Abstract

Objective: The study aims to analyze clinical and laboratory data differences, also the correlation of Visceral Adiposity Index (VAI) with other observed parameters in type 2 diabetics with or without ischemic stroke. Methods: This study was conducted as analytic observational study with cross-sectional design. Clinical data were directly collected from patients, followed by laboratory data collection. There was a comparison (Mann-Whitney or independent T-tests) and correlation analysis (Pearson or Spearman). Results: In the ischemic stroke group, the total-to-HDL cholesterol ratio and VAI were significantly higher (p=0.033 and 0.002, respectively) than the non-ischemic stroke group. The total-to-HDL cholesterol ratio and VAI had a significant correlation (p<0.001). Clinical (WC and VAI) and laboratory data (total cholesterol, triglyceride, LDL-C, and total-to-HDL cholesterol ratio) differ significantly in type 2 diabetics with or without ischemic stroke. Conclusion: In both diabetes groups, with or without ischemic stroke, there was a significant correlation between VAI and the total-to-HDL cholesterol ratio.

Abstract

Introduction: Sepsis is a group of symptoms caused by infection, characterized by organ dysfunction due to compromised hosts response to infection hence may lead to a life-threatening condition. One of the treatments for sepsis stated in one hour-bundle is the administration of broad-spectrum antibiotics before the culture results obtained. Improper use of antibiotics may lead to antibiotic resistance. The purpose of this study was to describe the microbes pattern and their sensitivity to antibiotics in patients with sepsis at Muhammad Hoesin Hospital, Palembang, to contribute to a useful treatment guideline and to provide a reference for further research. Methods: This study was a descriptive observational study with a cross-sectional design using medical record of patients diagnosed with sepsis whose blood culture results were positive. This study was conducted at Muhammad Hoesin Hospital, Palembang, from January 2017 to December 2018. The data was processed and analyzed by univariate analysis using the SPSS 21.0 computer program. Results: The study subjects were predominantly children with Gram-positive bacteria (24.6%) as the most common cause. The most common bacteria observed were Staphylococcus epidermidis (35.7%), Staphylococcus aureus (27.3%), Staphylococcus haemolyticus (24%), and Staphylococcus hominis (21.4%). Linezolid (100%), nitrofurantoin (100%), and quinupristin/dalfopristin (100%) were found to be sensitive to gram-positive bacteria. Conclusion: Gram-positive bacteria were the most common cause of sepsis in Muhammad Hoesin Hospital, Palembang. The bacteria were sensitive to linezolid, nitrofurantoin, and quinupristin/ dalfopristin.

Abstract

Background: The pathogenesis of COVID-19 involves complex immunological processes that can predispose to disease severity. In COVID-19 patients, there will be changes in the number of CD4 + and CD8 + cells which are part of lymphocyte cells in the specific immune system. This study aims to see the relationship between CD4+ and CD8+ cell counts in COVID- 19 patients at Dr. Mohammad Hoesin Hospital Palembang, Indonesia. Methods: This research is an observational study with a case series approach. A total of 30 study subjects who were COVID-19 patients participated in this study. The number of CD4+ and CD8+ cells was assessed, then correlation analysis was performed with the Spearman test, p<0.05. Results: The correlation between CD4+ and CD8+ has a value of R = 0.875 and p<0.01. This shows that CD4+ and CD8+ have a strong positive correlation and are statistically significant. Conclusion: The increase in the number of CD4+ cells will be followed by an increase in the number of CD8+ cells in COVID-19 patients at Dr. Mohammad Hoesin General Hospital Palembang, Indonesia.

