Abstract

Chronic spontaneous urticaria (CSU) is a mast cell driven skin disorder that is characterized by the development of recurrent itchy wheals for 6 weeks or more due to unknown causes. Diagnosis is purely clinical while assessment of disease activity, its impact on quality of life, as well as treatment efficacy are based on history, physical examination, and the use of questionnaires. However, questionnaires have the disadvantage of being subjective. In the last decade, studies have been focusing on understanding of chronic spontaneous urticarial pathomechanism and identification of potential biomarkers to determine disease activity, clinical course, and treatment efficacy. Some of the biomarkers that have been studied are D-dimer, C reactive protein, interleukin-6, FcεRI, and vitamin D. Biomarkers will then be classified into clinical and molecular biomarkers. With identification of biomarkers that considered to be more objective and reliable, personal approach in evaluating the course of the disease and the most suitable treatment for patients can be achieved.

Abstract

Background: Hair loss is a normal experience, but if the amount of hair loss exceeds what is considered normal, it will usually cause anxiety for the patient. In Indonesia, almost every Muslim woman wears a hijab. The hijab is one of many factors believed to cause hair loss. Telogen effluvium (TE) itself is a hair cycle disorder in which the anagen phase terminates prematurely, resulting in diffuse club hair loss without scarring, and TE itself has many risk factors. The purpose of this study was to determine the relationship between the incidence of TE in women who wear hijabs and those who do not. Methods: This cross-sectional study was conducted from August 2019 to April 2021 involving 188 healthy women aged 18 years and over who had not yet reached menopause. In the hijab group, it was determined that they should wear a minimum of 8 hours a day for at least 5 years. The test was carried out on hair that had not been washed using a trichogram, to look for hair characteristics in each group (hijab-wearing and non-hijab-wearing). Analysis of the data obtained was carried out using the Chi-square test and odds ratio. Results: Telogen effluvium was higher in subjects wearing hijab (26.8%) than in subjects who did not wear hijab (18.1%), (p-value 0.040, OR 2.036). Conclusion: There was a significant relationship between TE in women who wear hijab compared to those who do not. Further studies should be conducted in other populations with a larger sample size to confirm these results.

Abstract

Background: Antibiotics are used in daily practice. To properly select antibiotics, it is important to pay attention to the definitive diagnosis, class of antibiotic, dose, interval, route of administration, and exact duration of use. Discussion: The World Health Organization (WHO) categorizes antibiotics into three groups: Access, Watch, and Reserve—known as the AWaRe classification—to help guide appropriate usage and combat antibiotic resistance. The Gyssens flowchart serves as a valuable tool for clinicians to qualitatively evaluate antibiotic utilization. In skin and soft tissue infections (SSTIs), treatment strategies are tailored according to the characteristics of the infection, including purulence, necrosis, and overall severity. For mild-to-moderate non-purulent SSTIs, beta-lactam antibiotics, such as penicillin and cephalosporin, alongside lincosamides like clindamycin, are recommended as first-line therapies. In cases of moderate-to-severe non-purulent SSTIs, empirical therapy must account for methicillin-resistant Staphylococcus aureus (MRSA). Meanwhile, purulent SSTIs require an initial incision and drainage, with subsequent treatment options including topical antibiotics for mild cases and systemic antibiotics for more complicated or severe infections. Extensive purulent SSTI in adults is empirically considered MRSA. Conclusion: Control of antimicrobial resistance can be achieved by using antibiotics according to the AWaRe classification and evaluating the use of antibiotics qualitatively using the Gyssens flowchart.

Abstract

Background: Psoriasis is a dermatological condition influenced by genetic and epigenetic factors, environmental triggers, as well as immune responses. Genomics refers to the study of individual genes (genomes) and their interactions with environmental influences. In the context of psoriasis genomics, the interplay between genetic predisposition and environmental factors contributes to the manifestation and variations of the disease. With advancements in molecular genotyping technology and genome-wide association studies (GWASs), researchers have identified 50 genetic markers linked to psoriasis. Discussion: Several genetic loci associated with psoriasis have been identified, including PSORS 1-15. The genetics of psoriasis is also linked to its onset, precipitating factors, associated comorbidities, and clinical variations. Advances in psoriasis genomics are leading to more targeted and innovative treatment approaches. The latest therapeutic developments for psoriasis mainly focus on the interleukin-23 (IL-23)/T-helper 17 (Th17) pathways. Conclusion: The development of GWASs provides new knowledge about psoriasis, including its pathogenesis and specific treatments.

