Abstract

Background: Squamous cell carcinoma of oral cavity is a malignant neoplasms derived from epithelia. The malignant neoplasms are cells that have changed their structure and function, and their number becomes increasing abnormally, invasive, and metastatic. Carcinoma can be caused by the resistance of malignant cell apoptosis. Bcl-2 is a proto-oncogene of Bcl family that inhibits the process of cell apoptosis and suppresses Bax protein (pro-apoptotic). The management efforts of cancer diseases, however, still have many obstacles. Thus, the researcher was triggered to explore more herbal plants, namely pomegranate. Pomegranate as a medicinal plant is accessible and cheap. Ellagic acid (EA) is a single active compound derived from whole pomegranate fruit extract (PGL), which has anti-cancer activity as in vitro, but EA is low concentration in plasma, low water solubility, and insoluble in intestinal. These facts prompted the researcher to compare between pomegranate extract, which consists of several active compounds, and that, which only consists of ellagic acid. Thus, this research is expected to know how some active compounds can work synergistically in the PGL, so the effect can be more potent. Purpose: The purpose of this research, therefore, was to compare EA with PGL in reducing the expression of Bcl-2. Methods: This laboratory experimental research was used 32 male mice (Balb/c) in the age of 5 months. They were randomly divided into 4 groups: 2 control groups (K0: which was not exposed with benzopirene and also untreated and K1: which was exposed with benzopirene and also untreated), 2 treatment groups (P1: which was exposed with benzopirene and also treated with EA and P2: which was exposed with benzopirene and also treated with the PGL). Next, an examination was conducted by using immunohistochemical techniques. results: The results then showed that the provision of the PGL could decrease the expression of Bcl-2 significantly higher than that of EA in the malignant epithelial cells of the oral mucosa of those mice. Conclusion: It may be concluded that the provision of the PGL can kill malignant cells in the oral cavity of mice by increasing apoptosis through decreasing Bcl-2 expression that was higher than the provision of EA.latar belakang: Karsinoma sel skuamosa rongga mulut adalah istilah yang digunakan menyebut neoplasma ganas berasal dari epitel. Neoplasma ganas adalah sel yang telah berubah struktur dan fungsi, sehingga mengalami peningkatan jumlah secara abnormal,invasif dan metastasis. Terjadinya karsinoma salah satu disebabkan oleh karena hambatan apoptosis terhadap sel ganas. Bcl-2 adalah protoonkogen keluarga Bcl yang berperan menghambat proses apoptosis sel dan bekerja menekan protein Bax (pro apoptosis). Berbagai upaya penatalaksanaan penyakit kanker masih banyak menemui kendala,sehingga peneliti menggali tanaman obat yaitu buah delima. Buah delima sebagai tanaman obat, mudah didapat dan harganya murah. Ellagic acid (EA) senyawa tunggal bahan aktif dari ekstrak buah delima yang memiliki aktivitas sebagai anti kanker secara in-vitro tetapi EA aktivitas dan konsentrasinya dalam plasma rendah, kelarutan dalam air rendah, metabolisme (EA) tidak larut dalam intestinal. Fakta ini mendorong peneliti untuk membandingkan dengan whole ekstrak delima (PGL) yang terdiri dari beberapa senyawa bahan aktif, tidak hanya ellagic acid, memungkinkan beberapa senyawa bahan aktif pada PGL bisa bekerja sinergis, sehingga efeknya lebih poten. tujuan: Tujuan penelitian ini membandingkan antara EA, dengan whole ekstrak buah delima (PGL) dalam menurunkan ekspresi Bcl-2. Metode: Metode penelitian yang digunakan adalah eksperimental laboratories, 32 ekor mencit (Balb/c), jantan, umur 5 bulan dibagi secara random menjadi 4 kelompok, 2 kelompok kontrol (K0: tidak dipapar benzopirene dan tidak diberi perlakuan, K1: dipapar benzopirene dan tidak diberi perlakuan), 2 kelompok perlakuan (P1: dipapar benzopirene dan diberi EA, P2: dipapar benzopirene dan diberi PGL). Pemeriksaan dengan menggunakan teknik imunohistokimia. hasil: Hasil penelitian menunjukkan bahwa pemberian PGL dapat menurunkan ekspresi Bcl-2 lebih tinggi dibandingkan EA pada sel epitel ganas mukosa rongga mulut mencit. Kesimpulan: Kesimpulan penelitian ini adalah pemberian PGL dapat membunuh sel ganas pada rongga mulut mencit dengan jalan meningkatkan apoptosis melalui penurunan ekspresi Bcl-2 lebih tinggi dibandingkan pemberian EA.

