Abstract

ABSTRAK Pasien lupus eritematosus sistemik (LES) memiliki risiko lebih tinggi terhadap infeksi tuberkulosis (TB) akibat penggunaan obat lupus dan penyakitnya. Di Indonesia, prevalensi TB aktif pada pasien LES mencapai 10,2%, jauh lebih tinggi dibandingkan populasi umum. Kurangnya pengetahuan pasien mengenai TB dapat menyebabkan keterlambatan diagnosis dan pengobatan yang optimal. Tujuan: Melakukan program edukasi Tuberkulosis pada pasien lupus yang diberi nama TULUS dalam meningkatkan pemahaman pasien LES tentang risiko dan pencegahan TB dan mengevaluasi efektivitas program edukasi tersebut. Kami melaksanakan kegiatan edukasi berbasis penyuluhan secara langsung dengan media pendukung berupa poster dan flyer. Pengetahuan pasien diukur dengan pre-test dan post-test. Kegiatan ini dilakukan di Poli Reumatologi, Rumah Sakit Dr. Hasan Sadikin Bandung. Hasil test dianalisis menggunakan uji Wilcoxon Signed Ranks untuk mengukur pengetahuan pasien. Sebanyak 21 pasien LES berpartisipasi dalam kegiatan edukasi TULUS. Terdapat peningkatan yang signifikan pada median skor pengetahuan dari 7 pada pre-test menjadi 9 pada post-test (p=0,001). Program edukasi TULUS terbukti efektif dalam meningkatkan pengetahuan pasien LES mengenai TB. Kegiatan lanjutan perlu dilakukan untuk menilai retensi pengetahuan tentang TB pada pasien LES. Kata Kunci: Lupus Eritematosus Sistemik, Tuberkulosis, Edukasi Kesehatan, Pencegahan TB.  ABSTRACT Systemic lupus erythematosus (SLE) patients have a higher risk of tuberculosis (TB) infection, due to the use of lupus drugs and the disease activity. In Indonesia, the prevalence of active TB in SLE patients has reached 10.2%, much higher than the general population. Lack of patient knowledge about TB can cause delays in diagnosis and optimal treatment. To evaluate the effectiveness of the Tuberculosis Education in Lupus Patients called TULUS education program in increasing the knowledge of SLE patients about TB risks and prevention. We carried out direct education methods with supporting media in the form of posters and flyers. Patient knowledge was measured before and after the education. The activity was conducted at the Rheumatology Clinic, Dr. Hasan Sadikin General Hospital, Bandung. The pre-test and post-test results were analyzed using the Wilcoxon Signed Ranks test to measure patient knowledge. A total of 21 SLE patients participated in the TULUS education. There was a significant increase in the median knowledge score from 7 on the pre-test to 9 on the post-test (p=0.001). The TULUS education program has proven effective in increasing SLE patients' knowledge about TB. Further activities are needed to assess retention of knowledge about TB in SLE patients. Keywords: Systemic Lupus Erythematosus, Tuberculosis, Health Promotion

Abstract

Systemic lupus erythematosus (SLE) is a chronic autoimmune disease often managed with immunosuppressants such as methylprednisolone (MP) and azathioprine (AZA), although therapeutic responses vary among individuals. Genetic variation, including polymorphisms in the ATP-Binding Cassette Subfamily B Member 1 (ABCB1) gene encoding the P-glycoprotein drug transporter, may influence treatment outcomes. The rs1045642 polymorphism has been linked to variable responses in SLE, but data in Indonesian populations are scarce. This study aimed to describe the distribution of the ABCB1 rs1045642 polymorphism in SLE patients from Bandung, Indonesia, and to explore its potential association with therapy outcomes using MP and/or AZA. We conducted a cross-sectional study of 84 SLE patients, collecting clinical data from medical records. Treatment outcome was defined as achievement of lupus low disease activity state (LLDAS). Genomic DNA was extracted and sequenced to determine rs1045642 genotypes. A total of 84 SLE patients were included, predominantly aged 26–35 years (34%). Almost half had a disease duration of 6–10 years (49%). The majority achieved LLDAS (69%), and all patients were receiving methylprednisolone, with 78.6% also receiving azathioprine. The genotype distribution of ABCB1 rs1045642 was AA 10.71%, AG 61.91%, and TT 27.38%, which deviated from Hardy–Weinberg equilibrium (p < 0.05). However, genetic variations were observed among patients with SLE. Further studies on other possible polymorphisms related to the outcome of SLE therapy are needed.

