Abstract

ABSTRACT Children with Autism Spectrum Disorder (ASD) often exhibit self-stimulation behaviors (stimming), such as repetitive movements, sounds, or actions, which can disrupt therapy sessions and social interactions. These behaviors can impede the development of social skills and focus. Given the importance of non-invasive interventions, modifying play-based activities such as LEGO and maze games (LEMA) has been explored as a potential method to reduce stimming behaviors. (LEMA) game on reducing self-stimulation (stimming) behaviors in children with Autism Spectrum Disorder (ASD). The research employed an A-B-A design, including a baseline phase (A1), an intervention phase (B), and a post-intervention phase (A2), conducted over 12 sessions. Two children, RFQ and HG, both of whom exhibited frequent stimming behaviors, were selected as participants. The intensity of their stimming behaviors was measured before and after the intervention using observational data. The data were analyzed using visual analysis to identify changes in the frequency and intensity of stimming behaviors across phases. Additionally, percentage of non-overlapping data (PND) was calculated to evaluate the effectiveness of the intervention data. The findings showed that stimming behaviors in both children, which originally lasted approximately 390 seconds during the baseline phase (A1), significantly decreased during the intervention phase (B), with durations reduced to 48-55 seconds. After the intervention was stopped, stimming behaviors remained reduced in the post-intervention baseline phase (A2), indicating the effectiveness of the LEMA game in reducing these behaviors. Modifying the LEGO and Maze (LEMA) game effectively reduced self-stimulation behaviors in children with ASD. The results align with previous studies suggesting that structured play interventions, like LEGO and maze activities, can enhance focus and reduce repetitive behaviors in children with ASD.

Abstract

Private nursing schools in South Kalimantan face increasing pressure to produce high-quality, ethically driven nursing professionals, aligning with Indonesia’s “Golden Vision 2045.” Within this context, the "Macbeth effect"—a psychological phenomenon where individuals experience guilt, anxiety, or a compulsion to "cleanse" themselves after confronting morally ambiguous situations—poses a challenge to ethical leadership. This study investigates the manifestation of the "Macbeth Effect"—the compulsion to morally cleanse oneself following ethically ambiguous situations—among leaders of private nursing schools in South Kalimantan, Indonesia. Using a qualitative case study approach, in-depth interviews were conducted with five school leaders to capture their experiences and perceptions of moral responsibility and ethical decision-making. Thematic analysis was employed to identify key themes, focusing on how the Macbeth Effect shapes leadership behavior, decision-making processes, and organizational dynamics within nursing education institutions. Findings reveal that leaders frequently exhibit moral cleansing behaviors, such as self-evaluation, transparency in decision-making, and using mediators to handle conflicts. While the Macbeth Effect enhances ethical leadership practices, it also introduces challenges that may impact mental well-being and leadership efficacy. Practical strategies, including promoting a culture of transparency, emotional support, and task delegation, are recommended to mitigate the psychological burden associated with moral cleansing. This research contributes to understanding ethical leadership in nursing education and highlights the need for support systems to sustain leaders’ mental resilience and ethical commitment.

Abstract

Hypertension is a major global challenge, contributing to the burden of heart disease, stroke and kidney failure, as well as causing early death and disability. This study was to determine the effect of progressive muscle relaxation on blood pressure stability in hypertension patients. The method used was a quasi-experimental pre-and post-test group design without a control group. The sample was 35 hypertension patients using a consecutive sampling technique data analysis using the paired t-test. Participants were excluded if they were receiving other treatments for hypertension or had musculoskeletal conditions, such as joint diseases or fractures, while those who had a history of hypertension within the previous 12 months, were not taking antihypertensive medication, and were not undergoing any other therapeutic interventions were included. Based on the results of the bivariate analysis, it was found that there was an effect of progressive muscle relaxation on the stability of blood pressure in hypertension patients (P value = 0.000 <0.05). Conclusions: Progressive muscle relaxation can reduce tension in the body, and increase the relaxation response. When the body feels the relaxation response, it will slow down the heart rate so that pumping blood throughout the body becomes effective and blood pressure decreases.

