Abstract

The paradigm changing in health care form “quality” to “quality and safety” aims to improve the quality of life of patient. One of the efforts is to minimize the occurrence of incidents. The largest number of the patient safety incidence is Nearly Injury Incidence, which is 18.53%. The increased risk of NPC can be caused by their presence of polypharmacy. Polypharmacy is often experienced in patients at the Internal Medicine Polyclinic at Roemani Muhammadiyah Semarang Hospital. Therefore, it is necessary to conduct research to determine the number of KNCs in the prescription service of poly internal medicine patients in the Outpatient Pharmacy Installation of Roemani Muhammadiyah Hospital Semarang. The study was a descriptive observational study with prospective data from prescription services for patients with internal medicine at the Outpatient Pharmacy Installation of Roemani Muhammadiyah Hospital Semarang in March 2020. Evaluations were carried out at each stage of prescription service. Stages consist of prescribing, taking drugs, and preparing medicines etiquette, checking. Data were analyzed and grouped according to the type of KNC at each stage of prescription service for patients with internal medicine, the number and percentage of KNC on each prescription sheet. The results showed that there were 25 KNCs on 375 sheets of prescribed poly patients in internal medicine at Roemani Muhammadiyah Hospital Semarang. The percentage of KNC is the drug entry stage of 9 KNC (2.4%), the taking stage of 15 KNC (4%), the preparation stage of 1 KNC (0.27%).

Abstract

One of the medicinal chemicals (BKO) that is abused in herbs preparations for pain reliever is mefenamic acid. Mefenamic acid is a class of non-steroidal anti-inflammatory drug (NSAID). The addition of mefenamic acid in herbs medicine aims to increase the effect of pain relief, but can caused harmful effects, including digestive tract disorders, hemolytic anemia, and seizures. The purpose of this study is to determine the presence of mefenamic acid in pain reliever herbs circulating in Ngawi and online shop.The samples in this study were several brands of pain reliever herbs from 4 herbal medicine shops (A,B,C, and D). This research is using purposive sampling method based on inclusion criteria. Samples was analyzed qualitatively by thin layer chromatography (TLC) method using cyclohexane:chloroform: methanol:glacial acetic acid (60:30:5:5) as mobile phase and silica GF254 as stationary phase. Quantitive analysis using spectrophotometry UV-Vis method. The results showed that 10 of 16 samples of herbal pain reliever were positive for mefenamic acid with levels ranging from 1.28% -7.30%. The UV-Vis spectrophotometric analysis method suitable for the analysis of mefenamic acid in herbal medicine for pain reliever based on validation results that meet the requirements includes linearity with r value 0.99956, accuracy 8 ppm was 100.32%, 10 ppm was 100.84%, 12 ppm was 100.61%, precision 1.33%, LOD 0.8914299023 ppm, and LOQ 2.701302734 ppm.

Abstract

Anemia complicated by CKD occurs 80-90% due to deficiency of erythropoietin. Erythropoietin Stimulating Agent (ESA) therapy is the main choice at Ciawi Hospital. Chronic Kidney Failure (CKD) is the 7th 10th biggest disease in Ciawi Hospital. The aim of the study was to evaluate the success of Erythropoietin Stimulating Agent (ESA) treatment in Hemodialysis (HD) patients at Ciawi General Hospital. This research is descriptive in nature, collecting retrospective data from secondary data from medical records, laboratory data from Hb patients. Sampling was taken using the total sampling method, namely all outpatient Hemodialysis (HD) patients at the Ciawi Hospital for the period October-December 2021 who met the inclusion and exclusion criteria totaling 47 patients. The results of this study show the sociodemographic picture of patients with Chronic Kidney Failure (CKD) with anemia complications, the most age range being 46-55 years, 18 patients (38.30%), the most sex being male, 25 patients (53.19%). The description of the duration of Hemodialysis (HD) therapy is that the most ? 12 months 27 patients (57.45%). The description of the highest frequency of ESA administration was at a frequency of 2 times a month with the Hb category increasing by 21 patients (63.6%) and the Hb category decreasing by 12 patients (36.4%). There is no relationship between the frequency of Erythropoietin Stimulating Agent (ESA) therapy with an increase in hemoglobin values in patients with chronic kidney failure undergoing hemodialysis and suffering from anemia as indicated by a significance value (P-value) of 0.659 > 0.05.

