Abstract

Basal cell carcinoma (BC) is a malignant neoplasm of non-katinized cells in the basal layer of the epidermis, is locally invasive, aggressive, destructive, and rarely metastasizes. KSB is more common in the elderly. The etiopathogenesis associated with KSB is genetic, environmental, and most commonly ultraviolet light exposure. Clinically, there are five types of SCI, namely nodular, superficial, morpheaform, pigmented, and Pinkus fibroepithelioma. Early detection of skin cancer can be done by skin self-examination (SAKURI). The purpose of this study was to determine clinical signs as predictors of basal cell carcinoma in traditional rubber farmers. Through 7 steps of occupational diagnosis, it can be concluded that the diagnosis of Basal Cell Carcinoma in patients is related to their work as traditional rubber farmers. Appropriate definitive and non-medicamentous management and patient compliance can restore the patient's health condition with a good prognosis.

Abstract

Arteriovenous fistula (AVF) merupakan akses vaskular pilihan utama pada pasien penyakit ginjal kronik (PGK) stadium akhir yang menjalani hemodialisis. Namun, kegagalan maturasi AVF dilaporkan terjadi pada 20–60% kasus dan menjadi penyebab utama morbiditas serta perlunya intervensi tambahan. Hingga saat ini, belum terdapat model prediksi maturasi AVF yang dikembangkan khusus untuk populasi Indonesia. Penelitian ini bertujuan untuk mengembangkan model prediksi berbasis skor risiko untuk memperkirakan keberhasilan maturasi AVF pada pasien PGK stadium akhir di RSUPN Dr. Cipto Mangunkusumo. Penelitian ini merupakan studi kohort retrospektif yang melibatkan 87 pasien yang menjalani pembuatan AVF periode Januari 2024 hingga Januari 2025. Data diperoleh dari rekam medis meliputi karakteristik demografi, komorbiditas, lokasi AVF, diameter anastomosis, riwayat akses vaskular, dan luaran maturasi. Analisis bivariat menggunakan uji Mann–Whitney dan kai kuadrat, dilanjutkan analisis multivariat dengan regresi logistik biner metode backward stepwise. Model prediksi dievaluasi menggunakan kurva ROC dan uji Hosmer–Lemeshow. Sebagian besar pasien berjenis kelamin perempuan (54%), dengan rerata usia 49,8 tahun (SB 12,3) dan rerata BMI 25,3 kg/m² (SB 4,5). Lokasi AVF proksimal (brachiocephalic) lebih banyak dilakukan (62,1%) dibandingkan distal (radiocephalic) (37,9%). Kombinasi diabetes melitus dan hipertensi merupakan komorbiditas tersering (49,4%). Keberhasilan maturasi dalam waktu ?2 bulan mencapai 77%. Analisis multivariat menunjukkan bahwa hanya indeks massa tubuh (BMI) yang berhubungan signifikan dengan maturasi AVF (p=0,019). Model regresi logistik yang diperoleh adalah logit(p) = 0,142 × BMI – 4,898 dengan nilai Nagelkerke R² sebesar 0,169. Probabilitas maturasi meningkat seiring peningkatan BMI, misalnya dari 20,6% pada BMI 25 menjadi 34,5% pada BMI 30.

Abstract

Endovenous Microwave Ablation (EVMA) is a minimally invasive thermal technique for treating venous insufficiency, known for its low complication rates and fast recovery. While compression stockings are commonly recommended after other thermal ablation procedures such as EVLA and RFA, their effectiveness following EVMA remains unclear and lacks standardized guidelines. This study aimed to compare great saphenous vein (GSV) occlusion success and changes in Venous Clinical Severity Score (VCSS) between patients who used compression stockings and those who did not after EVMA. This prospective cohort study included patients with chronic venous insufficiency (CVI) who underwent EVMA at Cipto Mangunkusumo Hospital between September and November 2025. Data analysis was conducted using Fisher’s exact test and the Mann–Whitney test. The results showed that all 40 patients achieved 100% GSV occlusion one week after EVMA, with no significant difference between the two groups (p=1.00). Preoperative VCSS scores were also comparable between the no-stocking group (10.40 ± 1.85) and the stocking group (11.25 ± 2.88) (p=0.274). Postoperatively, both groups experienced a decrease in VCSS scores; however, the reduction was significantly greater in the stocking group (median 6 vs. 8; p=0.015). The median decrease in VCSS was also higher in the stocking group (4 [2–10]) compared to the no-stocking group (2.50 [0–7]; p=0.012). Component analysis revealed that compression stockings significantly improved only the pain parameter (p=0.004). In conclusion, compression stockings do not affect GSV occlusion rates after EVMA but provide better clinical improvement, particularly in reducing postoperative pain.

