Abstract

Diarrhea is one of the main causes of death in the world. Environmental cleanliness, sanitation and access to clean water are still factors that trigger diarrhea, especially in developing countries such as Indonesia. Diarrhea can attack anyone at any time, both adults and children. This Community Service aims to determine the effectiveness of using the control reminder information system for diarrhea patients in increasing repeat visits at the Mayang Community Health Center. The implementation method is carried out by providing health education by providing information about diarrhea through posters, sending health education videos and Control message reminders using the SKEDit Application Program. Evaluation before health education was carried out using SKEDit, customer satisfaction was 67.8% and after health education was carried out using SKEDit, it was 80.1%. Meanwhile, the rate of repeat visits for diarrhea patients after receiving a control reminder increased by 70%. Based on the results of observations, it shows that this Community Service activity is able to increase customer satisfaction and increase repeat visits of diarrhea patients at the Mayang Community Health Center.

Abstract

Background: Feelings of fatigue and stress can arise in caregivers when caring for patients with type 2 diabetes mellitus (T2DM) due to responsibility, pressure, and increasing demands. Therefore, the ability to bounce back from difficult situations, known as resilience, is needed. Purpose: This study aims to analyze the effect of the Family Caregiver Empowerment Model (FCEM) intervention on the resilience of caregivers of patients with type 2 diabetes mellitus. Methods: This study is a randomized control group pretest and posttest study with a sample of 74 respondents, randomly selected using cluster sampling (treatment = 35, control = 39). The independent variable is FCEM, and the dependent variable is resilience. The instrument used is the Connor-Davidson Resilience Scale 25 (CD-RISC 25) questionnaire. Statistical analysis was performed using the Wilcoxon Signed Rank Test and Independent T-test. Results: The results showed that there was a significant difference in depression scores between the pretest and posttest in both the intervention and control groups (p = 0.000 and p = 0.002, < α = 0.05). Additionally, there was a significant difference in resilience between the intervention and control groups (t = 13.644, p = 0.000). Conclusions: The findings of the study demonstrated the effect of the FCEM intervention on the resilience of caregivers of T2DM patients. It is essential to increase the resilience of family caregivers by providing empowerment as a form of social support.

Abstract

Background: Pain is a primary complaint among patients in healthcare services. The failure in pain management can increase morbidity and mortality rates. There is a need to improve nursing students' competencies through innovative Web-Based Learning (WBL) models, which leverage technology to create a comprehensive and interactive learning environment. This study aims to develop CALM (Case-based Advanced Learning for Pain Management) to enhance the attitudes and knowledge of final-year nursing students regarding pain.Method: The research design used was Research and Development (R & D), involving final-year nursing students from the Faculty of Nursing, University of Jember, who met the inclusion and exclusion criteria. The sampling technique used was purposive sampling. The research instruments used were the Knowledge and Attitude Survey Regarding Pain (KASRP), Nurse Behavior Questionnaire in Pain Management (NBQ-P), Focus Group Discussion (FGD) guide, and System Usability Scale (SUS). Data analysis was performed using descriptive statistical analysis.Results: The results of the study indicated that the knowledge and attitudes of final-year nursing students regarding pain fall into the low category (53.8%). In terms of pain management behavior, the majority of respondents were in the good category (55.90%). The development of CALM learning media resulted in six modules to be developed: pain assessment and evaluation; pain pharmacology; non-pharmacological interventions; pain in special populations; and cultural, spiritual, and ethical aspects of pain management. The feasibility test of CALM learning media showed positive results.Conclusions: The development of CALM learning media was highly accepted, functions optimally, facilitating access to pain-related information, and aligning with previous evidence regarding the effectiveness of e-learning and interprofessional curricula, which have the potential to enhance knowledge, assessment skills, and learning satisfaction among students

Abstract

Chronic kidney disease (CKD) is a global health problem with multifactorial etiologies that may clinically manifest as leg edema, affecting patient comfort and mobility. Although pharmacological and non-pharmacological therapies can be administered simultaneously, evidence regarding the application of combined ankle pump exercise and 30° leg elevation in nursing care for edema reduction in CKD patients remains limited. To analyze nursing care through the application of combined ankle pump exercise and 30° leg elevation therapy to reduce leg edema in a patient with CKD. This study employed a case report design involving a CKD patient with bilateral leg edema. Nursing interventions consisted of a combination of ankle pump exercise and 30° leg elevation administered twice daily for 8 minutes per session, accompanied by furosemide therapy. Edema was assessed through daily observation of edema grading. A reduction in edema level was observed, with the left leg showing improvement on the third day and the right leg on the fourth day, decreasing from grade II to grade I edema. The application of combined ankle pump exercise and 30° leg elevation, accompanied by furosemide administration, demonstrated a reduction in the degree of leg edema in a patient with CKD. However, further studies using larger samples and controlled study designs are needed to confirm the effectiveness and generalizability of this intervention in clinical nursing practice. Abstract

Pleural effusion is the accumulation of fluid in the pleural cavity or membrane that separates the inner chest wall from the lungs. Pleural effusion can affect hemodynamics because increased intrapleural pressure can push organs in the mediastinum. Providing a semi-fowler position is one of the nursing interventions that is expected to improve hemodynamic status in patients with pleural effusion. This study aims to determine the implementation of nursing care for patients with pleural effusion by applying the semi-fowler position to hemodynamic status in the Catleya Room, dr. Soebandi Hospital, Jember. The research method in this study uses a case study. The respondents and place in this study were Mr. T. with pleural effusion in the Catleya Room of RSD dr. Soebandi Jember. Semifowler position intervention was given for 3 days with a duration of 2 hours. The results of this study indicate that on the first day, there was a decrease in blood pressure, MAP, and respiratory rate. While the pulse rate and oxygen saturation increased. On the second and third day, of the application of the semifowler position, the results showed that there was a decrease in respiratory rate and an increase in pulse rate and oxygen saturation. While there were no hemodynamic changes in blood pressure and MAP. The conclusion of this study is that the implementation of the semi-Fowler position has an effect on hemodynamic status, especially on reducing respiratory rate and increasing pulse rate and oxygen saturation.