Abstract

Pendahuluan: Dermatitis atopik (DA) memiliki karakteristik pruritus, eritema, indurasi, skuama. Manifestasi klinis ini dapat pula ditemukan pada berbagai penyakit kulit yang menyerupai, terjadi bersamaan, atau merupakan komplikasi DA. DA awitan dewasa dapat menunjukkan variasi klinis yang tidak biasa. Kasus: Laki-laki berusia 36 tahun, mengeluhkan bercak kemerahan yang terasa gatal di badan dan ekstremitas sejak 6 bulan lalu. Keluhan disertai bercak tebal di kedua tungkai. Pasien memiliki riwayat asma serta riwayat akalasia dengan malnutrisi berat. Pasien awalnya didiagnosis dermatitis yang disebabkan defisiensi asam lemak esensial dengan infeksi sekunder, dermatitis numularis, liken simpleks kronikus, dan dermatitis seboroik. Setelah evaluasi lebih lanjut, pasien memenuhi kriteria Hanifin-Rajka disertai kadar IgE yang tinggi, sehingga ditegakkan diagnosis DA awitan dewasa. Diskusi: Laporan kasus ini menunjukkan penegakkan diagnosis DA awitan dewasa tipe generalisata berdasarkan klinis. Pemeriksaan penunjang untuk menyingkirkan diagnosis banding dermatitis kontak tidak dapat dilakukan karena pasien dalam terapi imunosupresan. Penatalaksanaan DA dilakukan secara sistematik disertai pemberian terapi sistemik. Kesimpulan: Penegakan diagnosis DA awitan dewasa berdasarkan klinis merupakan sebuah tantangan. Manifestasi klinis seringkali tidak sesuai dengan kriteria diagnosis yang umum digunakan terutama pada kasus de novo. Pemeriksaan penunjang seperti kadar IgE, eosinofil, uji kulit, dan biopsi kulit terkadang diperlukan untuk penegakan klinis secara tepat sehingga pasien dapat diberikan tata laksana yang sesuai.

Abstract

Red palm oil (RPO) contains antioxidants due to its carotenoid and vitamin E content. However, the lipophilicity and instability of these two compounds may reduce RPO's bioavailability and efficacy of RPO. This study aimed to develop RPO submicroemulsions and evaluate the effects of magnetic stirrer (M) and homogeniser (H) on their characteristics and physical stability. A submicroemulsion was developed using the D-phase emulsification method. Formulas with globule sizes less than 200 nm, polydispersity index (PDI) less than 0.4, and zeta potential less than -30 mV were tested for stability at room temperature for 3 months and analysed for Ostwald ripening rates. The homogenizer method produced smaller globule sizes and lower PDI (p<0.05). Ten formulations from both the magnetic stirrer and homogeniser methods met the initial globule requirements. After 3 months, F4H and F9H showed the best physical stability with globule sizes of 181.23±0.96 and 175.23±1.88 nm, PDI of 0.077±0.034 and 0.035±0.023, and zeta potential of -33.80±0.56 and -30.57±0.15 mV (p>0.05). However, F9H had a lower Ostwald ripening rate than F4H, namely 2.54×105 and 6.04×105 nm3/month, respectively. In conclusion, the homogeniser produced more stable RPO submicroemulsions than the magnetic stirrer.

Abstract

   Kuku memiliki fungsi penting sebagai pelindung ujung jari, penunjang fungsi sensorik, serta aspek estetika yang berperan dalam menunjang penampilan dan kepercayaan diri. Seiring meningkatnya minat terhadap perawatan dan keindahan kuku, penggunaan kosmetik kuku seperti cat kuku, kuku akrilik, cat kuku gel, dan berbagai teknik rias kuku semakin meluas. Meskipun secara umum relatif aman, penggunaan cat dan rias kuku dapat menimbulkan berbagai kelainan kuku akibat faktor mekanis, traumatis, infeksi, maupun reaksi terhadap bahan kimia yang terkandung di dalam produk. Tinjauan pustaka ini bertujuan untuk membahas jenis-jenis kelainan kuku yang dapat terjadi akibat penggunaan cat kuku dan rias kuku, termasuk kuku rapuh, onikolisis, dermatitis kontak, infeksi, perubahan warna kuku, hingga risiko jangka panjang akibat paparan sinar ultraviolet pada penggunaan cat kuku gel. Berbagai bahan seperti formaldehid, akrilat, metakrilat, dan pelarut organik diketahui berperan dalam timbulnya efek samping tersebut. Pemahaman mengenai teknik perawatan kuku, bahan kosmetik yang digunakan, serta potensi komplikasinya sangat penting bagi klinisi agar dapat melakukan diagnosis yang tepat, memberikan edukasi pencegahan, dan menentukan tata laksana yang sesuai pada pasien dengan kelainan kuku akibat kosmetik.