Abstract

Background: The compressive strength of orthodontic tooth movement will be distributed throughout the periodontal ligament and alveolar bone, resulting in bone resorption on the pressure side and new bone formation on the tension side. Caffeine, a member of the methyl xanthine family, represents a widely-consumed psychoactive substance that can stimulate osteoclastogenesis through an increase in RANKL. A 3D Micro-Computed Tomography (µ-CT) x-ray device can be used to measure orthodontic tooth movement and changes in periodontal ligament width. Purpose: The purpose of this research was to analyze the effects of caffeine on the distal movement distance of two mandibular incisors using 3D µ-CT. Methods: The research subjects (guinea pigs) were randomly divided into four groups. Of the two control groups created, one received two weeks of treatment and the other three weeks. The members of these two control groups were subjected to orthodontic movement but received no caffeine. Meanwhile, the other two groups were treatment groups whose members also received either two or three weeks of treatment. In these two treatment groups, the subjects were subjected to orthodontic movement and received a 6 mg/500 BM dose of caffeine. The orthodontic movement of the subjects was induced by installing a band matrix and orthodontic bracket on each mandibular incisor to move distally by means of an open coil spring. Observations were then conducted on days 15 and 22 with µ-CT x-rays to measure the distal movement distance of the two mandibular incisors and the width of the periodontal ligament. Results: The administration of caffeine increased the tooth movement on day 15 (p<0.05) and day 22 (p<0.05). The increase in the tooth movement on day 22 was greater than that on day 15 (p<0.05). The width of the periodontal ligament on the pressure side of the treatment groups experienced greater narrowing than that of the control groups (p<0.05). Meanwhile, the width of periodontal ligament on the tension side of the treatment groups widened more than that of the control groups (p<0.05). Conclusion: µ-CT x-ray can be used to evaluate the extent of orthodontic movement in addition to the width of the mandibular incisor periodontal ligament during orthodontic tooth movement. Moreover, it has been established that the administering of caffeine can improve orthodontic tooth movement.

Abstract

Background: Trauma-induced ulcers constitute one of the painful abnormalities affecting the oral cavity about which numerous individuals complain. The prevalence of traumatic ulcers (15-30% of cases) is relatively high compared to other oral lesions. Unfortunately, the use of anti-inflammatory drugs classified as steroids can lead to contraindications and serious side effects. Hence, the use of natural ingredients represents an alternative treatment. One such ingredient is red pomegranate containing bioactive elements acting as anti-inflammatory, antibacterial, antimicrobial, and antioxidant agents which accelerate the healing process in traumatic ulcers. Purpose: The aim of this study was to determine the effects of the topical application of red pomegranate (Punica granatum Linn) extract on the healing process in traumatic ulcers in Wistar rats. Methods: 24 male rats, aged 2-3 months, were divided into six groups, namely; two control groups (PC: triamcinolone acetonide and NC: sterile aquades) and four treatment groups (red pomegranate extract gel at respective concentrations of 12.5%, 25%, 50% and 75%). A traumatic ulcer was subsequently created using a flame-shaped burnisher tip 2mm in diameter and heated for ±15 seconds. The diameter traumatic ulcer of each research subject was measured and observed until it healed within the remedial parameters determined by means of a UNC-15 periodontal probe which had been placed on it. The data obtained was analyzed using Kruskal-Wallis and Mann-Whitney tests. Results: Topically applied red pomegranate extract gel can reduce the width of the ulcerated area, while also accelerating traumatic ulcer healing. There was a significant difference in the healing time between the group using red pomegranate extract gel at a concentration of 75% and the positive control group using 0.1% triamcinolone acetonide with p=0.44 (p<0.05). Conclusion: Red pomegranate extract gel possesses the ability to accelerate the healing process in traumatic ulcers. The most effective concentration of red pomegranate extract gel at accelerating the healing process in traumatic ulcers is 75%.

