Abstract

Title: Relationship of Some Factors with the Utilization of the 4th Visit Services (K4) in Pregnant Women (Study in Public Health Services of Kalimas, Randudongkal, Pemalang District).Background: In 2018 Public Health Services of Kalimas had the highest K4 achievement and had met the Minimum Service Standards (100.71%). In the implementation of K4 there are differences in the standard K4 coverage, K4 access and K4 target. This can be seen from the May 2019 standard K4 coverage of 31.24% K4 coverage of access is 35.59% and the target of 41.67%. Methods: This study aims to analyze the relationship of several factors with the use of K4 services in Public Health Services of Kalimas. This study is an explanatory research with cross sectional design. The study population numbered 112 pregnant women. By using simple sampling random technique, 93 sample were selected. Statistical test was bivariate analysis (Chi Square; Continuity Correction). Result: The result show 75.3% of mothers have low parity, 71.0% of mothers have high family income (≥UMK), 71.0% of mothers have ease in accessing accessibility, 81.7% of mothers have high decision making authority, 73.1% have support from cadres, 64.5% the availability of pregnancy information, 53.8% of mothers get incomplete services, 82.8% of mothers have never had a miscarriage, and 82.8% of mothers use K4 services with access. Conclusions: There is no relationship of parity, family income, accessibility, women’s authority in decision making, health cadre’s support, and history of miscarriage with the use of K4 services. There is relationship between the availability of pregnancy information (p-value = 0.006), completeness of service (p-value = 0.032) with the use of K4 services. Optimizing the provision of information through consoling to mothers, providing IEC (Communication, Information and Education), conducting interpersonal communication, and making regular visits to mother with help of midewives.

Abstract

Abstrak: Tujuan dari kegiatan pelatihan E-Marketing di IKM Sanan Kota Malang ini adalah memberikan wawasan, pengetahuan, dan pendampingan bagi para pelaku usaha kripik tempe untuk melakukan pengembangan usaha melalui pemasaran online yang harapannya dapat meningkatkan memperluas jaringan pesaran dan mampu meningkatkan omst pendapatan pelaku usaha. Metode yang dilakukan adalah melalui sosialisasi, kegiatan pelatihan dan pendampingan kepada para pelaku usaha sampai dengan pelaku usaha mempraktekkan langsung bentuk-bentuk promosi online. Hasil yang diperoleh dari kegiatan pelatihan ini adalah dapat meningkatkan pengetahuan dan keterampilan wirausaha di dalam membuat membuat media pemasaran online, sehingga meningkatkan motivasi di dalam berusaha. Pelatihan E-Marketing ini mendapat respon yang positif dan pelaku usaha mendapat pengalaman berharga dengan mempraktekkan sendiri model pemasaran online yang mudah diaplikasikan.Abstract: The aim of the E-Marketing training activity at IKM Sanan Malang is to provide insights, knowledge, and assistance for tempe kripik businesses to develop business through online marketing which hopes to increase network expansion and be able to increase the revenue of business actors. The method used is through outreach, training activities and mentoring to business actors to business practitioners directly practicing online forms of promotion. The results obtained from this training activity are able to increase entrepreneurial knowledge and skills in making making online marketing media, thus increasing motivation in doing business. This e-marketing training received a positive response and businesses gained valuable experience by practicing their own online marketing models that were easy to apply.

Target of: This research aim to know therapeutic communications influence to behavioral degradation hardness behavior at schizophrenia patient at home Psychopath Area Surakarta. Method research use design experiment true, with pretest type control design group. Intake of Sample by using non sampling purposive type sampling probabiility matching with inklusi. Result criteria: result of research, indicating that: with value test Paired t test there are behavioral degradation of hardness at schizophrenia patient at treatment group and group control by significant. And percentage only there is difference equal to mean value 6 % among two groups. and Value difference between group control and this treatment group in test by using is Independent of t Test discovered by value 0,324, this value is compared to smaller of p significant value 0,05 meaning the result have a meaning of or significant. This matter answer hypothesizing early, meaning there is influence applying of therapeutic communications to behavioral degradation of hardness.

