Abstract

Backgraound: Labor process begins with the process of cervical dilatation, which is commonly accompanied by pain. Labor pain caused by uterine contractions can cause thinning of the cervix. Effects of labor pain includes inflammation in uterus and labor dystocia. Effleurage and counterpressure are complementary treatment to stimulate uterine contractions. Objective: To analyze the effectiveness of effleurage and counterpressure massages on the progress of cervical dilatation. Method: This was a quasi-experiment study with pre-post test nonequivalent control group. Population was the women in labor in stage I active phase. Research subjects were 68 people that were divided into two groups. Sampling technique was consecutive sampling. Instrument used in this study was the internal examination. Data were analyzed using Wilcoxon and Mann Whitney. Results: The progress of cervical dilatation before and after the intervention of the effleurage was 4.74±0.83 vs 7.47±1.21 with p=0.00 and the result for the counterpressure group was 4.59±0.66 vs 8.03±0.96 with p=0.00. The average improvement of cervical dilatation on the effleurage and counterpressure groups was 2.73 vs 3.44 with p=0.00. Conclusion: The counterpressure has the more significant effect than the effleurage on improving the cervical dilatation. Keywords: Cervical dilatation, counterpressure, effleurage

Abstract

Background: Postpartum hemorrhage is define as 500 cc or more bleeding after the completion of third stage (after the placenta is born). Maternal mortality due to postpartum hemorrhage occurs a lot in health services, one of the factors that influence it is the behavior of health workers in the prevention of postpartum hemorrhage. The behavior of midwives in preventing postpartum hemorrhage is closely related to the competencies seen from the training attended. Objective: To find out the relationship between training and the behavior of midwives in preventing postpartum hemorrhage in Bantul Regency. Methods: We used a correlation study with a cross sectional design. The sampling technique in this study used probability sampling with cluster sampling totaling 68 midwives in five community health centers. Retrieval of data in this study used an observation sheet to see the actions of midwives in conducting active management in the third stage as a preventive measure for postpartum bleeding and the identity sheet of respondents to see the training that had been attended by midwives. Results: There was no relationship between training with the behavior of midwives in the prevention of postpartum hemorrhage in Bantul Regency with a p value of 0.093. A total of 40 midwives (58.8%) had good behavior and 53 midwives (77.9%) had attended training. Abstract

Sexual activity during pregnancy is one of the significant issues for both pregnant mother and healthcare provider, though rarely disclosed. Studies on this topic have been done broadly, though still no conclusion. The aim of this study was to analyze the change of mother’s sexual activity before and during pregnancy and to describe resources used by mother to find sexual activity information. A descriptive cross sectional study was conducted on pregnant mothers who undergo antenatal care at Tegalrejo public health center, Yogyakarta. Sample was determined using quota sampling which total sample was 78 pregnant mothers (26 pregnant mothers per trimester) without significant health problem related to their pregnancy, such as high risk pregnancy condition and vaginal bleeding. Data were collected using Indonesian version of Pregnancy Sexuality and Response Inventory (PSRI) questionnaire which reliable to used (Cronbach’s alpha: 0.714). Data on sexual activity (biological, emotional, and physical aspects) in each trimester were analyzed by paired t test. Data on sexual activity among trimesters in several sub-aspects (frequency, desire, and pain during intercourse) were analyzed by Kruskal wallis and followed by post hoc analysis on significant result. Result showed the significant changes of biological aspect of mother’s sexual activity before and during pregnancy, especially in the second (p 0.002) and third trimester (p 0.000) and the changes tend to decline. Significant decline also found in emotional aspect of all trimesters (p<0.05). In line with biological aspects, physical aspect also showed significant decline in the second (p 0.000) and third trimester (p 0.000). Pregnant mother got numerous resources related to sexual activities during pregnancy, yet the most common was from healthcare provider (35.9%). Providing the right information related to this topic will help the mother to understand their sexual activity changes and to choose the right decision in performing sexual activity.

