Abstract

Background:The ketogenic diet (KD) is a high-fat, low-carbohydrate, and normal-protein diet that has been used for the treatment of medically refractory childhood epilepsy since the 1920s.The KD includes 80% fat, 15% protein, and 5% carbohydrate; the ratio of fat to carbohydrate plus protein ranges from 2:1 to 4:1.The purpose of the case report was to learn benefits and factors that influence the administration of the ketogenic diet in intractable epilepsy. Case:A 2-years 9 months old boy since 3 month of age the child begins seizure. Five month the child was diagnosed with epilepsy received one type of anti epileptic drug (AED). Seven months of age the child began control in outpatient clinic Neurology Department of Dr. Kariadi Hospital with a diagnosis of general epilepsy, were given 2 type of AEDs. Since10 month of age the child was given 3 type of AEDs. The child still often seizure, at 15 months was diagnosed intractable epilepsyand at 29monthof age, was programed to have long term EEG and KD during hospitalization. Conclusion:The administration of KD in 2-years9 months old boy with intractable epilepsyshowed benefits in reducing the frequency of seizures. Key word : Ketogenic Diet, Intractable Epilepsy, Child

Abstract

Latar Belakang : Paparan sinar matahari pada kulit merupakan cara terbaik untuk sintesis vitamin D. Kadar vitamin D yang adekuat dalam tubuh merupakan proteksi terhadap berbagai penyakit seperti penyakit degeneratif, kanker dan juga infeksi saluran napas. Beberapa penelitian menghubungkan kadar vitamin D yang rendah dengan morbiditas dan mortalitas COVID-19. Hal ini menyebabkan fenomena baru pada masyarakat yaitu kebiasaan berjemur. Tujuan : Artikel ini akan membahas tentang metabolisme vitamin D, peran sinar matahari dalam mengaktifkan vitamin D di dalam tubuh, dan peran vitamin D dalam berbagai penyakit, khususnya mekanisme imunitas untuk COVID-19. Diskusi : Vitamin D meningkatkan kekebalan alami seluler terutama dengan cara menginduksi peptida antimikroba, yang meliputi cathelicidin, LL-37, 1,25-dihdroxyvitamin D dan defensins. Selain itu vitamin D akan meningkatkan sekresi hidrogen peroksida pada sel monosit. Pemberian vitamin D dosis tinggi sebanyak 10.000 IU/hari selama beberapa minggu dilanjutkan 5000 IU/hari bermanfaat untuk mencegah COVID-19, walaupun hasilnya masih memerlukan penelitian lebih lanjut. Absorpsi sinar matahari ke dalam tubuh manusia dipengaruhi oleh warna kulit, penggunaan bahan pakaian dan tabir surya , dan luas pajanan. Paparan sinar matahari sebesar satu Minimal Erythemal Dose (MED) pada orang dewasa dapat meningkatkan konsentrasi vitamin D setara dengan suplementasi 10.000 – 25.000 IU. Penelitian pada bayi yang diberi paparan 3 kali seminggu @ 5 menit pada jam 10.00-14.00, dengan paparan 50% area tubuh selama 2 bulan, mendapatkan kenaikan 25(OH)D sebesar 8,9 ng/mL. Simpulan : Vitamin D yang diaktifkan oleh paparan sinar matahari sangat bermanfaat sebagai proteksi berbagai penyakit termasuk juga pada COVID-19, walaupun efektifitasnya masih memerlukan penelitian lebih lanjut. Kata Kunci : COVID-19, vitamin D, paparan sinar matahari Background : The exposured of sunlight on the skin is the best way for vitamin D synthesis. Adequate vitamin D levels are protection against various diseases such as degenerative diseases, cancer and also respiratory infections. Several studies have linked between low vitamin D levels with COVID-19 morbidity and mortality. This causes a new phenomenon in the community, namely sunbathing. Purpose : This review rearticle will discuss about vitamin D metabolism, the role of sunlight in activating vitamin D in the body, and the role of vitamin D in various diseases, specifically the immune mechanism for COVID-19.Discussion : Vitamin D increases cellular innate immunity mainly by inducing antimicrobial peptides, which include cathelicidin, LL-37, 1,25-dihdroxyvitamin D and defensins, and also increase the secretion of hydrogen peroxide in monocyte cells. The administration of high-dose vitamin D of 10,000 IU / day for several weeks followed by 5000 IU / day is useful to prevent COVID-19, although the results still require further research. The sun exposure to activated vitamin D body is affected by skin color, using of clothing and sunscreen, and area of ??exposure. Sun exposure of one Minimum Erythemal Dose (MED) in adults can increase vitamin D concentrations equivalent to 10,000 - 25,000 IU vitamin D supplementation. Study on infants who were given exposure 3 times a week @ 5 minutes at 10:00 to 14:00, with exposure 50% of body surface area for 2 months, increased 25(OH)D of 8.9 ng/mL. Conclusion : Vitamin D which is activated by sun exposure is very useful as protection for various diseases including COVID-19, although its effectiveness still requires further research. Abstract

