Abstract

The evaluation of IUD contraceptive services is still lack of quality until now. Many IUD  acceptors choose to stop use contraception because they don't accept side effects such as bleeding, and fear of interfering with sexual intercourse. Adequate information from professional health personnel, in the form of counseling, is a very important step in helping women choose the best contraceptive method and according to their needs. The purpose of this study was to determine the relationship between education level and work period of midwives to the knowledge of midwives about IUD counseling. The research method uses a quantitative approach with a crossectional design. The number of respondents was 124 midwives Public Health Center in Bandung. The sampling technique uses consecutive sampling. Measurement of IUD counseling knowledge using a questionnaire that has been tested for validity and reliability using the Rasch Model with alpha chron-bach 0.87 (Good). Data were analyzed using SPSS 20 chi-square. The results of the study found that almost all of the 96% of respondents were lack of knowledge about counseling IUDs. The chi-square analysis results did not have a significant relationship between the level of education and knowledge of IUD counseling p = 0.548 (> 0.05). There is no significant relationship between the period of work with knowledge of IUD counseling p = 0.081 (> 0.05). Communication and counseling have been included in the Midwifery Diploma III education curriculum, but the level of education does not have a significant relationship to the knowledge of midwives about IUD counseling. This shows that the learning process in educational institutions has not been able to produce midwives who have knowledge of IUD counseling as expected.

Abstract

Objective: To determine the risk factors that played roles in the incidence of multidrug-resistant tuberculosis (MDR-TB) in such patients. Multidrug-Resistant Tuberculosis is a form of tuberculosis caused by Mycobacterium tuberculosis that is resistant to at least isoniazid and rifampicin.Methods: This was a case control study to compare MDR-TB to non-MDR-TB pulmonary tuberculosis outpatients in Dr. Hasan Sadikin General Hospital, Bandung on August–September 2014. Fifty MDR-TB outpatients were included as the cases and 50 non-MDR-TB outpatients as controls. Data was collected by questionnaires and patient’s registration forms. Bivariate and multivariate analyses were performed using chi-square test and multiple logistic regression test, with p<0.05 considered significant. Results: From bivariate analysis, number of previous tuberculosis treatments, regularity of previous treatment, and burden of cost were significant risk factors for developing MDR-TB (p<0.05); while from multivariate analysis, number of previous TB treatments was the only risk factor that played a significant role in the incidence of MDR-TB (OR 24.128 95% CI 6.771-85,976). Conclusions: Patients and medication factors are risk factors that play roles in the incidence of MDR-TB. The significant risk factor is the number of previous TB treatment. Abstract

