Abstract

Introduction: Nosocomial infections remain a major patient safety issue, particularly in Emergency Departments (ED) with high patient flow. Hand hygiene is the most effective preventive measure; however, compliance among healthcare workers and patient families at the Emergency Department of Hospital Nacional Guido Valadares (HNGV) remains low due to limited facilities, inadequate education, and high workload. Objective: To improve hand hygiene compliance among healthcare workers and patient families through a multimodal intervention as part of nosocomial infection prevention efforts in the ED of HNGV. Method: This residency program with anylisis fishbone, SWOT, USG are implemented assessment, intervention, and evaluation stages. Compliance was measured using direct observation based on the WHO Five Moments before and after intervention. Interventions included structured education, improved hand hygiene facilities, visual reminders, and audit-feedback mechanisms. Results: Pre-intervention compliance was 48% among healthcare workers and 42% among patient families, with an overall rate of 45%. Post-intervention results showed significant improvement: healthcare worker compliance increased to 82%, family compliance to 76%, and overall compliance reached 79%. The greatest improvement occurred in the moments before patient contact and after touching patient surroundings. Conclusion: The multimodal intervention effectively enhanced hand hygiene compliance in the ED of HNGV. Sustained improvement requires ongoing education, adequate facilities, and regular monitoring to support a strong patient safety culture and reduce nosocomial infection risks.

Abstract

Introduction: Hypertension is one of the most prevalent non-communicable diseases among older adults, and low health literacy often becomes a major barrier to effective self-management. Health education plays an important role in improving the knowledge and ability of older adults to control blood pressure and prevent complications.  Objective: This activity aimed to evaluate the effectiveness of health education in improving hypertension self-management knowledge among older adults in Babakan Anyar Village, Kadipaten.  Methods: A pre-experimental one-group pre-test–post-test design was used, involving 32 older adults with hypertension identified through screening. The intervention consisted of a 60-minute educational session delivered through interactive lectures, discussions, and visual media covering low-salt diets, physical activity, medication adherence, blood pressure monitoring, and warning signs of hypertension. Knowledge was measured using a questionnaire before and after the session.  Results: Findings showed a significant improvement in knowledge levels, with participants categorized as having good knowledge increasing from 31% at pre-test to 78% at post-test. Wilcoxon analysis indicated a statistically significant difference between pre-test and post-test scores (z = –4.064; p < .001).  Conclusion: These results demonstrate that structured health education is effective in enhancing older adults’ knowledge regarding hypertension self-management. Similar programs are recommended to be implemented regularly as part of community-based efforts to empower older adults and improve hypertension control.

Abstract

Introduction: High-quality nursing services are critical to hospital performance; however, they are frequently constrained by imbalances between nurse availability, competency, and workload. Simpang Lima Gumul Kediri Regional Hospital experiences such challenges, as indicated by excessive workload conditions in the Gelatik Ward and potential fatigue risks in the Perkutut Ward. Additionally, administrative issues arise from nurses’ limited awareness of the validity periods of their Work Plan Requirements (RKK) and Practice Permits (SIP), which can lead to temporary deactivation from clinical duties and reassignment to non-clinical tasks, thereby reducing effective nursing capacity. Objective: This community service program aims to develop and implement a digital system to support nursing workforce management by addressing staffing workload gaps and improving administrative compliance related to RKK and SIP validity. Methods: The program employed the introduction and implementation of E-PROLIC (Electronic Professional License), a digital platform designed to monitor the validity status of nurses’ RKK and SIP. The system integrates administrative tracking with workload analysis using the Workload Indicator of Staffing Need (WISN) method to support staffing planning and prevent permit expiration. Results: The WISN analysis showed that the Gelatik Ward requires 20.21 nurses but has only 12 available, resulting in a WISN Ratio of 0.59, which indicates an excessive workload. In contrast, the Perkutut Ward requires 12.78 nurses and has 13 available, yielding a WISN Ratio of 1.00, indicating adequate staffing levels. Conclusion: The implementation of E-PROLIC provides a digital mechanism for monitoring RKK and SIP validity while supporting workload-based staffing analysis, thereby contributing to improved administrative management and nursing workforce planning.

Abstract

Introduction: Education about HIV/AIDS is a crucial component in strengthening prevention and control efforts within Voluntary Counseling and Testing (VCT) services. Observations in the VCT Clinic of Elim Hospital Rantepao revealed that education was mainly delivered verbally without routine use of audiovisual media, resulting in less effective engagement and comprehension among clients. Objective: This community service activity aimed to improve clients’ knowledge about HIV/AIDS through the implementation of educational video media at the VCT Clinic of Elim Hospital Rantepao. Methods: The activity applied an educational and participatory approach including a 4-minute HIV/AIDS video covering definition, transmission, prevention, VCT testing, ARV therapy, and stigma reduction. Knowledge was measured using a pre-test and post-test questionnaire consisting of 10 items. Situation analysis was also performed using Fishbone analysis to identify root causes of educational gaps, SWOT analysis to map potential implementation strategies, and USG (Urgency, Seriousness, Growth) analysis to determine priority issues. A total of 20 clients participated (60% female; aged 17–45 years). Results: The intervention led to a significant increase in clients’ knowledge, with the mean score rising from 6.0 in the pre-test to 8.7 in the post-test, reflecting a 23.75% improvement. Positive shifts were also observed in attitudes toward PLWHA, with a 20% increase in attitude-related indicators. Client satisfaction was high, with 95% reporting satisfied or very satisfied experiences. Fishbone and USG analysis identified the lack of audiovisual educational media as the top priority problem (score 14), while SWOT analysis strengthened the feasibility of sustainable implementation based on service support and community opportunities. Conclusion: Educational videos are proven to be an effective and well-accepted health promotion tool to improve HIV/AIDS knowledge and reduce stigma among VCT clinic clients. This intervention is recommended for continuous use as an innovative strategy to enhance the quality of health education and support HIV/AIDS prevention efforts in the community.

Abstract

Introduction: Stunting, a chronic nutritional problem caused by lacking nutritional intake over a prolonged period, particulary during the First 1000 Days of Life (1000 HPK), remains a significant challenge in Indonesia. In Bogor Regency, the prevalence reaches 18.9%, largely due to the low consumption of animal protein. Objective:  This community service points to increase the knowledge and awareness of the community in Sukaharja Village, Bogor Regency, regarding the importance of nutritional fulfillment by promoting the Fish Eating Movement (GEMARIKAN) to prevent stunting. Method: Involving 25 participants (mothers/women in reproductive age), the activity utilized a four-stage process (Planning, Preparation, Implementation, Evaluation). The method includes interactive nutrition education (presentations/videos) assessed by pre/post-tests, supplemented by a cooking demonstration (Pepes Ikan Lele Kemangi) and recipe book distribution. Result: The results of the quantitative evaluation demonstrated a significant increase in the respondents' nutritional knowledge scores after the intervention. Although an improvement in attitude was observed, the changes were not significant. The program successfully met its participant target and fully documented all activities. Conclusion: This program is a vital intervention that tackles high stunting rates by leveraging catfish, a local food source. This by delivering targeted education that successfully boosts knowledge, it is expected to positively influence consumption behaviors and mitigate stunting risk.
