Abstract

The expense of managing CKD patients ranks second highest behind cardiovascular illness, with hypertension being the primary contributor to renal impairment. Maximizing the efficiency of therapeutic expenditures is essential. This study is to evaluate the cost-effectiveness of the combination of amlodipine 10 mg - candesartan 16 mg compared to the combination of candesartan 16 mg - furosemide 40 mg in patients with chronic kidney disease and hypertension. The research employed a non-experimental observational cohort design. The study was performed at both a private and a governmental hospital in Yogyakarta, Indonesia. Data were gathered retroactively using complete sampling. The inclusion criteria comprised outpatients with CKD-hypertension from January 2019 to October 2022, aged 18 years or older, who were undergoing combination therapy. The efficacy of therapy was evaluated by determining the number of samples achieving therapeutic objectives after 2 to 4 weeks. The mean expenditure was derived from monthly antihypertensive costs. The analysis of cost-effectiveness employed the Average Cost-Effectiveness Ratio (ACER) and the Incremental Cost-Effectiveness Ratio (ICER). The study comprised 50 samples, with 51% male participants and 66% aged 60 or older. Amlodipine and candesartan demonstrated superior efficacy (77%) and a reduced average cost of 101,095 IDR. The ACER for amlodipine and candesartan was 1,306 IDR, whereas for furosemide and candesartan it was 1,571 IDR. The amalgamation of amlodipine and candesartan is more economically advantageous than furosemide and candesartan.

Abstract

Boesenbergia rotunda, Zingiber cassumunar, and Hibiscus sabdariffa are potential medicinal plants to be developed as weight loss agent. This research aimed to determine weight loss activity of the 3 plants in combination. The model used was zebrafish wild type. The protocol was approved by the Ethics Committee of Health Research, Faculty of Pharmacy, Mulawarman University. The extraction method was decocta. The flavonoid and phenolic contents were calculated using standard method of analysis in Indonesian Herbal Pharmacopoeia.  The results of rendemen and total flavonoid/phenolic content (TFC/TPC) calculations are Boesenbergia extract 2,7% with the TFC of 49,7 mg/g; Zingiber cassumunar extract 6,5% with the TPC of 41,2 mg/g; Hibiscus extract 31,6% with the TFC of 91,2 mg/g. It forms as dry extracts, brown colour, and hygroscopic. The average of weight loss of zebrafish in 2 weeks treatment is 9-16% in sample 1 group; 28,5% in sample 2 group; and no lowering body weight on sample 3 group. The sample 1 and 2 are promising compared to orlistat activity, 32% weight loss. Abstract

The escalating utilization of antipsychotics undeniably contributes to the heightened risk of drug side effects. This study aims to identify antipsychotic side effects and their causality. This study belongs to observational research with a descriptive cross-sectional design. The participants were patients prescribed antipsychotic drugs at Hospital X Yogyakarta between April and July 2022 who met the specified inclusion criteria. This study utilized accidental sampling, generating 61 patients as the participants. Data collection was carried out prospectively through interviews and medical records. The Naranjo algorithm was used to analyze causality. The predominant drug type administered to patients was atypical antipsychotics, with a total of 72 prescriptions. Risperidone accounted for the highest number of prescriptions, totaling 37 (22.16%). Weight gain contributed to the highest percentage of reported side effects, with 16 (16.33%) instances. The side effects with the highest causality score of 7 (probable) were breast secretion, tremors, palpitations, weight gain, and decreased blood pressure. Weight gain was the most frequently observed side effect when taken together. The maximum score for the causality association of side effects was an average of 7, indicating a probable connection. This study provides insight into the numerous side effects associated with prescription antipsychotic drugs.

Abstract

Daun gambir (Uncaria gambir Roxb.) merupakan obat tradisional yang dapat berfungsi sebagai antioksidan, anti-inflamasi, antibakteri dan antihiperlipidemia. Pemanfaatan daun gambir didasari oleh aktivitas famakologi. Fermentasi daun gambir dilakukan untuk melengkapi kebutuhan nutrisi dan meningkatkan fungsi kesehatan. Penelitian ini bertujuan untuk mengidentifikasi aktivitas antioksidan, kadar katekin, kadar tannin, protein kasar dan serat kasar pada daun gambir setelah difermentasi akan diaplikasikan untuk broiler sebagai pengganti AGP. Penelitian menggunakan penelitian experimental dengan Rancangan Acak Lengkap yang terdiri dari 10 perlakuan 3 ulangan. Penelitian ini mengevaluasi efek fermentasi daun gambir menggunakan Rhizopus oligosporus dan Lactobacillus plantarum terhadap aktivitas antioksidan dan komposisi fitokimia. Hasil menunjukkan bahwa kombinasi L. plantarum 1,5% + R. oligosporus 1% menghasilkan aktivitas antioksidan tertinggi (25,415 ppm) serta kadar tanin dan serat kasar terendah. Peningkatan kadar katekin tertinggi ditemukan pada kombinasi L. plantarum 2,5% + R. oligosporus 2% (326,87 µg/ml). Penelitian ini menunjukkan bahwa fermentasi dapat meningkatkan nilai nutrisi daun gambir, sehingga berpotensi sebagai aditif pakan broiler.

