Abstract

Introduction. Sepsis is defined as organ dysfunction caused by dysregulation of the host’s response to infection. The majority of sepsis are experienced by elderly patients with high mortality rates. In the condition of sepsis, there are various body mechanisms that cause a decrease in albumin and hemoglobin. This study is to determine the predictive value of albumin and hemoglobin levels on 28-day mortality of elderly sepsis patients.

Abstract

Introduction. Geriatric patients have multipathological characteristics, low physiologic reserves, atypical clinical signs and symptoms, decreased functional status, and nutritional disturbances. This situation is a risk factor for polypharmacy. Polypharmacy is closely related to drug side effects, drug interactions, and patient compliance. Polypharmacy is thought to have a relationship with the length of stay in geriatric patients. The purpose of this study was to determine the correlation between polypharmacy and length of stay in geriatric patients at Moehammad Hoesin Hospital Palembang. Methods. This was a cross-sectional study with a correlation test design conducted at Dr. Mohammad Hoesin Hospital (RSMH) Palembang from April 2021 to July 2021. The sample was geriatric patients aged > 60 years. We collected data on the number and types of drugs given and the length of hospitalization. All data processing and analysis in this study were conducted by using SPSS version 22 for Windows. Results. Of 56 geriatric patients, there was 28 female (50%) and 28 male (50%). The median value of the number of drugs was 7 (3-32). Patients without polypharmacy were 7 (12.5%), and patients with polypharmacy were 49 (87%). The median length of hospitalization for geriatric patients at RSMH was 11.5 (range 1-47) days. There was a correlation between polypharmacy and length of stay (r=0.277; p value=0.039). Conclusion. There is a weak correlation between polypharmacy and length of stay (r=0.277; p=0.039).

Abstract

Background: The aging process is physiological, but the onset, rate, and extent of the progression of aging at the cell, tissue, organ, and system level are very heterogeneous because they are influenced by genetic and environmental factors. The aging process is known to involve the entire body, including musculoskeletal and cardiovascular components. Several theories explain this association, including reduced muscle mass associated with chronic heart failure, decreased serum myostatin, and cross-talk between skeletal and cardiac muscle mediated by inflammatory cytokines. This study aimed to determine the relationship between skeletal muscle strength and cardiac performance in elderly patients at Dr. Mohammad Hoesin General Hospital, Palembang. Methods: This research is an analytical observational study with a cross-sectional design. The total sample size was 65 individuals who met the inclusion and exclusion criteria. Data analysis was carried out using the SPSS 25 for Windows program. Results: Handgrip strength has a significant relationship with diastolic dysfunction on a numerical scale (p=0.009). Low handgrip strength has a 3 times greater risk of experiencing diastolic dysfunction than patients with Normal Handgrip Strength. Conclusion: Skeletal muscle strength has a relationship with cardiac senescence performance in elderly patients at Dr. Mohammad Hoesin General Hospital, Palembang.

Abstract

Background: Systemic inflammation associated with high hs-CRP levels constitutes one pathway through which diastolic dysfunction develops in older adults. This study aims to determine the relationship between high-sensitivity c-reactive protein (hs-CRP) and diastolic dysfunction in older adults at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia. Methods: This research is an observational analytical study using a cross sectional design. The research was conducted at the geriatrics polyclinic at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia from June to August 2023. A total of 65 people took part in this study. The relationship between hs-CRP and diastolic dysfunction was analyzed using SPSS version 25, univariate and bivariate. Results: Hs-CRP had a significant relationship with diastolic dysfunction on a numerical scale (p=0.012), with a median value of 1.6 mg/dL in subjects with diastolic dysfunction and 0.9 mg/dL in subjects without diastolic dysfunction. On the categorical scale, the hsCRP less than 1 mg/dL group found nine subjects (23.7%) with diastolic dysfunction and 14 subjects (51.9%) without diastolic dysfunction, the hsCRP 1-3 mg/dL group found 21 subjects (55.3%) with diastolic dysfunction and 12 subjects (44.9%) without diastolic dysfunction, in the hsCRP more than 3 mg/dL group (p=0.026) there were eight subjects (21.1%) with diastolic dysfunction and one subject (3.7%) without dysfunction diastolic. Conclusion: There is a significant relationship between hsCRP and left ventricular diastolic dysfunction in geriatrics patients at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia.

