Abstract

This study aims to determine the relationship between breast self-examination (BSE) and the stage of breast cancer at RSUP Dr. M. Djamil Padang in 2020. The method used is a mixed-method with a Sequential Explanatory Design. Sampling technique using Consecutive Sampling. Quantitative data was obtained through questionnaires and qualitative data using interviews. The results showed that the number of respondents who were not good at breast self-examination (BSE) was 64 (58.2%) and respondents who were good at breast self-examination (BSE) were 46 (41.6%). The most common breast cancer stages were at stage 3 of 67 (63.9%), stage 2 of 30 (27.3%) and the least at stage 4 was 13 (11.8%) with a p-value = 0.020. In conclusion, there is a relationship between breast self-examination (BSE) and the stage of breast cancer. Keywords: Breast Self-Examination (BSE), Breast Cancer Stage

Abstract

Introduction Corona Virus Disease 19 (COVID-19) infection was first reported inWuhan, China, in December 2019. The author wants to know how the OncologySurgery Division elective surgery patient services before and during early pandemi cera of COVID-19 at Dr. M Djamil Padang Hospital. M ethod: This research is acomparative study with a quantitative approach to elective surgery patients in theOncology Surgery Division before and during early pandemic era of COVID-19 at Dr.M Djamil Padang Hospital. The sampling technique in this study was total sampling.Data analysis will be carried out using statistical analysis in accordance with theexisting data scale and using SPSS software. Results: In this study, 137 sampleswere obtained consisting of 80 samples before the COVID-19 pandemic and 57samples during early pandemic era of COVID-19. Of the 57 patients who underwentelective surgery from the Surgical Oncology Division during early pandemic era ofCOVID-19, all patients were not suspicious / confirmed. The time for oncologyservice assessment was 14.26 days before COVID-19 pandemic and 8.93 days duringearly pandemic era of COVID-19. Conclusion: More patients who underwent electivesurgery in the surgical oncology division were female, both before and during earlypandemic era of COVID-19. The mean age of patients before COVID-19 pandemicwas 48.72 years and 49.53 years during early pandemic era of COVID-19. There isa disparity between the length of time the assessment of elective surgery patients inthe surgical oncology division before and during early pandemic era of COVID-19pandemic.

Abstract

BACKGROUND: Interaction between estrogen and estrogen receptor (ER) takes part in the regulation and differentiation of breast tumorigenesis. Some ERα polymorphisms, including ERα rs3798577, are reported to be associated with the risk and aggressiveness of breast carcinoma since the site was reported to be targeted by microRNA, which can further modulate the ERα expression. Hence, this study was conducted to disclose the possible role of ERα SNP rs3798577 on breast carcinoma patients.METHODS: Samples were taken from the post-mastectomy breast carcinoma tissues of female patients and screened based on the completeness of medical and histopathological records. DNA isolation was proceeded using real time-polymerase chain reaction (RT-PCR) then analyzed for high resolution melting (HRM). The nucleotide base sequence was then analyzed based on rs3798577 ERα polymorphism. ER immunohistochemistry test was carried out and counted quantitatively based on the staining intensity and the percentage of the stained cells.RESULTS: Out of 65 samples, there were 33 samples as wild type and 32 samples as variant type. Most variant and wild type had >80% ERα percentage. Most variant type had middle ERα intensity, while wild type had strong ERα intensity. Higher percentage of variant type (52.2%) was found with weak ERα histoscore, meanwhile higher percentage of wild type (52.4%) was found with strong ERα histoscore, but not significant (p=0.725).CONCLUSION: ERα rs3798577 variant type had a lower ERα intensity and weaker ERα histoscore compared to the wild type, suggesting that ERα rs3798577 polymorphism might play a role in breast carcinoma risk determination.KEYWORDS: breast cancer, ERα, rs3798577, polymorphism, immunoexpression

Abstract

Latar Belakang: Pembedahan kanker daerah kepala dan leher umumnya menimbulkan defek luas dan biasanya memerlukan flap. Meskipun free flap saat ini merupakan gold standard untuk rekonstruksi daerah kepala leher, pectoralis major myocutaneous flap (PMMC) masih digunakan. Kasus: Perempuan usia 47 tahun, dengan kanker tiroid papiler T4aN0M0 dengan ulserasi di kulit leher, menjalani tiroidektomi total dan defek operasi direkonstruksi dengan pectoralis major myocutaneous flap. Hasil rekonstruksi dapat diterima secara fungsional dan estetik. Tidak ada komplikasi hematom ataupun abses post operasi. Simpulan: Pectoralis major myocutaneous flap masih merupakan salah satu metode utama untuk rekonstruksi operasi kepala leher dan dapat diterima secara fungsional dan estetik jika free flap tidak dapat dilakukan. Background: Surgery for head and neck cancer generally leaves a wide defect that usually needed a flap. Although free flap is currently the gold standard for reconstruction of the head and neck, the pectoralis major myocutaneous flap is still popularly used. Case: A 47-year old female with thyroid carcinoma, underwent total thyroidectomy and the surgical defect was reconstructed with pectoralis major myocutaneous flap. The results were viable, functional, and aesthetically acceptable. No postoperative complications such as hematoma or abscess observed. Conclusion: Pectoralis major myocutaneous flap was still one of the main methods for head and neck reconstruction surgery.

