Abstract

Physical activity programs offered in Indonesian hospitals are often inadequate to help type 2 diabetes (T2D) patients meet international physical activity recommendations. This study aimed to identify T2D patients’ physical activity enablers and barriers, their preferences for, and experiences with, physical activity programming, with a view to developing future programs that are suited to these patients’ needs and preferences. Four focus groups were conducted with 28 patients (50% female) of the T2D clinic at the Local General Hospital of Yogyakarta. Discussions were thematically analysed in NVivo 10. The most reported types of physical activity were walking and participation in the hospital-based exercise program. Participants were motivated to be physically active for the health benefits and for social interactions. The main barriers to physical activity were a lack of enjoyment, the absence of knowledge about appropriate activities for T2D patients, and a shortage of time due to social and family obligations. Based on study participants’ preferences, future physical activity programs need to include educational components, regular clinical evaluations, consultations with health educators, and efforts to increase the reach of the programs. This study provides practical suggestions for physical activity programming that meet the needs of T2D patients in Indonesia.

Abstract

Physical inactivity is a significant public health concern among adolescents. Social cognitive factors are suggested to influence physical activity including in students, However, it is unclear how these factors influence physical activity levels among students in the Education and Training Center in Riau. This study aimed to develop a prediction model of physical activity levels based on social cognitive predictors in this population. The study was a cross-sectional study of 107 students from the Education and Training Center in Riau who completed online questionnaires on physical activity and social-cognitive correlates. Physical activity levels were measured using the Physical Activity Vital Sign, while the social cognitive correlates included self-efficacy, outcome expectations, stress levels, and motivation, which were measured using validated scales. Simple and multiple linear regression analyses were conducted to analyze the data. The results indicated that self-efficacy, outcome expectations, stress levels, and motivation significantly predicted physical activity levels among students. The results of the multiple linear regression indicated regression coefficients were 0.26, 0.31,-0.33 and 0.27 for self-efficacy, outcome expectation, stress level and motivation practice, respectively. The multiple linear regression model, which included all variables, predicted 32.9% of the variance in physical activity levels. The findings suggest that physical activity promotion programs for students should focus on improving self-efficacy, outcome expectations, stress management, and motivation. Future research can focus on developing and evaluating the efficacy of physical activity promotion interventions that target social cognitive predictors among students.

Abstract

Atrial fibrillation (AF) significantly increases the risk of stroke, necessitating anticoagulation therapy. Warfarin, a commonly prescribed anticoagulant regimen, requires careful monitoring to ensure patient safety. This study aimed to assess the impact of dose switching, dose variation, and potential interactions with warfarin on the incidence of stroke recurrence in stroke patients with AF. The study retrospectively analyzed the treatment records of stroke patients with AF in outpatient settings over one year. The subjects comprised 314 patients who received warfarin prescriptions at two Indonesian Hospitals from January 1, 2015, to December 31, 2019. Out of these patients, 50 had recorded data regarding dose adjustments, variations, and interactions. They were divided into two groups: a case group (n=11) with stroke recurrence and a control group (n=39) without recurrence. Statistical analysis, including chi-square tests and odds ratio calculations, revealed that both warfarin dose switching (OR=7.6) and dose variation (OR=6.6) significantly influenced the incidence of stroke recurrence. It implies that inconsistencies or alterations in warfarin dosing substantially elevate the likelihood of experiencing another stroke, potentially due to inadequate anticoagulation leading to clot formation. Interestingly, the analysis of drug interactions did not significantly impact stroke recurrence. In summary, the recurrence of stroke in patients with AF is notably influenced by warfarin dose adjustments and variations rather than drug interactions. This study highlights the critical importance of precise dosing strategies and vigilant monitoring to enhance the efficacy of anticoagulant therapy in this high-risk population.

Abstract

The incidence of persistent respiratory symptoms after acute post-COVID-19 infections, referred to as post-COVID-19 syndrome, is high. This study aimed to determine the feasibility and the acute effects of the combination of aerobic, strength and breathing exercises on pulmonary function and oxygen saturation among women post-COVID-19. This study involved 36 women post-COVID-19, with a mean age of 34.4±9.1 years, in a 45-minute aerobic, strength, and breathing exercise with an intensity of 65-75% maximum heart rate. The pulmonary function was measured with spirometry, while oxygen saturation was measured with an oximeter. These measurements were taken pre and post-exercise. Feasibility was determined as participants’ ability to complete the exercise and the incidence of adverse effects. A pair-t-test and effect size were assessed to estimate the acute effects of the exercise on Cohen’s d estimation. All patients complied, and no adverse events were reported, thus, supporting the feasibility of the exercise protocol. There was an increase in most post-exercise pulmonary function and oxygen saturation compared to pre-exercise (p <.001 to 0.03), with effect sizes ranging from 0.5 to 0.8, indicating medium to large effects of the exercise program on pulmonary functions and oxygen saturation. In conclusion, the combination of aerobic, strength, and breathing exercises is feasible and safe for women post-COVID-19 and potentially improves most pulmonary functions. Further research is recommended to evaluate the longer adaptation of the exercise on the pulmonary function in post-COVID-19.

