Abstract

Chronic Kidney Disease (CKD) is a condition resulting from a decline in the kidneys' ability to maintain bodily balance. In CKD patients, fluid volume overload can lead to edema; specifically, edema is caused by low blood albumin levels. Ankle pump therapy is a non-pharmacological approach used to reduce edema. Objectives To determine the effect of ankle pump therapy on edema levels in chronic kidney disease patients at RSU Diponegoro Dua Satu Klaten. This study employed a pre-experimental design using a one-group pretest-posttest approach. The sample consisted of 60 respondents selected via purposive sampling. Data collection utilized a pitting edema measurement sheet and a Standard Operating Procedure (SOP) for ankle pump exercises; data analysis was performed using the Wilcoxon test. The Wilcoxon test for the 60 respondents yielded a p-value of 0.001 (p < 0.05), indicating that ankle pump therapy reduces edema levels in patients with chronic kidney disease. Abstract

Patients with chronic kidney disease (CKD) undergoing hemodialysis frequently experience anxiety, which can negatively impact their quality of life and treatment adherence. Complementary therapy combining simple acupressure and lemon aromatherapy is suggested to induce relaxation, thereby helping to reduce anxiety levels. This study aimed to analyze the effect of combined simple acupressure and lemon aromatherapy on reducing anxiety levels in hemodialysis patients. A quasi-experimental study with a pretest-posttest control group design was conducted at the Hemodialysis Unit of RSU Diponegoro Dua Satu Klaten in June 2026. A total of 42 respondents were selected using purposive sampling and divided equally into an intervention group (n = 21) and a control group (n = 21). The instruments utilized included Standard Operating Procedures (SOPs) for simple acupressure, SOPs for lemon aromatherapy, and the Hamilton Anxiety Rating Scale (HARS) questionnaire. Data were analyzed using the Shapiro–Wilk normality test and the paired t-test. Prior to the intervention, the majority of respondents in the intervention group experienced moderate anxiety (95.2%). Following the combination therapy, anxiety levels significantly decreased, with the majority shifting to mild anxiety (76.2%) and showing a mean score reduction of 6.81 on the HARS scale. Conversely, the control group showed no significant change in anxiety categories, with a minor mean score reduction of 1.24. The paired t-test confirmed that the combination therapy had a statistically significant effect on reducing patient anxiety (p < 0.001). Combined simple acupressure and lemon aromatherapy is effective in reducing anxiety levels and is recommended as a safe, independent complementary nursing intervention for hemodialysis patients.