Abstract

The high production of durian fruit gives an environmental problem if it is not used properly. Durian fruit skin contains flavonoids, phenolic compounds, saponins, and tannins which have antioxidant activity. Its antioxidant activity makes durian husk a candidate as a basic ingredient for natural cosmetics. Producing a formulation of scrub preparations as natural cosmetics based on the results of the characteristic test was the purpose in this research. Experimental research with a post-test design technique was carried out by making the changes of the concentration of durian husk as an active ingredient in the scrub preparations used in this study. The form of an oil-in-water cream scrub was used as the formulation in a change of concentration range of 1% (F1), 1.5% (F2), and 3% (F3) for each formula. Furthermore, the characteristic test was carried out which included organoleptic test, homogeneity test. pH test, viscosity test, spreadability test, and adhesion test. In this research also used statistica tests. The results showed that the best formulation is obtained from formulation 3 (F3) with a concentration of 3% durian rind. The characteristic test results include: stable organoleptic test for storage in terms of shape, color and odor; homogeneity test formed granules and there was no separation between the oil and water phases; Stable pH test according to skin pH at 5.05; spreadability test 7.3 cm; viscosity test 64 dPa.s; and 2.41 minutes of adhesion test. It pointed that all formulations fulfilled the requirements for topical scrubs in each characteristic test.

Abstract

Therapeutic Drug Monitoring (TDM) is a clinical pharmacy service aimed at increasing the effectiveness of therapy and improving patient safety. TDM is widely used in the use of drugs with a narrow therapeutic index or can cause toxic effects in small dose changes, such as theophylline. Theophylline is a xanthine class of drugs which is still often used in Indonesia and has a bronchodilatory effect so that it can be used in the treatment of asthma or chronic obstructive pulmonary disease (COPD). This drug has a narrow therapeutic index with a therapeutic dose of between 10-20 mg/L, concentrations above 20 mg/L can cause dangerous side effects such as nausea, vomiting, metabolic acidosis, gastrointestinal bleeding, and even death. Theophylline is one of the drugs that needs to be monitored related to the improvement of symptoms and drug levels in plasma. Therefore, the purpose of writing this article is to look at theophylline monitoring that has been carried out in Indonesia with a literature study approach using inclusion and exclusion criteria. The results obtained are that there are still a few hospitals that carry out therapy drug monitoring related to theophylline levels in the blood.

Abstract

Metabolic syndrome is an increasingly prevalent health problem in both developed and developing countries, including Indonesia. The treatment of metabolic syndrome often involves polypharmacy, which increases the potential for drug interactions. Drug interactions are considered clinically important if they can cause increased toxicity or reduce the therapeutic effects of drugs, leading to a failure to achieve the desired therapeutic effect. Reported cases of drug interactions are more common in elderly patients. This study aims to determine the potential Drug-drug Interactions (DDIs) in elderly patients at risk of metabolic syndrome at Puskesmas Aikmel. The study uses a descriptive observational method with prospective data collection. The population in this study consists of outpatient patients at Puskesmas Aikmel from May to July 2021, meeting the inclusion criteria: patients diagnosed at risk of metabolic syndrome, elderly patients (age ? 60 years), and using a minimum of 2 drugs. Sampling was done using total sampling technique. The results of the study indicate 99 cases of potential drug interactions identified in the prescription of elderly patients. The drug classes with potential interactions include antihypertensive drugs in 51 (51.51%) cases, antihyperglycemic drugs in 42 (42.42%) cases, and dyslipidemia drugs in 6 (6.06%) cases. The types of drug interactions observed were pharmacokinetic interactions in 37 (37.37%) cases, pharmacodynamic interactions in 58 (58.58%) cases, and unknown interactions in 4 (4.04%) cases. The severity of these potential interactions was classified as minor in 5 (5.05%) cases, moderate in 86 (86.87%) cases, and major in 8 (8.08%) cases.

Abstract

Blood disorder caused by unbalanced globin synthesis resulting in damage to hemoglobin and recessive inheritance, handling do regular blood transfusions. Repeated blood transfusions in the long term lead to the accumulation of iron in the body. The purpose of this study was to determine the content and of mangiferin activity extract M. indica L leaf as an iron chelating in vitro. indica L leaf extract obtained by maceration method with 96% ethanol. Testing mangiferin content by TLC methods and iron chelating effectiveness in vitro using methods Ferrous ion chelating (FIC), the principle of the method is to measure the ability of a compound to compete with ferozine the metal chelate iron. Identification of TLC revealed that there is content mangiferin on leaf extract M indica L, by the similarity of color patches and Rf extracts and mangiferin, by using three types of the mobile phase is n-hexane: ethyl acetate: water (6: 3: 1) with Rf 0.1, ethyl acetate: formic acid: water (8: 2: 1) with Rf of 0.34 and methanol: chloroform: water (7: 2: 1) with Rf 0.8, Test result the ability of the metal chelate extract M. indica L leaf of ethanol has an iron chelating effect as in vitro methods FIC. Concentration higher of the iron chelating higher percentage. Extract at concentrations of 1, 10, 20, 30 and 40 ug / ml respectively have iron chelating percentage 6,44%, 29,87%, 58,85%, 75,74% and 92,99%, with values (IC50) it is 18,73 µg/ml.

