Abstract

Typhoid fever is an acute infection of the digestive tract caused by Salmonella typhi bacteria. The main therapy for typhoid fever is the administration of antibiotics. The high incidence of typhoid fever in Indonesia has led to increased antibiotic use and triggered antibiotic resistance. Antibiotic resistance will cause various problems such as incurability of the disease, suprainfection, increased risk of side effects, mortality, morbidity and health costs. This study aims to determine the pattern of antibiotic use and evaluate the rationality of antibiotic use in typhoid fever patients at RSUD Dr. Soedono Madiun qualitatively using the Gyssens method. This study uses a qualitative descriptive method by collecting patient medical records retrospectively with a purposive sampling method. The obtained medical records will be analyzed qualitatively using Gyssens criteria. The results of the qualitative analysis show that from 57 medical records there are 14 types of antibiotics used with thiamphenicol as the most common antibiotic (22%). The results of the qualitative analysis are antibiotic prescriptions for typhoid fever at Dr. Soedono Madiun Regional General Hospital. Soedono Madiun is in accordance with 72% accuracy, namely category IV A as many as 9 (15%), category IV B as many as 2 (4%), category IV D as many as 5 (9%), category III B as many as 9 (15%), category II A as many as 1 (2%), category II B as many as 4 (7%), category I as many as 1 (2%) and category 0 as many as 26 (46%).

Abstract

Populasi lanjut usia (lansia) meningkat secara global, dengan prevalensi kondisi multi-morbiditas yang memengaruhi pengelolaan obat. Penelitian ini bertujuan mengidentifikasi polifarmasi pada pasien geriatri di Poliklinik Penyakit Dalam RS Islam PKU Muhammadiyah Palangka Raya. Penelitian deskriptif kuantitatif ini menggunakan data sekunder dari e-rekam medis pasien geriatri periode Juni–Desember 2024. Data yang dikumpulkan mencakup usia, jenis kelamin, diagnosis, dan jumlah obat yang diberikan. Hasil menunjukkan 63 pasien geriatri, terdiri dari 29 laki-laki dan 34 perempuan, semuanya memenuhi kriteria polifarmasi dengan lebih dari 5 jenis obat per resep. Sebanyak 80,95% pasien menerima lebih dari 10 jenis obat selama perawatan. Temuan ini mengindikasikan perlunya manajemen terapi obat yang lebih hati-hati untuk meminimalkan risiko efek samping dan interaksi obat.

Abstract

 Hypertension increases with age, especially in the elderly. This study aims to determine the relationship between family support and medication adherence in elderly patients with hypertension in the outpatient unit of the Durenan Community Health Center. This study is a non-experimental, prospective, observational quantitative study with descriptive analysis. The sample consisted of 98 elderly respondents selected using purposive sampling. Primary data were obtained from the Perceived Social Support Questionnaire Family (PSS-Fa) to assess family support and the Adherence to Refills and Medications Scale-Short Form (ARMS-SF) to assess adherence. Secondary data were obtained from patient medical records. The results showed that of the 54 respondents with good family support, 51 respondents were compliant and 3 were non-compliant. Conversely, of the 44 respondents with poor family support, only 7 were compliant and 37 were non-compliant. The chi-square test showed a p-value of 0.000 (p 0.05), which means there is a significant relationship between family support and medication adherence. A correlation coefficient (CC) of 0.982 indicates a very strong relationship, meaning that the better the family support, the higher the medication adherence in hypertension patients. Family support has been shown to be a crucial factor in the success of hypertension therapy in the elderly.