Abstract

Introduction: It is known that pregnancies with autoimmunity have a higher risk of complications in the mother and fetus compared to pregnancies without autoimmunity. The purpose of this study was to determine the comparison between pregnancy outcomes with autoimmune rheumatic disease and without autoimmune rheumatic disease.Methods: This study is an observational analytic with a retrospective cross-sectional design. Data were obtained from all patients with pregnancy outcomes with autoimmune rheumatic disease and without autoimmune rheumatic disease at Hasan Sadikin Hospital Bandung 1 January - 31 December 2021-2023.Results: During this period, 71 pregnant women were found to be accompanied by autoimmune rheumatic diseases and then data on pregnant women without autoimmune rheumatic diseases were randomly taken as controls. In this study, it was found that pregnant women with autoimmune diseases experienced more neonatal outcomes of stunted fetal growth, namely 11(15.5%) compared to pregnant women without autoimmune rheumatic diseases, namely 2(2.8%) with a p-value of 0.009.Conclusion: This study found that pregnant women with autoimmune rheumatic disease experienced more neonatal outcomes of FGR compared to pregnant women without autoimmune rheumatism.Perbandingan Antara Luaran Kehamilan Dengan Penyakit Rhematik Autoimun Dan Tanpa Penyakit Rematik AutoimunAbstrakPendahuluan: Diketahui bahwa kehamilan dengan autoimun memiliki risiko komplikasi pada ibu maupun janin lebih tinggi dibandingkan dengan kehamilan tanpa autoimun. Tujuan penelitian ini untuk mengetahui perbandingan antara luaran kehamilan dengan penyakit rematik autoimun dan tanpa penyakit rematik autoimunMetode: Penelitian ini bersifat analitik observasional dengan desain potong lintang retrospektif. Data diperoleh dari seluruh pasien luaran kehamilan dengan penyakit rematik autoimun dan tanpa penyakit rematik autoimun di Rumah Sakit Hasan Sadikin Bandung 1 Januari – 31 Desember 2021-2023Hasil: Dalam periode tersebut ditemukan 71 orang ibu hamil yang disertai dengan penyakit rematik autoimun kemudian diambil data ibu hamil tanpa penyakit rematik autoimun secara random sebagai kontrol. Pada penelitian ini didapatkan ibu hamil dengan penyakit autoimun lebih banyak mengalami luaran neonatal pertumbuhan janin terhambat yaitu 11(15.5%) dibandingkan ibu hamil tanpa penyakit rematik autoimun yaitu 2(2.8%) dengan p- value0.009.Kesimpulan : Penelitian ini menemukan bahwa ibu hamil dengan penyakit rematik autoimun lebih banyak mengalami luaran neonatal pertumbuhan janin terhambat dibandingkan dengan ibu hamil tanpa rematik autoimun.Kata kunci : Luaran Maternal, Luaran Neonatal, Autoimun

