Abstract

Hipertensi merupakan salah satu masalah kesehatan global yang berisiko menimbulkan komplikasi serius seperti penyakit jantung dan stroke, dll. Penanganan non-farmakologis, termasuk terapi herbal jus tomat menjadi suatu alternatif yang baik karena efek samping yang minimal Penelitian ini menggunakan metode penelitian Kuantitatif dengan desain penelitian Quasy-Experiment Pretest-Postest Non-Equivalen With Control Group. Penelitian ini dilaksanakan di wilayah UPT Puskesmas Aek Natolu Toba Samosir, dengan total sampel sebanyak 60 responden yang terdiri dari 30 kelompok intervensi dan 30 kelompok Kontrol yang dipilih dengan menggunakan Teknik simple random sampling, data yang telah dikumpulkan dianalisis dengan menggunakan uji statistic T-test. Terdapat perbedaan tekanan darah penderita hipertensi sebelum dan sesudah pada kelompok intervensi dan kelompok kontrol dengan dengan nilai P-Value sistol 0,000 dan P-Value diastole 0,000 dan nilai rerata selisih sistol kelompok kontrol dan intervensi adalah 13,73, pada diastol 4,69. Hal ini menunjukkan adanya pengaruh terapi herbal jus tomat terhadap penurunan tekanan darah pada penderita hipertensi. Terapi herbal jus tomat memiliki pengaruh yang signifikan dalam menurunkan tekanan darah pada penderita hipertensi. sehingga jus tomat ini dapat dipertimbangkan sebagai terapi tambahan non-farmakologis dalam manajemen hipertensi.

Abstract

ABSTRAK Kecemasan merupakan respons psikologis yang umum dialami oleh pasien kanker payudara selama menjalani kemoterapi, yang dapat berdampak negatif terhadap kualitas hidup dan keberhasilan terapi. Salah satu intervensi non-farmakologis yang dapat digunakan untuk mengurangi kecemasan adalah relaksasi autogenik. Penelitian ini bertujuan untuk mengevaluasi efektivitas relaksasi autogenik terhadap tingkat kecemasan pada pasien kanker payudara yang menjalani kemoterapi di Rumah Sakit Murni Teguh Medan. Penelitian ini menggunakan desain quasi eksperimen dengan pendekatan pretest-posttest control group. Jumlah sampel sebanyak 64 responden yang dibagi ke dalam dua kelompok, yaitu kelompok intervensi dan kelompok kontrol, masing-masing terdiri dari 32 orang. Intervensi relaksasi autogenik diberikan selama 15–20 menit selama tiga hari berturut-turut. Pengukuran kecemasan dilakukan dengan menggunakan kuesioner Zung Self-Rating Anxiety Scale (ZSAS). Hasil analisis menunjukkan adanya penurunan signifikan tingkat kecemasan pada kelompok intervensi dibandingkan kelompok kontrol. Hasil ini menunjukkan bahwa relaksasi autogenik efektif digunakan sebagai intervensi keperawatan mandiri dalam mengurangi kecemasan pasien kanker payudara yang menjalani kemoterapi. Kata Kunci: relaksasi autogenik, kecemasan, kanker payudara, kemoterapi ABSTRACT Anxiety is a common psychological response experienced by breast cancer patients undergoing chemotherapy, often affecting their quality of life and treatment adherence. One non-pharmacological intervention that can reduce anxiety is autogenic relaxation. This study aimed to evaluate the effectiveness of autogenic relaxation on the anxiety levels of breast cancer patients undergoing chemotherapy at Murni Teguh Hospital Medan. A quasi-experimental design with a pretest-posttest control group approach was used. A total of 64 respondents were selected using purposive sampling and divided equally into intervention and control groups. The autogenic relaxation intervention was administered for 15–20 minutes over three consecutive days. Anxiety levels were measured using the Zung Self-Rating Anxiety Scale (ZSAS). The results showed a significant decrease in anxiety levels in the intervention group compared to the control group. These findings suggest that autogenic relaxation is effective as an independent nursing intervention to reduce anxiety in breast cancer patients undergoing chemotherapy. Abstract

Type 2 diabetes mellitus (T2DM) is a group of metabolic diseases that occur when the body cannot effectively use the insulin it produces, resulting in hyperglycemia. T2DM mostly affects adults and is usually caused by age, overweight and inactivity. To manage diabetes appropriately or achieve adequate glycemic control, patients must perform diabetes self-care activities by organizing a proper diet, doing physical exercise, monitoring blood sugar levels, complying with prescribed medications, performing foot care, and quitting smoking. The purpose of this study was to determine the relationship of diabetes self-care activities with glycemic control in type 2 diabetes patients in the Padang Bulan Medan Health Center. This research method is quantitative non-experimental with correlation design. The majority of respondents carried out insufficient self-care activities (47.1%) with 79.3% uncontrolled glycemic control. The relationship between diabetes self-care activities and glycemic control was significant with p = 0.000 (<0.001) with a value of r = 0.643. Health center workers are expected to provide diabetes self-care education. Improving the implementation of diabetes self-care activities is aimed at preventing further complications and improving the quality of life of patients. Keywords: Diabetes. Abstract

