Abstract

Uncaria nervosa Elmer leaves are known to contain bioactive compounds with potential as natural drug candidates, yet their pharmacokinetic profiles, drug-likeness, and molecular affinity toward target proteins remain limitedly explored. This study aimed to evaluate the pharmacokinetic properties, drug-likeness, and molecular interaction potential of bioactive compounds from the ethanol extract of Uncaria nervosa leaves. An in silico approach was employed using SwissADME to analyze pharmacokinetic parameters (absorption, distribution metabolism, and excretion) and drug-likeness based on Lipinski’s and Veber’s rules, followed by molecular docking against proteins 6O0K and 2W3L using MOE software. The results showed that most compounds exhibited high gastrointestinal absorption, did not inhibit major CYP enzymes, and several complied with drug-likeness rules. Docking analysis revealed that ceratodictyol displayed the closest binding energy to the positive controls (doxorubicin and obatoclax), indicating strong molecular affinity, while N-formamide showed superior pharmacokinetic and drug-likeness properties. In conclusion, the combination of pharmacokinetic and docking data highlights ceratodictyol as the main candidate supported by N-formamide as a potential compound, suggesting that Uncaria nervosa leaf extract may serve as a promising natural source for anticancer drug development.

Abstract

Uncaria guianensis is a medicinal plant from the Rubiaceae family, known for its wide range of pharmacological activities, particularly as an anti-inflammatory and immunomodulator. The leaves are rich in secondary metabolites such as alkaloids, flavonoids, tannins, and triterpenoids, whose potential is strongly influenced by the extraction method employed. This study aimed to compare the yield and characteristics of 70% ethanol extracts of Uncaria guianensis leaves obtained through four extraction methods: maceration, percolation, soxhlet extraction, and sonication. Leaf samples were collected from Kampar Regency, Riau, processed into powdered simplicia, and extracted using 70% ethanol. The results of the study showed that the soxhletation method produced the highest yield, namely 65.16%, followed by the maceration, percolation, and sonication methods with yields of 46.42%; 38.20%; and 35.82%, respectively. Phytochemical screening confirmed that all extracts tested positive for the presence of flavonoids, alkaloids, tannins, saponins, steroids, and triterpenoids. In terms of physicochemical characteristics, the soxhlet extract exhibited the best quality, with the lowest ash content (1.00%) and moisture loss (0.53%). In conclusion, soxhlet extraction is the most effective method for producing 70% ethanol extracts of Uncaria guianensis leaves, both in terms of quantity (yield) and quality (ash content and moisture loss).

Abstract

Red shoot leaves are known to contain secondary metabolic compounds such as alkaloids, flavonoids, tannins, triterpenoids, steroids, saponins, and phenolic compounds. Flavonoids are known to have cardioprotective activity, including the ability to inhibit platelet aggregation. This study aims to determine the potential of red shoot leaf extract (Syzygium myrtifolium Walp.) as an anticoagulant. The method used is the determination of coagulation time, carried out on days 0, 3, and 6, following the initial wounding of the mice’s tails. The tail is wounded using a sterile lancet at a distance at 2 cm from the tip, and blood is subsequently absorbed using a capillary tube. Coagulation time is measured by observing the formation of fibrin threads on the broken sections of the capillary tube every 15 seconds. Coagulation time is the time required for the formation of the fibrin thread. The study results on day 6 showed that the 100 mg/KgBW dose group of red shoot leaf ethanol extract demonstrated an effect of increasing coagulation time equivalent to the 100 mg acetosal group. Meanwhile, the 125 mg/KgBW dose group of red shoot leaf ethanol extract showed a stronger effect than the 100 mg acetosal group.

Abstract

Patients with type 2 Diabetes Mellitus accompanied by hypertension commonly receive multiple medications, increasing the risk of polypharmacy and potential drug interactions. This study aimed to analyze the relationship between the number of drugs used and the occurrence of potential drug interactions among patients with type 2 Diabetes Mellitus and hypertension at Hospital Ini Pekanbaru City. This study employed a non-experimental observational design with retrospective data collection. Samples were selected using purposive sampling, and data were analyzed using IBM SPSS Statistics version 27. A total of 98 medical records met the inclusion criteria. The results showed that 57 patients experienced potential drug interactions, with a total of 67 interaction events. All identified interactions were classified as moderate in severity (100%). Based on the interaction mechanisms, pharmacodynamic interactions were predominant, accounting for 64 events (95.52%), while pharmacokinetic interactions accounted for 3 events (4.48%). Statistical analysis demonstrated a significant association between the number of drugs prescribed and the occurrence of potential drug interactions (p = 0.001). Close monitoring of potential drug interactions is essential to ensure the effectiveness and safety of therapy, particularly in patients undergoing polypharmacy.

Abstract

Hipertensi merupakan salah satu masalah kesehatan utama yang memerlukan pengelolaan terapi yang rasional untuk mencapai outcome klinis yang optimal. Penelitian ini bertujuan untuk menganalisis pola penggunaan obat antihipertensi, tingkat rasionalitas terapi, serta hubungannya dengan outcome klinis pada pasien hipertensi di RSUD Rokan Hulu Provinsi Riau. Penelitian ini menggunakan desain cross-sectional retrospektif dengan data rekam medis pasien rawat jalan periode Januari–Desember 2024. Sebanyak 104 pasien yang memenuhi kriteria inklusi dianalisis. Evaluasi rasionalitas terapi meliputi ketepatan indikasi, pemilihan obat, dosis, dan interval pemberian. Outcome klinis dinilai berdasarkan pencapaian target tekanan darah. Hasil penelitian menunjukkan bahwa ketepatan indikasi mencapai 100%, ketepatan dosis 98,1%, dan ketepatan interval 99%, namun ketepatan pemilihan obat masih 80,8%. Kombinasi dua obat merupakan regimen paling banyak digunakan. Tidak terdapat hubungan yang signifikan antara karakteristik pasien dengan ketepatan penggunaan obat, namun terdapat hubungan antara ketepatan terapi dan outcome klinis. Evaluasi penggunaan obat antihipertensi penting untuk meningkatkan kualitas pelayanan dan hasil terapi pasien hipertensi.

Abstract

Pemberian banyak obat dalam satu resep dapat meningkatkan risiko terjadinya interaksi obat yang berpotensi menurunkan efektivitas terapi atau menimbulkan efek samping yang merugikan. Penelitian ini bertujuan untuk menganalisis potensi interaksi obat pada resep polifarmasi di Apotek Keluarga 1 Kota Pekanbaru periode Mei 2024–Juni 2025. Penelitian ini menggunakan desain observasional retrospektif dengan teknik purposive sampling. Analisis potensi interaksi obat dilakukan menggunakan aplikasi Medscape, situs Drugs.com, dan literatur Stockley’s Drug Interaction. Data dianalisis menggunakan uji Chi-Square dengan perangkat lunak SPSS versi 26. Dari 127 resep yang memenuhi kriteria inklusi, sebanyak 46 resep (43%) mengalami potensi interaksi obat dengan total 57 kejadian interaksi. Interaksi farmakodinamik merupakan mekanisme yang paling dominan (77%) dan tingkat keparahan terbanyak adalah moderate (67%). Nilai p sebesar 0,037 (p < 0,05) menunjukkan adanya hubungan yang signifikan antara jumlah kombinasi obat dengan potensi terjadinya interaksi obat.