Abstract

BACKGROUND: Systemic lupus erythematosus (SLE) is an autoimmune disease characterized by inflammation. The pathogenesis of SLE involves key proteins, including B cell-activating factor (BAFF) and the ubiquitin enzyme A20, both serving as negative regulators of inflammation and contributing to B cell homeostasis. In this review, focused on interventions directed at BAFF and the A20 enzyme, utilizing monoclonal antibodies either independently or in conjunction with conventional therapy for SLE patients.METHODS: A literature search was conducted on the PubMed platform by combining various terms, including "B-cells activating factor", "TNFAIP3 protein (human)", "therapeutics" or "drug therapy", and "lupus erythematosus, systemic" (limited to the last 10 years). From total of 104 articles discovered in thr search, the total number of articles collected after being filtered was 27 articles.RESULTS: Clinical development and evaluation have been conducted regarding the use of appropriate therapy for SLE patients. Selective BAFF inhibitor has been tested in clinical trials as a blocking agent in BAFF receptor (BAFF-R) and signaling nuclear factor-kappaB (NF-κB) by A20 bindings to inhibit the activation of autoreactive B cells. Just like other antimonoclonal therapies, BAFF and the A20 enzyme can be used as therapeutic targets with a single use or combined with the standard therapy in patients with SLE. In addition, the use of BAFF and A20 also shown to have safe side effects in patients with SLE. CONCLUSION: BAFF protein and A20 enzyme present promising therapeutic targets for managing autoimmune diseases like SLE. Therapeutic interventions can be administered individually or in conjunction with standard treatments.  KEYWORDS: systemic lupus erythematosus, therapeutic targets, BAFF, A20

Abstract

Background: One of the causes of the increase in hospitalized SLE patientsis infection, and it is an important factor in morbidity and mortality, so it isnecessary to conduct a research to identify factors related to infection andthe type of infection caused in hospitalized SLE patients. Methods: Thisstudy is a retrospective, categorical descriptive study utilizing medicalrecords of SLE patients diagnosed with and treated for infection both onadmission and during their stay in Hasan Sadikin General Hospital betweenJanuary 2016 to June 2018. Results: Seventy- four patients were involvedinto this study. Female were 70 (94.6%), aged <40 years were 69 (93.2%)patients, and all 74 (100%) were entirely in an active disease condition witha mean Mexican systemic lupus erythematosus disease activity (MexSLEDAI) score of 9 ± 5.2. Fifty-three (71.6%) subjects experienced majorinfections. Mucocutaneous and kidney were the most organs involvementfound in SLE patients during infection, 63 (85.1%) and 57 (77.0%)respectively. Sixty-two (83.7%) subjects had received corticosteroids beforethe onset of infection, while pneumonia was the most common infectionfound in 33 patients (44.6%). This study also noted 12 (16.2%) patientsdeceased related to infection with most causes were respiratory failure(66.7%). Conclusion: Age less than 40-year-old, mucocutaneous and kidneyinvolvement and previously on corticosteroids were found higher among inhospitalized SLE patients. Most experienced major infections andpneumonia is the most common type of infection.

Abstract

Background: B-Cell Activating Factor (BAFF) is a cytokine that plays a rolein systemic lupus erythematosus (SLE) pathogenesis.  BAFF increases B cellfunction, which will affect disease activity.  In addition to decreasing diseaseactivity, good quality of life (QoL) is one of the goals to be achieved in SLEtherapy, which is also affected by disease activity.  The purpose of this studyis to know the correlation of the BAFF profile with the QoL of SLE patients.Methods:  This was a cross-sectional study using secondary data from aprevious study.  The subjects of this study were SLE patients who had visitedthe rheumatology outpatient clinic or were hospitalized in Hasan SadikinHospital Bandung from September 2016 until February 2017.  Subjects wereasked to complete the Short Form-36 and measure their BAFF level inserum.  Demographic data were collected, and disease activity data wereassessed by Mexican Systemic Lupus Erythematosus Disease Activity Index(MEX-SLEDAI).  The statistical analysis used in this study was the Spearmantest. Results: There was a positive correlation between BAFF and MEXSLEDAI (r 0.238, p-value 0.038).  There was also a negative correlationbetween MEX-SLEDAI and PCS, as well as the MCS score (r -0.392 and 0.371, p-value <0.05). Conclusion:  BAFF levels will increase in higherdisease activity which will affect a poorer quality of life.

