Abstract

Smoking behavior remains a health challenge in Indonesian society. Riskesdas data shows an increase in smoking prevalence in adolescent groups. One of the control efforts is the implementation of PHW (Pictorial Health Warnings) on cigarette packs in accordance with Permenkes RI No. 28 of 2013. This study aims to determine the effectiveness of cigarette packs in influencing smoking intention and behavior PHW among adolescents in Tanah Bumbu Regency, Banjarbaru City and Banjarmasin City. The study used a quantitative method with a cross-sectional design. Samples were obtained through an accidental sampling technique, obtaining 315 smoking adolescents and 62 non-smoking adolescents. The instrument was a modified version of the Global Youth Tobacco Survey questionnaire. The analysis used was a linear regression test. The results further showed that 64.5% of respondents who did not smoke stated that their intention was very likely due to fear (89.03%) after seeing the warning and the image of smoking causing lung cancer, hence they did not smoke. Furthermore, there is a relationship between pictorial warnings on cigarette packaging with fear (p-value = 0.0001, r = 0.430) with a positive influence of 16.3%. In adolescent smokers, smoking pictures causing throat cancer 2 caused the most fear (71.3%) and the highest urge to quit smoking (66.48%). There is a relationship between pictorial warnings with fear (p-value = 0.0001, r = 0.195) and smoking cessation intensity (p-value = 0.0001, r = 0.728). The positive effect on fear was 3.5% and smoking cessation intensity was 52.9%.

Abstract

The high prevalence of smoking in Indonesia poses a substantial financial burden on the national health insurance system. At the local level, regional governments frequently encounter a fiscal dilemma between relying on revenues generated from the tobacco sector and suppressing the escalating costs of curative services driven by the adverse impacts of tobacco. This study aims to empirically examine the relationship between tobacco fiscal instruments, regional revenues, and the healthcare cost burden resulting from smoking-related diseases in Tebing Tinggi City. A quantitative approach utilizing a descriptive-comparative study design was employed to analyze secondary data spanning the 2022–2024 period. Data were obtained from the Regional Development Planning Agency of Tebing Tinggi, including realized figures for Cigarette Taxes and the Tobacco Excise Revenue Sharing Fund, as well as medical service data from Dr. H. Kumpulan Pane Regional General Hospital in Tebing Tinggi City. In 2024, the total regional revenue generated from the tobacco sector in Tebing Tinggi City reached only IDR 14.48 billion (consisting of IDR 14.19 billion from Cigarette Taxes and IDR 288.68 million from Tobacco Excise Revenue Sharing Fund. Conversely, the total medical expenditure for 11 types of smoking-related diseases swelled massively, draining the regional budget by IDR 33.05 billion in the same year, predominantly driven by catastrophic cases such as stroke and tuberculosis. There exists a hidden fiscal deficit in Tebing Tinggi City, whereby revenues from the tobacco sector fail to cover even half of the total local healthcare expenditure caused by these negative externalities. To mitigate this hidden fiscal deficit and reduce long-term curative health expenditure, a comprehensive local policy enforcing a total ban on cigarette advertising is highly recommended as a direct upstream preventive intervention. Keywords: Fiscal Deficit, Cigarette Tax, Healthcare Costs, Negative Externalities, Tobacco Excise Revenue Sharing Fund

Abstract

Background: Smoking is a unique social phenomenon among adolescents because it begins with high curiosity to try cigarettes and eventually develops into a habit that is difficult to stop. Objectives: This research aims to explore adolescents' perceptions of pictorial health warnings (PHWs) on cigarette packs. Method:  This research is qualitative, using a purposive sampling technique with predetermined criteria.  The population was male adolescents aged 10-19 years. This research involved 35 adolescents from Banjarmasin City, Banjarbaru City, and Tanah Bumbu Regency, South Kalimantan Province. Data were collected using an open-ended questionnaire. Results: The findings of this research can be grouped into several main thematic areas, namely perceptions of visual content, effectiveness of pictorial health warnings (PHWs), suggestions for improvement, emotional responses, and intensity of attention. Participants generally thought that the images displayed on cigarette packs should more explicitly highlight the health effects of smoking. Perceptions of the effectiveness of current PHWs varied, ranging from the view that PHWs are less attractive to quite effective in conveying health messages. Some of the suggestions that emerged included typographical changes, selection of more visually disturbing images, and the addition of elements such as information on medical costs, unappealing colors, and warnings about death. Regarding the intensity of attention to PHWs, participants' responses varied, ranging from not paying attention to seeing them frequently. Most participants believed that PHWs can influence adolescents' decisions not to smoke through emotional responses in the form of fear and concern about their impact. Conclusion: This research indicates that the effectiveness of PHWs on cigarette packs for adolescents is limited, and design improvements as well as cross-sector collaboration are needed to increase awareness of their impact.