Abstract

Indonesia is one of the countries with a high prevalence of diabetes, with total cases of diabetes in adults reaching 10 million cases, 6.2% of the total adult population. Diabetes mellitus (DM) is a group of metabolic diseases characterized by hyperglycemia due to defects in insulin secretion, insulin action, or both. Insulin resistance in muscle and liver cells and pancreatic beta-cell failure has been recognized as the pathophysiology of major damage of type 2 diabetes. The blood glucose test is an important laboratory examination that can be used to diagnose, screen, and monitor DM type 2. There are several types of blood glucose levels, including random blood glucose tests, fasting blood glucose tests, post prandial blood glucose tests, oral Glucose Tolerance Test (OGTT), and HbA1c. Type 2 DM is often called the silent killer because sufferers are often late for self-examination and treatment so that complications arise early. Early symptoms of type 2 diabetes can be identified through blood sugar examination and risk factor screening. Screening for type 2 DM can detect abnormalities as early as possible so that interventions can start as early as possible to minimize the complication rate so that the quality of life can be maintained or better.

Abstract

Data Centers for Disease Control and Prevention (CDC) tahun 2016 menunjukkan sekitar 110 juta orang menderita penyakit menular seksual (PMS). PMS sering kali dipandang sebelah mata, padahal dampaknya bagi kesehatan jangka panjang cukup signifikan terutama pada wanita dan bayi, seperti infertilitas, kelainan kongenital, kanker dan lain-lain. Penjaja seksual merupakan kelompok berisiko tinggi untuk tertular PMS. Karena itu, perlu diketahui prevalensi PMS terutama pada mereka yang berisiko tinggi (wanita penjaja seksual) sehingga dapat dibuat kebijakan terkait program pengendalian dan pencegahan PMS. Jenis penelitian yang digunakan adalah studi prevalensi dengan pendekatan survai klinis secara cross sectional. PMS yang dihitung prevalensinya pada penelitian ini adalah infeksi HIV, Hepatitis B, Hepatitis C dan sifilis. Serum dari subjek penelitian dikumpulkan dan diperiksa anti-HIV untuk infeksi HIV, anti-HCV untuk Hepatitis C, HbsAg untuk Hepatitis B dan VDRL untuk sifilis. Didapatkan prevalensi PMS pada wanita penjaja seksual di Palembang adalah sebagai berikut yaitu: HIV sebesar 4.9%, sifilis sebesar 3.3%, Hepatitis B sebesar 9.8% dan Hepatitis C sebesar 1.6%. Edukasi, sosialisasi terkait bahaya dan dampak dari PMS perlu dilakukan terutama pada mereka yang berisiko tinggi seperti pada wanita penjaja seksual sebagai bentuk dari program pengendalian dan pencegahan PMS.

Abstract

Keganasan adalah penyakit yang ditandai dengan pertumbuhan sel-sel abnormal yang berlebihan dan menyebar ke organ tubuh lain. Demam neutropenia merupakan salah satu komplikasi pada keganasan karena mekanisme dasar penyakit tersebut dan efek samping dari kemoterapi. Leukosit, neutrofil dan neutrophil extracellular traps (NETs)berperan penting dalam proses keganasan dan infeksi. Penelitian ini bertujuan untuk mengetahui korelasi hitung leukosit dan neutrofil dengan kadar neutrophil extracellular traps (NETs) pada pasien keganasan dengan demam neutropenia. Penelitian ini adalah penelitian analitik observasional dengan desain cross sectional. Pasien keganasan dengan demam neutropenia yang memenuhi kriteria inklusi sebanyak 30 pasien pada bulan Maret-Agustus 2018 di Ruang Perawatan Departemen Kesehatan Anak RSUP Dr. Mohammad Hoesin Palembang diambil sebagai sampel dengan teknik consecutive sampling. Hitung leukosit dan neutrofil diambil dari data sekunder pasien, dan kadar NETs diukur dengan menggunakan ELISA. Analisis menggunakan uji korelasi Spearman, hasil bermakna apabila p<0,05. Hasil penelitian menunjukkan tidak terdapat korelasi signifikan antara hitung leukosit (p=0,207;r=0,273) dan neutrofil (p=0,388;r=189) dengan kadar neutrophil extracellular traps (NETs) pada pasien keganasan dengan demam neutropenia.Tidak terdapat korelasi yang bermakna antara hitung leukosit dan neutrofil dengan kadar neutrophil extracellular traps (NETs) pada pasien keganasan dengan demam neutropenia.