Abstract

Pericardial effusion is a fatal and life-threatening condition. If it is not addressedthoroughly, complications such as constrictive pericarditis may occur. Etiologies ofpericardial effusion varied and one of the most common etiologies is tuberculosis.Other etiologies include inflammation, malignancy or other autoimmune disordersuch as sarcoidosis. Sarcoidosis is a diagnosis of exclusion and is confirmed bybiopsy result that showed non-caseating epitheloid-cell granuloma, with no otherorganism or particles. Epidemiology of sarcoidosis in Japan is 1-2 case per 100.000patients, with the peak incidence between the age of 20 to 39 years old. We reporteda case of 37 years old woman presented to our emergency department withshortness of breath and signs of pericardial tamponade. She was previously healthywith no other significant past medical. She was first treated as a case ofextrapulmonary tuberculosis and shown no improvement with anti-tuberculosismedications. Several work ups were then done in search of other etiologies of herpericardial effusion. A biopsy form one of her abdominal lymph nodes wasperformed, which pathologically revealed sarcoidosis. She was placed oncorticosteroid and methotrexate with improvement of symptoms. One monthfollowed up showed complete resolution of her pericardial effusion.

Abstract

Background: Systemic Lupus Erythematosus (SLE) is an autoimmune disorder commonly complicated by hematological disorders and lupus nephritis (LN). Kidney impairment and chronic inflammation may affect erythropoiesis and red blood cell parameters. However, there is limited information on the short-term changes of hematological parameters in patients with and without LN. This study aimed to compare the short-term changes of hematological parameters between two clinical visits in patients with SLE with and without LN. Methods: This retrospective observational study used data from the Hasan Sadikin Lupus Registry in 2023. A total of 202 female patients with SLE aged 18 years and older with complete hematological data from two clinical visits were included. Hematological parameters included hemoglobin, hematocrit, mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC), and red cell distribution width coefficient of variation (RDW-CV). Comparison of changes in parameters between the first and second visits was done separately for patients with and without LN using the Wilcoxon Signed Rank Test. Results: In patients with LN, there was a statistically significant reduction in MCHC between the first and second visits, whereas there were no significant changes in hemoglobin, hematocrit, MCV, MCH, and RDW-CV. In patients without LN, small but statistically significant changes were noted in hemoglobin, MCV, and MCHC. The degree of change was small and remained within the normal range. Conclusion: The short-term hematological parameters in patients with SLE are generally stable between two clinical visits. Only MCHC showed a significant change in patients with LN, indicating a selective and subtle hematological change rather than a global effect of LN. These results indicate a cautious interpretation of the short-term hematological changes and suggest that MCHC may be used as an auxiliary parameter for hematological assessment in patients with SLE and LN.

Abstract

Background: Awareness of Systemic Lupus Erythematosus (SLE) is critically low among Indonesian adolescents, despite clinical evidence showing that approximately 52% of pediatric cases occur between the ages of 11 and 15. This study evaluated the effectiveness of a health education seminar conducted during a community service program entitled “LUPUS CERDAS: Cekatan, Energik, Responsif, dan Sadar Sehat” in improving health literacy regarding SLE, photoprotection, and physical activity among junior high school students. Methods: A quasi-experimental one-group pre-test/post-test design was conducted at SMPN 2 Jatinangor, involving 35 students from Grades 7 to 9. Data were collected using a validated 20-item multiple-choice questionnaire and a thematic qualitative feedback form. Results: There was a statistically significant improvement in knowledge scores, increasing from a mean of 72.29 ± 12.97 to 81.71 ± 15.34 (p < 0.001), representing a 13.04% gain from the baseline. Qualitatively, 57.1% of students identified the pathology of Lupus as the most engaging topic due to its novelty, while 85.7% rated the visual educational aids as "Very Helpful." Notably, the study identified a "Silent Knowledge Gap," with 28.6% of participants remaining uncertain about their family medical history regarding autoimmune symptoms. Conclusions: Targeted community service interventions are powerful tools in enhancing adolescent health literacy. By providing students with an evidencebased framework for understanding autoimmune triggers, these programs increase awareness regarding early recognition and family health communication. Future outreach should prioritize the integration of mental health and lifestyle management into school-based health curriculum.