Abstract

Background: The establishment of macrovascular (cardiovascular) event is initiated in the period of prediabetic. Metformin plays role in the carbohydrate and lipid metabolism as well as vascular protection. The mechanism of the cardiovascular event risk increased in the pre-diabetic individual has not been clearly established.Objective: The study was to determine if insulin resistance control by low-dose short-course metformin administration in the non-obese prediabetic individual might decrease the risk of atherosclerosis.Methods: It was a prospective experimental double blind study with pretest-posttest control group design. It was conducted on the first degree relatives of type-2 Diabetes Mellitus patients who met the criteria of non-obese prediabetic individuals. Subjects were randomly classified into treatment group (designed for metformin administration), and the control group (designed for placebo administration). The prediction of atherosclerosis risk was based on the hsCRP, PAI-1, VCAM-1, and fibrinogen levels.Result: The results showed that the margin between pre- and post-metformin administration term were 1.89 vs. 0.06 mg/L (p=0.001), 1.42 vs. 0.84 IU/mL (p=0.0151. 180.85 vs. 03.81 mg/L (p=0.061) and 80.15 vs. 31.42 mg/dL (p=0.001) for hsCRP, PAI-1, VCAM-1, and Fibrinogen levels in the treatment and control group, respectively.Conclusion: The study concluded that the low-dose short-course metformin administration might decrease the atherosclerosis risk significantly in the non-obese prediabetic individuals.Key words: metformin - non-obese prediabetics - hsCRP - PAI-T- Fibrinogen - VCAM-T

Abstract

Introduction. Multi Drug Resistance (MDR) antibiotics have become a global health threat to the community. The use of appropriate antibiotics makes preventive and curative measures very important for the success of overcoming MDR and intervening the complexity of resistance, at least slowing the rate of occurrence of MDR. This study aimed to identify the difference in the quality of antibiotic use quality of use using gyssens flowchart and the success in sepsis patients’ treatment due to gram-negative MDR bacteria with non-MDR bacterial infections in tertiary hospitals. Methods. A retrospective cohort study was conducted using secondary data from patient with age more or equal than 18 years who were hospitalized in inpatient unit or ICU in Indonesia Army Central Hospital Gatot Soebroto from 2017-2019. Both the MDR and non-MDR gram-negative inclusion groups were assessed for the quality of antibiotic using the Gyssen flowchart. Results. There was a significant difference between antibiotic quality on gram negative MDR bacteria and non-MDR (20.6% vs. 13.6%; RR 1.517 (CI 95% 1.1 – 2.1); p value = 0.015) which showed that gram-negative MDR experienced 1.517 times better quality than non-MDR. This study also found that there was a significant difference between therapeutic success on gram negative MDR bacteria and non-MDR (57.4% vs. 39.1%; RR 1,431 (CI 95% 1.0 – 2.1); p value = 0.02). Conclusions. There is a better quality on the utilization of antibiotics with Gyssens index patients resulted from MDR negative gram bacterial infection in comparison to non-MDR in tertiary hospital. There is also a better success in treating the sepsis patient with MDR negative gram bacterial infection in comparison with non-MDR.

Abstract

Introduction. Mortality rate of invasive candidiasis is still high, approximately 30-70%. Every study has a variety mortality rate depend on study design and sample. There is no data in Indonesia about profile and mortality factors analysis in critically ill patients with invasive candidiasis. Methods. The Study design was cross sectional. We studied 102 hospitalized critically ill patients with invasive candidiasis. The demographic, clinical and laboratory data, the risk factors for invasive candidiasis and the outcome of each patient in 30 days were recorded. An analysis bivariate with chi square or Fisher’s test was carried out to analyse some factors such as age > 60 years old, severe sepsis, APACHE score > 20, respiratory failure, renal failure, delayed antifungal treatment > 72 hours after positive culture, Charlson index score, and ICU or non ICU patients. The logistic regression of multivariate analysis was carried out to identify the most influence of all mortality factors. Results. Among 102 identified sample, the majority was male (52.9%), the median age was 53 years old and the mortality rate was 68,6%. Laboratory candida findings came from blood sample (candidemia) (98.03%), liquor cerebrospinal (1.5%) and retina exudate (1.5%). The most common candida species were candida non albicans especially Candida tropicalis (34.3%) and Candida parapsilosis (34.3%). The risk factors for invasive candidiasis from this study related to underlying disease were sepsis (78.9%), malignancy (42.15%), diabetes mellitus (29.4%) and related to therapy or treatment were the usage of broad spectrum antibiotic (99%), central vein catheter (77.5%), and parenteral nutrition (70.6%). The result from multivariate analysis, severe sepsis (p 0.001, OR 7.7, IK95% 2.4 – 24.7), Charlson Index ≥3 (p 0.022, OR 3.5, IK95% 1.2–10.2), and respiratory failure (p 0.066, OR 2.7 IK95% 0.9 – 8.0) were independently associated with mortality. Conclusions. In Cipto Mangunkusumo hospital, most critically ill patients with invasive candidiasis was male, median age was 53 years old, and mortality rate was 68,6%. The most species candida caused infection were Candida tropicalis and Candida parapsilosis. The most risk factors of invasive candidiasis from underlying disease was sepsis and from the treatment was the usage of broad spectrum antibiotic. Severe sepsis, and Charlson index ≥3 were associated with a 30 day mortality in critically ill patients with invasive candidiasis.