Abstract

The research about isolation and antifungal activity test of ethanol extract of Punica granatum L peel towards Candida albicans’ has been done. The objectives of this study are to determine any secondary metabolite compounds that can be isolated from Punica granatum L peel, to determine antifungal activity of Punica granatum L peel extract and to determine the most effective concentration to inhibit Candida albicans’ growth. Based on phytochemical test result, it can be concluded that secondary metabolite compounds contained in Punica granatum L peel extract are flavonoide and tannin. The process was continued by TLC and PTLC. Based on TLC process, eluent with best separation was methanol:ethyl acetate (3:1) with 0,44 and 0,51 as the Rf values. The eluent from the following TLC was used for PTLC process. PTLC results showed one round spots, the yellow one with Rf value = 0,77. The following process was antifungal activity test using agar diffusion method and Candida albicans was used for this purpose. The antifungal testing process was done by varying the concentration of Punica granatum L peel extract to 10%, 20%, 40% and 50%. The result showed that the four concentrations can inhibit the Candida albicans’ growth, indicated by transparent zone surrounding the concentration. One way Anova results Fcount> Ftable = 116,709727> 5,064, it indicates that there is significant influence of various concentration to resistivity zone diameter. The process continued by analyzing the most effective concentration using multiple Tukey comparative test with 1% significance level. The result showed that the most effective concentration to inhibit Candida albicans’ growth was 50% Punica granatum L peel extract with its resistivity capacity is 22,26 mm.

Abstract

Curcumin was reported can suppress the growth of blood tumor cells. This study aims to determine the physical and chemical properties of curcumin derivatives that give affect to antitumor activity of blood cells which were expressed as the best HKSA equation model. This   study   used   curcumin   and   twelve   derivatives   which   have   antitumor   activity parameters of blood cells (K562) as IC50. Parameters of physicochemical properties of curcumin compounds series and their derivatives as descriptors include net charge of atoms in the curcumin framework structure, EHOMO, ELUMO, Log P, E Hid, a, MR, Volume, Surface area (SA), Total Energy, dipole moment (u), Hf, and Mass which are the results of geometric optimization calculations using the PM3 semiempirical method with hyperchem application. HKSA equation model was built using the BuilQSAR statistical calculation analysis. The best HKSA equation model is then validated internally and externally. Calculation of statistical analysis with BuldQSAR, the descriptor that has the most influence is  obtained, namely net  atomic charge, EHOMO,  surface area, dipole moment and  Hf with  linear regression equation: Log 1/IC50 : + 1.7622 (± 0.3655) qC4 - 0.9457 (± 0.1047) E Homo + 0.0013 (0.0002) SA + 0.0575 (± 0.0216) u - 0.0068 (± 0.0004) Hf - 11.4161 (± 1.0440) , n = 10; R = 0.999; R2= 0,9990; SPress = 0.0232. as well as non-linear regression equation : Log 1/IC50 = + 15.1400 (±4.9008) qC7 + 7.4959 (± 5.6081) qC8 - 0.5839 (± 0.1397) qC15 + 0.0005 (± 0.0003) SA - 0.0054 (±0.0008) Hf - 5.7246 (±3.2476), n = 10; R = 0,999, R2 = 0,9977; SPress = 0.0341.

Abstract

Standardization is needed to obtain uniform raw materials that can guarantee the pharmacological effects of singkil plants. Phytochemical screening is the initial stage in a phytochemical research that aims to provide an overview of the class of compounds contained in the plant under study. The purpose of this study was to determine the standardization profile and class of chemical compounds in the ethanol extract of singkil leaves. This type of experimental research. Singkil leaf extraction was carried out by maceration method with 70% ethanol solvent, then followed by standardization test of singkil leaf ethanol extract. The next step is testing phytochemical compounds by identifying alkaloids, flavonoids, saponins, tannins, triterpenoids/steroids through phytochemical screening which are analyzed descriptively.The standardization results showed that the ethanol extract of singkil leaves had a dry shrinkage of 3.43%±1.28% (w/w), the extract content was soluble in water in singkil leaf powder was 20.01%±0.22% (v/w), the water soluble extract content of singkil leaf extract was 19.81 ± 0.22% (v/w), the ethanol soluble extract content of singkil leaf powder was 24.48 ± 0.24 %(v/w), ethanol soluble extract content of singkil leaf extract was 26.88 ± 1.16 %(v/w), specific gravity of singkil leaf extract was 0.91 ± 0.00 %(v/v), and ash content of singkil leaf extract. was 30.40 ± 0.00 %(w/w).