Abstract

Pineapple peel waste can be processed into pharmaceutical dosage form, but research related to the toxicity of pineapple peel is  still limited. Based on the LC50 value and the description of morphological damage in Artemia salina larvae, this study seeks to establish the acute toxicity of pineapple peel aqueous extract. The method used is the Brine Shrimp Lethality Test (BSLT). Preliminary tests were carried out using each of 10 Artemia salina larvae in 10 mL of pineapple peel water extract at concentrations of 10000;1000;100;10;1; 0.1 ppm. Based on the results of the preliminary test, the definitive test was conducted with seven concentration variations, plus a saltwater negative control and a potassium dichromate 39.06 ppm positive control. After 24 hours, observations were performed on the number of deaths and morphological damage of Artemia salina larvae. An LC50 value will be generated by performing a probit analysis on data regarding the number of larvae deaths using Microsoft Excel. A morphological damage of Artemia salina larvae were analyzed descriptively. Results of the preliminary test are presented as concentration ranges for the final test, including 100, 251, 630, 1583, 3975, 6987, and 10,000 ppm. The LC50 value obtained based on the definitive test was 1185 ppm. Morphological damage to Artemia salina larvae due to exposure to water extract of pineapple peel occurred in the antenula, appendages, abdomen, and digestive tract. Pineapple peel water extract belongs to the non-toxic category with LC50 value is > 1000 ppm. Morphological damage to Artemia salina larvae occurred at concentrations of 630-10000 ppm.

Abstract

Alopesia androgenetik adalah bentuk kebotakan yang umum terjadi pada pria dan wanita, dipicu oleh faktor genetik dan hormonal, terutama dihidrotestosteron (DHT). Minoksidil dan finasterid adalah terapi yang paling sering direkomendasikan untuk mengatasi alopecia androgenetik; namun, pengobatan ini sering kali menghasilkan efek samping dermatologis, termasuk iritasi kulit, ruam, dan dermatitis kontak. Selain itu, metode pengobatan konvensional memiliki keterbatasan dalam penetrasi kulit dan efektivitas optimal. Oleh karena itu, dibutuhkan sistem penghantaran obat yang lebih efektif, seperti Nanostructured Lipid Carriers (NLC). Penelitian ini bertujuan untuk mengeksplorasi potensi NLC sebagai sistem penghantaran obat inovatif untuk alopecia androgenetik. Metode yang digunakan melibatkan literatur yang diperoleh dari platform seperti Google Scholar, ScienceDirect, dan PubMed. Data yang dikumpulkan dianalisis dan dikelola menggunakan Mendeley® untuk tujuan referensi. Temuan dari literatur menunjukkan bahwa NLC dapat meningkatkan penetrasi obat ke dalam folikel rambut, meningkatkan retensi obat di kulit, dan mengurangi efek samping. Temuan ini menegaskan potensi NLC sebagai sistem penghantaran obat yang menjanjikan untuk meningkatkan respons terapeutik dalam mengobati alopecia dan berbagai kondisi kulit lainnya.

Abstract

Propolis ear drops are natural antioxidants that have been developed for various health purposes. There are many types of propolis ear drops available today, including the Indonesian Mount Lawu Propolis ear drop (IMLPED). Indonesian Mount Lawu Propolis ear drop is proven to contain many antioxidant substances such as flavonoids, phenolic acids, caffeic acid pethyl ester, and quercetin. However, to date, no study has assessed the antioxidant activity of IMLPED, especially the 2% IMLPED. This study aims to assess the antioxidant activity of the 2% IMLPED. This is an in vitro study to assess the antioxidant activity of the 2% IMLPED with 2,2-Diphenyl-1-Picrylhydrazyl (DPPH) method based on the inhibitory concentration at 50% (IC50) value. The IC50 represents the concentration required to reduce 50% of the free radicals. The result of this study showed that the IC50 value of the 2% IMLPED is 30.71 µg/mL. Based on the IC50 value, the 2% IMLPED formula exhibits a robust antioxidant effect. The implications of this revelation establish IMLPED as a promising candidate in combating oxidative stress.