Abstract

Introduction. The virulence factors from community acquired-methicillin resistant Staphylococcus aureus (CA-MRSA) mainly due to toxins like Panton Valentin Leukocidin (PVL) and Phenol Soluble Modulin (PSM). Both of toxins cause decrease of value through neutrophil lysis. This study aimed to identify different value of absolute neutrophil count between hospital and community acquired MRSA. Methods. A cross sectional was conducted which included subjects who were infected by MRSA and hospitalized during 2012-2017. Classification of MRSA were divided due to its sensitivity and resistance to non-beta lactam antibiotics. Isolate that resistance to ≤ 2 antibiotics were classified as CA-MRSA. The others with resistance to ≥ 3 antibiotics were classified as hospital acquired MRSA. Absolute neutrophils count (ANC) were collected 24 hours from the positive MRSA culture. Data were analyzed by using independent T test and Mann-Whitney test. Results. We collected 62 subjects infected by MRSA which 35 subjects were HA-MRSA and 27 subjects were CA-MRSA. The median of ANC from CA-MRSA was 7,410.7 (1,147.3-26,560.2) and HA-MRSA was 16,198.0 (3,921.6-28,794.1) with p value < 0.001. Conclusion. There was a different value of absolute neutrophil count in infections due to community and hospital acquired MRSA.

Abstract

Phylogentic Relationship among Limnonectes (Ranidae: Amphibia) found in West Sumatra with Other Species from South East Asia based on the based on the 16S rRNA Gen. The objective of this research was to study the phylogenetic relationship among Limnonectes species found in West Sumatra and with other species from South East Asia based on the partial DNA sequences16S rRNA sequences. DNA sequences were aligned using ClustalX version 1.64b, and the phylogenetic relationship within samples were analyzed using PHYLIP version 3.5c program. The alignment showed that, from 805 sites, there are 250 parsimony informative polymorphism sites. The phylogenetic tree showed that all of the Limnonectes spesies were divided in two clusters, the L. blythii complex and L. kuhlii complex. L. kuhlii and L. sp1 clustered into L. kuhlii complex and L. shomponerum and L. macrodon were clustered to L. blythii complex. This result showed that L. kuhlii and L. blythii are species complexes that are actually constituted of several species.

Abstract

Sumur DA-011 merupakan sumur yang menggunakan artificial lift berjenis gas lift. Terdapat permasalahan yang terjadi dalam pengoperasian sumur gas lift. Salah satu permasalahan pada sumur DA-011 yaitu kebocoran pada valve yang terpasang. Kebocoran valve ditandai dengan titik injeksi sumur DA-011 berada pada gas lift valve ke-1 yang seharusnya beroperasi pada valve ke-7. Sehingga, perlu dilakukan evaluasi dan analisa kondisi sumur DA-011 sebelum dilakukan desain ulang untuk mengganti valve yang terpasang sebelumnya. Desain ulang dilakukan menggunakan spreadsheet untuk mendapatkan laju gas injeksi, letak kedalaman masing-masing valve, dan mengetahui titik injeksi yang optimum. Selain itu, software pipesim digunakan Gas Lift Performance Curve untuk mengetahui laju gas injeksi dari valve yang terpasang. Saat kebocoran valve, sumur DA-011 menginjeksikan gas sebesar 0,418 mmscfd, laju produksi sebesar 247 blpd dan laju produksi minyak sebesar 9,88 bopd. Sedangkan saat optimasi sumur dapat menginjeksikan gas sebesar 0,8 mmscfd, laju produksi sebesar 1900 blpd dan laju produksi minyak sebesar 76 bopd. Kata Kunci : Artificial Lift, Desain ulang, Gas Injeksi, Gas Lift Valve.   ABSTRACT The DA-011 well is a well that uses an artificial lift type gas lift. There are problems that occur in the operation of the elevator gas well. One of the problems in the DA-011 well is a leak in the installed valve. The valve leak is indicated by the injection point of the DA-011 well being in the 1st gas lift valve which is supposed to operate on the 7th valve. Thus, it is necessary to evaluate and analyze the condition of the DA-011 well before redesigning to replace the previously installed valve. The redesign was carried out using a spreadsheet to obtain the injection gas rate, the depth of each valve, and find out the optimum injection point. In addition, pipesim software is used Gas Lift Performance Curve to determine the rate of injection gas from the valve installed. During the valve leak, the DA-011 well injected 0.418 mmscfd of gas, a production rate of 247 blpd and an oil production rate of 9.88 bopd. Meanwhile, when optimizing the well, it can inject gas of 0.8 mmscfd, the production rate of 1900 blpd and the oil production rate of 76 bopd.   Key Words : Artificial  Lift, Redesign, Gas Injection, Gas Lift Valve
