Abstract

The JKN program in Pancur Batu District, Deli Serdang Regency, has not been running optimally, with a low number of participants utilizing health services. This is due to various factors, including access, service quality, and public understanding. This study aims to identify the relationship between factors that influence the utilization of health services in the area. The method used is a quantitative approach with a cross-sectional design, involving 97 respondents selected by simple random sampling. Data were collected through questionnaires and analyzed using chi-square analysis. The results showed a significant correlation between the use of JKN services and education level (p = 0.015), availability of facilities and infrastructure (p = 0.009), accessibility (p = 0.002), and complaints of illness (p = 0.011). In contrast, no significant correlation was found between employment status (p = 0.805) and salary (p = 0.622) with service utilization. The conclusion of this study is that increasing the utilization of the JKN program requires a holistic approach, which includes improving access to health facilities, improving service quality, and educating the public about the benefits of the JKN program.

Abstract

Anxiety is a problem characterized by refusing to be given nursing care, sleep disturbances, restlessness, worry, fear, crying easily, decreased appetite. The presence of family support can reduce the patient's anxiety level and influence the level of support they receive. The aim of this research is to identify the relationship between family support and the anxiety level of children suffering from chronic kidney failure aged 5-18 years at Haji Adam Malik Hospital. This type of research is correlation analysis with a Cross Sectional approach. The population in this study were families and children with GGK, the sample in this study was 73 people using a total sampling technique. The tool used to measure family support and the anxiety level of children with chronic kidney failure aged 5-18 years is a questionnaire. From the data, 18 people (24.7%) had good family support with anxiety in children, 10 people (13.7%) in the non-anxious category, 6 people in the mild anxiety category (8.2%), 2 people in the moderate anxiety category (2.7%). Of the 37 people (50.7%) who have sufficient family support, 5 people (6.8%) are not anxious in the child's anxiety category, 22 people are in the mild anxiety category (30.1%), 10 people are in the moderate anxiety category (13.7%). Of the 18 people (24.7%) who had less family support with anxiety in children in the non-anxious category, 3 people (4.1%), in the mild anxiety category, 5 people. It was concluded that there was a relationship between family support and the level of anxiety in children with CKD aged 5-18 years at Haji Adam Malik Hospital (p value = 0.005). It is hoped that the suggestions in this study can increase family support in reducing anxiety in children by providing religious education to children, always accompanying children in the treatment of chronic kidney failure. Abstract

The number of chronic kidney disease (CKD) patients undergoing hemodialysis in Indonesia continues to increase, putting great pressure on the national health financing system, especially the National Health Insurance (JKN). This study is a literature review that analyzes five scientific articles published between 2021 and 2024. Articles were selected through targeted searches in Google Scholar and PubMed, then synthesized narratively to describe trends and financing implications. The synthesis results show a significant increase in the number of hemodialysis patients each year, with therapy costs reaching trillions of rupiah. Payment rates set in the INA-CBGs system are often not in accordance with real costs in hospitals, risking a deficit in health facilities and BPJS Kesehatan. The increasing trend of hemodialysis patients places a large economic burden on the national health system. Cost control strategies are needed through primary prevention, early detection of CKD, adjustment of INA-CBGs rates, and development of alternative therapies to maintain service sustainability.

Abstract

Primary dysmenorrhea is common among adolescent girls and affects their quality of life and daily activities. Therefore, effective and safe treatments are needed. This study aims to evaluate the effectiveness of red ginger consumption in reducing the intensity of primary dysmenorrhea pain in adolescent girls. The research design used was a pre-experimental design with a one-group pretest-posttest design. The study was conducted at SMK "P" Batu City from September to December 2024, involving 32 adolescent female participants selected through a total sampling technique. The independent variable was the consumption of red ginger, 200 ml, twice daily (morning and evening), and the dependent variable was the intensity of dysmenorrhea pain. Pain levels were measured before and after the intervention using direct observation instruments. Data analysis was performed by calculating the frequency distribution and the Wilcoxon statistical test. The results showed that before the intervention, most respondents experienced moderate pain (59.4%) and severe pain (21.9%). After red ginger consumption, there was a significant decrease in pain intensity. On the first day of intervention, 68.6% had mild pain, and 25% had moderate pain. On the second day, 46.8% reported no pain, and 40.6% experienced only mild pain. Wilcoxon test results revealed a significant difference in pain intensity between the pre-intervention and post-intervention periods (p < 0.001). Thus, red ginger consumption is proven to be significantly effective in reducing primary dysmenorrhea pain intensity in adolescent girls.