Background: Non alcoholic fatty liver disease (NAFLD) has been associated with cytokines and inflammatory mediators. Adiponectin has insulin sensitizing effects and has correlation with severity of NAFLD disease. However, the study about the relationship between adiponectin level and NAFLD is lacking. The objective of the study was to determine the association between adiponectin level and NAFLD in obese adolescents through the role of insulin resistance. Methods: This was a cross-sectional study, that was done in August 2007. The inclusion criteria were obese adolescents aged 11-14 years, and normal weight adolescent as control group. Adiponectin was assessed by using ELISA, insulin resistance was obtained by Homeostasis Model Assessment Insulin Resistance (HOMA-IR). NAFLD was confirmed by abdominal ultrasound, which represented by fatty liver imaging. The comparison of adiponectin level and HOMA-IR among 3 groups were analyzed by Kruskal Wallis test, meanwhile the correlation between adiponectin level and some variables were analyzed by spearman correlation. Result: There were 73 subjects, consisted of 37 obese and 36 non obese. Among obese subjects, 54.1% got NAFLD. All of our obese subjects were insulin resistance, the HOMA-IR level of obese non NAFLD was 6.1 and obese with NAFLD was 6.8. The adiponectin levels in normal children was (5.1mg / ml), obese non NAFLD (4.1mg / ml) and obese with NAFLD (4.0 mg / ml) (p <0.001). There were no association between adiponectin level and other variables. Conclusion: There were significant differences of adiponectin levels and insulin resistance measured by HOMA-IR between normal and obese subjects, but no significant differences between the obese groups with or without NAFLD. Abstract

Background: Physical fitness affected by aortic elasticity which is increased appropriately with insuline growth factor-1 (IGF-1). Channa striata extract (CSE) supplementation increased arginine serum level and further lead to increase of IGF-1, endothelial dysfunction remodeling, and physical fitness. The current study aimed to explore the difference of physical fitness of short stature children with and without CSE supplementation. Method: This cross sectional study conducted to 100 short stature children (male 58; female 42; CSE 50; placebo 50) aged 8-12 years in Brebes, Central Java Indonesia. Short stature was defined as the height/age Z score between -2 and -3 based on WHO 2007 growth chart standard. Physical fitness was measured by modified Harvard step test as stated as VO2max, physical activity by physical activity record and stated as physical activity level (PAL). Data was analyzed by independent t-test and Pearson correlation. Results: VO2max level was higher in CSE 41.67 ± 6.967 ml/kg/minutes and placebo 41.16 ± 5.238 mL/kg/minutes, p=0.682. Hemoglobin level was higher in CSE than placebo (13.12 ± 0.932; 12.99 ± 0.878) g/dL; p=0.5020. All children revealed active category on PAL. The results showed a significant relationship between PAL and physical fitness, hemoglobin and PAL on CSE (p<0.05), however there was no correlation between hemoglobin level and physical fitness. Conclusion: Physical fitness in short stature children with CSE supplementation or placebo was good category. There was no differences between hemoglobin level and PAL in both of groups. Abstract

BACKGROUND Niemann-Pick disease (NPD) types A result from the deficient activity of sphingomyelinase. NPD type A is characterized by early-onset, progressive neurodegenerative course; systemic disease manifestations, including massive hepatosplenomegaly, interstitial lung disease, and cherry-red macula; and death in early childhood. The objective: to enhance the recognition of health care providers about the potential undiagnosed NPD because nonspecific clinical manifestation CASE PRESENTATION A 18-months-old boy was admitted to Dr. Kariadi Hospital with enlarged abdomen since seventh month old with failure to thrive. He also showed progressive loss of neurologic function, microcephaly with open major fontanelle, recurrent pulmonary and systemic infection. Physical examination revealed facial dysmorphic, milestone regression, under-nutrition, crackles in both lungs, hepatosplenomegaly with petechial in extremities and floppy infants. Laboratory investigations showed anemia (9.4 g/dL) and thrombocytopenia (73.000/mm3). The lactate dehydrogenase (482 U/L) and alkaline phosphatase (159, 03 IU/L) were higher than normal. Abdominal ultrasound revealed hepatomegaly with normal parenchyma and splenomegaly without nodule. Skeletal survey revealed Erlenmeyer flask deformity. Foam cell are detected in bone marrow puncture. Retcam examination showed cherry red spot at the macula. Bera revealed auditory neuropathy. The enzyme activity showed normal ?–Glucosidase (5.55 uM/hr) and chitotriosidase (105,8 nmol/ml) but low sphingomyelinases activity (0.30 uM/hr) which confirmed the diagnosis of NPD. DISCUSSION Niemann-Pick disease (NPD) types A result from the deficient activity of sphingomyelinase. NPD type A is characterized by early-onset, progressive neurodegenerative course; systemic disease manifestations, including massive hepatosplenomegaly, interstitial lung disease, and cherry-red macula; and death in early childhood. Type A is very rare and a severe infantile form with neurologic degeneration resulting in death usually by 3 years of age. No treatment available for type A and It’s a rare disease in Indonesia. CONCLUSION These investigations were able to diagnose this child as a NPD-Type A. Patient was closely monitored and symptomatic treatment was provided.

