Abstract

Background: Nucleus Pulposus Hernia (HNP) in the cervical region is a degenerative condition that can cause pressure on the spinal nerve. Magnetic Resonance Imaging (MRI) is the primary imaging modality for the diagnosis of HNP, but it has a long acquisition time limitation, which risks causing artifacts due to patient movement. The use of GRAPPA parallel imaging techniques can speed up acquisition, but lower the signal-to-noise ratio (SNR). This study aims to evaluate the effect of wavelet denoising filters on improving the quality of images and cervical MRI information of T2 TSE GRAPPA sequences in HNP patients. Methods: The study used a pre-experimental was conducted on 16 cervical HNP patients at Budi Rahayu Hospital Pekalongan. Acquisitions are carried out with acceleration factors 2, 3, and 4. Subjective analysis of image quality by three radiologists was performed on five anatomical structures (disc, corpus, spinal cord, CSF, and spinesus process). Data were assessed using the Cohen's Kappa test for fit between assessors and the Friedman test to compare SNR and CNR values between acceleration factors.Result: The application of wavelet denoising filters significantly increased SNR and CNR values (p = 0.0174 and p = 0.0175). R-factor 3 produced the best overall image quality with the highest Kappa value of 0.681 in the R1*R2 grader pair, indicating substantial conformity. The highest mean rank for visual quality was found in the spinal cord (R-factor 2 = 1.17), while the CSF, corpus, and disc showed the best quality in R-factor 3. This technique has been proven to retain critical diagnostic information with more efficient acquisition times. Conclusions: The combination of GRAPPA with wavelet denoising filters on T2 T2 sequence cervical MRI GRAPPA significantly improved image quality, especially at R-factor 3. These results show that this method is effective in reducing noise and preserving anatomical details, thereby improving the accuracy of cervical HNP diagnosis and patient comfort during the examination.

Abstract

Background: Mental health, diet, and physical activity play an important role in adolescents development. However, previous studies have reported inconsistent findings on the relationship between mental health, eating disorders, and physical activity. The COVID-19 pandemic has negatively impacted mental health, potentially contributing to eating disorders and reducing physical activity. Objectives: This study aimed to examine the association between mental health, eating disorders, and physical activity in adolescents during the COVID-19 pandemic. Methods: A cross sectional quantitative study was conducted in 2022, involving 236 high school adolescents. Mental health was  assessed using the Depression, Anxiety, and Stress Scale (DASS-21), eating disorders were evaluate with the Eating Attitudes Test (EAT-26), and physical activity levels were measured using the International Physical Activity Questionnaire (IPAQ). Results: More than 50% of adolescents experienced depression, anxiety, or stress with severity ranging from mild to very severe. While 82.6% of adolescents did not exhibit eating disorders, 63.6% had low physical activity levels. A significant association was found between depression and eating disorders (p=0.024), but no significant relationship was observed between anxiety (p=0.080) or stress (p=0.232) and eating disorders. Additionally, depression (p=0.411), anxiety (p=0.547) and stress (p=0.097) were not significantly associated with physical activity. Conclusion: During the COVID-19 pandemic, most adolescents experinced mental health challenges, maintened normal eating behaviors, and exhibited low physical activity. Depression was significantly correlated with eating disorders, while no significant associations were found between mental health variables and physical activity levels.

Abstract

Background: Cesarean section (CS) delivery should only be performed with medical indications. It is important to analyze the socioeconomic role in CS and determine how to address socioeconomic-related factors in optimizing CS coverage in the Philippines using the latest national data.Purpose: This study analyzed the socioeconomic roles in CS delivery in the Philippines. Methods: This study used secondary data from the 2022 Philippines National Demographic and Health Survey (NDHS). The cross-sectional study included 4,452 women aged 15–49 who had given birth within the previous three years. Seven control factors were examined: employment, antenatal care (ANC), age, education, marital status, residence, and parity. The mode of delivery was considered the outcome variable, while socioeconomic status was the exposure variable. Binary logistic regression was used for the final data analysis. Results: Cesarean section was performed in 18.45% of deliveries in the Philippines. Analysis of socioeconomic status showed that women in the “poorer” group were 1.758 times more likely than the poorest to undergo CS (AOR 1.758; 95% CI 1.757–1.758). Women in the middle-income group were 2.164 times more likely than the poorest to have a CS (AOR 2.164; 95% CI 2.163–2.165). Those in the richer group were 2.718 times more likely (AOR 2.718; 95% CI 2.717–2.719), and the richest were 4.787 times more likely to deliver by CS compared to the poorest (AOR 4.787; 95% CI 4.785–4.789).Conclusion: Socioeconomic disparities are strongly associated with CS delivery in the Philippines. The wealthier the mother, the more likely she is to have a CS. Efforts should focus on reducing unnecessary CS in the rich and richest groups. Optimizing ANC education should include addressing psychological needs, promoting positive values, and providing a sense of security and comfort in normal childbirth. At the same time, equitable access to CS should be ensured for the poorest groups through education and insurance coverage.

Abstract

Sedation of propofol, chloral hydrate, midazolam, and ketamine is commonly used in MRI examinations. There are still risky side effects from the previous 4 sedations, so melatonin tablets are a safer option. Melatonin synthesized in tablets can still cause quite adverse side effects. Pistachios processed into milk can be used as an alternative to natural melatonin oral sedation. Research Objective: To determine the potential of pistachio milk as an alternative to oral melatonin sedation to support MRI examinations. Research Method: This research uses the R&D (Research and Development) method with 4 stages, namely finding potentials and problems, data collection, product design, and design validation. The product in the form of 200 mL pistachio milk with concentration variants of 35 g, 45 g, 55 g, 65 g was tested in a laboratory to determine its potential from the test results with a reference to melatonin tablets of 10 mg. Research Results: Laboratory tests showed the presence of melatonin and other accompanying ingredients such as phenolics, carotene, and polyphenols. The melatonin content of the 35 g, 45 g, 55 g, 65 g variants is 5.5 mg, 6.1 mg, 8.7 mg, 10.5 mg, respectively. Conclusion: Pistachio milk with a 65 g variant has a melatonin content of 10.5 mg which is close to the reference for sedation of melatonin tablets of 10 mg, so it has the potential to be an alternative to oral sedation of melatonin to support MRI examinations.