Masyarakat memiliki hak untuk mendapatkan pelayanan kesehatan yang bermutu dan terjangkau. Sejak awal tahun 2014, pemerintah berupaya meningkatkan akses pelayanan kesehatan melalui program Jaminan Kesehatan Nasional. Pelayanan kesehatan tersebut tentunya harus tetap berkualitas. Peneliti bertujuan merumuskan konsep kualitas pelayanan kesehatan berdasar atas ekspektasi pasien. Desain penelitian ini adalah kualitatif dengan paradigma konstruktivisme. Partisipan terdiri atas 17 pasien rawat jalan peserta Jaminan Kesehatan Nasional dan 7 petugas kesehatan Rumah Sakit Umum Daerah Al-Ihsan dan Soreang Kabupaten Bandung, Pengumpulan data dilakukan dengan cara wawancara mendalam selama bulan Agustus?November 2016. Pelayanan kesehatan yang berkualitas terdiri atas 11 dimensi. Dimensi sarana prasarana mengutamakan kecukupan kapasitas fasilitas. Dimensi karyawan mengutamakan kesesuaian jumlah kapasitas tenaga kerja. Dimensi pelayanan medis mengutamakan komunikasi. Dimensi pelayanan administrasi mengutamakan sistem antrian yang tertib. Dimensi keamanan pelayanan mengutamakan minimalisasi risiko bahaya. Dimensi kepercayaan menunjukkan loyalitas. Dimensi akses mengutamakan kemudahan menjangkau rumah sakit. Dimensi kesetaraan merupakan perlakuan yang sama antara pasien peserta JKN dan non-JKN. Dimensi transparansi informasi merupakan penyajian informasi yang jelas. Dimensi iur biaya mengungkapkan tidak ada penambahan biaya dalam tindakan medis maupun pengobatan. Dimensi kualitas antar bagian merupakan pelayanan yang sama di setiap titik bagian pelayanan. Pelayanan kesehatan yang berkualitas berdasar atas harapan pasien meliputi dimensi sarana prasarana, karyawan, pelayanan medis, pelayanan administrasi, keamanan pelayanan, kepercayaan terhadap rumah sakit, akses, kesetaraan, transparansi informasi, iur bayar, dan kualitas antarbagian. [MKB. 2017;49(2):102?9] Kata kunci: Ekspektasi pasien, Jaminan Kesehatan Nasional, kualitas pelayanan kesehatan  Health Service Quality Concept based on Expectation of the National Health Insurance Participants Community has the right to receive affordable and qualified health care. Since the early 2014, the government has attempted to increase health care access through the implementation of the National Health Insurance (Jaminan Kesehatan Nasional, JKN) scheme that still requires quality health care. The aim of this study was to formulate the concept of health care quality based on patients? expection. This was a qualitative study using constructivism paradigm on 17 JKN-member outpatients and 7 health care workers of Al-Ihsan General Public Hospital and Soreang Public District Hospital, Bandung District. Data were collected through in-depth interviews during the period of August?November 2016. Quality health care consists of 11 dimensions: facility and infrastructure dimension that prioritizes on adequacy of facility capacity; employee dimension that prioritizes on the number and capacity of human resource; medical service dimension that prioritizes on communication; administrative service dimension that prioritizes on orderly queuing system; service safety dimension that prioritizes on minimizing hazard risk; trust dimension that shows loyalty; access dimension that prioritizes on easy access to hospital; equality dimension that emphasizes same treatment for JKN and non-JKN patients; information transparency that prioritizes on clear information provision; cost sharing dimension that underlines no additioanl fee for medical actions and treatments; and inter-department quality dimension that includes same services in each service point. It is revealed that quality health care based on patients? expectation includes facility and infrastrucutre, employee, medical service, administrative service, service security, trust towards hospital, access, equality, information transparency, cost-sharing, and inter-department quality. Abstract

Urolitiasis merupakan kondisi terbentuknya berbagai jenis batuan dalam saluran urinaria. Identifikasi jenis batuan yang dilakukan saat mendiagnosis urolitiasis perlu dilakukan untuk mencegah terjadinya penyakit yang berulang. Penelitian ini bertujuan untuk mendapatkan gambaran mengenai prosedur diagnosis kasus urolitiasis pada kucing dan anjing dalam praktek dokter hewan di Kota Bandung dan kasus urolitiasis berulang. Sampel responden diambil menggunakan purposive sampling dan didapatkan sembilan responden dokter hewan. Variabel yang diamati adalah prosedur diagnosis dan kasus urolitiasis berulang. Pengambilan data dilakukan menggunakan kuesioner berupa google formulir kemudian diolah secara deskriptif. Variabel prosedur diagnosis dikategorikan menjadi prosedur baik dan kurang baik, sedangkan variabel keberulangan dikategorikan menjadi berulang dan tidak berulang. Hasil didapatkan bahwa enam dari sembilan responden memiliki prosedur diagnosis baik. Seluruh responden dengan prosedur diagnosis baik maupun kurang baik mendapati dan menangani kasus urolitiasis berulang. Dalam kasus urolitiasis dokter hewan praktek melakukan prosedur diagnosis menggunakan berbagai alat penunjang diagnosis. Kasus urolitiasis yang berulang terjadi dengan interval yang berbeda, prosedur diagnosis bukan satu-satunya faktor yang bisa mendukung terjadinya keberulangan, faktor lain seperti pakan sangat mendukung terbentuk urolit kembali.