Abstract

Hydronephrosis occurs due to a disorder in the urinary tract that causes the flow of urine from the urinary tract to be obstructed, causing the dilation of the channels in the kidneys. The main symptoms that hydronephrosis patients commonly feel are pain in the lower back, nausea, and fatigue. Deep breath relaxation therapy, as a non-pharmacological therapy, has the effect of reducing pain. This study aims to determine the effect of deep breath relaxation techniques in patients with hydronephrosis and pain complaints. The method used in this study was a case study of one patient who was given an intervention for three days. The pain intensity scale was measured using the Numeric Rating Scale (NRS). This study showed a significant reduction in pain intensity in hydronephrosis patients from a scale of 6 to a scale of 3, where the deep breath relaxation technique intervention was given for three consecutive days for three sets per day in patients. Based on the results of the study, the researcher suggests the use of deep breath relaxation techniques as a non-pharmacological therapy in hydronephrosis patients with pain complaints in clinical practice, which can be provided by health workers, especially nurses or as patient-independent therapy because deep breath relaxation techniques are easy to do therapy and do not require additional tools and materials to do so.

Abstract

Dengue fever or hemorrhagic fever is an acute infectious disease that can be transmitted by the Aedes aegypti mosquito. The virus carried by the Aedes aegypti mosquito will attack cells in the liver, disrupting liver function and causing swelling and bleeding. Symptoms that can occur are high fever, muscle and joint pain, and rash. One of the nonpharmacological therapies is deep breath relaxation therapy to reduce pain. This study aims to analyze deep breath relaxation techniques to reduce pain. This research design used a case report approach.   The pain can decrease after being given deep breath relaxation therapy for 3 days. On the first day, on a scale of 7, it decreased to a scale of 2 on the third day of hospitalization. Applying deep breath relaxation techniques can help reduce pain intensity by relaxing skeletal muscles that experience spasms due to increased prostaglandins, so that vasodilation of blood vessels will increase blood flow that is spasmed and ischemic. This technique can help a person or patient control themselves when feeling emotional discomfort, physical stress, or pain. Deep breath relaxation techniques can be given to patients who experience pain as a nonpharmacological therapy. The application of this relaxation technique is proven to reduce pain intensity.

Abstract

Clean and Healthy Behavior (PHBS) is a behavior carried out on consciousness as a result of learning that makes a person. Problems of clean and healthy living behavior are most commonly found in groups, for instance, in the Training Houses.  This is often demonstrated by inappropriate environmental codes, including the availability of post-training health posts, the low level of knowledge of healthcare professionals such as managers and healthcare providers about clean and healthy living behaviour, and the high number of frequently occurring medical conditions such as scabies, diarrhea, and cold cough. This dedication to the community aims to provide knowledge and understanding of Clean and Healthy Living Behavior to the centurion so that there is a change in behavior and able to practice in his daily life in the Islamic boarding school Az Zahro Curahdami Bondowoso district. The method of implementation is done by means of giving education developed through viewing the public health podcast Primary Health Service Curahdami and is presented with questions and answers. Before conducting the education on Clean and Healthy Living Behavior, 75% of respondents had less knowledge, and 25% had sufficient knowledge about clean and healthy living behavior. Based on the results of observations showed that the activities of devotion to the community can improve the central knowledge about the behavior of a clean and healthy life through watching the Podcast Health Society Primary Health Service Curahdami district of Bondowoso.

Abstract

Peptic ulcer disease frequently precipitates acute pain and gastrointestinal hemorrhage, yet conventional analgesic use remains constrained by bleeding risks and mucosal irritation. This study evaluated the clinical effectiveness of integrating handgrip exercise and lavender aromatherapy into nursing care for pain reduction in a patient presenting with melena secondary to peptic ulcer disease. Employing a prospective single-patient case design, the intervention was administered over three consecutive days using standardized nursing protocols. Pain intensity was quantified using the Numerical Rating Scale (NRS) immediately before and after each 30-minute session, with continuous hemodynamic and clinical monitoring. Results demonstrated a progressive attenuation of pain intensity, with NRS scores declining from 7 to 4 across the intervention period, concurrent with improved physiological stability and enhanced patient-reported comfort. These outcomes suggest that the combined modalities operate through complementary neurophysiological pathways, leveraging diffuse noxious inhibitory controls and limbic-mediated autonomic regulation to modulate both sensory and affective pain dimensions. The findings underscore the clinical feasibility, safety, and analgesic potential of structured, nurse-delivered nonpharmacological interventions in acute gastrointestinal settings. Systematic integration of such multimodal protocols into routine nursing practice offers a viable strategy for optimizing pain management while minimizing pharmacological adverse effects. Further randomized controlled investigations are recommended to establish dose-response parameters, validate long-term efficacy, and develop standardized implementation guidelines for broader clinical adoption.
