Abstract

Background and purpose: The prevalence of obesity among adolescents is increasing significantly. One of the predictors of this problem is unhealthy eating pattern due to peer and social media influence. This study aims to explore in depth the experience of college students in watching food content on YouTube. Methods: This study used a qualitative method with a phenomenological approach. Data were obtained through in-depth interviews with ten informants and one focus group discussion with seven female students aged 18-22 years. The informants were female students who have been watching food content on YouTube for more than one year. Data collection was carried out over April-May 2019. Data were analyzed thematically and presented using anarrative approach. Results: There were three themes that emerged in this study, namely the reasons for watching food content, the benefits, and the effects of watching food content on adolescent health habits. Food content on YouTube is imagined to be educational and provides new knowledge related to food and culture in various regions of Indonesia and other countries. Watching YouTube has become a routine teen activity where they can spend one to four hours per day and can cause teens to form sedentary lifestyle. YouTube watching habits are influenced by peers and social media. It can increase adolescent appetite, thus, they more often opt for unhealthy foods. Conclusion: Watching food content on YouTube has a positive and negative impact on adolescent health. One negative impact is that the content or influencers can cause the selection of unhealthy foods. Attractive education method should be applied in order to counteract unhealthy food content and promote healthy eating habits.

Abstract

Background and purpose: The national tuberculosis (TB) prevalence study showed that between 2013 and 2014 there was significant proportion of pulmonary TB patients received treatment from private healthcare facilities. Data from Bali Province and Badung District Health Offices revealed that of all suspected TB patients only a few were referred by their private general practitioner. A reward system was created in 2012 by providing credit points to every referral made by the private general practitioner. This study aims to examine association between the reward system, national health insurance scheme and willingness to refer suspected TB patients. Methods: A cross-sectional study was conducted from February to April 2017. A total of 111 private general practitioners who practice at Badung District were interviewed. In addition, medical record from these private general practitioners were also examined. Results: Our study found that as many as 55 (45.95%) of private general practitioners had ever referred suspected TB patients over the last year. The total suspected TB patient being reffered in the last year was 132 cases. A cross checking with the laboratory data revealed that only 47 cases (35.61%) presented at referral laboratory facilities. Multivariate analysis showed that the willingness of private general practitioners to refer suspected TB patient was associated to reward system (AOR=4.62; 95%CI: 1.23-17.32) and supervision from TB officials (AOR=13.07; 95%CI: 3.78-45.13). The enrollment of private general practitioners as a primary healthcare facility under the national insurance scheme was not associated with the willingness to refer suspected TB patients. Conclusions: About half of the private general practitioners have ever referred a suspected TB patient. Supervision from TB officials and a reward system in terms of credit points were associated with the willingness to refer suspected TB patients. Ongoing supervision, providing credit points, and improving referral systems are required to improve case findings and referral of suspected TB patients by private general practitioners

Abstract

Background and purpose: Studies on determinants of nutritional status in under-five children in Indonesia have been widely explored. However, most studies utilised only one out of three nutritional status indicators–weight/age, height/age, or weight/height. These studies also show inconsistent results. This present study aims to examine nutritional status in under-five children by using all three indicators and associated factors which include mother’s characteristics, nutrition intake, and child’s history of infectious diseases. Methods: A cross-sectional study was conducted with a total sample of 100 under-five children who were randomly selected. Data on the mother’s characteristics, nutrition intake, and child’s history of infectious diseases were collected through structured interview with the mother, while data on nutritional status were obtained through measurements. Data were analysed using logistic regression to identify association between the nutritional status of under-five children with the mother’s characteristics, nutrition intake, and child’s history of infectious diseases. Results: There was no association between the nutritional status of under-five children based on weight/age, height/age, and weight/ height with the mother’s characteristics and nutrition intake. However, there was an association between the nutritional status of under-five children and child’s history of infectious diseases with an adjusted odd ratio (AOR)=41.0 (95% CI: 12.10-139.14) for weight/age, AOR=3.52 (95%CI: 1.53-8.05) for height/age, and AOR=10.69 (95%CI: 1.31-87.11) for weight/height. Conclusions: Child’s history of infectious diseases is the only determinant associated with nutritional status in under-five children based on weight/ age, height/age, and weight/height. Prevention measures are required to prevent infection among children by improving environmental hygiene as well as providing prompt treatment for infectious diseases especially diarrhoea and upper-respiratory tract infection.

Abstract

Background and purpose: Early marriage through merariq or eloping tradition is a common practice in West Nusa Tenggara particularly in Central Lombok. Early marriage leads to several negative consequences on the reproductive health of female adolescents. This study aims to explore early marriage practices and its impacts on the reproductive health of female adolescents in Central Lombok. Methods: A qualitative study with a case study approach was conducted, and seven early married female adolescents purposively selected as the study participants. Interviews were also conducted with two family members of the female adolescents, one community leader, and a midwife who worked as an adolescent health coordinator at a public health centre. Data were analyzed using a thematic approach to map the reasons for early marriage and its impacts on the reproductive health of female adolescents. Results: Our in-depth interviews revealed that several reasons associated to early marriage practices were: lacking in knowledge regarding the effects of early marriages, perceived benefits of early marriages including perception on maturity and logical thinking, and social sanctions from the society because it considered a disgrace when the eloped girl returned home. Several negative consequences of early marriages emerged from this study were complicated pregnancy/delivery, low birth weight, and undernourished children. Our informants concerned about the psychological effects of early marriages which include fear, regret, and depression. Eloped female adolescents lose their agency for decision making including the right for further education. Conclusions: Early marriage practices bring negative health consequences to female adolescents. An active collaboration across health sector and community leaders is required to provide sufficient information regarding the harmful effects of early marriages. Also, a more effective communication strategy using a family-based approach is required to appropriately target female adolescents with health information associated with the impacts of early marriage.

Abstract

Background and purpose: Disaster preparedness is crucial for health workers in order to provide relief to communities affected by disasters quickly and precisely. However currently there are only few health workers who are prepared to face disasters. This study aims to determine the association between knowledge in disaster management, perception of disaster preparedness, participation in training and experience in disaster management with disaster preparedness in health workers. Methods: A cross sectional survey was conducted among health workers at Petang and Abiansemal public health centres (PHCs). Of the six PHCs in Petang and Abiansemal Sub-districts, four were randomly selected consisting of one PHC in Petang Sub-district and three PHCs in Abiansemal Sub-district. All health workers (271 people) in the four PHCs were selected as respondents. Individual face-to-face interviews were conducted by the first author during April 2018 in the workplaces of each respondent using a pre-tested questionnaire. Data collected consisted of socio-demographic characteristics, knowledge in disaster management, perceptions of disaster preparedness, participation in training, experiences in disaster management and disaster preparedness. Questions consisted of three components namely knowledge in disaster management (12 items), perception of disaster preparedness (32 items) and disaster preparedness (25 items). Bivariate analysis was conducted with chi square test and multivariate analysis with binary logistic regression to determine the association between knowledge, perception, participation in training and experience in disaster management with disaster preparedness. Results: The results showed that 70.9% of respondents had attended disaster training, 40.6% had good knowledge, 24.7% had participated in disaster management and 49.1% had a high level of disaster preparedness. The variables significantly associated with disaster preparedness were perceptions of disaster preparedness (AOR=6.40; 95%CI: 3.71-10.99) and participation in disaster training (AOR=2.68; 95%CI: 1.44-4.97). Conclusion: Disaster preparedness among PHC health workers remains low. Perception of disaster preparedness and participation in training are significantly associated with disaster preparedness. Continuous training is needed to increase disaster preparedness among health workers.
