Abstract

Background: Tuberculosis is a contagious disease that is still a public health problem. Indonesia is the country with the second most frequent tuberculosis cases in the world. One of the element that play a role in tuberculosis control is qualified human resources, including health cadres.Objectives: To determine the effectiveness of training to increase knowledge and change the attitude of the health cadres against tuberculosis in Gunungkidul.Methods: This study uses the Quasy Experiment research design, with a pretest-posttest design. Before the given intervention is given a pretest, then intervention in the form of training, after that do the posttest. After that analysis and test significance.Results: There were 70 health cadres, 35 were given intervention (training) and 35 as controls. There was a significant increase in knowledge in training cadres (p < 0.05) and there was a difference in the level of knowledge between trained and untrained (p < 0.05), there was a change in attitude on the trained and untrained cadres (p < 0.05).Conclusions: With the training, it can be improve knowledge and influence the attitude of health cadres against tuberculosis.

Abstract

Background: Cough cases are increasing every year. The most common cause is a virus. Coughing is influenced by endurance, Cough Ethics, and Handwashing with Soap/Cuci Tangan Pakai Sabun (CTPS). Transmission occurs through droplets in the air from the patient when coughing or sneezing. We need to educate the community to minimize transmission, especially among children. Ultimately, this speeds up the healing process. Objective: This study aimed to provide an understanding of Cough Ethics and behavior change using narrative in action to kindergarten teachers. Method: This study used descriptive-analytic qualitative methods with narrative in action. The subjects of the study were a population of kindergarten teachers in the working area of the Puskesmas Sedayu 1, selected purposive snowball sampling. Retrieval of data was done through the observation stage, pre-narrative in action activities, the narrative in action, post narrative in action activities, and the final observation stage in kindergarten. Result: Observations were done at the school to find out the infrastructure. Observation before and after the demonstration showed there were improvements and concluded that all can demonstrate the Cough Ethics and CTPS. Watching the Cough Ethics video, CTPS, followed by the dialogue, motivation, education, persuasion, promotion were empowering for the teachers to change their behavior. All groups when coughing or sneezing covered their nose and mouth with a tissue, mask, or the inner side of elbow arm or expelled phlegm on the ground, lodong/special place like a jar, and toilet. Conclusion: Narrative in action activities are an alternative way of providing understanding to change behavior. Watching videos makes it easy to explain the Ethics of Cough. Practice is needed to improve the skills of narrative in action.

Abstract

The UI Makara Satellite Clinic (KSUI) is a university clinic that serves UI residents and the public as well as a vehicle for  education  and  health  research  in  primary  services.  It has been  a  BPJS  Outpatient  Service  Clinic  (KPRJ)  since October  2018.  It  is  called  a Satellite  Clinic  because  it  is planned  to  become  a  satellite  for  university  hospitals  in the academic health system at the University of Indonesia. Currently, KSUI is located on the Depok campus and the Salemba campus. In its daily services at the Depok campus, KSUI provides  services  for  doctors,  dentists,  nursing, pharmacy,  laboratories,  action  rooms, and psychological counseling.KSUI’s mission is not the same as an ordinary outpatient clinic, but because KSUI is a health service facilitated by the university, KSUI has a mission to maintain the health of the academic community, and not only treat them when they fall  ill.  Therefore,  in  addition  to  services that  respond  to patients coming to the clinic, KSUI also organizes periodic health check services, facilitates Posbindu activities in the faculties and office buildings on campus, and participates in medical care at any gathering or sports activities provided on campus.With  the  Covid-19  case  being  found  in  Indonesia,  KSUI decided  to  change  its service  method  as  quickly  and  as firmly  as  possible  from  face-to-face  services  to online services,  following  the  Chancellor’s  Decree  to  close  the campus  from  face-to-face  learning  and  other  student activities.  With  the  return  of  the  UI  dormitory students and  the  implementation  of  work  from  home  (WFH)  for lecturers  and education  staff,  KSUI  must  provide  safe services during a pandemic and can be accessed by service users  without  having  to  come  to  campus.  Therefore,  this article is a brief piece of information that has been done by KSUI when it decided that services should be carried out online.  It  is  not  easy  for  the  officer,  because  it  has  never been done, and it is not easy for the patient.