Abstract

Pendahuluan: Merokok merupakan salah satu kebiasaan yang sering dilakukan oleh masyarakat, termasuk kelompok lanjut usia (lansia). Kebiasaan merokok dapat mempengaruhi sekresi saliva pada lansia yang secara fisiologis mengalami penurunan sekresi saliva dan penipisan epitel mukosa mulut karena proses penuaan. Perubahan sekresi saliva dan penipisan epitel mukosa mulut dapat meningkatkan resiko infeksi rongga mulut seperti kandidiasis oral. Tujuan penelitian ini adalah untuk menganalisis perbedaan sekresi saliva melalui pengukuran laju aliran saliva serta terjadinya kandidiasis oral pada pasien lansia perokok dan bukan perokok, di Rumah Sakit Gigi Mulut  Universitas Jember (RSGM UNEJ). Metode: Penelitian ini merupakan analitik observasional pada pasien lansia RSGM UNEJ di Bagian Penyakit Mulut meliputi status merokok, pengukuran laju aliran saliva unstimulated, dan insidensi kandidiasis oral berdasarkan hasil pemeriksaan mikrobiologi oral swab. Populasi dalam penelitian ini adalah seluruh pasien lansia RSGM UNEJ Bagian Penyakit Mulut periode September 2019-Januari 2020. Metode pengambilan sampel yang digunakan adalah  total sampling. Hasil: Hasil uji Mann-Whitney menunjukkan tidak ada perbedaan signifikan antara sekresi saliva unstimulated pasien lansia perokok dan bukan perokok. Terdapat perbedaan kejadian kandidiasis oral antara pasien lansia yang merokok dengan yang tidak merokok (nilai p=0,065; derajat kepercayaan=90%). Simpulan: Tidak terdapat perbedaan laju aliran saliva pada lansia perokok dan bukan perokok, namun terdapat perbedaan terjadinya kandidiasis oral antara lansia perokok dan bukan perokok.Kata kunci: Laju aliran saliva, merokok, kandidiasis oral, lansia. ABSTRACTIntroduction: Smoking is one of the habits often carried out by the community, including the elderly. Smoking habits can affect salivary secretion in the elderly, who physiologically experience decreased salivary secretion and thinning of the oral mucosal epithelium due to the ageing process. The salivary secretion change and thinning of the oral mucosal epithelium can increase the risk of oral infections such as oral candidiasis. The purpose of this study was to analyse the differences in salivary secretion by measuring the salivary flow rate and the occurrence of oral candidiasis in elderly smokers and nonsmokers at the University of Jember Dental Hospital. Methods: This study was an observational analytic study in elderly patients of the Department of Oral Diseases University of Jember Dental Hospital, including the smoking status, measurement of unstimulated salivary flow, and the incidence of oral candidiasis based on the results of the oral microbiological swab. This study’s population were all elderly patients at the Department of Oral Diseases University of Jember Dental Hospital for September 2019-January 2020. The sampling method used was total sampling. Results: The Mann-Whitney test results showed no significant difference between the unstimulated saliva secretion of elderly smokers and nonsmokers. There was a difference in the incidence of oral candidiasis between elderly smokers’ patients and nonsmokers (p-value = 0.065; confidence level = 90%). Conclusion: There is no difference in the salivary flow rate in elderly smokers and nonsmokers, however, there are differences in the occurrence of oral candidiasis between elderly smokers and nonsmokers.Keywords: Salivary flow rate, smoking, oral candidiasis, elderly.

Abstract

Recurrent aphthous stomatitis (RAS) is an oral mucosal lesion in the form of a single or multiple recurrent ulcer. The etiology is still not known, usually occurs in the second decade. RAS is more likely to strike in early adulthood with a tendency more often in women than men (3:1). The RAS precipitating factors are infection, trauma, hormones, emotional effect, fatigue, immunological disorders, nutritional disorders, and hereditary. Clinically RAS is in the form of ulcers that are round, single or multiple, varying in diameter, white-centered, redness in the edge, and painful. The RAS Classification: aphthous ulcers major, minor aphthous ulcers and herpetic form. Ulceration is a mucosal damage to the stratum basale, painful and interfere while chewing food. RAS can heal itself between 7-14 days and even more but the excruciating pain will torture the patient. Wound healing is a complex process that involves many elements which work in sync to achieve wound healing. Pomegranate is one of the widely studied medicinal plants for health. Pomegranate has benefits as anti-inflammatory, anti-oxidant, anti-microbial, immunostimulatory, cheap, easy to obtain, have no side effects. Pomegranate has very complex potential that can eliminate various predisposing factors in a single therapy and also can increase endurance. Based on this fact the purpose of this research is to theoretically study pomegranate as an alternative therapy for RAS biomolecularly. The conclusion is the active ingredient of pomegranate theoretically has the functions of anti-inflammatory, anti-oxidant, anti-microbial, immune-stimulatory and can regulate the fibrosis process, so that pomegranate can cure RAS.