Abstract

Pendahuluan: Dermatomiositis (DM) merupakan penyakit jaringan ikat langka yang ditandai manifestasi kulit khas, kelemahan otot progresif, serta keterlibatan organ multisistem. Pemeriksaan myositis-specific autoantibodies (MSA) berhubungan dengan fenotip klinis dan prognosis. Kasus: Laki-laki 36 tahun didiagnosis DM klasik dengan lesi khas berupa Gottron sign, Gottron papules, v-neck sign, shawl sign, holster sign, serta kelemahan otot progresif bilateral. Pemeriksaan laboratorium didapatkan peningkatan enzim lactate dehydrogenase (LDH), alanine aminotransferase (ALT), aspartate transferase (AST), C-reactive protein (CRP), ANA IF dengan titer >1:1000 pola homogen, creatinine kinase (CK), CKMB, serta troponin I. Panel MSA didapatkan hasil positif terhadap anti-Mi-2a dan anti-Mi-2b. Pemeriksaan elektromiografi (EMG) sesuai dengan immune-mediated myopathy. Pemeriksaan histopatologis didapatkan akantosis tidak teratur, ortokeratosis, basal vacuolar alteration, keratin plug, pigment incontinence, kolagen papila dermis homogen, sebukan perivaskular ringan, deposisi musin sesuai dengan gambaran DM. Lesi kulit perbaikan dan tidak ada lesi baru, namun kelemahan pada tangan, tungkai atas, dan tungkai bawah masih dirasakan sama dengan pemberian terapi metilprednisolon 32 mg setara prednison 0,5 mg/kgBB dan metotreksat 10 mg/minggu selama 2 minggu. Diskusi: Patogenesis penyakit DM sangat kompleks dipengaruhi oleh genetik, lingkungan, dan gangguan sistem imun dengan peran autoantibodi, adanya inflamasi jaringan, kerusakan sel parenkim, dan vaskulopati. Pemeriksaan MSA membantu penegakan diagnosis DM atipikal, mengidentifikasi manifestasi keganasan, pilihan tata laksana, dan memengaruhi prognosis. Anti-Mi-2 antibody positif berhubungan dengan angka kejadian penyakit paru interstisial dan keganasan lebih rendah, serta respon terapi dan luaran klinis yang lebih baik. Simpulan: Pemeriksaan MSA dengan hasil anti-Mi-2 antibody positif pada DM dapat digunakan sebagai penanda diagnostik dan prognostik yang baik.

Abstract

Background: Skin diseases represent a major burden in outpatient dermatology settings in Indonesia. The pattern of dermatological diagnoses varies substantially by age group and sex, reflecting differences in hormonal milieu, environmental exposures, and immune function. Objective: To describe and analytically assess the distribution of dermatological diagnoses stratified by age group and sex among patients attending in a private health and beauty clinic in Menteng, Central Jakarta, from January 2024 to December 2025. Methods: A cross-sectional, analytic observational study was conducted using secondary data from the clinic’s visit records. A total of 8,105 patient visits were included. Diagnoses were classified by ICD-10 code and stratified into eight broad age groups: Infants (<1 years), Young Children (1-4 years), Older Children (5-9 years), Young Adolescent (10-14 years), Older Adolescents (15-19 years), Young Adults (20-24 years), Adults (25–59 years), and Elderly (≥60 years). Chi-square (X²) goodness-of-fit was used for hypothesis testing. Results: Female patients predominated (74.9%; x2 = 2,016.761, p < 0.001). The most common diagnoses were acne vulgaris (n = 1,230; 15.2%), atopic dermatitis (n = 756; 9.3%), chloasma (n = 740; 9.1%). Acne vulgaris peaked in the Adults group (65.2% of all acne cases), chloasma in the Adults group (87%), and atopic dermatitis in the Adults group (34.9%). Chi-square analysis revealed that all top-10 diagnoses showed statistically significant non-uniform age distribution (all p < 0.001). Conclusion: Dermatological diagnoses at this private health and beauty clinic are age- and sex-patterned. These findings provide a basis for age-targeted prevention, early detection, and clinical resource allocation in urban private dermatology practice.
