Abstract

Background: Human Immunodeficiency Virus (HIV) is a virus that attacks lymphocytes, leading to a gradual decline in the body's immune system and increasing susceptibility to various diseases, including Acquired Immunodeficiency Syndrome (AIDS). Men who have sex with men (MSM) are at a higher risk of contracting HIV/AIDS due to unsafe sexual behavior. Objective: This scoping review aims to explore the factors associated with safe sexual behavior in MSM Living with HIV. Method: The scoping review is based on the framework by Arksey and O'Malley. A literature search involved various databases, such as EBSCO, PubMed, Science Direct, and CINAHL I, with the keywords: "Factors Associated" AND "Safe Sex Behavior" AND "Men Who Have Sex With Men OR MSM OR LSL". The selection process for research articles involved the use of inclusion criteria. The results were analyzed by extracting data into a data mapping table (PRISMA Flow Diagram). Results: 20 articles discussed factors related to safe sexual behavior among men with HIV/AIDS. These factors include individual perception, knowledge, and values. Discussion: It can be concluded that individual perception strongly influences safe sexual behavior among men, as individual perceptions can guide and motivate behavior.

Abstract

Post-surgery fracture patients have discharge planning needs that must be completed, among others are pain management, aids, nutrition, medication, wound treatment, and control time. Fracture patients are prone and have a high risk to infection, complication and length of hospitalization, so that post-surgery patients need comprehensive information related to discharge planning needs. This research is aimed to describe the need of discharge planning on patients with lower extremity fracture in Marjan Atas Room dr. Slamet Garut Hospital. The methodology used in this research is descriptive quantitative. The population of this research is the post-surgery patients with lower extremity fracture, the sample of this research is 30 respondents gained by using total sampling technique. The data of the research is gained with Instrument made by the researcher based on Potter & Perry theory. The instrument contains 28 questions with measurement scale: strongly needed and less needed. The research data was analyzed using frequency distribution based on question items at each sub variable. The results showed that most of the post-surgery discharge planning needs with the mostly need category is of 60% based on sub variables of nutrition, medication, mobilization, wound treatment, pain management, and aids. The sub variable that is less needed is control time. From the research above, it can be concluded that the need for post-surgery discharge planning for fracture is nutrition, medication, mobilization, treatment care, pain management, and aids. It is suggested that the patient need and be give nutrition officer, pharmacist or nurse to explain patient care at home using leaflet media.

Abstract

ODS (One Day Surgery) service is an innovation in surgical treatment in recent decades and is claimed to have many advantages. However, increased use of anesthetic drugs, pain management during postoperative, increased length of recovery or hospitalization, increased risk of infection and increased costs are complications that can result from poorly managed preoperative anxiety. This study aimed to identify anxiety preoperative ODS patients in Dr. Hasan Sadikin Bandung. This research used quantitative descriptive design with sample of 60 patients recruited by purposive sampling technique. Data collection used Spielberger State-Trait Anxiety Inventory (STAI: Y-6) instrument and APAIS (Amsterdam Preoperative Anxiety And Information Scale) are valid and reliable. Data analysis used descriptive statistics. The results showed that all respondents experienced preoperative anxiety with mean 53,94 (SD ± 9,38). Based on the source of preoperative anxiety, the preoperative anxiety picture associated with the mean operating procedure 5.65 (SD ± 1.94), anxiety concerning the mean anesthesia procedure of 5.37 (SD ± 1.83) and the anxiety associated with the need to obtain the mean information of 7.43 (SD ± 1.98). It can be concluded that all preoperative patients in ODS experience preoperative anxiety in relation to the need for information. It is recommended that hospitals improved the quality of providing information to patients and families by involving interdisciplinary teams, thereby improved the overall quality of ODS services and improved patient and family satisfaction. Abstract