Abstract

BACKGROUND Basic knowledge of sexual and reproductive health is important to be introduced during the premarital period, to reduce high-risk sexual behavior among adolescents, as well as to increase the correct attitude in their future marriage. This study explored the knowledge of HIV/AIDS, sexually transmitted diseases (STDs) and anemia among unmarried adolescents. METHODS This was a cross-sectional analytical study on secondary data from the Indonesian Demographic and Health Population Survey 2017, initially conducted by the National Population and Family Planning Board (BKKBN). Knowledge of HIV/AIDS, STDs and anemia among unmarried adolescents aged 15-24 years in West Java Province and Jakarta, the capital city of Indonesia, was compared and analyzed using the chi-square test. RESULTS In total, 5,389 weighted data were retrieved. In general, adolescents in Jakarta had better knowledge of all diseases compared to those in West Java (p<0.001). Knowledge of anemia and STDs was poor compared to that of HIV/AIDS which was moderate to good (p<0.001). Interestingly, although the adolescents had a middle-low wealth index (43%), the majority (>90%) had their own mobile phone with almost daily internet access, however most of their information sources on health education were from their school period. CONCLUSION Premarital education in schools is imperative to enhance the knowledge of STDs and anemia. Moreover, poor knowledge of anemia is a challenge for introducing the topic of thalassemia major, an inherited autosomal recessive type of anemia due to hemoglobinopathy. Since Indonesia is harboring 6-10% of thalassemia carriers, integrative premarital education is needed for better family planning.

Abstract

Introduction: Iron deficiency anemia in developing countries is common. However, knowledge on the etiology of anemia among young adults needs to be further explored, including hemoglobinopathy, since Indonesia is located in the thalassemia belt, harboring 6-10% thalassemia carriers. The study aimed to explore the knowledge about anemia among young adults in Jakarta and the province of West Java. Methods: A descriptive analytical study design using secondary data from the National Demographic and Health Survey 2017 was employed. A questionnaire was distributed to unmarried young adults (n=5389) aged 15-24, including questions about anemia. Knowledge was further compared between places, genders, and age groups. Result: Only 70% of young adults have heard about anemia. However, the knowledge was poor. The definition of anemia was lack of blood (48%), and only 5% stated it was due to iron deficiency. The etiology of anemia was lack of vegetables (21%) and meat (18%) consumption. As a treatment, only 9% mentioned the iron tablets. None mentioned thalassemia carrier as an alternative cause of anemia. Conclusion: Young adults in Jakarta and West Java are poorly informed about anemia. Socialization to increase knowledge about anemia is imperative. Thalassemia carriers need to be introduced among unmarried young adults for better family planning in the future.

Abstract

Background: Diabetes Self-Management Education (DSME) is a structured self-care program for managing type 2 diabetes mellitus (T2DM), including treatment adherence and complications prevention. This study aimed to evaluate the effect of DSME on diabetes-related knowledge and hemoglobin A1c (HbA1C) levels among patients with T2DM.  Methods: A quasi-experimental pretest-posttest non-equivalent control group design was conducted from August 2023 to January 2024, involving 65 patients with T2DM in occupational health care in Cikarang and Tangerang, Indonesia.  The DSME program included five key components: (1) diabetes mellitus education, (2) medical nutrition therapy, (3) physical exercise, (4) pharmacological intervention; and (5) blood glucose self-monitoring. The intervention group received DSME in six sessions over three weeks (45–60 minutes each), while the control group only reviewed 40 educational slides for 30 minutes. Knowledge was assessed using the Diabetes Knowledge Questionnaire (DKQ-24), and HbA1c levels were measured twice, six months apart. Data were analyzed using independent t-tests, dependent t-tests, and ANOVA. Results: The intervention group (n=26) and control group (n=39) had similar baseline characteristics. A significant improvement in diabetes knowledge was observed in both groups. Interestingly, only the intervention group showed a notable reduction in HbA1c levels (−1.9%), from 8.5% to 6.6%. Post-intervention HbA1c levels differed significantly between groups (p<0.05), with the control group remaining at 8.2%.Conclusions: The DSME program effectively improves knowledge and reduces HbA1c levels in patients with T2DM. It is recommended for integration into occupational health care settings to promote healthy lifestyles and enhance diabetes management.