Abstract

Patient compliance in adhering to stroke treatment recommendations becomes one of the factors influencing the occurrence of recurrent strokes. Depression in post-stroke patients inhibits functional brain recovery, increases the potential for recurrent strokes, lowers the quality of life, and raises the risk of death compared to patients without depression. The aim of this study is to analyze the relationship between drug therapy compliance and the level of depression on the occurrence of recurrent strokes in post-stroke patients. This cross-sectional study involved 60 patients aged between 30 and 85 years, all undergoing outpatient stroke treatment at a hospital in Indonesia. Recurrent stroke incidents were defined as experiencing a secondary stroke within 30 days after the initial stroke occurrence. Out of the total participants, 12 individuals had experienced recurrent strokes after their initial episode, while 48 patients had not experienced recurrent strokes up to the point of data collection. Medication adherence data were evaluated using the MMAS-8 (Morisky Medication Adherence Scale), and depression levels were measured using the PHQ-9 (Patient Health Questionnaire-9). The research results show that in the post-stroke patient group, the level of patient adherence is high at 41.7%, and in the recurrent stroke group, the level of patient adherence is moderate at 41.7%. Meanwhile, in the post-stroke patient group, the dominant depression level is low (50%), while in the recurrent stroke group, the dominant depression level is mild, followed by severe depression. The analysis indicates that medication adherence is not associated with stroke recurrence, while depression is related to stroke recurrence with an OR value of 2.060, p <0.05. Further research on more efficient monitoring methods and proper depression management is recommended to reduce the risk of stroke recurrence.

Abstract

International Physical Activity Questionnaire (IPAQ) and the Global Physical Activity Questionnaire (GPAQ) are the two most widely used to measure physical activity (PA). Yet, their convergent validation with the maximum oxygen capacity (VO2Max) among Indonesian young adults is unknown. This study compared physical activity (PA) assessments using IPAQ and GPAQ and their correlations with the maximal oxygen capacity (VO2Max) in the young adult population. This study was a cross-sectional descriptive study on 63 Sports Science students (84.13% Male age ranged 18-22 years). PA was measured using both the GPAQ and IPAQ to calculate the total PA and PA in each domain (i.e. work-related, transport-related, and leisure). One additional domain which was household-related was assessed exclusively for the IPAQ based on the scoring guideline. The VO2Max was measured using a standard equation based on participants’ performance on a 1-mile run. Paired sample t-test was conducted to compare total PA based on GPAQ and IPAQ while the Wilcoxon sign rank test was conducted to compare GPAQ and IPAQ domains. Pearson or Spearman correlation test assessed the correlations among the VO2Max, total PA, and PA on each GPAQ’s and IPAQ domain. No significant difference in the total PA per week between IPAQ and GPAQ was found. Significant differences, however, were found in work and transport domains, but not in the leisure domains. A moderate correlation among GPAQ and IPAQ (r=0.56, p=0.01), a moderate correlation between GPAQ and VO2Max (r=0.41, p= 0.01) but a weak correlation between IPAQ dan VO2Max (r=0.25, p<0.05) were demonstrated. While, this study has indicated convergent validity of both GPAQ and IPAQ, GPAQ has a higher correlation with VO2Max compared to the IPAQ. Therefore, GPAQ is more recommended to be used to measure PA levels among Indonesian young adults compared to IPAQ.

Abstract

Background Problems: Histology is a critical component of the sports science curriculum, as it thoroughly explains the connection between the structure and function of cells, tissues, and organs. Research Objective: This study investigates the improvement of histology learning among sports science students by employing a functional approach and practical sessions with virtual microscopy. Methods: The study adopted a quasi-experimental design with 103 students from the Sports Science program, enrolled in a histology course with two parallel classes. The students were divided into experimental (n = 56) and control groups (n = 46) using cluster random sampling based on their classes. The experimental group received instruction using virtual microscopy, while the control group was taught using light microscopy throughout the semester. Both groups were assessed on their learning outcomes at the end of the semester through a written exam. Experimental group students also provided feedback on the courses. Using the independent t-test, the learning outcomes were compared between groups. Students’ feedback was analysed descriptively. Findings and Results: The results revealed that the average test scores for participants in the experimental group were significantly higher than those in the control group (74.9 ± 7.8 and 68.9 ± 9.6, respectively). Student feedback indicated that the majority concurred that the functional approach to histology teaching enhanced their understanding of histological structures. They also noted that practical virtual microscopy sessions improved their comprehension of the topics studied. Additionally, nervous tissue was identified as the most challenging tissue to learn, primarily due to visualisation difficulties in relating microscopic and macroscopic structures and the abundance of new terminology to remember. Conclusion: The findings suggest that combining a functional approach with virtual microscopy notably enhances the quality of histology education, equipping sports science students with a comprehensive understanding and essential practical skills for their academic and professional development.

Abstract

Background : Transplantation provides near-normal life and excellent rehabilitation compared to dialysis and is the preferred method of treating end-stage renal disease (ESRD) patients. Methods: We conducted a retrospective analysis of anesthesia management from 20 cases of live renal transplants carried out between August 2017 and April 2019 at Dr. Sardjito Central General Hospital, Yogyakarta. Preoperative patient status, anesthesia management, and postoperative care of the subjects were assessed.Result : Most patients had preoperative anemia, normal serum potassium, normal serum creatinine, and normal ejection fraction. Anesthesia management began since 24 hours before surgery, in which the patients were hospitalized, had peripheral IV access and fluid maintenance, and hemodialysis, followed by premedication 1 hour before surgery. Prior surgery, anesthesia induction and intubation were done, followed by maintenance of anesthesia and intaoperative monitoring. Postoperative care consisted of administration of analgesia and management of complications. Conclusion: Optimization of preoperative status, proper anesthesia management, and good postoperative care are keys for a successful renal transplant program.