Fraktur adalah kerusakan pada kontinuitas tulang, penanganannya dengan operasi yang dapat menimbulkan kecemasan. Ketika pasien mengalami kecemasan maka akan mengalami gangguan pada beberapa sistem tubuh diantaranya tekanan darah meningkat, gelisah dan sulit tidur. Jika hal tersebut terjadi ketika menjelang operasi dapat menyebabkan kegagalan operasi. Penelitian ini bertujuan untuk mengetahui gambaran tingkat kecemasan preoperative pada pasien fraktur di Ruang Marjan Atas RSUD dr. Slamet Garut. Jenis penelitian ini menggunakan metode deskriptif dengan pendekatan cross sectional. Populasi dalam penelitian ini adalah pasien preoperative dengan fraktur dengan jumlah sampel sebanyak 32 responden, dengan teknik total sampling. Data diambil dengan menggunakan instrumen Hamilton Anxiety Rating Scale (HARS), dianalisis secara deskriptif kuantitatif dan diolah dengan menggunakan software komputer, kemudian disajikan dalam bentuk distribusi frekuensi. Hasil penelitian menunjukkan sebagian besar responden mengalami kecemasan ringan (65,6%). Kecemasan ringan terjadi pada orang yang baru pertama kali melakukan operasi karena ketakutan dan kurangnya pengetahuan, sedangkan pasien yang tidak cemas sudah mempunyai pengalaman operasi sebelumnya dan ketakutan menjadi berkurang. Simpulannya adalah tingkat kecemasan pada pasien preoperative dengan fraktur di Ruang Marjan Atas RSUD dr. Slamet Garut mayoritas cemas ringan. Dapat disarankan intervensi preoperative teaching kepada pasien sehingga dapat meminimalisir terjadinya kecemasan preoperative.

Abstract

Fraktur merupakan salah satu penyebab cacat diantaranya akibat kecelakaan. Fraktur ekstremitas bawah sering terjadi terkait dengan morbiditas yang cukup besar dan meenyebabkan perawatan panjang di rumah sakit. Gangguan akibat fraktur berdampak pada toleransi aktivitas sehingga mengurangi produktivitas. Selama ini karakteristik penderita fraktur ektermitas bawah belum diketahui, sehingga jika tidak dilakukan tidak dapat diketahui pencegahan resiko fraktur. Penelitian ini bertujuan untuk mengetahui karakteristik pasien fraktur ektermitas bawah. Metode penelitian ini merupakan penelitian deskriptif kuantitatif dengan observasional. Tekhnik sampling yang digunakan yaitu total sampling, sehingga didapatkan responden sebanyak 40 orang. Analisa hasil yaitu menggunakan analisis statistik distribusi frekeunsi. Hasil dari penelitian ini yaitu semua responden berjenis kelamin laki-laki (100%), sebagian besar responden berusia 36-45 (42,5%), untuk jenis fraktur sebagian besar yaitu fraktur terbuka sebanyak 25 (62,5%) dan lokasi fraktur terbanyak yaitu pada tibia sebanyak 26 responden (62,5%). Fraktur dapat mempengaruhi produktivitas penderita akibat adanya gangguan ektermitas akibat cedera yang menganggu fungsi tubuh. Dengan mengetahui karakteristik pasien fraktur ektermitas bawah, maka perawat dapat melakukan intervensi keperawatan yang sesuai. Abstract

Pressure ulcers are a complication that becomes more complex when experienced by patients with multimorbidity such as Congestive Heart Failure (CHF), Chronic Kidney Disease (CKD), and thrombocytopenia. These conditions can lead to prolonged healing, high risk of infection, systemic complications, and even death. Interventions must be accompanied by appropriate tools to evaluate wound healing optimally. The Pressure Ulcer Scale for Healing (PUSH) is a tool for accurately, practically, and simply assessing the progression of pressure ulcers. However, its use for evaluating pressure ulcers remains limited. The objective to describe the use of PUSH in patients with pressure ulcers and multimorbidity. Method: This study used a case study design involving a 56-year-old patient with a grade III pressure ulcer on the buttocks measuring 121.5 cm2 with granulation tissue, minimal necrosis tissue, pus, and active bleeding. Wound progression was evaluated using PUSH over 4 days during wound care. The results that the PUSH score increased to 16 with a wound size of 150 cm2 moderate exudate, and extensive necrosis tissue. This indicates that the wound condition has not improved, prompting an evaluation of the wound care and factors hindering wound healing. Although PUSH aids clinical decision-making, it has limitations in assessing wound depth and systemic conditions as it only evaluates three parameters. The PUSH instrument is a simple and efficient tool for monitoring the healing of pressure ulcers and has the potential to indicate changes in wound condition as a result of an intervention, thereby aiding clinical decision-making. PUSH is recommended for routine use in clinical practice, while continuing to evaluate systemic conditions comprehensively and involving a multidisciplinary team.
