Abstract

Background: The occurrence of ineffective airway clearance in pulmonary TB patients begins with disease transmission which occurs when a patient with positive BTA sneezes or coughs without covering his nose or mouth so that germs will spread into the air in the form of phlegm or droplets. In the pathophysiology of pulmonary TB, it can cause destruction or fibrosis of the airways and damage the lung parenchyma, causing tracheal/pharyngeal edema, increased secretion production, sputum obstruction, rupture of blood vessels or even hemaptoea, resulting in the airway being ineffective and causing shortness of breath. Purpose: The aim of this research isexplain the study aims to determine the effect of a combination of Active Cycle of Breathing Techniques (ACBT) and chest physiotherapy on the effectiveness of airway clearance in pulmonary TB patients. Methods: The method used was quasi experimental with a pretest-posttest control group design approach. This design involved two groups, namely a combination intervention group of Active Cycle of Breathing Techniques (ACBT) and Chest Physiotherapy and a control group. Data collection was carried out using direct observation sheets and medical records. Results: Based on the test results using Mann Whitney, a p value of 0.000 was obtained, where the value was <0.05, which means there was a difference in the airway clearance score between the intervention and control groups in the delta/change data, where based on the mean value, the airway clearance score for the intervention group was obtained. the increase in score was greater than the control group when delta/change data. Conclusion: Combination of Active Cycle of Breathing Techniques (ACBT) and Chest Physiotherapy on the Effectiveness of Airway Clearing in Pulmonary TB Patients and is easy to do.

Abstract

Background: A person's anxiety due to an illness is one of the causes of hypertension or an increase in blood pressure above the normal limit in all age groups. Hypertension that is not recognized and is not treated immediately can cause various kinds of comorbidities. Therefore, apart from pharmacological treatment, non-pharmacological therapy is also needed to reduce anxiety and blood pressure and improve the quality of sleep for hypertension sufferers. Purpose: The aim of this research is Explain the influence of Combination Of Spiritual Benson Relaxation and Lavender Aromatherapy (SBR-LA) on anxiety, sleep quality and blood pressure in hypertension patients. Methods: This research is quantitative research that uses a quasi-experiment research design with a pretest - posttest control group design approach. The entire population of hypertension patients in TNI AL Dr.Oepomo Hospital many as 97 people. The sample size was 44 respondents. The sampling technique is purposive sampling. The research independent variables are: Combination Of Spiritual Benson Relaxation And Lavender Aromatherapy  (SBR-LA) and the dependent variables are anxiety, sleep quality and blood pressure. Analysis used the paired t-test with a significant p value <0.05. Results: The results of research using the paired t-test show that there is an effect of giving The Effect of the Combination of Spiritual Benson Relaxation and Lavender Aromatherapy (SBR-LA)which is significant for anxiety with p value = 0.000, sleep quality with p value = 0.000 and systolic blood pressure p value = 0.000, diastolic blood pressure p value = 0.000. Conclusion: The Effect of the Combination of Spiritual Benson Relaxation and Lavender Aromatherapy (SBR-LA) Effectively reduces anxiety levels, improves sleep quality and lowers blood pressure in hypertension patients. This technique is very easy to do, so it can be applied as a nursing intervention in inpatient rooms to develop nursing knowledge in hospitals.

Abstract

Background: Ischemic stroke causes serious ongoing problems in the lives of sufferers, including biological, psychological and spiritual changes so that comprehensive and multidisciplinary nursing care is necessary. Purpose: The purpose of this research is to analyze the effect of Mindfulness and Spiritual Combination Breathing Exercises towards anxiety, depression and blood pressure of ischemic stroke patients. Methods: This research is a quantitative research that uses a quasy experimental research design with a pretest - posttest control group design approach. The population of all ischemic stroke patients at RSAU dr. M.Munir Lanud Abdulrachman Saleh Malang as many as 108 people. The sample size is 50 respondents. The sampling technique is purposive sampling. The intervention group was given a combination of Mindfulness and SpiritualityBreathing Exerciseswithin 10-15 minutes 2 times a day for 5 days and the control group was given intervention according to hospital standards. Analysis used the Manova test with a significant p value <0.05. Results: The results of the study using the Manova test showed that there was a significant effect between anxiety, depression and blood pressure after giving the Combination of Spiritual Mindfulness and Breathing Exercise sshowed that p value = 0.000, with the highest partial Eta Squared value of 0.643 anxiety followed by 0.579 depression and 0.263 blood pressure. Conclusion: A combination of Spiritual Mindfulness and Breathing Exercises effectively reduce the level of anxiety, depression and blood pressure of ischemic stroke patients.

Abstract

Background: Currently, the trend of patients admitted to the hospital is increasingly complex and has many co-morbidities, some of which the patient's condition will suddenly worsen or become critical, due to various interrelated causes such as weak monitoring systems and staff reactions to emergencies. Delays in treatment or inadequate care for patients who are treated in the Hospital Ward can result in several adverse conditions, namely increased patients transferred to the ICU (Intensive Care Unit), increased LOS (Lenght of Stay) in the hospital, heart attacks, or patient death. Purpose: The purpose of this study to analyze the differences in the modification of the NEWS observation sheet on the emergency conditions of patients at the hospital. Methods: The type of research used is quasi-experimental with a pre-test-post-test control group design. With a population of 160 patients and a total patient sample of 62 patients with 31 patients for the intervention group and 31 patients for the control group. How to take samples with purposive sampling technique. The independent variables in this study were the NEWS observation sheet and the NEWS modification, while the dependent variable was the patient's emergency condition. The instruments in this study were NEWS observation sheets and NEWS modifications, integrated patient development record sheets, pre-test and post-test sheets. Data analysis used Wilcoxon test and Mann Whitney test with a value of α = 0.05. Results: The results showed that the modification of the NEWS (National Early Warning Score) observation sheet with a graphic was more influential than the NEWS observation sheet in increasing vital sign monitoring with p=0.00, while the NEWS observation sheet as a control had an influence with a significant level of p=0.043, both the model shows that it is equally influential in detecting changes in patient conditions and as an aid in communicating with other health workers. Conclusion: The conclusion of this study is that after being given a modified NEWS observation sheet which is a scoring system that is simple, practical and proven to increase alertness and response to signs of bad physiological changes so that patient safety can be monitored optimally and mortality rates can decrease.

Abstract

Background: Stroke causes neurologic disturbances that can cause changes in quality of life. Quality of life is mental, physical, social health and regardless of disease. In order to achieve a good quality of life, there must be a role for health workers. Discharge planning in stroke patients is preparing patients for treatment in healing and improving the health status of stroke patients. Purpose: The purpose of the literature review is to find out the importance of discharge planning in stroke patients. Methods: The research stage is to make a selection by paying attention to the year of publication and with full text. Furthermore, articles and journals that are considered less relevant are excluded. Literature was obtained from 10 articles relevant to Google Scholar, Indonesia One Search, DOAJ, and Garuda with the keywords Discharge planning, Quality of life and Stroke. Results: After reviewing the selected articles, it was found that discharge planning has a good impact on increasing family readiness in caring for patients, as well as improving the quality of life of stroke patients, namely being able to increase rehabilitation activities, prevent disease complications and adherence to taking medication and routine control. Conclusions: Improving the quality of life of stroke patients, which can improve rehabilitation activities, prevent disease complications and comply with taking medication and routine control.