Abstract

Lupus Eritematosus Sistemik (LES) merupakan penyakit autoimun yang mengenai hampir semua sistem organ dan memiliki manifestasi klinis yang bervariasi. Perjalanan penyakit LES ini ditandai dengan remisi dan eksaserbasi sehingga memerlukan pemantauan yang ketat akan aktivitas penyakitnya. Untuk menilai aktivitas penyakit lupus maka digunakanlah skor SLEDAI (Systemic Lupus Erythematosus Disease Activity Index). Tujuan dari penelitian ini adalah untuk mengetahui hubungan hasil laboratorium pasien LES dengan skor SLEDAI dari pasien LES yang berobat di Rumah Sakit Dr. Mohammad Hoesin Palembang periode Januari 2013-Juli 2014. Penelitian ini berbentuk analitik observasional dengan desain cross sectional serta menggunakan data sekunder, yaitu rekam medik subbagian Alergi Imunologi Bagian Penyakit Dalam Rumah Sakit Dr. Mohammad Hoesin Palembang. Sampel yang diperoleh sebanyak 43 sampel penderita LES yang memenuhi kriteria inklusi. Hasil yang diperoleh dianalisis dengan menggunakan uji statistik Fisher’s Exact. Dari 43 pasien, 16 pasien (37,3%) memiliki skor SLEDAI ringan dan 27 pasien (62,7%) memiliki skor SLEDAI berat. Hasil penelitian ini menunjukkan bahwa terdapat hubungan yang sangat bermakna antara anti-dsDNA dan skor SLEDAI (nilai p=0,005). Tidak terdapat hubungan yang bermakna antara anemia, trombositopenia, leukopenia, proteinuria dan ANA dengan skor SLEDAI (nilai p>0,05). Terdapat hubungan yang bermakna antara hasil laboratorium anti- dsDNA dan skor SLEDAI.

Abstract

Background: The rRT-PCR examination is very effective as a diagnostic tool for COVID-19 but cannot be a full reference as an evaluation examination because rRT-PCR is able to capture the genetic material of the virus without ensuring that the virus is still alive or only dead particles. This study aims to explore the correlation between absolute lymphocyte values and the severity of COVID-19 at Dr. Mohammad Hoesin General Hospital Palembang. Methods: This study is an observational study involving 413 research subjects at Dr. Mohammad Hoesin General Hospital. Data analysis was carried out univariate and bivariate. Results: The absolute number of lymphocytes with the severity of COVID-19 has a negative correlation, with a strong correlation strength, which is expressed by the value of r = -0.617 and the degree of significance of p = 0.000. Conclusion: There is a correlation between absolute lymphocyte count and the severity of COVID-19 at Dr. Mohammad Hoesin General Hospital Palembang.

Abstract

Production of Extended-Spectrum Beta-Lactamase (ESBL) by Enterobacteriaceae continues to be a problem of infectious diseases, especially in hospitals. The main causes of ESBL-producing bacteria colonization are urinary tract infections, length of hospital stay, invasive medical equipment, and antibiotics usage. This study aims to compare the incidence of ESBL based on the type of organism in Dr. Mohammad Hoesin Hospital for the 2017 and 2018 periods. The research design used was descriptive with a cross-sectional approach, which used secondary data at the Clinical Pathology Department of Dr. RSUP. Mohammad Hoesin Palembang. The findings of this study showed a decreasing pattern in the incidence of ESBL in 2017 and 2018, but with a similar pattern which was dominated by Klebsiella pneumoniae (followed by Escherichia coli and Klebsiella ozaenae), inpatients in pediatric wards, internal medicine, and intensive care units, and on sputum specimens. This study showed the presence of high levels of ESBL-producing bacteria (>60%) in Dr. Mohammad Hoesin Hospital, which was mainly caused by Klebsiella pneumoniae.
