Abstract

Diarrhea is a health problem that most often occurs in children. Various etiologies of diarrhea can cause prolonged diarrhea and become malnourished (malnutrition). The etiology of diarrhea can be caused by a bacterial infection and requires antibiotics, so that detection is needed as early as possible. Examination of soluble triggering expressed receptors on myeloid cells-1 (sTREM-1) can predict bacterial infection in children with acute diarrhea. The study aims to know how much the diagnostic value of fecal leukocytes test compared to sTREM-1. A cross-sectional study of children aged 6-60 months with acute diarrhea without complications and other diseases. In the subjects, clinical manifestation was performed (fever, vomiting, abdominal pain), fecal leukocyte test and sTREM-1 test as a standard reference. Children, aged 6-60 months with acute diarrhea due to bacterial infections with sTREM-1 > 470 pg/mL as many as 2 of 64 subjects and fecal leukocytes > 10/HPF as many as 14 of 64 subjects, most male, normal nutritional status and had clinical manifestation of fever, vomiting and without abdominal pain. Increased fecal leukocytes > 10 / HPF has a sensitivity of 50%, specificity 79.1%, positive predictive value 7.1%, negative predictive value 98%, accuracy 78%, positive likelihood ratio 2.18 and negative likelihood ratio 0.63. Fecal leukocyte test > 10/HPF as a diagnostic confirmation is not good in diagnosing acute diarrhea due to bacterial infection.

Abstract

Tuberculosis is a chronic infectious disease prevalent in Indonesia caused by Mycobacterium tuberculosis. It primarily affects the lungs but can also invade other organs. It is essential to gather information on the patient's medical history, close contact with TB patients, environmental conditions, and occupation as a healthcare worker to assess the risk of infection transmission. Pulmonary TB can potentially relapse and lead to post-tuberculosis chronic lung disease, which can negatively impact the patient's prognosis and quality of life. If left untreated, it may also result in heart-lung disease, causing disruptions in cardiorespiratory function. Proper treatment is crucial to effectively manage TB and prevent complications.

Abstract

Prostate cancer remains one of the most prevalent cancers globally, with traditional treatments like surgery, chemotherapy, and radiation often causing significant side effects and lengthy recovery. This study explores emerging non-invasive therapies—such as androgen-receptor pathway inhibitors (ARPI), radioligand therapy with lutetium, immunotherapy, high-intensity focused ultrasound (HIFU), and cryoablation—evaluating their efficacy, side effects, progression-free survival (PFS), overall survival (OS), and tumor reduction. A systematic review of recent studies revealed that ARPIs and lutetium-based radioligand therapy significantly improve PFS and OS in metastatic cases. Meanwhile, HIFU and cryoablation show promise in localized prostate cancer, offering effective tumor control with shorter recovery and fewer long-term complications than surgery. Immunotherapy, notably immune checkpoint inhibitors, demonstrated efficacy in select patient groups but posed risks of immune-related adverse events. While these therapies highlight a shift toward personalized treatment—leveraging genomic and clinical markers to optimize patient care—long-term data on their outcomes and side effects remain limited. This review underscores the potential of non-invasive therapies to transform prostate cancer management by enhancing quality of life and reducing treatment burdens. However, extensive research and clinical trials are essential to validate these approaches as standard care.

Abstract

Children with Anorectal Malformations (ARM) may experience lifelong physical impairment if not treated properly, and may adversely affect the personal and economic existence of affected individuals, as well as families and communities. The purpose of this study was to determine the factors of the incidence of anorectal malformations in neonates from maternal aspects and fetal aspects. By using research methods in the form of literature studies from the search results of 62 findings and 38 literature based on Electronic Based. The results of the discussion state that maternal risk factors that contribute to the incidence of ARM include exposure to teratogens such as cigarettes, alcohol, pesticides, drugs, and radiation during pregnancy. And fetal risk factors that contribute to the incidence of ARM include genetic and chromosomal factors, including related syndromes such as Down syndrome and VACTERL, which are the main causes of ARM.

Abstract

Women of Reproductive Age (WRA) as prospective mothers are a vulnerable group whose health status, particularly nutritional status, requires attention. The quality of future generations is determined by the mother's condition before pregnancy, as malnutrition during the preconception period can lead to both maternal and fetal malnutrition. This study aims to analyze the factors associated with preconceptions of nutritional readiness among WRA in Kerinci Regency. The study uses an analytical design with a cross-sectional approach. It was conducted at 5 Religious Affairs Offices in Kerinci Regency (The Office of Religious Affairs of East Air Hangat, Depati Tujuh, Sitinjau Laut, Danau Kerinci, and Keliling Danau) from 1 November to 30 November 2023. The sample consisted of 80 pre-marriage WRA registrants. Data were collected using questionnaires and analyzed using Chi-Square tests and Multiple Logistic Regression. The results showed significant associations between age (p-value 0.645), attitudes (p-value 0.005), occupation (p-value 0.006), physical activity (p-value 0.449), lifestyle (p-value 0.001), income (p-value 0.002), culture (p-value 0.018), food access (p-value 0.004), and the role of health workers (p-value 0.015) with preconceptional nutritional readiness. Multivariate analysis indicated that the most dominant factor was lifestyle, with an odds ratio (OR) of 17.75. In conclusion, the study found significant associations between various factors and preconceptions of nutritional readiness, with lifestyle being the most influential. Cross-sectoral cooperation and education are needed to intervene during the preconception period to prepare women for a healthy pregnancy and a positive pregnancy outcome.

