitian quasi-eksperimental dengan pre- dan post- intervensi, pengukuran yang berulang kali, dan kontrol yang tidak merata. Kelompok pertama akan menerima pelatihan SLLO (Xa) dan pendampingan (Xb). Kelompok kedua hanya akan berfungsi sebagai kelompok pengontrol. Penelitian ini menemukan: (1) Pelatihan LO efektif untuk meningkatkan pengetahuan, dan pemahaman peserta dalam perspektif kerja, masalah, keinginan mempebaiki diri dan menyebarkan kepada teman kerja. (2) Perubahan tingkat individu meliputi; keinginan memperbaiki perilaku kerja; keterbukaan dan kesiapan pencatatan perilaku; kesediaan menyediakan waktu; lebih banyak mempergunakan analisis penyebab masalah; lebih banyak dialog dan tidak terjebak dengan gejala; keinginan dan upaya menyebarkan LO dalam seminar dan pelatihan. (3) Bentuk perubahan kelompok dan institusi belum terlihat, berupa upaya untuk menyampaikan informasi pelatihan LO kepada peserta lintas sektor yang lebih luas. (4) Pengaruh lingkungan birokrasi pemerintah tingkat kabupaten mengakibatkan; perubahan individu berpengaruh pada perubahan kelompok sedikit; komunikasi membutuhkan waktu banyak; Sulit melakukan dialog antar anggota tim. Pada tingkat kecamatan keadaan berubah lebih baik; Perubahan individu yang berpengaruh pada perubahan kelompok lebih banyak; komunikasi antar anggota lebih cepat; dialog lebih mudah. In order to support local government capacity to implement their role and capacity after the implementation of decentralization policy, The Decentralization Unit of Ministry of Health RI has developed several strategic plans which directed to anticipate decentralization transition, such as learning organization. To objective of this study is to understand the effect of training program on individual and institutional changes in a more strict bureocratic environment. This study is a quasi-experimental study with pre and post intervention study design, several times measurements, and unequally distributed control. The first group receive SLLO training (Xa) and assistance (Xb). The second group is a control group. The study result show that: (1) the LO training is efective to increase knowledge and understanding of the trainees on job perspective, problem solving, self improvement need and distribution to group member (2) The individual level changes include the need to job behavioral improvement, the openness and readiness to record behaviour, more problem analysis, more dialogue and not trapped in surface symptoms, want and need to distribute LO in seminar and training (3) the group and institutional changes has not been seen yet (4) only small effect of individual changes to group level changes, communication needs plenty of time, difficult to conduct dialogue among team members. In sub-district level situation has chenged to a better situation, more effect of individual level cahnges to group level changes, faster communication between group members and easier dialogue.

Abstract

Pemberdayaan masyarakat adalah salah satu strategi dalam pembangunan kesehatan yang digunakan oleh pemerintah Indonesia. Pemberdayaan masyarakat ini menjadi salah satu fungsi puskesmas yang wajib dijalankan oleh seluruh puskesmas di tanah air. Walaupun strategi pemberdayaan masyarakat sudah lama digunakan, sampai sekarang belum ada instrumen spesifik untuk mengukur tingkat pemberdayaan masyarakat pada sektor pembangunan kesehatan di Indonesia. Penelitian ini bertujuan untuk mengukur tingkat pemberdayaan masyarakat pada level komunitas sebagai wilayah kerja puskesmas di Depok dan Jakarta Selatan. Pemberdayaan masyarakat diukur menggunakan tujuh potensi masyarakat meliputi kepemimpinan, organisasi, dana, sumber daya, teknologi, pengetahuan, dan pengambilan keputusan. Metode pengukuran dilakukan dengan membandingkan ketujuh potensi masyarakat di wilayah kerja puskesmas di Depok (32 puskesmas) dan wilayah kerja puskesmas terpilih Jakarta Selatan (28 puskesmas) dengan potensi standar yang dikembangkan peneliti. Hasil penelitian memperlihatkan bahwa tingkat pemberdayaan masyarakat di wilayah kerja puskesmas di Depok umumnya banyak yang memenuhi kategori baik, sebaliknya di Jakarta Selatan banyak kategori kurang. Provinsi Daerah Khusus Ibu Kota Jakarta berupaya pemberdayaan masyarakat dan promotif preventif lebih mudah dilakukan oleh kantor kelurahan daripada puskesmas. Community empowerment is one of strategies in health development that is used by government of Indonesia. It is also one of puskesmas (primary health center) function that must be run by every primary health center in Indonesia. Though community empowerment has been used for a very long time, there is not any specific instrument to measure level of community empowerment in health sector development in Indonesia. This research aimed at measuring community empowerment at community level using neighbourhood association as work area in two cities in Indonesia, Depok and South Jakarta. Community empowerment is measured using seven community potentials including leadership, organizations, fund, resource, technology, knowledge, and decision making. The measurement method is comparing those community existed potentials with potential standard developed by researcher in 32 primary health center in Depok and 28 selected primary health center in South Jakarta. The result shows that level of community empowerment in primary health center work area in Depok is generally in good categories, but South Jakarta is generally in less category. In Jakarta, the effort of community empowerment and promotive preventive is conducted easier by village administration office than primary health center.

Abstract

Secara nasional, konsumsi garam beryodium cukup adalah 62,3% dan di Provinsi Jawa Barat adalah 58,3%. Cakupan konsumsi garam beryodium tingkat rumah tangga di Kota Bekasi hanya sekitar 62,14%. Pemantauan garam beryodium di tingkat rumah tangga oleh Dinas Kesehatan Kota Bekasi tahun 2004 menunjukkan bahwa garam yang mengandung yodium cukup adalah 51%. Penelitian ini dilakukan untuk mengetahui analisis faktor demand dan supply terhadap konsumsi garam beryodium tingkat rumah tangga di wilayah kerja Puskesmas Kecamatan Bekasi Barat Kota Bekasi dengan menggunakan desain cross sectional. Populasi yang diteliti yaitu 110 orang ibu dengan menggunakan uji chi square. Pada faktor demand didapatkan hasil bahwa ada hubungan pengetahuan dengan konsumsi garam beryodium tingkat rumah tangga. Namun, tidak ada hubungan antara pendapatan dengan konsumsi garam beryodium tingkat rumah tangga. Pada faktor supply didapatkan hasil bahwa tidak ada hubungan antara ketersediaan di pasar dan harga dengan konsumsi garam beryodium tingkat rumah tangga. Untuk meningkatkan cakupan konsumsi garam beryodium tingkat rumah tangga diperlukan kerja sama dari berbagai pihak. Nationaly, the consumption of iodized salt is 62,3% and in Province of West Jawa is 58,3%. The coverage consumption of iodezed in household level in Bekasi city only about 62,14%. The monitoring iodezed salt in household level by district health departemen in 2004 showed that the enough iodezed salt is 51%.This research was conducted to determine the factor analysis of demand and supply of iodized salt consumption at household level in the District of West Bekasi. This iodesed salt udy used cross sectional design. The population that was studied was 110 mothers using chi square test. On the demand factor, the result shows that there is a relationship between knowledge and the consumption of iodized salt at household level. However, there is no relationship between the revenue and the consumption of iodized salt at household level. While in the supply factor, shows that there is no relationship between availability and price in the market and the consumption of iodized salt at household level. To improve the coverage of iodized salt consumption at household level, it is required cooperation from various parties.