Abstract

Kanker payudara merupakan kanker yang paling banyak pada perempuan di dunia. Pada beberapa dekade terakhir, pengobatan adjuvan tamoxifen banyak digunakan. Penelitian ini bertujuan mengevaluasi disease-free survival, survival rate, dan efek samping pengobatan tamoxifen. Penelitian deskriptif retrospektif pada bulan September hingga Desember 2016 di RSUP Dr. M. Djamil Padang menggunakan data sekunder 100 sampel dari rekam medik bagian Bedah Onkologi RSUP Dr. M. Djamil Padang dan data primer melalui wawancara. Analisis dilakukan secara univariat dan metode Kaplan-Meier. Hasil penelitian ini didapatkan kelompok usia terbanyak adalah 50-59 tahun dengan rata-rata 52,3 tahun. Disease-free survival 5 tahun adalah 0,727 dan survival rate 5 tahun adalah 0,777. Efek samping paling banyak adalah hot flush (65%). Terdapat hubungan antara efek samping pengobatan tamoxifen dan disease free survival dengan nilai p=0,001 (p < 0,05) kekuatan hubungan sedang (r=0,376). Breast cancer is the most commonly diagnosed cancer among women worldwide. For the last few decades, adjuvant tamoxifen is the most preferred therapy. This research aims to evaluate the disease-free survival, survival rate, and side effects of tamoxifen therapy. This retrospective descriptive research was conducted in September until December 2016 at Dr. M. Djamil Padang Hospital using 100 secondary data samples from medical records of Surgical Oncology Dept., Dr. M. Djamil Padang Hospital and primary data through interviews. Univariate analysis and Kaplan-Meier method are used. The result shows that the largest age group is 50-59 years old with average 52.3 years old. The 5-year disease-free survival is 0.727 and 5-year survival rate is 0.777, the most frequent side effect is hot flush (65%). There is medium strength of the relationship (r = 0.376) between side effects of tamoxifen treatment with disease-free survival (p = 0.001 <0.05).

Abstract

Diabetic ulcer is a disease that greatly affects the patient's quality of life, where the end of the course of the disease is that the patient must undergo an amputation. In Indonesia, diabetes shows an increasing incidence. Diabetic wounds that don't heal are a risk factor for infection and a major cause of amputations and death. One way of treatment is by treating wounds using the Negative Pressure Wound Therapy technique for healing diabetic ulcers. The research method used is a systematic review by identifying journal articles published online consisting of English in 2018-2021. Articles were retrieved using search engine tools PubMed, ScienceDirect, and JSTOR. There are 5 journals based on inclusion criteria. The results showed that there was an effect of wound care using the Negative Pressure Wound Therapy technique on the healing of diabetic ulcers.

Abstract

Background Fine-Needle Aspiration Biopsy (FNAB) thyroid nodules is not easy to distinguish betwen follicular malignancy and follicular benign lession. Malignancy diagnosis can only be made from histopathology examination by examing tumor invation to capsule and vaskular. Matrix Metalloproteinase 9 (MMP-9) examination can be added to help making initial diagnois. MMP-9 is a proteolytic enzyme that plays important role in cancer progressivity, especially in thyroid. High MMP-9 expression will lead to earlier degradation of thyroid and its will make the cell metastasist easier. This study is to examine MMP-9 expression of thyroid follicular neoplasm related tomalignancy through histopathology examination and the diagnostic test of FNAB MMP-9 expression and histophatologic examination. Methods We used 42 sample FNAB follicular thyroid in Anatomic Pathology laboratories RSUP M Djamil Padang and histopathologic examination was performed. Immunohistochemical was staining with primary antibody MMP-9 then its expression were assessed in stroma and cytoplasmic cell tumour. Results MMP-9 expression was found positive in malignancy histopathologic (80.9%). Expression of MMP-9 at FNAB folikullar neoplasm thyroid is significant in malignant histopathologic thyroid (p=0.017). MMP-9 Sensitivity in thyroid follicular noduleFNAB is quite good (88.575) and its specificity is 51.14%. Conclusion Increased MMP-9 expression of FNAB follicular thyroid neoplasm is associated with histopathologic malignancy.