Abstract

The implementation of the Human Papillomavirus (HPV) vaccination program is still experiencing obstacles in several developing countries. This setback is related to the lack of knowledge and the high cost of the vaccine so that it affects people's behavior about the HPV vaccine. HPV vaccination in adolescents itself as primary prevention has not been widely conducted by adolescents in Indonesia. The results of previous studies showed that not many teenagers had vaccinated against HPV by using self-financing guarantees such as health savings. The obstacles faced in implementing HPV vaccination are that the vaccine is still expensive and there is lack of knowledge of parents and adolescents about the importance of HPV immunization. This formative research using qualitative methods was conducted to analyze the implementation plan of the HPV vaccination program. Data were collected through focus group discussions (FGD) and in-depth interviews. At the end of each FGD activity or interview, transcription and thematic analysis based on content were done. The FGD participants were ten teachers, while the in-depth interview participants were one school committee. This study found two main themes, namely the scheme of implementing the HPV vaccination program and obstacles to the implementation of HPV vaccination. Schools generally supported the program implementation. However, parents still question the importance of the HPV vaccination for their daughters and consider the vaccine costs expensive. Implementation of the HPV vaccination program must consider the existing program schemes in schools and possible obstacles. The results suggested that for the next HPV vaccination program, the facilitators should provide intense and relevant education to parents about the importance and side effects of HPV immunization, and are expected to build a willingness of parents to vaccinate their daughters and seek vaccination costs, one of which can be through a savings program.

Abstract

Background: Adolescents are vulnerable to reproductive health problems. National Family Planning Coordinating Board (or Badan Kependudukan dan Keluarga Berencana Nasional/BKKBN in Bahasa Indonesia’s term), as government agency, has implemented reproductive health services program called Pusat Informasi dan Konseling Kesehatan Reproduksi Remaja (PIK-KRR) in certain schools. The problem was there are differences in the utilization of PIK-KRR in some areas in Indonesia. Knowledge about reproductive health and attitude on the program were some factors that may cause the differences in utilization of PIK-KRR.Objective: To identify the correlation between knowledge and attitude on reproductive health with the utilization of  the PIK-KRR.Methods: It was a non-experimental study using cross sectional approach. Sample of the research were 157 students among XI and XII grader of public high school 1 Srandakan. The research variables were knowledge about reproductive health, attitude on reproductive health, and PIK-KRR utilization. This research used total sampling technique and data were analysed using univariate and bivariate analysis. Fisher and Chi-Square test were used for bivariate analysis.Results: Respondents in this study were 143 of 157 students in XI and XII (91,07%). The result of the analysis showed that there were 95,1% of the respondents with a high level of knowledge about reproductive health, 53,1% of  respondents had the positive attitude to the reproductive health, and the utilization of PIK-KRR was in a high category (48,2%). There was a significant correlation between reproductive health knowledge and the utilization of PIK-KRR (p= 0,002) and between the attitude of reproductive health with PIK-KRR (p= 0,006).Conclusion: There was a correlation between knowledge about reproductive health and attitude on it with the utilization of PIK-KRR. ABSTRAKLatar Belakang: Masa remaja memerlukan perhatian serius karena rentan terjadi permasalahan kesehatan reproduksi. Pemerintah melalui Badan Kependudukan dan Keluarga Berencana Nasional (BKKBN) telah melaksanakan program kesehatan reproduksi bernama Pusat Informasi Kesehatan Kesehatan Reproduksi Remaja (PIK-KRR) di sekolah. Masalah yang dihadapi yaitu adanya perbedaan pemanfaatan PIK-KRR di beberapa wilayah Indonesia. Banyak faktor yang memengaruhi pemanfaatan PIK-KRR, antara lain pengetahuan dan sikap tentang kesehatan reproduksi.Tujuan: Untuk mengetahui hubungan pengetahuan dan sikap tentang kesehatan reproduksi dengan pemanfaatan PIK-KRR.Metode: Penelitian ini merupakan penelitian non-eksperimental dengan rancangan studi cross sectional. Teknik pengumpulan data menggunakan total sampling. Sampel penelitian sebanyak 157 siswa SMA kelas XI dan XII. Variabel yang diteliti adalah pengetahuan, sikap tentang kesehatan reproduksi, dan pemanfaatan PIK-KRR. Analisis data terdiri dari analisis univariat dan bivariat. Analisis bivariat menggunakan Uji Fisher dan Uji Chi-Square.Hasil: Responden dalam penelitian ini adalah 143 responden dari 157 siswa kelas XI dan XII (91,07%). Hasil analisis menunjukkan bahwa remaja dengan tingkat pengetahuan tentang kesehatan reproduksi dalam kategori tinggi sebanyak 136 orang (95,1%), remaja yang memiliki sikap positif terhadap kesehatan reproduksi sebanyak 76 orang (53,1%), tingkat pemanfaataan PIK-KRR dalam kategori tinggi sebanyak 69 orang (48,2%). Ada hubungan yang bermakna antara pengetahuan tentang kesehatan reproduksi dengan pemanfaatan PIK-KRR (p=0,002) dan sikap tentang kesehatan repoduksi dengan pemanfaatan PIK-KRR (p=0,006).Kesimpulan: Pengetahuan dan sikap tentang kesehatan reproduksi berhubungan dengan pemanfaatan PIK-KRR.