Abstract

The counseling process requires engaging media to effectively enhance understanding and promote behavioral change within the target population. This study aimed to evaluate the impact of health counseling delivered through video and leaflet media on adolescents’ readiness to face menarche. A quasi-experimental design with a nonequivalent control group was employed. The study population consisted of students in grades 4 to 6 at SDN 101816, totaling 136 individuals. From this population, 60 respondents were selected and divided equally into two groups: one group (n = 30) received counseling via video media, while the other group (n = 30) received counseling through leaflets. Results indicated that adolescents' readiness to face menarche increased following health counseling with video media, with mean scores rising from 26.47 pre-intervention to 31.20 post-intervention, representing an improvement of 4.73 points. Statistical analysis confirmed a significant effect of video-based counseling on readiness (p < 0.001). Similarly, counseling using leaflet media resulted in an increase in readiness, with mean scores improving from 24.73 before intervention to 27.10 after, an increase of 2.34 points, also statistically significant (p < 0.001). Furthermore, comparison between the two groups revealed a significant difference in the effectiveness of video versus leaflet counseling approaches, favoring video media (p = 0.002). These findings suggest that health workers should prioritize the use of engaging media, such as videos, in health promotion efforts aimed at preparing adolescents for menarche.

Abstract

Dehydration poses a significant health risk to construction workers due to heavy physical work and exposure to hot working conditions. This study aimed to determine the prevalence of dehydration and identify associated risk factors among construction workers. A cross-section study was conducted involving 32 construction workers in a construction project for the construction of a wheat flour and soybean mill owned by PT. Agri First which operates at KIM 2 Medan. Data were collected using questionnaires, fingertip pulse oximeters, urine refractometers, and observation sheets. Dehydration is categorized based on the level of urine specific gravity (ultrasound). The data was statistically tested using the Fisher Exact test (α=0.05). The majority of workers (75%) were dehydrated. There was no significant relationship between dehydration and age. However, workers with more than 3 years of experience were more likely to be dehydrated compared to workers with less than 3 years of experience (OR = 8,333, 95% CI: 1,392-49,872). Workers with moderate workloads were also more likely to be dehydrated compared to workers with light workloads (OR = 18,333). There was a significant association between water intake and dehydration (p = 0.000). All workers who did not drink enough water were dehydrated, while only 66.7% of those who drank enough water were dehydrated. Workers with more than 3 years of experience, moderate workload, and inadequate water intake have a higher risk of becoming dehydrated. Interventions to improve hydration among construction workers are urgently needed.

Abstract

Pneumonia remains a leading cause of morbidity and mortality in children, especially those with weakened immunity due to factors such as insufficient breast milk, malnutrition, HIV or measles infection, premature birth, or incomplete immunizations. Environmental exposures—including dust, cigarette smoke, and air pollution further elevate the risk. This study aimed to systematically review literature (2019–2024) on the effectiveness of chest physiotherapy in improving clinical outcomes, specifically respiratory rate, oxygen saturation, length of hospital stay, and symptomatic recovery in children with pneumonia. We conducted electronic searches on Google Scholar, NCBI, and accredited Indonesian journals using the keywords “chest physiotherapy” and “pneumonia in children.” From approximately 60 initially identified articles, eight met our inclusion criteria and were reviewed in full. Most studies were quasi‑experimental; one was a randomized controlled trial. Study populations ranged from infants (0–5 years) to older children (6–12 years). Interventions comprised postural drainage, clapping, vibration, percussion, deep‑breathing exercises, and effective coughing techniques. Results from eight studies indicated modest improvements in oxygen saturation and reductions in respiratory rate in some small trials. However, findings were inconsistent, with substantial heterogeneity in methodology, intervention protocols, and outcome reporting. Overall evidence quality was low to moderate. Chest physiotherapy may offer potential benefits in terms of oxygenation and respiratory rate in pediatric pneumonia patients. Nevertheless, current evidence remains limited and heterogeneous. High‑quality randomized controlled trials using standardized protocols are needed to establish stronger recommendations
