Abstract

Latar Belakang: Pembedahan kanker daerah kepala dan leher umumnya menimbulkan defek luas dan biasanya memerlukan flap. Meskipun free flap saat ini merupakan gold standard untuk rekonstruksi daerah kepala leher, pectoralis major myocutaneous flap (PMMC) masih digunakan. Kasus: Perempuan usia 47 tahun, dengan kanker tiroid papiler T4aN0M0 dengan ulserasi di kulit leher, menjalani tiroidektomi total dan defek operasi direkonstruksi dengan pectoralis major myocutaneous flap. Hasil rekonstruksi dapat diterima secara fungsional dan estetik. Tidak ada komplikasi hematom ataupun abses post operasi. Simpulan: Pectoralis major myocutaneous flap masih merupakan salah satu metode utama untuk rekonstruksi operasi kepala leher dan dapat diterima secara fungsional dan estetik jika free flap tidak dapat dilakukan. Background: Surgery for head and neck cancer generally leaves a wide defect that usually needed a flap. Although free flap is currently the gold standard for reconstruction of the head and neck, the pectoralis major myocutaneous flap is still popularly used. Case: A 47-year old female with thyroid carcinoma, underwent total thyroidectomy and the surgical defect was reconstructed with pectoralis major myocutaneous flap. The results were viable, functional, and aesthetically acceptable. No postoperative complications such as hematoma or abscess observed. Conclusion: Pectoralis major myocutaneous flap was still one of the main methods for head and neck reconstruction surgery.

Abstract

Kanker payudara merupakan kanker yang paling banyak pada perempuan di dunia. Pada beberapa dekade terakhir, pengobatan adjuvan tamoxifen banyak digunakan. Penelitian ini bertujuan mengevaluasi disease-free survival, survival rate, dan efek samping pengobatan tamoxifen. Penelitian deskriptif retrospektif pada bulan September hingga Desember 2016 di RSUP Dr. M. Djamil Padang menggunakan data sekunder 100 sampel dari rekam medik bagian Bedah Onkologi RSUP Dr. M. Djamil Padang dan data primer melalui wawancara. Analisis dilakukan secara univariat dan metode Kaplan-Meier. Hasil penelitian ini didapatkan kelompok usia terbanyak adalah 50-59 tahun dengan rata-rata 52,3 tahun. Disease-free survival 5 tahun adalah 0,727 dan survival rate 5 tahun adalah 0,777. Efek samping paling banyak adalah hot flush (65%). Terdapat hubungan antara efek samping pengobatan tamoxifen dan disease free survival dengan nilai p=0,001 (p < 0,05) kekuatan hubungan sedang (r=0,376). Breast cancer is the most commonly diagnosed cancer among women worldwide. For the last few decades, adjuvant tamoxifen is the most preferred therapy. This research aims to evaluate the disease-free survival, survival rate, and side effects of tamoxifen therapy. This retrospective descriptive research was conducted in September until December 2016 at Dr. M. Djamil Padang Hospital using 100 secondary data samples from medical records of Surgical Oncology Dept., Dr. M. Djamil Padang Hospital and primary data through interviews. Univariate analysis and Kaplan-Meier method are used. The result shows that the largest age group is 50-59 years old with average 52.3 years old. The 5-year disease-free survival is 0.727 and 5-year survival rate is 0.777, the most frequent side effect is hot flush (65%). There is medium strength of the relationship (r = 0.376) between side effects of tamoxifen treatment with disease-free survival (p = 0.001 <0.05).

Abstract

Background: The concern surrounding the physical and mental health-related quality of life (HRQoL) among patients with breast cancer emerged during COVID-19. However, there is a lack of sufficient studies that have specifically addressed this issue. It is crucial to emphasize the evaluation of this aspect as it significantly impacts the enhancement of future physical and mental HRQoL for these patients.Purpose: This study investigated the determinants of physical and mental HRQoL among patients with breast cancer during COVID-19.Methods: In a cross-sectional study conducted between September and October 2022, participation involved 260 patients diagnosed with breast cancer, selected through convenience sampling. Our observation focused on assessing the outcomes of physical and mental HRQoL using the Short Form (SF-12) questionnaire for data collection. Additionally, the data underwent analysis via binary logistic regression.Results: Respondents aged <50 years (p=0.010; aOR=2.08; 95% CI=1.18–3.67), unemployed (p=0.022; aOR=2.29; 95% CI=1.16–4.53), and had high depression (p=0.026; aOR=2.41; 95% CI=1.13–5.12) have a more likelihood to have low physical HRQoL. In addition, unemployed respondents (p=<0.001; aOR=4.15; 95% CI=2.14–8.04) and had high anxiety (p=0.004; aOR=2.71; 95% CI=1.38–5.33) have more likelihood to have low mental HRQoL.Conclusion: In this study, it was found that anxiety, depression, and unemployed respondents were associated with a higher likelihood of experiencing lower physical and mental HRQoL. Nurses and healthcare workers should prioritize addressing psychological issues such as anxiety and depression among breast cancer patients, emphasizing the necessity for interventions aimed at alleviating these concerns.

Abstract

Background: The BRCA1/2 mutation rate was found to be higher in triple-negative breast cancer (TNBC) patients depending on young age, and the presence of a family history of breast cancer and ovarian cancer. Research on BRCA1/2 germline mutations that focused on premenopausal TNBC patients in Indonesia has never been carried out. The objective of this research is to determine the BRCA1/2 germline gene mutation status in premenopausal TNBC patients in West Sumatra.Methods: This is a descriptive study conducted at the Surgical Oncology Division of DR. M. Djamil Hospital, Padang from April to July 2023. This study included patients aged 50 years who were diagnosed with the TNBC subtype at Dr. M. Djamil Hospital, Padang from January 2016 to July 2023. Secondary data was collected through the patient’s medical records to observe the TNBC patient’s characteristics. A blood test of TNBC patients was carried out to determine germline mutation of BRCA1/2.Results: 22 patients with TNBC participated in the study. In this study, 4/22 (18.2%) had BRCA2 pathogenic mutations and no BRCA1 pathogenic mutation was found. All patients with BRCA2 pathogenic mutation have the same mutation characteristics, i.e exon 3, genetic variation (c.262_263delCT), amino acids (p.Leu88AlafsTer12), and type of mutation (Frameshift Deletion). Conclusions: TNBC patients in West Sumatra have BRCA2 pathogenic mutations with the same characteristics as the mutations

Abstract

Introduction: Premenopausal breast cancer (PBC) is often more aggressive than postmenopausal cases, with prolactin playing a key role in tumor development. This study examines the relationship between serum prolactin levels, prolactin receptor (PRLR) expression, and clinicopathological features in PBC patients. Methods: This cross-sectional study was conducted from 2021 - 2022 at multiple hospitals in Pekanbaru, Indonesia. Thirty-five PBC patients and 35 healthy controls were included. Serum prolactin levels were measured using ELISA, and PRLR expression was assessed via IHC. Statistical analysis was performed using independent t-tests and correlation analysis (p < 0.05). Results: PBC patients had higher prolactin levels (51.96±80.40 ng/mL) than controls (20.83±15.74 ng/mL, p = 0.031). PRLR expression was detected in over 90% of tumors, with a significant correlation between prolactin levels and cancer stage (p = 0.035). Discussion: These findings suggest prolactin contributes to PBC progression, with higher levels linked to advanced stages, possibly through JAK2/STAT5 signaling. Conclusion: Serum prolactin levels and PRLR expression are associated with cancer stage, supporting their potential as biomarkers for early detection and disease progression. Further studies are needed for validation.