Abstract

Betaine yang dikenal juga sebagai trimetilglisin, tersebar luas di tanaman, hewan, mikroba, dan sumber nutrisi yang melimpah seperti makanan laut, bayam, bit, dan dedak gandum. Dalam bidang nutrisi hewan, termasuk untuk unggas, betain sering digunakan sebagai tambahan pakan untuk mengurangi stress oksidatif. Penelitian ini bertujuan untuk mengidentifikasi analisis proksimat, rendemen ekstrak, aktivitas antioksidan dan kadar betain pada tiga jenis sumber nabati yaitu bayam, bit dan dedak gandum yang mengandung betain pada pakan unggas. Penelitian menggunakan penelitian eksperimental dengan Rancangan Acak Lengkap yang terdiri dari 3 perlakuan dan 6 ulangan. Proses ekstrasi ketiga bahan nabati menggunakan metode refluks. Parameter yang diamati diantaranya analisis proksimat yaitu kadar air, kadar abu, protein, dan lemak serta nilai rendemen, aktivitas antioksidan dan kadar betain. Hasil penelitian menunjukkan bahan nabati dedak gandum memiliki kandungan kadar air 8.45%, abu 5.01%, lemak 0.39%, nilai rendemen 19.60% dan aktivitas antioksidan 1,365.79 ppm yang lebih baik dibanding kedua bahan nabati lainnya. Sedangkan bayam menunjukkan kadar protein 26.17% dan bit menunjukkan hasil kadar betain 1138 mg/100g lebih tinggi dibanding kedua bahan nabati lainnya. Sehingga pada penelitian ini penggunaan dedak gandum direkomendasikan karena memiliki kadar air, abu dan lemak yang rendah, nilai rendemen serta aktivitas antioksidan yang lebih baik dibanding kedua bahan lainnya.

Abstract

Astaxanthin adalah karotenoid teroksigenasi yang memiliki aktivitas antioksidan yang kuat. Aktivitas antioksidan astaxanthin 10 kali lebih tinggi dibandingkan karotenoid lainnya dan lebih dari 500 kali lebih efektif dibandingkan α-tokoferol. Aktivitas antioksidan yang kuat ini menyebabkan pemanfaatan astaxanthin secara luas dalam makanan, obat, suplemen makanan (nutraceuticals), kosmetik, dan pakan. Di balik manfaatnya yang besar, senyawa astaxanthin memiliki  kekurangan yaitu kelarutannya yang rendah dalam air, ketidakstabilannya pada suhu tinggi, pH asam, oksigen atau cahaya dan bau/rasanya yang kuat. Bentuk sediaan yang banyak digunakan dalam pengobatan yang memanfaatkan astaxanthin adalah sediaan oral dan topikal. Literature review ini mengulas kemajuan terkini dalam pembuatan sediaan farmasi dengan komponen astaxanthin, yang dikembangkan untuk meningkatkan stabilitas dan kelarutannya dalam air, terutama pada sediaan oral dan topikal. Metode yang digunakan dalam penyusunan artikel riview ini adalah pencarian database melalui Scopus, PubMed, dan Google Scholar. Dari penelusuran literatur, diketahui bahwa sistem penghantaran baru yang dikembangkan untuk senyawa astaxanthin antara lain, liposom, nano partikel, sistem partikulat, kompleks inklusi, emulsi, dan film. Sistem tersebut terbukti mampu meningkatkan sifat astaxanthin, di antaranya adalah peningkatan stabilitas, potensi antioksidan, aktivitas biologis, pelepasan obat, dan perlindungan dari degradasi di saluran pencernaan.

Abstract

Young coconut husk is one of the wastes that can be processed as a source of herbal medicine. Previous research on young coconut husk has provided information about its tannin content, antioxidant activity, and other bioactivities. This study aims to measure antioxidant activity and total phenolic and flavonoid contents from young coconut husk decoction. The determination of antioxidant activity and total phenolic and flavonoid contents used successively were DPPH, Folin-Ciocalteu, and aluminium chloride methods. The antioxidant activity test of young coconut husk decoction revealed an IC50 value of 47.69 µg/mL. The analysis of total phenolic and flavonoid content in the decoction showed 46,43 mg GAE/gram of husk powder and 4.04 mg RE/gram of husk powder, respectively. Collectively, young coconut husk extracted by decoction has strong antioxidant activity and can be used as a source of phenolics and flavonoids.
