Abstract

This research was conducted at the employee cooperative of PT. XYZ to identify the quality of service provided by the company cooperative to employees of PT. XYZ and provide suggestions for improving service quality which is the main priority for improvements. The method used in this research is the service quality method and Importance performance analysis with a quantitative observation model, this research begins in July 2023 - August 2023. The respondents in this research are all employees of PT. The sampling method was taken based on Slovin's formulation and then the results were obtained from 95 people. The results of research that had been carried out throughout the GAP on five Servqual dimensions had negative values including tangible (-0.11), reliability (-0.35), responsiveness (-0.10), assurance (-0.11), and empathy (-0.39), so the entire gap has a negative value, namely employee satisfaction with the PT cooperative. XYZ has not been fulfilled. Corrective actions that must be implemented by the management of the PT employee cooperative. XYZ, based on the results of the Importance performance analysis on the Cartesian diagram, can be seen in quadrant A which is the quadrant with the main priority that needs improvement. There are 2 attributes in quadrant A including employees not looking at customer status in providing services (E2) and recording orders and transactions accurately (R2).

Abstract

Sinom drink product "FA DRINK" is a traditional drink that has many benefits for the body. In the development of its business, this beverage product has constraints in developing products to be accepted by the community. One of the problems encountered is the large number of similar beverage products on the market. The development strategy that will be carried out is based on strengths, weaknesses, opportunities and threats with the SWOT (Strengths, Weakness, Opportunities, and Threats) method, which is then sorted by the QSPM matrix to get a priority strategy. The results showed the total weight of the IFE matrix score of 3.319 and EFE of 2.990 so that the company was in a grow and build condition. Based on the QSPM matrix, the highest value is 6.863 and the lowest value is 5.17. Product development strategy recommendations based on the QSPM matrix are to collaborate with traders to increase sales, marketing by participating in exhibitions, improving product packaging to make it more attractive strengths

Abstract

Occupational Safety and Health is one of the most important elements that must be applied in the working stages. The implementation of K3 in the workplace can create a safe, comfortable, and efficient workplace. PT Paku Jaya as one of the companies engaged in the construction sector is always committed to implementing K3 in the work area. One of the jobs that are often done by PT Paku Jaya is Cleaning Scrubbing. During the work process there are violations in the implementation of K3 in the work area. This can cause accidents in the work area. The methods used to conduct occupational safety and health analysis in this study are HIRARC and HEART. Based on the results of the study using HIRARC method, 9 types of hazards were found. Based on the analysis using HEART method, the largest HEP value is 0.192, which is opening the Blind Sprayer line. Risk control can be carried out in accordance with the hierarchy of hazard control that can be applied to Cleaning Scrubber work, namely Engineering, Administrative Control, and PPE.

Abstract

Penelitian ini bertujuan menganalisis risiko ergonomi pada operator mesin bubut di CV Sumber Rezeki yang menghadapi potensi gangguan muskuloskeletal (Musculoskeletal Disorders/MSDs) akibat postur kerja statis dan repetitif. Menggunakan metode Rapid Entire Body Assessment (REBA) dan Rapid Upper Limb Assessment (RULA), studi ini mengevaluasi postur tubuh pada aktivitas produksi utama. Hasil analisis menunjukkan tingkat bahaya yang kritis, di mana skor tertinggi tercatat mencapai 11 untuk REBA dan 9 untuk RULA, yang diklasifikasikan sebagai tingkat risiko sangat tinggi (very high risk). Kondisi ini mengindikasikan perlunya intervensi perbaikan segera untuk mencegah cedera jangka panjang. Penelitian ini berkontribusi dalam memberikan bukti empiris mengenai bahaya postural di lingkungan bengkel bubut serta merumuskan dasar perbaikan ergonomi untuk meminimalkan risiko fisik dan meningkatkan keselamatan kerja operator.