Abstract

Maternal death rate is one of the important health development indicators. Indonesian maternal mortality is still high due to both direct and indirect causes that occur during pregnancy and childbirth. High-risk pregnancy can present complications for both the mother and fetus, and demands early detection. Early detection requires involvement of the community, health cadre, medical officers, and government. There is a need to increase the community health cadre competency in the detection of pregnancy complications, especially high-risk pregnancy knowledge. This study used a quasi-experimental design without control group to determine the effect of training regarding pregnancy complications on community health worker’s knowledge in Wijimulyo, Nanggulan, Kulon Progo, Yogyakarta in October 2018. The research subjects were community health cadre workers chosen by purposive random sampling. There were 43 research subjects who were given pretests, training about complications of pregnancy, and posttests. The instrument used was a knowledge questionnaire about complications during pregnancy that consisted of 23 items with reliability of 0.865. There was significant difference between community health cadre’s knowledge about complications of pregnancy at pretest and posttest. These results indicate that training about pregnancy complications increases the knowledge of community health cadre workers concerning complications of pregnancy. Further research is needed to assess community health cadre’s skills in making early detection of pregnancy complications.

Abstract

ABSTRAKLatar Belakang: Self efficacy ibu menyusui merupakan salah satu kunci keberhasilan pemberian ASI. Banyaknya tempat kerja yang dirasakan oleh ibu bekerja belum menyediakan ruang laktasi dan fasilitas serta kebijakan untuk ibu bekerja yang menyusui Institusi Pendidikan Kesehatan di wilayah Kotamadya Yogyakarta. Kemungkinan kendala lainnya adalah jauhnya jarak tempat kerja dan waktu cuti bagi ibu bekerja. Tujuan: Mengekplorasi self-efficacy menyusui pada ibu bekerja. Metode: Desain penelitian yang digunakan adalah kualitatif dengan pendekatan fenomenologi. Populasi dalam penelitian ini adalah ibu-ibu yang memiliki anak usia 0-6 bulan yang bekerj a di Institusi Kesehatan di Wilayah Kotamadya Yogyakarta. Jumlah sample sebanyak 6 informan inti dan 1 informan pendukung, dengan tehnik purposive sampling. Instrument dalam penelitian ini adalah peneliti sendiri, alat perekam, alat tulis, pedoman wawancara mendalam, dan lembar observasi, dengan waktu wawancara dilakukan selama 25-30 menit, yang dilaksanakan antara bulan Oktober- November 2016. Data dianalisis dengan tahapan teori . Hasil: Ditemukan lima tema yaitu adanya harapan dan motivasi ibu dalam memberikan ASI eksklusif, perlunya persiapan & ketrampilan yang dimiliki ibu, kesulitan fisik yang dialami ibu, beban psikologis yang dialami ibu, keterbatasan waktu & fasilitas yang dirasakan ibu bekerja dalam memberikan ASI eksklusif. Kesimpulan: Ibu bekerja dalam memberikan ASI eksklusif, mengalami hal-hal yang menyenangkan dan hal-hal yang kurang menyenangkan. Hal menyenangkan yang mereka rasakan adalah adanya harapan, motivasi, memiliki persiapan, dan ketrampilan. Sedangkan hal-hal yang kurang menyenangkan adalah adanya masalah fisik, kondisi psikologis, minimalnya support sistem, kurangnya fasilitas, dan keterbatasan waktu bersama bayinya.Kata Kunci: Self-Efficacy, ibu bekerja, ASI eksklusifMother’s Self-Efficacy in Exclusive Breastfeeding at Health Education Institution in Yogyakarta ABSTRACTBackground: Self efficacy of breastfeeding mothers is one of the keys to the success of breastfeeding. Working mothers still thinks that many workplaces do not provide lactation rooms and facilities as well as policies for working mothers who breastfeed at the Health Education Institutions in Yogyakarta Municipality. Another possible obstacle is distance of workplace and leave time for working mothers. Objective: To explore self-efficacy of breastfeeding in working mothers. Methods: The research is qualitative with phenomenological approach. The population was mothers who had children aged 0-6 months who worked at the Health Institutions in Yogyakarta Municipality. The number of samples was 6 key informants and 1 supporting informant, whom were taken using with purposive sampling. The instruments of the research included researcher, recording devices, stationery, in-depth interview guidelines, and observation sheets, with the interview duration of25-30 minutes, which was held between October-November 2016. Data were analyzed using Creswell’s theory of stages. Results: Five themes were found, namely expectation and motivation of mothers in exclusive breastfeeding, need for preparation and skills possessed by mothers, physical difficulties experienced by mothers, psychological burden experienced by mothers, limited time andfacilities felt by mothers working in exclusive breastfeeding. Conclusion: Working mothers in exclusive breastfeeding had both pleasant and unpleasant experiences. The pleasant experiences include hope, motivation, preparation, and skills. Meanwhile, unpleasant experiences include physical problems, psychological conditions, minimum system support, lack of facilities, and limited time with the baby.Keywords: Self-Efficacy, working mothers, exclusive breastfeeding