Diabetes mellitus is characterized by elevated blood glucose levels above normal, with random blood glucose levels ≥ 200 mg/dl and fasting blood glucose levels ≥ 126 mg/dl. This study employed a quantitative research method with a quasi-experimental design and an equivalent control group design approach. The study utilized power analysis with consecutive sampling techniques. Data collection techniques involved distributing questionnaires to 74 diabetes mellitus patients in the Onolalu Health Center Working Area, South Nias District. Data analysis techniques used independent t-test statistical analysis and presented in the form of a T-test table. The results of the study indicate that there is an influence of self-care behavior on blood sugar control levels in type II diabetes mellitus patients in the Onolalu Health Center Working Area, South Nias Regency, with a Sig. (2-tailed) of 0.024 < 0.05. Therefore, in the independent sample t-test, it can be concluded that the null hypothesis (HO) is rejected and the alternative hypothesis (Ha) is accepted. Thus, it can be concluded that there is a significant (statistically significant) difference between the average blood sugar levels of type II diabetes mellitus patients in the intervention group and the control group.

Abstract

One of the important components in the management of patient care in hospitals is the role of nurses in implementing discharge planning, which is a structured process of preparing patients to return to their environment after they get treatment in the hospital. Nurses have a significant role in helping families prepare to care for patients after discharge from psychiatric hospitals. Through evaluation of the patient's condition, coordination with multidisciplinary care teams, patient and family education, and preparation of a detailed discharge plan, nurses can positively influence Family Readiness. Objective: to evaluate the relationship between the role of nurses in implementing discharge planning and family readiness to care for patients at home. Method: The sample used was a purposive sample of 78 implementing nurses working in the inpatient room and the design of this study was cross-sectional. Data was collected through questionnaires as well as statistical correlation Chi-Square was used to analyze it. The validity test for the nurse role questionnaire was 0.629 to 0.777, while the family readiness questionnaire was 0.456 to 0.765. The Cronbach's alpha value for the nurse's role variable was 0.962 and family readiness was 0.967 so the questionnaire was declared reliable. Results: The results showed a relationship between the role of nurses in implementing discharge planning and family readiness to care for patients at home (0.020 < 0.05). Conclusions: Thus, the role of nurses in the discharge planning process plays an important role in improving family readiness in caring for patients at home.

Abstract

According to applicable laws and regulations, the law regulates professional behavior in the relationship between doctors and nurses based on the responsibility for delegating tasks and attributive authority. This study aims to analyze juridically the delegation of authority of doctors to nurses in hospitals and juridical analysis of the delegation of authority of doctors to nurses in terms of the existing lex specialist legislation. This study uses secondary data from literature review through online searches of scientific articles using three electronic databases, namely: Google Scholar, Science Direct, and Proquest, with data related to issues of delegation and authority of doctors to nurses in Indonesia. Nurses as health workers need legal protection in carrying out their duties and obligations according to their authority which has been regulated in related laws and regulations. The legal responsibility of nurses is based on the field of law in State Administrative Law, Civil Law, and Criminal Law. Meanwhile, the nurse's attribution authority is based on nursing care and is attached to the independent function of nurses carrying out tasks based on their authority. The mandate authority in the nurse's interdependent function in teamwork and delegation authority is attached to the dependent function where the nurse's interventions are the doctor's responsibility, but the duties and responsibilities are left to nurses who have met certain requirements.

Abstract

Health problems in the elderly begins with the process of deterioration of body cells, which causes a decrease in function and immune system so that it can increase risk factors for disease. Some diseases that are often experienced by the elderly include, hypertension, diabetes mellitus, rheumatism, gout, hearing loss, vision problems, osteoporosis, and others. These conditions will have an impact on increasing morbidity and mortality, declining quality of life, and increasing health financing. The method of implementing community service activities is carried out by educating health issues, providing guidance, counseling and mentoring to 20 elderly groups. The purpose of community service activities is to increase health care efforts which are expected to implement programs to create or improve the ability to live healthy in the community both individuals and groups to be able to solve various problems related to health improvement and disease prevention on an ongoing basis. Education is carried out with health education about general elderly diseases that often occur, namely Diabetes and Gout, then conduct guidance and counseling by dividing into groups, and provide assistance by teaching foot exercises and foot care. Results The implementation of community service activities shows the results that Good Knowledge about degenerative diseases (85.0%), after the mentoring activities namely guidance, counseling, foot gymnastics simulation and foot care. Paired t-test results before and after the assistance was given p value of 0.007. It is expected that the elderly group can be independent in efforts to prevent degenerative diseases that are focused on diabetes mellitus