Abstract

Latar Belakang:Dokter layanan primer berperan sebagai gatekeeper untuk mengurangi biaya dengan membatasi rujukan ke pelayanan spesialis yang tidak sesuai. Pembatasan penggunaan pelayanan spesialis yang tidak perlu bisa meningkatkan kualitas pelayanan.Pembatasan rujukan dalam jaminan kesehatan nasional (JKN) antara lain dengan diberlakukannya 144 diagnosis penyakit level kompetensi 4 yang harus dilayani di fasilitas kesehatan tingkat pertama (FKTP).Ada 13 kasus terbanyak dari 144 diagnosis penyakit nonspesialistik yang dirujuk ke pelayanan kesehatan tingkat lanjut (FKTL) berdasarkan hasil monitoring BPJS Kesehatan Jawa Tengah-DIY tahun 2014.Meningkatnya kasus penyakit yang dirujuk ke FKTL meningkatkan biaya pelayanan kesehatan.Tujuan penelitian ini adalah mengetahui faktor-faktor yang mempengaruhi dokter merujuk 13 penyakit nonspesialistik berdasarkan persepsi dokter.Metode:Penelitian ini merupakan studi kualitatif dengan pendekatan “grounded theory”. Penelitian dilakukan pada bulan Juli-Agustus 2015 di DIY.Sampel dipilih secara purposive.Jumlah sampel 20 FKTP.Responden adalah dokter.Pengumpulan data dengan wawancara mendalam.Triangulasi dilakukan kepada sumber yang berbeda dan observasi.Hasil:Faktor yang mempengaruhi dokter dalam merujuk penyakit meliputi faktor eksternal dan internal. Faktor eksternal merupakan faktor yang menurut dokter dirasa cukup dominan meliputi kurangnya ketersediaan obat seperti obat-obat yang masuk dalam program rujuk balik (PRB); kurangnya ketersediaan alat medis seperti alat fisioterapi, alat pemeriksaan mata, alat penyedot serumen, permintaan pasien; kebijakan BPJS tentang penjaminan resep kacamata hanya bisa di dokter spesialis mata di FKTL; dan perilaku dokter spesialis rumah sakit yang tidak mengembalikan pasien PRB ke FKTP. Faktor internal antara lain faktor penyulit penyakit seperti tidak respon dengan pengobatan, multidrug resistan tuberculosis (MDR-Tb); dan kompetensi dokter yang kurang pada penyakit Bell’s palsy dan presbiopia.Kesimpulan: Faktor-faktor yang mempengaruhi dokter merujuk penyakit adalah kurangnya alat medis, kurangnya ketersediaan obat, permintaan pasien, kebijakan BPJS Kesehatan, perilaku dokter spesialis, dan faktor penyulit penyakit, serta kompetensi dokter yang kurang. Kata kunci: dokter layanan primer, rujukan, pelayanan kesehatan primer, jaminan kesehatan nasional

Abstract

Background: A large number graduates of midwife even now accompanied with low competency isues in associated with quality of clinical learning. Now implementation of clinical learning with caseload model has problems. Clinical learning model in clinical practice that student, teacher and clinical midwife experienced provide greater insight to develop an effective clinical teaching strategy in midwifery education. The main objective of this study was to investigate student midwife, teacher and clinical midwife’ insight about an ideal clinical learning model in midwifery education.Method: A qualitative study by Focus Group Discussion (FGDs). By selected randomly, this study was conducted 32 from 76 final year midwifery students at Estu Utomo Boyolali Midwifery Academy, 14 lecturer and 13 clinical midwife who involve in Estu Utomo Boyolali Midwifery Academy clinical practice program at 2013-2014. There are 4 groups of student midwife, 2 groups of lecturer and 2 groups of clinical midwife. FGDs were arranged in 3 session differently between students, lecturer and clinical midwife. FGDs were facilitated by researcher and 3 research assistant. Data were analyzed using Atlas.Ti 6.1 software to support the coding process and identifying the main categories from verbatim transcripts.Results: Six themes emerged from the focus group data, “student caseload”, “duration of clinical practice”, “clinical placement”, “clinical mentorship”, “documentation”, and “clinical assessment”. From the sixth themes, demonstrated that an ideal clinical learning model in midwifery education to design preparation clinical learning should consider about that components. An ideal clinical learning model in midwifery education should give priority to quality than quantity especially to suggest decreasing student caseload.Conclusion: This study showed that midwife student, teacher and clinical midwife suggest that clinical learning model should give priority to quality than quantity to gain midwifery care competence. Clinical learning model therefore applied with student case loading that each student have different need. They are suggest that midwifery clinical learning should be consistent with the midwifery care philosophy “women center care” that provide midwifery continuity of care.

Abstract

Background: Every medical education institution in Indonesia must used SPICES model as a strategy/approach on their curriculum. This accord with what is state in Standar Pendidikan Profesi Dokter Indonesia (SPPDI). Assessment of implementation of SPICES model in educational curriculum is highly important in determining the direction of learning process underway. The aim of this study is to develop a questionnaire that helps medical educational institution to assess degree of implement of SPICES model on their curriculum.Method: Questionnaire of Problem-based learning (PBL) learning process with SPICES model development made the two stages. First stage purpose was to develop questionnaire, while second stage was to validate of the questionnaire construct. First stage consists of three steps, i.e literature study, arrangements the items, and content validity. Construct validity of the questionnaire is done by used reliability test, and factor analysis test.Results: Result from questionnaire development in the first stage was 29 items. Construct validity process in the stage two produced 17 items.Conclusion: Assessment of PBL learning process with SPICES model questionnaire proven to be valid and reliable. Further investigation needs to assess stability of the questionnaire in other region.