Abstract

Sleep disturbances are common among older adults, especially those residing in nursing homes, and can significantly affect their quality of life and overall well-being. Non-pharmacological interventions, such as guided imagery and virtual reality (VR), have shown promise in improving sleep quality by promoting relaxation and reducing cognitive arousal. The SI-VIGUR (Virtual Reality Guided Imagery) app combines VR technology with guided imagery to offer a therapeutic solution for improving sleep in older adults. This study aimed to evaluate the effectiveness of the SI-VIGUR VR app in improving sleep quality among older adults in nursing homes. This study aimed to evaluate the effectiveness of the Virtual Reality Guided Imagery (SI-VIGUR) application in improving sleep quality among older adults in nursing homes in Palembang City, South Sumatra, Indonesia, in 2024. Using a quasi-experimental design with pre- and post-tests, 80 participants aged 60 or older with sleep disturbances were selected. They received 15-minute SI-VIGUR sessions three times a week for 1 months. Sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI). Participants in the experimental group showed a significant improvement in sleep quality, as evidenced by a decrease in their PSQI scores (from 14.20 ± 1.86 to 7.31 ± 2.81). The intervention group demonstrated a marked reduction in poor sleep quality, with a higher percentage of individuals reporting fair or good sleep quality after the intervention compared to before. These findings were statistically significant (p = 0.0001), confirming the effectiveness of the SI-VIGUR app. The SI-VIGUR VR app proved to be an effective non-pharmacological intervention for improving sleep quality among older adults in nursing homes. The results suggest that VR-guided imagery can significantly reduce sleep disturbances, offering a promising solution for enhancing the well-being of older adults in institutional settings.

Abstract

Continuous Ambulatory Peritoneal Dialysis (CAPD) is an effective alternative for renal failure patients to undergo treatment with more independence and flexibility. However, the success of CAPD is highly dependent on the patient's skills and knowledge. Many patients lack sufficient information, which may hinder the effectiveness of the dialysis process. The Objectives of this study is to determine the best way to use digital interventions to enhance self-management capabilities in peritoneal dialysis patients. A systematic review following the PRISMA 2020 guidelines was conducted. Article selection used PICO. The research focused on peritoneal dialysis patients and how the use of applications to improve self-management skills. article searches in 2015-2024 in 4 databases, namely EBSCO, Proquest, Scopus, and Pubmed databases. MeSH was used in the medical domain for keyword search. Papers were evaluated for quality using the Jonna Briggs Institute (JBI) framework. There were 7 relevant papers found in the literature search. Implementing digital interventions in healthcare in peritoneal dialysis patients has been used worldwide. Digital interventions used include the use of mobile health text messages, websites and apps. Significant improvements in peritoneal dialysis-related knowledge as well as patient satisfaction are characteristic of most therapies. The resulting impact of digital interventions varies widely with the attitudes and decision-making of individuals undergoing peritoneal dialysis. Digital interventions present a new approach to enhancing self-management capabilities in patients with chronic kidney disease who use peritoneal dialysis. The use of technology can provide the necessary support to manage their condition effectively.

Abstract

Community knowledge in health empowerment strategies is a crucial element for the success of health programmes. However, many factors influence the level of public knowledge about a programme. This study aims to identify the relationship between demographic factors such as gender, age, education, and occupation, and the level of community knowledge about the Desa SIAGA programme as a community empowerment approach to improve maternal and neonatal health. This research employs a descriptive correlational design with a quantitative survey method. The study involved 107 respondents from Desa Jati, Sukoharjo District, using a simple random sampling technique. The data was collected through a questionnaire survey containing questions to explore the public's knowledge about the SIAGA Village program to improve maternal and neonatal health. The collected data was then analyzed using bivariate analysis and tested using the Pearson Product Moment. The results indicate that the majority of respondents have sufficient knowledge about the Desa SIAGA programme. Age and gender were found to have a significant relationship with knowledge level, with older age and female gender being associated with better knowledge. In contrast, no significant relationship was found between education level and occupation with the level of knowledge about the Desa SIAGA programme. These findings highlight the importance of more intensive outreach and community-based approaches to improve public understanding of the Desa SIAGA programme, particularly among older age groups and men.

