Abstract

Introduction. Old age is related to geriatric syndrome where there are several health problems that often occur related to a decrease in body function and an increase in the inflammatory process. Decrease in muscle mass, muscle strength and physical performance will cause a condition of sarcopenia and frailty syndrome in the elderly. The importance of prevention so that sarcopenia does not occur in the elderly by evaluating the factors that can cause it, one of which is controlling interleukin 6 (IL-6) as proinflammatory. The purpose of this study was to determine the correlation between serum IL-6 levels with muscle mass, muscle strength and physical performance in the elderly gymnastics-community in Mohammad Hoesin General Hospital, Palembang. Methods. This research is an observational analytic study with a cross-sectional approach which was conducted at the Integrated Geriatric Clinic Internal Medicine RSMH Palembang from November 2019 to November 2020. A sample of 31 people aged > 60 years was examined for muscle mass, muscle strength, physical performance and IL-levels. 6 serum. All data processing and analysis in this study used the SPSS version 25 for Windows program. Results. of the 31 subjects, 27 were female (87.1%) and 4 male (12.9%). The mean value of muscle mass is37.9 kg/m2, muscle strength 23 kg, and physical performance 6.1 m/sec.There were 28 (90.3%) samples with elevated serum IL-6 levels, while 3 (9.7%) samples had normal levels. There was a significant negative correlation (p = 0.000) with a strong correlation strength (r = -0.722) between serum IL-6 levels and muscle mass and a significant negative correlation (p = 0.050) with a weak correlation strength (r = -0.354) between IL levels -6 serums with physical performance. Conclusions. There was a significant negative correlation between serum IL-6 levels and muscle mass and physical performance. So, it is very important to prevent an increase in serum IL-6 levels so that the elderly do not fall into a state of sarcopenia.

Abstract

Background. Delirium is a common condition in geriatric patients. One of the trigger factors for this condition is an infection, such as COVID-19 infection. Elderly with COVID-19 show atypical symptoms such as delirium. Elderly patients with COVID-19 who present with delirium, either as a primary symptom or showing symptoms or signs, have a poor prognosis. This study were aimed to presents covid-19 elderly patient with comorbid delirium. Case presentation. A 77-year-old woman with disorientation for one day came to Emergency Department with her family. She had no history of headaches, blurred vision, or seizures. However, she had a fever, did not want to eat for three days, and had a purulent decubitus ulcer. The patient was diagnosed with acute delirium syndrome, confirmed COVID-19 with sepsis, malnutrition, hypercoagulation, grade III decubitus ulcer, suspected dementia, immobilization, total dependence. The patient admitted to the isolation ward. The patient had meropenem 500 mg every 12 hours, anticoagulants and favipiravir according to the dose and parenteral nutrition. Conclusion. Patients with COVID-19 who present with delirium, either as a primary symptom or presenting symptoms or signs, have a worse outcome. Delirium relationship with comorbid factors can increase mortality and morbidity in the elderly with COVID-19.

Abstract

Background: The elderly and geriatric population is the population most at risk for complications of COVID-19. Preliminary data show that individuals aged >59 years are estimated to experience five times more severe COVID-19 symptom onset than those aged 30 to 59 years. Nutritional status is believed to play a role in the body's ability to produce antibodies after COVID-19 vaccination. Methods: A cross-sectional analytic observational study to determine the role of nutritional status on levels of IgG SARS-CoV-2 after COVID-19 Vaccination in Palembang. IgG levels were assessed by the ECLIA method. Data analysis was carried out univariate and bivariate. Results: The mean quantitative IgG of SARS-CoV-2 in underweight nutritional status was higher with a value of 5757.42±2594.08 U/mL and with the lowest mean quantitative IgG SARS-CoV-2 in overweight nutritional status with a value of 2998.13±4386.95 U/mL. Conclusion: Nutritional status did not play a role in SARS-CoV-2 IgG levels after the COVID-19 vaccination in Palembang.

Abstract

As the population aging increasing rapidly worldwide, including in Indonesia, the health problems come as consequences as well as delivering health and social care. At this moment 14% of Indonesian consist of aging population, defined as those above 60-year-old of age. Aging ameliorates normal hormonal and immunology system, consequently elderly are prone to infection as well as metabolic disorder, namely diabetes. Good glycemic control in elderly can prevent cognitive decline such as mild cognitive impairment and dementia. Abstract

The causes of sarcopenia are thought to be multifactorial including: environmental factors, illness, activation of inflammatory pathways, mitochondrial dysfunction, loss of neuromuscular junctions, reduced number of satellite cells and changes in the quality of hormonal function. The mechanism of sarcopenia occurs because the aging process coupled with reduced physical activity can cause the formation of cytokines to increase so that inflammation is easy, insulin resistant, anabolic resistance and neuromuscular impairments.

Abstract

Omega-3 fatty acids are polyunsaturated fatty acids that have multiple double bonds, the first double bond is located on the third carbon atom of the omega-methyl group, the next double bond is located at the third carbon atom from the previous double bond. The methyl omega group is the last group in the fatty acid chain. Omega-3 fatty acids are nutrients that play a vital role in the growth and development process of brain neuron cells for the intelligence of the born baby.

