Abstract

Background: Patients with cardiovascular diseases (CVD) are vulnerable to experiencing drug-drug interactions (DDIs). DDIs are a concern among patients receiving multiple drug regimens but they are also an avoidable cause of adverse drug reactions. The study of potential DDIs (pDDIs) would make it easier for the healthcare provider to deliver better patient care and mitigate pDDIs incidence. Objective: This study aimed to assess the frequency, severity level and risk factors associated with pDDIs among medications used to treat hypertensive ambulatory patients in Universitas Airlangga hospital. Methods: A retrospective observational study was carried out from electronic prescriptions received by hypertensive patients in March 2021. Data collection includes demographic data, the profile of antihypertensive drug use, and pDDIs. pDDIs were identified by severity using Lexicomp Drug Interaction Checker (Application). Univariate logistic regression analysis was used to find associated factors of major pDDIs. A p-value less than 0.05 (≤ 0.05) was considered statistically significant. Results: From 704 patients, 53.98% women and 46.02% men, 89.06% (n = 627) patients had minor to major pDDIs; 1354 pDDIs were identified, 89.4% (n = 1,210) were moderate and 9.8% (n = 133) were major class. Multiple antihypertensive drug regimens had significance associated with the major pDDIs occurrence. Conclusion: We found a high prevalence of pDDIs among hypertensive patients. The majority of pDDIs were of moderate severity. Multiple antihypertensive drug regimens were associated factors in the presence of major pDDIs.

Abstract

Background: Isoprinosine is an immunomodulator that is now being used to treat Covid-19 patients. Objectives: To evaluate Isoprinosine with Favipiravir or Oseltamivir in moderate Covid-19. Methods: In a retrospective observational analysis, in-hospital moderate Covid-19 patients treated between June 2020 and June 2021 were included. Results: Inclusion criteria for 364 patients were met, with 135 receiving Favipiravir-Isoprinosine (Group 1) and 229 receiving Oseltamivir-Isoprinosine (Group 2). In group 1, the majority of patients (58.50%) were female (35.60%), had no comorbidities (71.60%), were discharged with a positive PCR (74.80%), did not require a breathing apparatus (99.26%), had leukocyte levels between 4,5-11,0 (82.22%), lymphocyte levels between 25-33 (34.07%), and were discharged with no ground-glass opacity (34.07%) (54.10%), LOS was 9-13 days (50.37%), while the mortality rate was 0.70%. In group 2, the majority of patients were male (54.10%), with the highest age range being 42-56 years (35.80%), without comorbidities (69.0%), discharged with a positive PCR (72.50 %), and without the need for a breathing apparatus (99.13%), with leukocyte levels ranging from 4.5 – 11.0 (81.22 %), with lymphocyte levels ranging from 25.0 – 33.0 (26.20 %), and were discharged with no ground-glass opacity (49.34 %), LOS was 9 - 13 days (34.06 %), and the mortality rate was 0.87%.Conclusion: In this trial, it was determined that combining isoprinosine with antivirals favipiravir or Oseltamivir could produce significant clinical improvement.

Abstract

Background: Cardiovascular disease is one of the chronic diseases with an increasing prevalence from year to year. The results of Indonesia's basic health research (RISKESDAS 2018) showed that it was experienced by 2.9% of the population. Ensuring the continuity of drug availability is very important in pharmacy services for this patient group. The history of drug use is considered in the planning dan procurement of drug products. Objective: This study aims to analyze the drug use pattern and cost of drugs for cardiology outpatients in the National Health Insurance scheme (JKN). Methods: A retrospective observational study was conducted using the prescription of cardiology outpatients JKN for March-May 2021. The ABC method carried out the analysis of drug use patterns and cost. Results: From 2,986 prescriptions for cardiovascular disease, there were 94 types of drugs from 37 therapeutic classes. In the ABC analysis of drug use for class A, 68.82% contained seven types of drugs, class B, 20.55% contained nine types of drugs, and class C, 10.63% with 78 types of drugs. The results of ABC analysis for investment value (cost) of drugs class A 66.96% contained two types of drugs, class B 19.81% with four types of drugs, and class C 14.23% with 88 types of drugs. Conclusion: ABC analysis of the drug use pattern and cost/investment showed different patterns which had value to consider in procurement planning to maintain service continuity.

Abstract

Background: Pharmacy students consist of undergraduate and professional pharmacy students. They are candidates for future pharmacist health workers who require practical experience. Offline learning with hands-on practice methods in health facilities supports their professional skills. Objective: This study aimed to determine pharmacy students' knowledge and attitude toward implementing offline learning methods during the new normal era. Methods: This study involved pharmacy students from Indonesia who were asked to participate in an e-questionnaire about the vaccination program, COVID-19 health protocols, pharmacist competence, and attitude toward implementing offline learning. The students' scores were based on their knowledge and attitude. Statistical analysis was performed to compare the scores between the two groups, and a correlation test was conducted to assess the relationship between the students' knowledge and attitudes. Results: A total of 652 pharmacy students were divided into two groups, undergraduate and professional pharmacy students, in a 3:1 ratio. About 74.6% of undergraduate students and 78.5% of professional pharmacy students had good and moderate knowledge. The level of knowledge was not different between the two groups (p=0.602; p>α). Professional pharmacy students were more ready to engage in offline learning compared to undergraduate students (p=0.001; p<α). However, there was a relatively low correlation between knowledge and attitudes, with r = 0.079 (p=0.043; p<α). Conclusion: Professional pharmacy students are more ready for offline learning methods than undergraduate students, particularly in hands-on field practice.