Abstract

Art therapy has been increasingly recognized as an effective therapeutic intervention for managing various psychological issues, including anxiety, depression, and stress, especially in pediatric and geriatric populations. Collage-making, a form of art therapy, has shown promise in helping individuals express emotions, reduce anxiety, and improve mental well-being. Objective to explore the effects of art therapy using collage media on reducing anxiety in children with Thalassemia. A case study approach was employed with two subjects, one child and one elderly individual, who underwent three sessions of collage art therapy over the course of three days. Each session lasted 30 minutes and focused on creating collages that allowed participants to express their emotions and experiences. Anxiety levels were assessed using the Revised Children’s Manifest Anxiety Scale (RCMAS) for the child and a similar anxiety assessment tool for the elderly participant. The child showed a significant reduction in anxiety, with the RCMAS score decreasing from 24 (indicating moderate anxiety) to 8 (indicating no anxiety) after three sessions. The elderly participant also exhibited a decrease in anxiety levels, with the final anxiety score dropping from 25 to 10. Both participants reported feeling more relaxed and emotionally relieved after the sessions. Art therapy using collage media effectively reduced anxiety levels in both children and elderly individuals. The creative process of collage-making provided a safe and engaging outlet for emotional expression, helping to alleviate anxiety.

Abstract

Patients with chronic kidney failure undergoing hemodialysis often experience decreased ability to perform activities of daily living. Fatigue is a common symptom that could influence the reduction of these activities. This study aims to analyze the relationship between fatigue and the level of independence of daily living activities in chronic kidney failure patients undergoing hemodialysis. This study used a descriptive correlative design and a cross-sectional approach with a population of 175 hemodialysis patients, which then obtained a sample of 121 people using the accidental random sampling technique. Data was collected using the FACIT Fatigue Scale and Katz index questionnaires, while the correlation test used Rank Spearman. The results showed a significant positive relationship between fatigue and the level of independence of daily living activities with a p-value = 0.001. The higher the fatigue value, the higher the level of dependence of a person on performing daily living activities. Early identification is needed to assess the fatigue status experienced by hemodialysis patients as a strategy to increase independence, which ultimately impacts quality of life.

Abstract

Tobacco Excise Tax (TET) plays a strategic role in increasing state revenue, which can be utilized to strengthen the National Health Insurance (NHI) system. This study aims to analyze the impact of raising tobacco excise taxes on the national health economy and the sustainability of health financing. A Systematic Literature Review (SLR) approach was employed, reviewing 20 journal articles published between 2018 and 2023. These articles were selected through a systematic process based on inclusion criteria such as topic relevance, applied methodology, and geographic focus. The findings reveal that increasing tobacco excise taxes is an effective strategy for enhancing public health and supporting economic development. Revenue from tobacco excise can serve as a sustainable source of funding for health initiatives, including addressing the JKN deficit, which amounted to IDR 31.7 trillion (USD 2.2 billion) as of May 2020. This funding shortfall, exacerbated by the COVID-19 pandemic, underscores the urgent need for alternative solutions. Additional revenue generated through increased tobacco taxes could be allocated to reduce the deficit and expand health service coverage.By raising excise tax rates, the government can simultaneously increase state revenue and encourage healthier behavior by reducing tobacco consumption. Thus, this policy has a dual role in improving public health indicators while strengthening the financial stability of NHI. A synthesis of the 20 reviewed articles indicates that such policies, if implemented consistently, could significantly reduce the JKN deficit and support the long-term sustainability of the national health system.