Abstract

Proyek manufaktur Replace 54in Sea Water Line di PT. SRK semula direncanakan selesai dalam 49 hari, tetapi terlambat 26 hari menjadi 75 hari. Penelitian ini menggunakan Fault Tree Analysis (FTA) untuk mengidentifikasi penyebab utama keterlambatan, yaitu pekerjaan drawing engineering, dan menghasilkan 4 langkah perbaikan bagi kontraktor. Penjadwalan ulang dengan Critical Path Method (CPM) menunjukkan bahwa penambahan jam kerja lembur 4 jam selama durasi proyek dapat mempercepat penyelesaian menjadi 32 hari dengan biaya tambahan sebesar Rp 16.000.000. Alternatif penambahan tenaga kerja mempercepat proyek menjadi 32 hari dengan biaya tambahan Rp 13.500.000. Kesimpulannya, penambahan tenaga kerja lebih efektif dari segi biaya untuk mencegah keterlambatan pada proyek Replace 54in Sea Water Line berikutnya.

Abstract

PT SWA is a steel fabrication service company operating in the construction sector, where productivity performance plays a critical role in meeting project schedules and cost targets. This study aims to measure weekly productivity levels in the steel fabrication process and to identify the dominant factors contributing to productivity fluctuations. The research employs the Objective Matrix (OMAX) method to quantify productivity performance using selected productivity ratios, which is then integrated with Root Cause Analysis (RCA) to identify the underlying causes of productivity decline. The results reveal significant weekly productivity dynamics during the ten-week observation period. Productivity increased during Weeks 1 and 2, with the highest improvement recorded in Week 2, achieving a productivity index of 190%. However, a continuous decline was observed from Weeks 3 to 10, with the lowest productivity index occurring in Week 8 at −78.33%. The analysis indicates that machine-related disruptions, substandard raw materials, and insufficient operator skills were the primary contributors to low-performing productivity ratios and overall productivity deterioration. The novelty of this study lies in integrating weekly OMAX-based productivity measurement with RCA to provide data-driven, actionable improvement insights within the context of steel fabrication services. The findings offer practical managerial implications, highlighting the importance of preventive maintenance planning, material quality control, and operator competency development to stabilize and improve productivity performance. Keywords: Productivity Measurement, Objective Matrix (OMAX), Root Cause Analysis, Steel Fabrication Services

Abstract

CV. XYZ is a supplier of waste wood powder, specifically utilizing Merbau wood. The company currently faces product quality issues, including powder mixed with other sizes (43.2%), foul odor and mold (16.2%), and excessive moisture/wetness (40.6%). This study aims to analyze and improve product quality using Six Sigma and FMEA methods. The results indicate an average DPMO value of 40,621 with a Sigma level of 2.46, suggesting that the process is not yet optimal. The FMEA analysis identified the machine factor as the primary cause of defects, with the highest Risk Priority Number (RPN) of 540. Improvement recommendations include machine recalibration, scheduled cleaning, and preventive maintenance. The implementation of these strategies is expected to reduce defects and enhance the company's competitiveness. This research contributes to quality control in the wood powder waste processing industry and opens opportunities for further research into developing more effective systems. Keywords: Defects, Six Sigma, DMAIC, FMEA, Quality Control, Product Defects