Abstract

LATAR BELAKANG : Insulin-like  Growth Factor-I (IGF-I) berperan dalam pertumbuhan janin terutama pada usia kehamilan lanjut. Adiponektin dan leptin berhubungan dengan depot adipose dalam modulasi metabolisme dan homeostasis energi. Ketiga hormon tersebut memiliki peran penting pada pertumbuhan janin. TUJUAN : Untuk mengetahui nilai cut off kadar IGF-1, leptin dan adiponektin pada bayi berat lahir rendah  (BBLR) sesuai masa kehamilan dan kecil masa kehamilan. METODE : Observasional analitik dengan desain belah lintang, dilaksanakan pada bulan November 2018 - Januari 2019. Subjek adalah  bayi baru lahir, kehamilan  tunggal dengan berat lahir >1000 gram dan <2500 gram, serta usia ibu saat hamil <40 tahun Sampel darah tali pusat diambil setelah bayi lahir, sebelum plasenta dilahirkan. Pemeriksaan IGF-1, leptin dan adiponektin menggunakan teknik immunoassay kuantitatif dengan metode ELISA. Data hasil penelitian merupakan data numerik, dilakukan uji ROC dan uji diagnosis untuk mendapatkan nilai cut-off. HASIL :  Selama periode penelitian, terdapat 46 subyek yang terdiri atas 11 bayi kecil masa kehamilan dan 35 bayi sesuai masa kehamilan. Jenis kelamin laki-laki sebanyak 24 bayi (52,2%) dan perempuan 22 bayi (47,8%). Sebanyak 41,3% bayi lahir dari ibu dengan preeklampsia. Didapatkan nilai cut-off IGF-1 adalah 51,12 ng/ml, nilai cut-off untuk leptin adalah 3,3 ng/ml, dan nilai cut-off adiponectin 19,60 ng/ml. KESIMPULAN : Terdapat perbedaan kadar IGF-1, leptin dan adiponektin darah tali pusat BBLR sesuai masa kehamilan dengan BBLR kecil masa kehamilan dengan nilai cut-off masing-masing. Abstract

Purpose : Analyze the cut-off point of fetal growth based on the Intergrowth-21, World Health Organization (WHO), and Hadlock’s estimated fetal weight (EFW) in pregnant women with normal or deficient vitamin D levels to predict neonatal outcomes. Method: This cross sectional study to develop a diagnostic test, included 120 of pregnant women who completed follow up until children aged 2 years, divided into normal and deficient vitamin D group. Ultrasound and maternal vitamin D level examined during the second trimester of pregnancy. EFW was calculated using Hadlock’s formula and plotted on the Intergrowth-21 and WHO curves. The reference standards were the neonatal outcome, LBW, stunting, and neurocognitive impairment. Significant odds ratio (OR) value and area under the curve (AUC) of 0.6 are used to determine the cut-off point to be used. Result: Fetal growth curve was based on the WHO at the 5th percentile to predict LBW to have an AUC of 0.6 and OR of 6, 95% confidence interval (CI) of 1.36–26.45. The AUC for predicting LBW based on Intergrowth and Hadlock were 0.45 and OR not significant. As well as the AUC estimated stunting based on Hadlock, the Intergrowth-21 and the WHO fetal growth curves is <0.6 with OR not statistically significant. The AUC predicted neurocognitive impairment based on WHO’s chart was 0.6 but OR not statistically significant. Conclusion: The WHO fetal growth curve can be used to predict LBW. The cut-off point of the fetal growth curve and which percentile is determined by the neonatal outcome.