Abstract

Introduction: The 2019 general election in Indonesia was the first general election to be held simultaneously and election officers (KPPS) had to complete the vote count on the same day with additional ballot papers. This situation caused high mortality and morbidity among KPPS officers due to stress and fatigue caused by job overload. This study was aimed to explore the factors related stress response in 2019 election KPPS officersMethods: A cross-sectional study design was conducted to the issue under the study included 80 data of KPPS officers at Polling Station (TPS) in Jakarta, Banten, and Yogyakarta. Occupational stressor and stress response was assesed with NBJSQ bahasa Indonesia. Multiple logistic regression models were used to explore factors associated with stress response.Results: The most perceived occupational stressor experienced by the 2019 General Election KPPS officers in this study were quantitative job overload (47,5%).The stress response that occurred in the 2019 General Election KPPS officers in this study was fatigue (17.5%). There was no relationship between occupational stressor and individual factors with stress response (p>0.05)Conclusion: This study showed that quantitative job overload was a major occupational stressor among KPPS officers in general election 2019. The stressor can trigger the incidence of heart attacks in predisposed individuals.

Abstract

Introduction: Among the many risk factors for stress in the workplace, bullying is a factor that is rarely discussed. As many as 19% of people in America experience bullying at work, followed by other European countries. Bullying is a strong link to worker stress, especially in blue-collar workers. Gender differences, especially in women, also have an important prevalence in workplace bullying. In Indonesia itself, there is no data regarding bullying in the workplace, especially among women and blue-collar workers. Objective: This study aims to find the relationship between bullying and stress in the workplace among female blue-collar workers. Method: This research is a cross-sectional analytical study using a negative act questionnaire-revised (NAQ-R) questionnaire on work stress in female blue collar workers in garment factories, in Bekasi city, Indonesia. Job stress assessment uses the Perceived Stress Scale-10 (PSS-10) questionnaire. Other variables assessed are age, salary, marital status, and subjective questions related to workload and work communication. Results: A total of 379 female blue collar workers in garment factories, in the city of Bekasi, Indonesia took part in this research. Workplace bullying is associated with job stress in female blue-collar workers. Workers who are often bullied tend to have medium-high levels of work stress. Bullying is the type of bullying with the highest scale. Conclusion: Workplace bullying is the factor that has the most influence on work stress in female blue-collar workers. Then, high workloads, poor communication, and lower salaries are related to bullying and stress in the workplace.

Abstract

Background: Work-related Hepatitis B is a significant health issue among healthcare workers, especially those at high risk of needle stick injuries (NSI). Needle stick injuries can act as a transmission route for the Hepatitis B virus, which, if not adequately managed, may lead to serious complications. Evaluating the Hospital Occupational Health and Safety System (OHSS) is crucial to prevent similar incidents. This study aims to evaluate the OHSS system in a hospital for preventing workplace injuries and to analyze a case of Hepatitis B resulting from a needle stick injury in a sanitarian.Case presentation: A 37-year-old male sanitarian was diagnosed with Hepatitis B following a Medical Check-Up that revealed reactive HBsAg without clinical symptoms. The incident occurred in April 2021 when the patient was pricked by a used needle while handling medical waste without using appropriate personal protective equipment (PPE) and without reporting the incident to the hospital’s occupational health unit. The patient had no history of Hepatitis B vaccination and was previously declared HBsAg negative during a 2020 medical check-up. Based on the analysis using the Seven Steps of Occupational Diagnosis, the incident was categorized as an Occupational Disease due to the failure of the OHSS system in preventing infections.Methods: This case report draws on clinical observations and a retrospective review of patient records, using the Seven Steps of Occupational Diagnosis framework to determine causal factors. Workplace safety protocols, including PPE availability and incident reporting processes, were assessed. The patient provided informed consent for the use of their data, with all identifying details removed to maintain confidentiality.Conclusion: Based on the evaluation using the managerial approach of OHSS, the Hepatitis B case in this sanitarian can be categorized as a work-related disease. The needle stick injury leading to Hepatitis B virus transmission highlights deficiencies in implementing workplace safety systems in the hospital. This study emphasizes the importance of strengthening the OHSS system through regular training, adequate PPE provision, improved Standard Operating Procedures (SOPs), and continuous monitoring to prevent similar incidents.