Abstract

Occupational Safety and Health (OSH) plays a critical role in maintaining safe and sustainable production, particularly in small-scale agro-industrial operations such as coffee processing. This study applies the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) framework to evaluate workplace hazards at Kopi Lanange Jagad, a micro-scale coffee producer in Malang Regency, Indonesia. Field observations and structured interviews were conducted to identify potential hazards across four production stages roasting, grinding, sieving, and packaging. Quantitative measurements were also taken, including ambient temperature, noise level, and airborne dust concentration, to provide empirical data for risk evaluation. The findings identified a total of 12 hazards, classified into three risk categories: high (25%), moderate (50%), and low (25%). The roasting process exhibited the highest risk score (R = 18.4), with average workplace temperatures reaching 85°C–120°C, significantly exceeding the national occupational limit of 30°C for moderate workloads (Permenaker No. 5/2018). The grinding stage also presented considerable hazards, with airborne dust concentrations averaging 2.4 mg/m³, approaching the Ministry of Manpower’s threshold limit value (TLV) of 3 mg/m³. Manual packaging was associated with ergonomic strain, where workers lifted 15–20 kg sacks repeatedly, leading to potential musculoskeletal disorders. Risk evaluation revealed that existing control measures such as natural ventilation and sporadic use of personal protective equipment (PPE) were insufficient to ensure worker safety. To reduce these hazards, the study recommends implementing engineering controls such as local exhaust ventilation and machine insulation, combined with administrative measures including job rotation, scheduled rest breaks, and regular safety training. Ensuring consistent PPE use especially heat-resistant gloves, dust masks, and ear protection can further mitigate residual risks. Overall, the application of the HIRARC framework at Kopi Lanange Jagad demonstrates its effectiveness as a low-cost and practical tool for identifying, prioritizing, and managing occupational hazards in small-scale coffee industries. Implementation of the proposed control strategies is projected to reduce total workplace risk by 35–40%, improving both worker well-being and operational productivity.

Abstract

The layout of facilities in the industrial world greatly affects the production process of a product. If the spatial layout used is good and appropriate, it will indirectly save distance, costs, time, and effort. Community service activities are carried out in partnership with SMK Manbaul Ulum. These activities aim to provide learning about facility layout to minimize production process time. The expected outcome of these activities is that students will be able to understand how to reduce excessively long production times and increase the number of products in a business unit, which impacts student productivity. The implementation methods include delivering material, simulating the production process layout, and conducting Q&A discussions. As a result, the activities are able to enhance students' understanding of the importance of layout in production facilities. Students can also simulate the production process to evaluate the existing layout in order to minimize production time. This activity has an impact on decision-making and problem-solving.

Abstract

Kualitas produk merupakan faktor penting dalam menjaga daya saing perusahaan manufaktur. CV. SMY merupakan perusahaan yang bergerak di bidang produksi tas ransel dengan tingkat permintaan yang cukup tinggi. Namun, dalam proses produksinya masih ditemukan berbagai jenis kecacatan produk yang berdampak pada meningkatnya biaya produksi serta menurunnya kepuasan pelanggan. Penelitian ini bertujuan untuk menganalisis pengendalian kualitas pada produk tas ransel serta mengidentifikasi faktor penyebab utama terjadinya kecacatan produk. Metode yang digunakan dalam penelitian ini adalah Failure Mode and Effects Analysis (FMEA) untuk menentukan prioritas risiko kegagalan berdasarkan nilai Risk Priority Number (RPN) dan Root Cause Analysis (RCA) untuk mengidentifikasi akar penyebab terjadinya kecacatan produk. Data penelitian diperoleh dari laporan produksi dan data produk cacat periode Januari–September 2025 melalui observasi, wawancara, dan dokumentasi perusahaan. Hasil penelitian menunjukkan bahwa jenis cacat dengan nilai RPN tertinggi adalah jahitan tidak rapi yang disebabkan oleh kurangnya ketelitian operator dan ketidakstabilan pengaturan mesin jahit. Selain itu, faktor lain yang memengaruhi kecacatan produk adalah kondisi lingkungan kerja yang lembap serta kualitas material kain dan resleting yang kurang baik. Berdasarkan hasil analisis RCA, usulan perbaikan yang diberikan meliputi peningkatan keterampilan operator melalui pelatihan berkala, penerapan SOP secara lebih ketat, perbaikan kondisi lingkungan kerja, serta peningkatan standar pemilihan bahan baku. Implementasi usulan perbaikan tersebut diharapkan dapat menurunkan tingkat kecacatan produk dan meningkatkan efektivitas sistem pengendalian kualitas pada proses produksi tas ransel.