Abstract

Introduction: The SARS-CoV-2 virus is the cause of coronavirus illness (COVID-19). The WHO classified the outbreak a pandemic on March 9, 2020. The COVID-19 vaccine has been widely utilized to manage the pandemic. However, some high-risk conditions that can compromise the vaccine's efficacy require careful consideration. Patients over 60 years old are at a greater risk for COVID-19. Due to the involution of the thymus and the transformation of the cortex and medulla tissue into adipose tissue, CD8+ T cell numbers decline. In addition, undernutrition reduces the number of CD8+ T cells. The purpose of this study is to examine the relationship between nutritional status and CD8+ T cell counts in elderly patients following the second COVID-19 vaccination. Methods: This study is a pilot project employing a cross-sectional design. The sampling employed a technique of sequential sampling. On day 28, following the administration of the second vaccine, the Mini Nutritional Assessment (MNA) and the amount of CD8+ T cells in the blood were utilized to evaluate the nutrition status. Results: Between May and November of 2021, forty subjects were collected. The average number of CD8+ T cells was 517.32±41.67. Therefore, five people (12.5%) were at risk of malnutrition, whereas thirty-five (87.5%) had a normal nutritional status. Conclusion: There is a significant relationship between MNA and CD8+ levels (P<0.05) and a positive correlation between nutritional status and CD8+ T cell counts in the elderly who have received the second covid-19 vaccination in Palembang.

Abstract

Introduction: Sarcopenia is a common problem found in the elderly. It is characterized by loss of muscle mass, muscle strength, and/or low physical performance. Currently, there’s little research on the association between serum albumin and muscle mass, muscle strength, and physical performance in the elderly, especially in Indonesia. This study aimed to find the correlation between serum albumin and muscle mass, muscle strength, and physical performance in the elderly. Method: A cross-sectional study was conducted from July 2022 to December 2022 towards elderly patients (>60 years) from the geriatric polyclinic at RSUP Moh. Hoesin Palembang. Serum albumin was examined for its correlation with muscle mass, muscle strength, and physical performance using Spearman’s test. Result: 41 elderly patients were included in this study (mean age 70.75 ± 7 years, 56.1% women). Correlation analysis showed that there’s a significant positive correlation between serum albumin with muscle strength (r=0.354, p=0.012) and a negative correlation with physical performance (r=-0.5, p <0.001). In comparison, there was no significant correlation between serum albumin and muscle mass (r=- 0.05, p=0.367). Conclusion: Muscle strength and physical performance have a significant correlation with serum albumin in the elderly. Therefore, even in the normal range, lower serum albumin is closely related to sarcopenia in the elderly. Key Words: albumin, muscle mass, muscle strength, physical performance, sarcopenia.

Abstract

Background: Xerosis and pruritus (XP) are the most common skin conditions in the elderly, manifesting clinically as roughness, scales, fissures, and mild-to-severe itching. Vitamin D improves skin hydration, transepidermal water loss (TEWL), and regulates the immune system. We aimed to determine the efficiency of topical 7-dehydrocholesterol (DHC)-vitamin D3 complex cream in elderly patients with XP. Methods: An experimental phase III study was performed on elderly women with XP from December 2021 to March 2022 at the Geriatric Dermatology Clinic of Dr. Mohammad Hoesin General Hospital, Palembang. Inclusion criteria were age older than 55 years, healthy or with comorbidity. The exclusion criteria include the use of moisturizer within two weeks before the study. DHC-vitamin D3 cream was applied on subject’s volar arms once a day. Overall dry skin score (ODSS), visual analog scale (VAS) for pruritus and adverse effects, skin hydration, TEWL, sebum level, serum vitamin D level, and dermatology life quality index (DLQI) were assessed at baseline and after 4 weeks. Statistical analysis was done with Wilcoxon and paired T-test. Results:Eighteen elderly women were included. Most subjects were 60 to 74 years old (83,3%). Half the subjects (50%) had comorbidity. There was a significant difference in skin hydration (p Conclusion: Topical 7-DHC vitamin D3 complex cream is proven to improve skin barriers, increase serum vitamin D levels, well-tolerated, and provided satisfaction and comfort in elderly women with XP.

Abstract

. Osteoporosis is a worldwide health issue. The microarchitecture of the bone is deteriorating, which increases the risk of fracture and disability. As the Indonesian people's life expectancy increases, the incidence of osteoporosis rises, making it more difficult to manage. This community service is carried out as a form of preventive treatment in the adult community in the village of Ibul Besar III Pemulutan Ogan Ilir, South Sumatra, which is one of Sriwijaya University's guided villages, with counseling methods to increase knowledge and health checks for early detection of osteoporosis, as well as calcium supplementation. There were 81 participants in this activity, with numerous dominant risk factors for osteoporosis identified, including female gender, menopausal state, and age above 50. Musculoskeletal symptoms, such as pain in the waist and back, which can be a consequence of osteoporosis, are more common than non-musculoskeletal symptoms.