Abstract

Background: Pharmacovigilance is administered to several pharmacological classes of drugs worldwide. However, there are still insufficient data regarding the prevalence and general characteristics of drug reactions, especially in developing countries. Objective:  This study aimed to determine the prevalence and characteristics of ADRs, including the pharmacological class involved, and report and classify the clinical manifestations associated with ADRs. Methods: This retrospective study was based on patient ADR reports during observation. Prevalence, patient demographics, and other data were evaluated using descriptive statistics. Results: Of 773 reports that met the inclusion criteria, most were doctors (80.6%), followed by pharmacists (18.7%). Of the total cases, 430 (55.6%) occurred in the women. Most suspected ADRs occurred in the 19-60 years age group (583; 75.4%). The highest incidence of ADR was observed in patients using antineoplastic agents (19.5%), systemic antibacterials (16.4%), or antihypertensives (12.5%). The majority of clinical manifestations were gastrointestinal disorders (41.7%), and approximately 309 (40%) ADR cases continued with antagonists/antidotes. Approximately 62% of the patients who experienced ADRs recovered. Conclusion: Antineoplastic, systemic antibacterial, and antihypertensive drugs appeared to be the most common drugs used for suspected ADR cases in this hospital. ADR reporting has been running well, but not all healthcare workers have participated actively. Hopefully, the results of this research will contribute to the upcoming strategies for pharmacovigilance activities in this hospital and other healthcare facilities to improve the quality and quantity of ADR reporting and increase the safety of medication usage.

Abstract

Background: Application of the ABC-VEN method in evaluating drug planning can increase efficiency and ensure optimal medicine availability and stable access to medications. Objective: To analyze ABC-VEN combinations to examine the profile of medicine use in the internal medicine department. Methods: This was an observational study with retrospective prescription data from outpatients in the Internal Medicine Department from January to March 2020. Collected data included the type, number of medicines, and medicine prices. Patients undergoing chemotherapy and retroviral therapy for HIV were excluded from the study. Subsequently, an ABC-VEN analysis was performed. Results: Of 4,242 prescription samples, 188 types of medicines were used. Based on the drug use evaluation with ABC analysis, category A contained 23 items (12.17%), category B contained 35 items (18.52%), and category C contained 130 items (69.31%). The ABC analysis for investment value found that category A contained eight items (4.23%), category B contained 22 items (11.64%), and 158 items (84.13%). Based on the VEN analysis, Group V had six medicine items, Group E had 152 medicine items, and Group N had 30 medicine items. The ABC-VEN investigation showed that there were eight, 151, and 29 items of medicines in Categories I, I, and III, respectively. Conclusion: Although there are medicines that are highly used, their investment value is quite low. The use of the ABC-VEN method to evaluate medicine use is crucial for organizing and controlling the medicine supply.

Abstract

Background: Patients with atrial fibrillation, venous thromboembolism, valvular disease, cardioembolic stroke, and acute myocardial infarction are prescribed the oral anticoagulant warfarin to treat thrombi. To guarantee its efficacy and safety, anticoagulants must be closely monitored. Furthermore, warfarin has a narrow therapeutic index, which increases the risk of side effects, particularly in the early stages of treatment. Objective: This study aimed to analyze the incidence of warfarin side effects in outpatients at Dr. Soetomo General Hospital. Methods: A retrospective cross-sectional design was adopted for outpatients at the Cardiology Department of Dr. Soetomo General Hospital from March to May 2023. Data were collected on the incidence of side effects in outpatients who received warfarin for a minimum of one month through an interview process. Other data, including age, sex, duration of warfarin therapy, comorbidities, and INR at the last scan, were extracted from medical records. The Chi-Square test was used to examine the data. Results: The results showed that 88 patients (42.7%) had side effects of bleeding (126 events). These included gum bleeding (22.3%), hematoma (20.4%), melena (7.7%), menometrorrhagia (2.4%), epistaxis (1.9%), hematuria (1.5%), hematemesis (1.0%), hemoptysis (1.0%), spontaneous venous bleeding  (1.0%), hematochezia (0.5%), hemostasis during blood sampling  (0.5%), tongue bleeding  (0.5%), and subconjunctival bleeding  (0.5%). Conclusion: The incidence of side effects during warfarin treatment was high, accounting for approximately 42.7% of cases. Furthermore, one patient experienced more than one side effect.