Abstract

Background: Low back pain (LBP) is pain that is felt in the lower back area, namely between the corners of the lowest ribs and the lower buttocks (lumbar or lumbo-sacral area) and can be local pain or radiate to the upper thighs/legs or both. Low back pain is a 10% debilitating condition that results in a change in life expectancy due to disability. Low back pain is not a malignancy and is a self-limiting condition, but patients always seek treatment to reduce the severity of their symptoms. Chronic Low Back Pain can alter lifestyle and increase the rate of prolonged absenteeism at work resulting in physical limitations. Discomfort due to chronic low back pain is a sign that requires immediate treatment. Methods: The purpose of this evidence-based case report is to get the right answer about McKenzie exercise in reducing low back pain in office workers. Article searches were conducted through PubMed, Embase and Cochrane Library. Inclusion criteria were Systematic Review, Meta-Analysis, Randomized Control Trial (RCT), Adult or Productive Age, Non Specific Chronic Low Back Pain and McKenzie Exercise. Then critically reviewed using Oxford CEBM criteria for intervention-therapy studies Results: 1 research article was obtained that met the inclusion criteria with the type of Randomized Control Trial (RCT). Conclusion:. The critical review showed that McKenzie exercise can reduce complaints of low back pain at 5 weeks of treatment, but this exercise did not show a clinical effect in the form of a reduction in complaints of low back pain after being implemented for 3,6 and 12 months. Abstract

The widespread use of Visual Display Terminals (VDTs) is associated with visual and musculoskeletal complaints, particularly Asthenopia, which may reduce productivity. A 32-year-old female financial worker presented with eye discomfort, including sore eyes, blurred vision, dizziness, and twitching on her left temple, particularly during extended computer use. The symptoms were relieved with rest. Most VDTs now use Light Emitting Diodes, which emit more blue light than LCDs and may cause eye fatigue and disrupt sleep. Currently, blue light filter lenses are available in the market, however, further study of existing scientific publications is necessary to ensure the effectiveness of blue light filter lenses, which are claimed to reduce asthenopia symptoms. This Evidence-based case report (EBCR) questions whether blue-light filter lens glasses alleviate symptoms of Asthenopia in workers A literature search in PubMed, Cochrane, and Embase identified Systematic Reviews, Meta-Analyses, and Randomized Controlled Trials (RCTs) evaluating blue light filter lenses in VDT users. These studies assessed objective measures (Critical Flicker Fusion Frequency—CFF) and subjective symptoms via questionnaires. The research papers were critically reviewed using the Oxford CEBM criteria for therapy (intervention) studies. Four Randomized Controlled Trial research articles met our criteria. However, the results did not provide conclusive evidence. There was no significant improvement in Critical Fusion Frequency (CFF), nor a reduction in subjective symptoms based on questionnaires. All studies varied in methodology, filter specifications, outcome tools, and subject characteristics. Therefore, blue light filter lenses cannot yet be clinically recommended for reducing asthenopia symptoms in VDT users. Further research is needed with a stronger design, larger sample size, and clearer standards and operational definitions.

Abstract

Objective: This study aims to examine the effect of computer-mediated communication (CMC) on occupational positive mental health (OPMH) mediated by work-family enrichment (WFE) in workers in remote areas using the CMC, OPMH-40, and WFE questionnaires. Method: The latent variables in this study are CMC, OPMH, and WFE. The observed variables include all items from CMC, OPMH, and WFE. The sample technique used was total sampling, revealing that 349 workers in the project department of an oil and gas company located in the West Papua Province of Indonesia expressed their willingness to participate as research participants. To test research hypotheses using structural equation modeling (SEM) analysis by testing measurement and structural models. Results: The results showed that the effect of WFE as a mediator strengthened the influence of CMC on OPMH in comparison to the direct effect. Conclusion: This study explains the mechanism by which WFE, acting as a partial mediator, increases the effect of CMC on OPMH. The communication between workers and their families will increase their mental health as it serves as a means of support for workers who are geographically separated from their family. Company management needs to support employees in implementing CMC with their families to increase OPMH and WFE, as well as reduce the negative influence of work-to-family conflict, so that work productivity will increase.