Abstract

ABSTRAKTujuan Penelitian: Mendeskripsikan kebutuhan pengguna untuk merancang pembelajaran kasusberbasis e-learning dengan menggunakan pedoman NANDA-I, NIC, NOC dalam asuhan keperawatanmaternitas. Metode: Desain penelitian yang digunakan ialah metode kualitatif dengan pendekatanstudi deskriptif analitik. Proses pengambilan sampel dengan cara purposive sampling. Partisipanterdiri atas 2 orang dosen dan 5 orang mahasiswa. Penelitian dilakukan pada bulan November 2015selama 3 minggu. Analisis data dengan pendekatan kualitatif dan penyajian data dilakukan secaradeskriptif. Hasil Penelitian: Hasil penelitian terdiri atas 4 tema utama, yaitu (1) permasalahan dalamproses pembelajaran asuhan keperawatan maternitas dengan menggunakan pedoman NANDA-I,NIC, NOC baik yang berasal dari mahasiswa, dosen, maupun sistem pembelajaran. Permasalahanmeliputi pengajaran asuhan keperawatan belum seluruhnya menggunakan NANDA-I, NIC, NOC,ketidakpahaman mahasiswa menggunakan NANDA-I, NIC, NOC jika diberikan kasus, fasilitas bukuperpustakaan terkait NANDA-I, NIC, NOC terbatas dan kurang lengkap; (2) tujuan pembelajaranasuhan keperawatan maternitas dengan menggunakan pedoman NANDA-I, NIC dan NOC; (3)tujuan pengembangan prototype e-learning, (4) spesifi kasi rancangan prototype E-learning yangdibutuhkan pengguna yang meliputi tampilan user friendly dan menarik; konten yang diinginkanberupa latihan penyelesaian kasus; kasus yang terdiri atas DM gestasional, preeklampsi, eklampsi,sindrom HELLP, solutio plasenta, perdarahan antepartum, dan masalah sistem reproduksi. Sistempengamanan berupa password dan account, serta dilengkapi proses download serta fl eksibeldan kompatibel. Diskusi: Faktor penyebab utama adanya masalah dalam pembelajaran asuhankeperawatan dengan menggunakan NANDA-I, NIC, NOC, karena fokus pembelajaran yang lebihditekankan pada pengumpulan pengetahuan tanpa mempertimbangkan keterampilan dalammelakukan asuhan keperawatan. Fokus rancangan prototype pembelajaran kasus berbasise-learning ini untuk selanjutnya bertumpu pada tampilan antarmuka serta pilihan skenario kasusyang dapat mengakomodasi kebutuhan mahasiswa dalam pembelajaran kasus. Kesimpulan:Pengembangan rancangan prototype pembelajaran kasus berbasis e-learning ini ditujukan sebagaipelengkap pembelajaran konvensional yang berfokus pada aspek pengetahuan mahasiswa dalammenerapkan penggunaan NANDA-I, NIC, NOC melalui latihan-latihan kasus yang diberikan.Kata Kunci: E-learning, asuhan keperawatan maternitas, NANDA-I, NIC, NOCTHE DEVELOPMENT OF E-LEARNING-BASED CASE LEARNING FOR MATERNITY NURSINGCARE USING NANDA-I, NIC, NOC TAXONOMY APPROACHABSTRACTObjective: To describe the development of E-learning-based case learning using the guidelinestaken from NANDA-I, NIC, NOC for maternity nursing care. Methods: This study was conductedusing a qualitative method with decsriptive analytical approach. Samples were taken usingpurposive sampling technique. Participants consisted of two lectures and fi ve students. The studywas conducted in November 2015 for three weeks. Data were analyzed qualitatively and presenteddescriptively. Results: The results of the study consisted of four major themes: (1) problems inmaternity nursing care learning process by using the guidelines taken from NANDA-I, NIC, NOCfrom students, lecturers and learning systems. The problems were that not all teachings of nursingcare used NANDA-I, NIC, NOC, students did not understand using NANDA-I, NIC, NOC if case weregiven, books related to NANDA-I, NIC, NOC in the library were limited and incomplete, (2) Objectivesof maternity nursing care learning by using the guidelines taken from NANDA-I, NIC and NOC,(3) Objectives of the development of E-learning prototype, (4) Specifi cation of E-learning prototyperequired by users such as user-friendly and interesting interface, contents consisting of caseexercises, cases including gestational DM, pre-eclampsia, eclampsia, HELLP syndrome, placentasolution, antepartum hemorrhage and reproduction system problems. Security system appliedpassword and account accompanied with a fl exible and compatible download page. Discussion:Problems arose in nursing care learning which employed NANDA-I, NIC, NOC because thelearning process focused on collecting knowledge without taking skills in performing nursing careinto consideration. The development of E-learning-based case learning prototype was focused oninterface and the options of case scenario that can accommodate students’ requirements in the caselearning. Conclusion: E-learning-based case learning prototype was developed as a complementarymedia for the conventional learning which focused on the cognitive aspects of students in employingNANDA-I, NIC, NOC through case exercises provided.Keywords: E-learning, Maternity Nursing Care, NANDA-I, NIC, NOC