Abstract

Improving the quality of life of the elderly through screening for risk of morbidity and promote the healthy eating practices. Aging results physiological decline in the organ function. Quality of life can be further decreased if there are other diseases that aggravate the health condition of the elderly. Chronic diseases such as diabetes and cardiovascular disease are the causes of disability in the elderly. Diet and physical activity are factors that can prevent this morbidity. Morbidity screening and education of healthy eating patterns are still needed for elderly, who are physically active, to support their health. Community service activities consist of nutrition counseling for elderly and health checking such as examine waist circumference, body fat mass, blood pressure, cholesterol and random blood sugar test. A total of fifty elderly, aged sixty years and over, that consist of forty-two women and eight men from the elderly community at Central General Hospital. Dr. Mohammad Hoesin Palembang, was included in this activity. The mean proportion of body fat mass was 35.01% + 9.03. As many as 6 (75%) men and 35 (83.3%) women had waist circumference exceeding normal. There were 2 people (4%) who had blood sugar levels> 200 g / dL, 18 people (36%) with cholesterol levels> 200 g / dL, and 12 people (24%) with blood pressure> 150/90 mmHg. Most of the elderly have central obesity and high cholesterol levels. There are participants who should receive therapy for having high blood sugar, cholesterol, and blood pressure. Respondents who have risk of disease are advised to carry out further examinations. A dietary assistance program could be proposed to improve obesity conditions in future community service activities.

Abstract

Ageing is associated with the geriatric syndrome, characterized by several health issues frequently associated with immunosenescence. In the elderly, immunosenescence is characterized by low-grade chronic systemic inflammation manifested as elevated levels of inflammatory molecular and cellular markers. One of the inflammatory markers is interleukin 6 (IL-6). In addition, omega-3 polyunsaturated fatty acids are a class of biologically active and anti-inflammatory fatty acids. This study aims to determine the relationship between omega-3 and interleukin-6 levels in the elderly in RSMH Palembang who are in good health. This investigation is a cross-sectional, analytic observational study. The sampling method employed was consecutive sampling. The participants in this study were members of the RSMH Palembang healthy elderly community who were 60 years of age or older. People who were malnourished, had severe illnesses like cancer or autoimmune diseases, or were taking Omega-3 supplements were not allowed in this study. As a result, 22 (55%) of 40 research subjects had low serum omega-3 levels. On the other hand, there were 35 (87.5%) samples with elevated serum IL-6 levels and 5 (12.5%) samples with normal IL-6 levels among the samples obtained. The correlation analysis showed that there was a strong negative correlation between the levels of serum omega-3 and interleukin 6 (p = 0.005). Omega 3 levels and interleukin-6 levels are correlated in the healthy elderly population of RSMH Palembang.

Abstract

Background: Sarcopenia is a syndrome characterized by decreased muscle mass and decreased muscle strength and muscle function. Oxidative stress and inflammatory processes are known to contribute to sarcopenia by releasing the catabolic stimuli interleukin-6 (IL-6) and insulin-like growth factor (IGF). Methods: The method used is a literature review by collecting appropriate articles from the PubMed and Google Scholar databases. Results: Low IL-6 can trigger satellite cell activity and myotube regeneration, whereas if IL-6 production increases chronically, it triggers muscle wasting. Changes in muscle size are the result of changes in the synthesis and degradation of muscle protein. IGF-1, where signal changes are strongly related to muscle size. IGF-1 binding to the receptor causes intracellular phosphorylation of the adapter protein Shc or insulin receptor substrate (IRS-1). Conclusion: The role of IGF-1 and IL-6 can be helpful as diagnostic and prognostic parameters for sarcopenia. Abstrak: Latar belakang: Sarkopenia adalah sindrom yang ditandai dengan penurunan massa otot disertai penurunan kekuatan otot dan atau fungsi otot. Stres oksidatif dan proses inflamasi dikenal sebagai faktor pemicu untuk sarkopenia dengan melepaskan rangsangan katabolik interleukin-6 (IL-6) dan IGF1. Metode: Metode yang digunakan adalah literature review dengan mengumpulkan artikel-artikel yang sesuai dari data base PubMed dan google sholar. Hasil: IL-6 yang rendah dapat mencetuskan aktivitas sel-sel satelit dan regenerasi myotube, sedangkan bila produksi IL-6 meningkat secara kronik mencetuskan muscle wasting. Perubahan ukuran otot merupakan dampak dari perubahan sintesis dan degradasi protein otot. IGF-1 dimana perubahan sinyal berhubungan kuat dengan ukuran otot. Ikatan IGF-1 pada reseptor menyebabkan fosforilasi intracellular adaptor proteins Shc atau insulin receptor substrate (IRS-1). Kesimpulan: Peran IGF-1 dan IL-6 dapat berguna sebagai parameter diagnostik dan prognostic terhadap kejadian sarcopenia.