Abstract

Background: Profesionalism has become a main competency for physician. Therefore, it is important to get an agreement from health provider about professionalism and it’s applicable learning method for medical institution. This research aimed to explore skills laboratory instructor and student’s perception of the learning method that can facilitate the mastery of professionalism and it’s professional behavior components.Method: A descriptive qualitative method was carried out in this research. Data was collected by indepth interview among instructors and FGD among two groups of students, and analyzed by grounded theory method approach.Results: Medical professionalism is an integrated ability of  knowledge, skill and attitude. Professional behavior formed by attitude. Professionalism can be learnt and taught by eksplisit, implisit and continuing education methods.Conclusion :. Medical professionalism can be collaborated in several learning methods. Role model is claimed as an effective learning method for medical professionalism and professional behavior mastery.

Abstract

Background: Student-centred learning is characterized by constructive, collaborative and contextual learning; derived from the constructivism theory. We studied the principle of “Tut Wuri Handayani” / TWH (to follow the learners to achieve their utmost potentials) which was initiated in 1922 by Dewantara, who was the first Ministry of Education, Republic of Indonesia.Method: A qualitative method with ‘content-analysis’ was used to study the articles written by Dewantara published in Indonesian newspapers between 1920-1960. We reflected the articles into current evidences of student-centred learning principles. Validations were done through three times focus group discussions (FGDs) by 30 students of a master’s of health professional education program and in-depth interview with two national experts in higher education.Results: In the period of early 20th century, Dewantara’s ideas had arrived at ‘contextual’ and ‘collaborative’ principles; the basis of current philosophy of student-centred learning. Dewantara’s exceptional initiatives were: (1) emphasizing equity between learners and teachers regardless the strong hierarchical social culture among Indonesian, (2) making the best use of cultural events and instruments as rigorous learning resources to enhance the sensory abilities and social sensitivity of the students towards community’s needs, (3) promoting observation-based learning to promote self-directed learning by helping the students to reflect on their mistakes, to be independent thinkers and to be responsible persons, to minimize gaps between attitudes and behaviours, and to provide role models and opportunities for students to actively participate in learning.Conclusion: “Tut Wuri Handayani” or ‘to follow the learners to achieve their utmost potentials’ revealed as a student-centred learning principle emphasizes life-long learning. Dewantara may be regarded as one of the world-wide leaders in education.

Abstract

Background: Indonesia medical education recruitment system recruits students directly from high school who had been influenced by mostly teacher centered learning. As part of pedagogical approach in higher education, those students will be obligatory to learn independently and effectively. Self-directed learning (SDL) is an important aspect in latest medical educational evidence which would determine students’ successfulness in learning. Self-directed learning readiness (SDLRS) is influenced by several factors including motivation, age, culture, and previous education experience. Today, the admission process in Indonesia is based on knowledge-test based only. This study aims to reveal correlation between several factors which influence students’ Self Directed Learning Readiness (SDLRS).Methods: This study used SDLRS questionnaire as a valid and reliable toll to measure the students’ self directed learning readiness and correlate with those several factors.Result: From 540 distributed questionnaires, 412 questionnaires returned back. The Spearman correlation showed significant ratio between students SDLRS score and students’ motivation. While, the analysis on age showed that age of 20 has significant ratio with students’ SDLRS score.Conclusion: among age, previous education experience, and culture; students’ internal motivation  should be considered as important factor for students’ SDLRS which will determine their successfulness in learning in medical education. These factors should be regarded as one requirement of students’ admission in medical education.

Abstract

Background: Feedback constitutes one of the important components in student Since feedback is not solely limited to student assignments, it can also be given during student learning, including tutorials. Therefore, the institution should pay particular attention to the process of feedback provision one of which is through evaluations on the process of providing feedback in tutorials. This study aimed to evaluate the process of providing feedback on the Problem-Based Learning tutorials in the Faculty of Medicine, Universitas Islam Sumatera Utara.Method: This study used mixed methods with a quantitative method performed first, followed by a qualitative method with a phenomenology approach. The subjects were the undergraduate medical students of the class of 2012 and tutors of the Problem-Based Learning tutorials. The quantitative data analysis was done by the descriptive analysis while the qualitative data analysis was done through verbatim analysis, coding, categorization and conclusion.Results: The students had received feedback but not routinely. Both the students and the tutors had perceived that feedback was important and gave them many benefits. In practice, feedback was given on the basis of tutors’ direct observation results. Action ;plan was often given but rarely involved discussion. Self-assessment/reflection was also not a matter of routine and the tutors did not know that it was part of the feedback. The contents of feedback were more often in the form of general comments. Feedback was often communicated in one direction. The students’ response to feedback varied. In giving and receiving feedback, the students, the tutors, the tutorial activities and the content of the feedback itself could influence one another. The students had  preferences so feedback can be more easily accepted and they focused on the structural aspects of feedback (the frequency of giving feedback, the detail and spesific feedback, two way communication in giving feedback and the feedback target). Tutors expected more trainings in giving feedback provided by institution to tutors and students.Conclusion: Despite its valuable benefits, tutorial feedback in the Faculty of Medicine, Universitas Islam Sumatera Utara,is still not in accordance with the principle of providing feedback and many factors have affected this condition.