Abstract

Hypertensive disorders during pregnancy, particularly preeclampsia, are a leading cause of maternal and perinatal mortality worldwide. Preeclampsia can progress into severe complications such as eclampsia and HELLP syndrome. Factors such as age, pregnancy history, and body mass index (BMI) are known to play a role in the occurrence of this condition. This study was conducted at RSUD Haji Makassar to identify determinant factors influencing the occurrence of preeclampsia in pregnant women. This study used a descriptive-analytical approach with a retrospective design, where secondary data were obtained from medical records of patients diagnosed with preeclampsia during a specific period (year 2024). Analysis was performed on factors influencing the incidence of preeclampsia in pregnant women at RSUD Haji Makassar. Age, parity, BMI, and height significantly influenced the occurrence of preeclampsia. Older age and higher parity increased the risk of preeclampsia. Higher BMI and shorter height were also associated with higher blood pressure, which worsened the condition. However, gravida and weight did not show significant effects. Age, parity, BMI, and height are significant determinants of preeclampsia. Management and prevention of preeclampsia should consider these factors to improve maternal and fetal safety during pregnancy.

Abstract

Nasal septal deviation is a common anatomical abnormality found in the general population and is thought to play a role in the pathogenesis of sinusitis. Some types of nasal septal deviation can impair sinus drainage and airflow, leading to inflammation and infection. However, there is no consensus on which type of septal deviation contributes most to sinusitis. Objective: This literature review aims to analyze and identify the types of nasal septal deviation that are most frequently associated with sinusitis. Methods: The data review was conducted using the PRISMA method, with data searches from PubMed, Google Scholar, ScienceDirect and Wiley with research data criteria between 2020-2024. The keywords used to search for data were "Type", "Nasal Septal Deviation", and "Sinusitis". A total of 23,943 articles were screened, with the final result being 20 articles that met the criteria for review. Results: Based on the review of the review in this study, nasal septal deviation (SND), especially types 3 and 5 according to the Mladina classification, contributes to chronic sinusitis through impaired ventilation and sinus drainage. Discussion : Nasal septal deviation (SND) is an anatomical abnormality that can interfere with airflow and sinus drainage, thus playing a role in the pathogenesis of sinusitis, especially types 3 and 5 according to the Mladina classification. These two types are often associated with osteomeatal complex obstruction and chronic mucosal irritation. The mechanisms involved include local hypoxia, mucociliary disorders, chronic inflammation, and changes in the microbiome.

Abstract

Procurement of laboratory equipment through operational cooperation is increasingly adopted by hospitals as a cost-effective and efficient strategy to support health services. This study examines the success of operational cooperation in procuring hematology analyzer equipment to replace damaged units, using a cost-benefit analysis (CBA) from the perspective of hospital management. Employing a sequential explanatory mixed-method approach, the study combined qualitative data from in-depth interviews with 12 purposively selected informants and observations, with quantitative analysis of secondary data using the CBA method. The analysis revealed a positive Net Present Value (NPV) of IDR 99,422,367.47 and a Benefit-Cost (B/C) ratio of 1.59, both exceeding the benchmarks of independent procurement. This indicates that operational cooperation is financially feasible and preferable. Non-financial benefits also strengthen this conclusion, including vendor-provided maintenance technicians, guaranteed supply of reagents and spare parts, and ongoing equipment upgrades aligned with technological advancements. These advantages minimize hospital disruptions and ensure service continuity. The study identifies key success determinants in the operational cooperation system: cost efficiency, human resources, regulatory compliance, vendor partnerships, equipment quality, historical laboratory data, and alignment with technological developments. In conclusion, operational cooperation for hematology analyzers is successful, sustainable, and offers significant benefits, both financial and operational, for hospitals.

Abstract

Diabetes Mellitus (DM) type II is a metabolic disorder caused by insulin resistance in muscle and liver cells, as well as pancreatic beta cell failure. Diabetic retinopathy is one of the diseases that is a microvascular complication of the retina due to chronic hyperglycemia in patients with Type II DM. Chronic hyperglycemia in patients with type 2 DM can change the physiology and biochemistry of cells, resulting in endothelial damage. In this condition, various biochemical pathways are activated, and this affects the occurrence of 4 pathophysiologies of diabetic retinopathy. Several risk factors suspected of triggering diabetic retinopathy are age, duration of disease, poor blood sugar control or hyperglycemia and blood pressure, dyslipidemia, hyperviscosity, kidney failure, anemia, and smoking. Blood sugar control is the most dominant risk factor for diabetic retinopathy among other factors in patients with type II DM. Based on the background, the author is interested in conducting research on risk factors that influence the incidence of diabetic retinopathy. The purpose of this study was to determine the risk factors that influence the incidence of diabetic retinopathy. The research conducted was a Literature Review with a Narrative Review design. Based on the search results, 7 relevant articles were obtained for use in this Narrative Review. The conclusion is that the risk factors suspected of causing diabetic retinopathy include the duration of diabetes mellitus, uncontrolled blood sugar levels, and Type II DM patients with hypertension or dyslipidemia.

Abstract

This study aims to analyze the effectiveness of using X-ray technology as a supporting tool in the forensic autopsy process. The method used is qualitative with a literature study approach that includes various scientific sources such as journals, books, and medical documents. The results showed that X-ray plays an important role in modern autopsy, especially as a non-invasive method to detect foreign bodies, fractures, and internal trauma that are not visible to the naked eye. In addition, technological developments such as postmortem CT (PMCT) and virtual autopsy (virtopsy) utilizing X-ray have also further expanded the application of imaging in the autopsy process, providing a three-dimensional view of the body without the need to perform surgery. Literature studies show that X-ray is effective in detecting internal lesions and foreign bodies, and aids in victim identification. However, this technique has limitations in visualizing soft tissue and microscopic abnormalities, so it should be used as a complement to traditional autopsy. In conclusion, the use of X-ray in autopsy improves diagnostic efficiency and accuracy, but should be combined with other imaging techniques for more comprehensive results.