Abstract

Smoking habit is a health problem that is found globally. Indonesia has the largest number of active smokers, reaching 65 million people. Fisherman is one of the occupation in Indonesia which have the largest proportion of active smokers, almost 44,5% compared to other occupation. Smoking habit triggers pathological conditions in the oral cavity. Smoking cause an inflammation of the palatal mucosa named nicotine stomatitis. The purpose was to determine the prevalence of nicotine stomatitis in fisherman at Puger Kulon and to find out the correlation between age, type of cigarette, the intensity of smoking and the incident of nicotine stomatitis in fisherman. Data were collected by interview with 301 respondents. Purposive sampling was used in this study. The chi- square test was used to examine the correlation between age, type of cigarette and the intensity of smoking with the incident of nicotine stomatitis. The prevalence of nicotine stomatitis in fisherman at Puger Kulon Village is 5.3% and age, type of cigarette, and intensity of smoking did not have a significant correlation to the incident of nicotine stomatitis in fisherman at Puger Kulon Village Puger District Jember Regency.

Abstract

The use of chemical insecticides in the long term and high frequency will suppress insects (mosquitoes) and vectors to be tolerant until resistant. Reports of resistance from various species of mosquitoes found in multiple countries even occur in Indonesia in Surabaya. This study aimed to analyze the effect of granules bioinsecticides on the mixture of srikaya seeds (A. squamosa) and betel leaf (P. beetle) on the histopathology of mice (M. musculus) using true-experimental (the post-test only group design). Sample taken by simple random sampling and grouped into each control group and the treatment group totaling 25 mice. Data were analyzed using simple linear regression test. The results showed an overall p-value >0.05, meaning that there was no bioinsecticide effect of a mixture of betel leaf granules (Piper beetle) and srikaya seeds (Annona squamosa) on the histopathology of the kidney of mice (Mus musculus). Required safety testing of target animals in addition to mice and a preliminary examination is needed before the research is conducted so that the results are valid Keywords: bioinsecticide; histopatology; Mus musculus

Abstract

Background: Chicken pox is the primary acute infectious disease caused by the Varicella-Zoster Virus (VZV) which attacks skin and mucosa accompanied by polymorphic skin disorders, mainly located in the central part of the body. Case: an 8- years-old woman came to the Dental Hospital, Jember University complaining about pain felt on the upper lip when used for eating for 7 days. The patient also had a fever and fluid-filled lumps appear in both hands and back since about 6 days ago. Clinical overview of the upper lip show erosion, irregular shape, clear borders, red, moist lesions, rough surfaces and pain. In both hands there are lesions in the formof erosion,the base is red,moist and the boundaryis clear. Management: Therapy applied to patients i.e. Aloclair gel as a topical analgesic medicine and moisturizer, Acyclovir tablet as an antiviral agent, paracetamol syrup as an antipyretic and multivitamin B complex as supporting therapy. Discussion: Chicken pox is a disease that can heal itself, but to speed up healing process and to reduce discomfort due to the symptoms that appear, causative and supportive cares are necessary. Conclusions: Management of a chicken pox patient aged 8 years old is conducted by providing Aloclair oral gel, Acyclovir tablets, paracetamol syrup and multivitamin of vitamin B complex and zinc to support healing process, and instruct the patient to avoid direct contact with other people.

Abstract

Traumatic ulcer is one of the disorders of the oral cavity that can be caused by mechanical, thermal, and chemical. Patch one of traumatic ulcer treatment choice are being developed, but the available patch is uneconomic and have side effects. Therefore, the use of natural ingredients is an alternative treatment, such as pomegranate. Pomegranate is known to contain bioactive elements, that can act as anti-inflammatory and antioxidant, that it can accelerate the healing of traumatic ulcers. The aim of this research is to determine the effect of the pomegranate extract patch on the acceleration of ulcer healing. Sample in this research were 25 traumatic ulcered wistar rats which were divided into 5 groups. All groups were condensed the pomegranate extract patch with a concentration of 12.5%; 25%; 50%, positive control (taisho patch), and negative control (placebo). The red pomegranate extract patch can accelerate the healing of traumatic ulcers. There was a significant difference in healing time between all groups (p<0.05), except between the positive control group with a concentration of 50%. pomegranate extract patch affected the acceleration of ulcer healing, with a concentration of 50% pomegranate extract patch was the fastest.

Abstract

Gadingan Village is located on the coast of Jangkar District, Situbondo Regency. Some residents have difficulty accessing clean water. In 2020, Gadingan Village received PAMSIMAS grants including drilling for water sources, deep water pumps, and reservoirs. Currently, these facilities have not been fully utilized because the management agency has not yet been formed. The Gadingan Village Government encourages BUMDes as PAMSIMAS managers, but BUMDes does not yet have knowledge of network management and planning. People who use water do not believe in the quality of the water and the quantity of the water. The problem solving method used is the socialization of PHBS and PAMSIMAS to the community, as well as BUMDes assistance for PAMSIMAS management and PHBS awareness. The results obtained are increased public awareness in the use of clean water and the provision of sanitation and hygiene facilities.