Abstract

This study aims to determine the relationship of daily physical activity with the quality of life of breast cancer survivors. The research design used is correlative with a cross-sectional approach. The results showed 53.9% of respondents aged > 60 years, 50.6% of respondents had higher education, 61.8% of respondents did not work, 49.1% of stage IIIA respondents, 55.1% of respondents had moderate daily physical activity, and 73,0% of respondents have a reasonable quality of life. Most respondents have moderate physical activity, 55.1%, with most of the quality of life being 73.0%. The results of the bivariate analysis showed that (p-value = 0.034). In conclusion, there is a relationship between daily physical activity and the quality of life of breast cancer survivors at RSUP Dr. M. Djamil Padang. Abstract

Background: Human Epidermal Receptor-2 (HER-2) is a member of the human epidermal growth factor receptor family that can be overexpressed or amplified in 15–20% of breast cancer cases. This overexpression or amplification is associated with excessive proliferation and differentiation resulting in more aggression and shortened disease-free survival (DFS) and overall survival (OS). Treatment strategies rely on several factors, including targeting the specific status of biomarkers like HER-2, one example is Trastuzumab. This study aims to evaluate trastuzumab treatment in HER-2 subtype breast cancer patients who received trastuzumab therapy for 6 months compared to patients who did not receive trastuzumab therapy in terms of DSF and OS from 2017 until 2022.Methods: This research employed a retrospective descriptive study using secondary data of 126 patients taken from the medical records of the Provincial General Hospital Dr. M. Djamil Padang and the Cancer Registry of the Indonesian Society of Surgical Oncology, Padang. The inclusion criteria for this study were data on breast cancer patients with HER-2 positive who received trastuzumab therapy for 6 months and patients who did not receive trastuzumab therapy. Patients have complete data including name, gender, age, immunohistochemistry results, and their classification can be determined. This study used univariate analysis and Kaplan-Meier for survival analysis methods.Results: The results of this study showed that the highest age group was over 50 years, the most common histopathological subtype was invasive Carcinoma Mammae of NST (Non-Special Type), and the most frequent TNM stage was Stage II. Patients who received 6 months of trastuzumab therapy had disease-free survival of 46 months (95% CI 39.95–51.15) and an overall survival of 53 months (95% CI 49.19–57.24). In the early stage, the disease-free survival rate was 56 months (95% CI 38.89–56.60), while in a locally advanced stage, the disease-free survival rate was 39 months (95% CI 51.52–61.21).Conclusions: There was a prolongation of disease-free survival and overall survival rates when trastuzumab therapy was given for 6 months. The prognosis of HER-2-positive breast cancer patients in the early stage was better than patients in the locally advanced stage.

Abstract

Background: Risk factors that cause breast cancer have been a study related to racial, cultural, and socioeconomic factors. Therefore, breast cancer risk factors become important for prognosis and predictive in management.  Currently, biomolecular breast cancer is divided into several subtypes, namely Luminal A, Luminal B, HER-2 overexpressive, and Triple-negative breast cancer. Objective: This study aims to determine the relationship between risk factors and the types of molecular subtypes of breast cancer in Padang City. Method: The type of research is analytical with a cross-sectional design. The research took place from April to August 2022 at RSUP Dr. M. Djamil Padang, RSI Ibnu Sina Padang, RSK Ropanasuri Padang and RSU Bunda BMC Padang. Results: The results showed that the most type of subtype was Luminal B (49.3%). Then, the type of TNBC (24.6%). Followed by HER-2 Overexpressive (11%). And the least is the type of Luminal A (10.1%). Most (89.9%) of patients were exposed to passive smoking. Most (87%) menopausal age is most at the age of under 50 years. Most (75.4%) have no family history of cancer. More than half (73.9%) did not use oral contraceptives for more than 12 months. More than half (72.5%) consumed fat and calories within normal limits. More than half (68.1%) were without sports activities. More than half (55.1%) of the age giving birth to their first child is found to be spanning the ages of 20 to 30 years. Almost half (47.8%) of breast cancer patients are obese. Based on the Chi-Square test and the Fisher test, showed that there was no association of risk factors with the type of molecular subtype. Conclusion: There is no relationship between risk factors and molecular subtypes of breast cancer in Padang City. Most types of molecular subtypes found Luminal B, followed by the type of TNBC, then HER-2 Overexpressive, and the least is Luminal A. The results of these findings are the same as other studies in Indonesia