Life experience of mother with preeclampsy pregnancy history in YogyakartaPurposeThe purpose of this paper was to determine the life experience of women with pre-eclampsia pregnancy history.MethodsThis research was a phenomenology study involving 7 postpartum mothers with history of pregnancy and delivery of preeclampsia in Dr. Sardjito Hospital conducted from June to September 2015.ResultsBased on this research, there were 4 themes which were found: 1) the symptoms that are felt to affect the psychological, spiritual, physical and behavior of the mother; 2) Awareness raising; 3) Source of power from family and community; and 4) Anxiety and fear have an impact on the mother's perception during the process of referral and childbirth.ConclusionThis study contributes to the knowledge that maternal conditions with preeclampsia pregnancy and childbirth in Yogyakarta still require the attention of health workers, especially from the aspects of alertness to perceived symptoms and the importance of information about pregnancy preeclampsia.

Abstract

Adolescent boys' knowledge and health education needs about sexually transmitted infections (STIs) Purpose: To determine the knowledge of adolescent boys and the need for health education about STIs in Yogyakarta Special Region.Methods: This research is a descriptive study with cross sectional design, with a total sample of 418 male students of class XII high school. This research was conducted in the Yogyakarta Special Region High School in September-November 2017. This study used univariate, bivariate and multivariate analysis. Results: The results showed that the average age of respondents 17 years as much as 61.5%, the most sources of information were lessons at school by 67.2%, 90.7% of students living with parents and the results of knowledge of adolescent boys in DIY bad by 52.4% and good by 47.6%. External variables related to knowledge are sources of information with p = 0.001 (p <0.05), the material that young boys want to know about health education about STIs is prevention done by 81.8% and 54.8% choosing media video as the desired method.Conclusions: The role of the world of education has a very big influence in providing information about STI to boys, the collaboration between the government and related parties will increase the knowledge of boys. One effort that can be done is by educating schools, especially on preventing STIs and using audiovisual media, besides that the role of the community and family, especially parents, will greatly influence the knowledge of adolescent boys.
