Abstract

Background: Stroke-associated Pneumonia (SAP) is one of the complications that occur in stroke patients and is associated with worse clinical outcomes. Risk factors for SAP and its impact on discharge status remain unclear.Purpose: This study aimed to determine the factors influencing the incidence of stroke-related pneumonia in the hospital and the patient’s discharge status. Methods: A case-control study was conducted at a private hospital in Surabaya from January 2021 to 2022. Medical record data were reviewed to identify stroke patients with suspected or confirmed pneumonia, after which risk factors (age, gender, stroke type, length of stay, stroke frequency, dysphagia, diabetes mellitus, hypertension, COVID-19, and discharge status) associated with the occurrence of SAP were examined. We excluded diagnoses of diseases with clinical manifestations such as tuberculosis-like pneumonia, lung tumor, pulmonary edema, pulmonary embolism, pulmonary atelectasis, and incomplete or missing medical record data. Data analysis used the Chi-square test to identify the risk factors for stroke-associated pneumonia. Results: One hundred sixty-three stroke patients treated in all inpatient rooms were diagnosed with pneumonia; 52.8% of them had confirmed pneumonia, while the rest were still suspected of pneumonia. Most pneumonia occurs in ischemic stroke patients (93.9%), first-time stroke patients (87.1%), experienced dysphagia (55.2%), length of stay less than 7 days (55.2%), hypertension (55.2%), diabetes mellitus (57.1%), and discharged home (84%). We also found as much as 12.9% inpatient mortality due to SAP. Dysphagia, DM, and COVID-19 were significantly correlated with the incidence of stroke-associated pneumonia (p=0.000).Conclusion: Stroke-associated pneumonia is a substantial medical complication among stroke patients affected by dysphagia, DM, and COVID-19. Screening for dysphagia is necessary for nurses to prevent SAP in stroke patients.

Abstract

Family as an informal caregiver has a very important role in the care of stroke patients. Families do not have sufficient knowledge and skills in caring for stroke patients because they are not trained. Families actively involved in care will feel more prepared to face new responsibilities in caring for stroke patients. This community service activity aims to improve knowledge and behavior to prevent recurrent stroke. The method used in this community service activity is a participatory education approach, including educational methods using the stroke self-management education module. This activity was conducted in May 2024 at RW 07, Banyu Urip Village, Surabaya. It was attended by 20 families of stroke patients. Evaluation of knowledge using the recurrent stroke prevention knowledge questionnaire and behavior using the stroke patient care behavior questionnaire. The community service results showed that family caregivers' knowledge and behavior increased after the family empowerment program. Family empowerment can improve knowledge and behavior in family caregivers of stroke patients in preventing recurrent stroke. Family empowerment programs can be provided periodically by community nurses to improve the ability of family caregivers to improve the quality of life of stroke patients.

Abstract

Introduction: Transitional Care (TC) was developed to reduce hospital readmission and improve the physical ability of stroke patients. The purpose of this literature review is to identify the application of Transcultural Care to the readmission rate in stroke patients. Method: Article searches were conducted using the same keywords in the Pubmed, CINAHL, SAGE and Science Direct databases published from January 2016 to September 28, 2021. A total of eight articles were identified in this study. Results: Eight articles were reviewed regarding the results of applying TC to the readmission rate in stroke patients. All reviewed articles showed a decrease in the readmission rate after TC was applied compared to usual care. There was substantive heterogeneity in (1) TC team coordinator, (2) hospital/clinic to home TC process, (3) TC follow-up duration, (4) satisfaction. Conclusions: Several articles support the positive results of using TC on readmission rate before 30 days and satisfaction with nursing services in patients with stroke. Standardization of interventions and research designs are needed to determine the most effective interventions.

Abstract

Background: Early readmission in stroke survivors remains a critical issue, particularly in patients with mild initial symptoms but who experience early recurrence within a short period. Evaluating both patient and caregiver readiness for discharge is essential to improve home care outcomes and prevent avoidable hospitalizations. Objectives: This case study aims to assess discharge readiness in a post-stroke patient and their caregiver using the PATH-s instrument and examine its implications for readmission prevention. Methods: A descriptive single-case study was conducted involving a 66-year-old male with a secondary ischemic stroke within 30 days of the initial attack. Data were collected via clinical records and interviews. Discharge readiness was evaluated using the Preparedness Assessment for the Transition Home after Stroke (PATH-s) instrument, consisting of 25 items. The assessment was guided through a structured interview due to the patient's limited ability to self-complete the questionnaire. Results: The PATH-s assessment yielded a total score of 69 out of 100, indicating a moderate level of discharge readiness. The average score per item was 2.67 out of 4, suggesting a need for additional support in specific areas. The domains identified for improvement included financial capacity, home and transportation accessibility, long-term prognosis, and patient insight about stroke. Conclusion: Evaluating discharge readiness in the patient and family caregiver helped increase awareness of home care limitations and guided appropriate nursing interventions. These findings highlight the value of structured assessments like PATH-s in supporting clinical decision-making and individualized discharge planning.

Abstract

Background: Stroke remains one of the leading causes of long-term disability worldwide, with hemiparesis being the most common motor deficit. Hemiparesis on the dominant side, such as sinistra hemiparesis, greatly impairs daily function. Motor Imagery Training (MIT) is a cognitive-based therapeutic approach that stimulates motor-related cortical areas through mental rehearsal of movement, offering a non-invasive, low-cost intervention to promote neuroplasticity during stroke rehabilitation. Objectives: This case study aims to evaluate the short-term effects of a 4-day Motor Imagery Training intervention on a post-stroke patient with hemiparesis sinistra, focusing on motor function improvement and cognitive engagement. Methods: Mrs SA, a 66-year-old female with a history of uncontrolled hypertension and prior stroke, presented with left-side weakness, facial asymmetry. The patient underwent MIT sessions for four consecutive days (30 minutes/day), guided by auditory scripts and visual imagery techniques targeting upper limb function. Motor performance was evaluated using the Fugl-Meyer Assessment for Upper Extremity (FMA-UE) and the Medical Research Council (MRC) scale. Results: After four MIT sessions, the patient demonstrated improved motor strength (MRC 4-/5), increased FMA-UE score from 29 to 36, and enhanced focus and engagement. Subjectively, the patient reported increased motivation and perceived movement initiation. These findings suggest early cortical activation and functional gains, even within a limited intervention period. Conclusion: This case supports existing evidence on the effectiveness of MIT in promoting neurofunctional recovery in stroke rehabilitation. Despite its brief duration, MIT contributed meaningfully to motor recovery and psychological readiness. Motor imagery is feasible for early rehabilitation and can be tailored to settings with limited resources.