Abstract

ISPA is a respiratory disorder that causes children to tend to be lethargic, fussy, and lack of appetite, thus reducing the child's immunity which affects other health problems. The use of Complementary Therapies can help to reduce the impact of disease incidence, especially ISPA in toddlers at home as an alternative therapy for self-medication for families. It is hoped that the involvement of village health worker in the mother group in the community is directly involved in implementing the program to create and increase the ability to live healthy in a sustainable manner. The method of implementing community service activities is carried out by providing education, training and mentoring to 41 people consisting of village health worker and mother groups. The implementation of activities for the mothers group began with distributing pre-test questionnaires, then continued with providing health education about complementary therapies, herbal therapy simulations and toddler massage training to village health worker and mother groups, the activities were evaluated by providing posttest sheets. The results of the implementation of community service activities after health education and training activities obtained good knowledge about complementary therapies (75.6%) and sufficient knowledge (24.4%), paired t-test results before and after health education and the training obtained p value 0.005. It is hoped that village health worker can become activists and health promoters for mothers in an effort to prevent health problems by using complementary therapies.

Abstract

Cervical cancer is the leading cause of cancer death for women in developing countries. The incidence of cervical cancer will increase four times greater in family planning acceptors who use hormonal contraceptives with a duration of more than five years. The high incidence of cervical cancer requires counseling for the community to conduct early detection of the possibility of cervical cancer. Through training and mentoring activities with comprehensive learning in productive age mother groups in the cervical cancer prevention and care Movement (GERCEP) efforts in the community that can increase knowledge as well as early prevention of cervical cancer and health maintenance efforts for productive age mothers. The method of implementing community service activities is carried out by providing education, learning training in the form of quizzing to 50 productive age mother groups. Education is carried out by providing health counseling about cervical cancer and how to prevent it through PHBS. The results of the implementation of this community service activity show that the mother's knowledge about cervical cancer and its prevention is in the good category (100%), after learning training activities. The results of the paired t-test test before and after being given learning training obtained a p value of 0.000. The results of this activity are expected that mothers are able to increase knowledge and insight in efforts to prevent cervical cancer through clean and healthy living behaviors.

Abstract

Abstrak Kadar asam urat yang tinggi pada penderita gout arthritis menjadi masalah kesehatan yang signifikan, terutama di wilayah kerja Puskesmas Sibolangit, Kabupaten Deli Serdang. Tujuan dari penelitian ini adalah untuk menganalisis pengaruh terapi herbal daun salam terhadap penurunan kadar asam urat. Penelitian ini menggunakan rancangan quasi-experimental dengan desain non-equivalent control group pretest-posttest, melibatkan 62 responden yang dibagi menjadi kelompok intervensi dan kontrol. Kelompok intervensi diberi terapi air rebusan daun salam dua kali sehari selama 14 hari. Hasil penelitian menunjukkan bahwa terjadi penurunan kadar asam urat yang lebih signifikan pada kelompok intervensi dibandingkan kelompok kontrol. Rata-rata kadar asam urat menurun dari 10,87 mg/dL menjadi 7,90 mg/dL. Terapi daun salam terbukti efektif menurunkan kadar asam urat melalui kandungan flavonoid dan efek diuretiknya. Penelitian ini mendukung penggunaan terapi herbal sebagai pendekatan non-farmakologis dalam keperawatan komunitas.Kata Kunci: terapi herbal, daun salam, asam urat, keperawatan komunitas, gout arthritis. Abstract High uric acid levels in gout arthritis patients are a significant health issue, particularly in the working area of Sibolangit Health Center, Deli Serdang Regency. This study aimed to analyze the effect of bay leaf herbal therapy on reducing uric acid levels. A quasi-experimental design with a non-equivalent control group pretest-posttest was applied, involving 62 respondents divided into intervention and control groups. The intervention group received boiled bay leaf water therapy twice daily for 14 days. Results showed a more significant decrease in uric acid levels in the intervention group compared to the control. The average uric acid level decreased from 10.87 mg/dL to 7.90 mg/dL. Bay leaf therapy proved effective in reducing uric acid levels due to its flavonoid content and diuretic effects. This study supports the use of herbal therapy as a non-pharmacological approach in community nursing. Keywords: herbal therapy, bay leaf, uric acid, community nursing, gout arthritis.
