Abstract

Based on data from the World Health Organization, it is estimated that around 15-20% of the world's population suffers from hypertension and Hypertension in Indonesia is the third cause of death in all age groups, namely around 6.7% of the total population of deaths in Indonesia. The prevalence of musculoskeletal diseases in Indonesia is 11.9% and based on symptoms, the prevalence of musculoskeletal diseases in Indonesia reaches 24.7%. Meanwhile, the prevalence of musculoskeletal disease in Lampung reached 18.9%. Apply the principles of family medicine services to patients and carry out holistic management based on the patient problem found with patient-centered approach, family focused, and community oriented based on Evidence Based Medicine. This study is a case report with Primary data obtained through history taking, physical examination and home visits to complete family, psychosocial and environmental data. Secondary data was obtained from the patient's medical record. The assessment is based on a holistic diagnosis from the beginning, process, and end of the study. Mrs. L 69 years old, came with complaints of headaches, heavy shoulders and lower back pain. The patient was diagnosed as hypertension and low back pain. In this case, the diagnosis and treatment were carried out according to the theory and related journals. After the intervention, there was a decrease in clinical symptoms and an increase in patient and family knowledge. Holistic management can improve the knowledge, attitudes, and behavior of patients and family in maintaining their health.

Abstract

Gout arthritis is a progressive disease due to the deposition of monosodium urate crystals in joints, kidneys, and other connective tissues due to chronic hyperuricemia. This disease needs to be managed comprehensively so that patients must know about their disease and have awareness and can make lifestyle modifications so that treatment goals can be achieved, prevent complications that may occur, and improve the patient's quality of life. This can be realized with family medicine-based management. The aim of this study is to identify internal, external risk factors and clinical problems found in patients, apply a holistic and comprehensive family doctor approach according to the problems found in patients, and carry out evidence-based medicine management that is family-approach, patient-centered and community oriented. The study is a case report. Primary data were obtained through history taking, physical examination and home visits. Secondary data were obtained from the patient's medical record. A 50-year-old female patient came with complaints of pain in the both knees. The patient has been diagnosed with uncontrolled gout since 2 months ago. In the knowledge assessment, the initial score before the intervention was 5 (low) and after the intervention it was 9 (high). In the aspect of uric acid levels there is a decrease in uric acid levels from 9,3 mg/dL to 5,6 mg/dL. In the aspect of diet there is a change in the type of food that is appropriate. In the aspect of physical activity there is a change in physical activity of 30 minutes of exercise per day. Enforcement of diagnosis and management of these patients has been done holistically, patient centered, family approach and community oriented based on several theories and the latest research. In the process of behavior change, the patient has reached the trial stage.

Abstract

Chronic obstructive pulmonary disease (COPD) is a chronic respiratory condition characterized by limited airflow. The Global Burden of Disease Study reports the prevalence of COPD is 251 million cases. One factor that plays a role is long-term smoking behavior, either actively or passively. The clinical manifestation of COPD is a cough with phlegm that persists for more than 3 months, is dry and accompanied by shortness of breath, especially when the patient is active. Management of COPD consists of medical and non-medical. COPD is a lung disease that can be treated and prevented. However, in treating COPD, there needs to be participation and support from the patient and family. This study is a case report. Primary data was obtained through history taking, physical examination and home visits. Secondary data was obtained from patient medical records. Assessment is based on a holistic diagnosis from the beginning, process and end of the study qualitatively and quantitatively. Based on the study. patient Mr. P is 76 years old with a diagnosis of COPD, Mr. P has concerns that the patient's illness could cause him to die. The patient hopes to recover and not take routine medication again. Knowledge about COPD and trigger factors such as trash fumes and cigarette smoke from the surrounding environment are problems that occur in patients. Holistic management is needed for patients with COPD to identify problems that occur in patients so that patients can have a better quality of life.

Abstract

Allergic contact dermatitis is dermatitis that occurs due to exposure to allergens outside the body, mediated by a type 4 hypersensitivity reaction. This disease is generally synonymous with pruritus has erythema, oedem,. This disease results in a high rate of morbidity and its prevalence has increased in recent decades. This article is a form of application of evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and management based on a problem-solving framework with a patient-centered and family-centered approach. This study is a Case Report. Primary data were obtained through autoanamnesis, physical examination, home visits to complete family and psychosocial data, as well as the environment. Assessment based on a holistic diagnosis from the beginning, process and end of the study quantitatively and qualitatively.Adult female patient aged 54 years with allergic contact dermatitis. On internal factors in cases such as genetic factors, age factors, and ignorance of the originator. External factors are the lack of family knowledge about kontak alergic dermatitis, its complications and the environment that has many allergens. Pharmacological and non-pharmacological interventions were carried out in the form of education in preventing the onset of symptoms of allergic contact dermatitis in patients. Diagnosis and management of these patients has been carried out in a holistic, patient-centered, family approach and based on several theories and recent research. In the process of change, patients and families have reached the adoption stage.

Abstract

Atopic dermatitis (AD) is a skin disorder that often occurs in infants and children, characterized by itching, frequent relapses, and a characteristic distribution of lesions. Management of DA contained in the 5 pillars which include patient education, prevention and modification of precipitating factors, improvement of optimal skin barrier function, management of inflammatory skin disorders, and control of the itch-scratch cycle. The aim of this study is to apply the principles of family medicine services to patients and carry out holistic management based on the patient problem found with patient-centered approach, family focused, and community oriented based on Evidence Based Medicine. This study is a case report with Primary data obtained through history taking, physical examination and home visits to complete family, psychosocial and environmental data. Secondary data was obtained from the patient's medical record. The assessment is based on a holistic diagnosis from the beginning, process, and end of the study. The result is there are internal and external factors happened to tje patient.  After the intervention, there was an improvement of symptom. Then there is an increase in knowledge through pretest and posttest. After holistic management was carried out based on the problems identified through family doctor approach, there was an increase in symptom as well knowledge abaout the disease.

Abstract

Asthma is a heterogeneous disease, which has characteristics of chronic inflammation of the airways characterized by a history of respiratory symptoms such as wheezing, shortness of breath, chest tightness, and coughing that vary in terms of time and intensity, accompanied by variations in expiratory airflow obstruction. This study is a descriptive study of case reports. Primary data were obtained through autoanamnesis, physical examination, and visits to the patient's home. Secondary data were obtained from patient medical records at Panjang Public Health Center. Patient Mrs. LM, 42 years old, diagnosed with moderate persistent Asthma was managed comprehensively, namely given medication in the form of bronchodilators and inhalers and carried out interventions using poster media and showing asthma exercise videos. The evaluation obtained results in the form of improved patient complaints, increased understanding of the disease as evidenced by a comparison of pre-test and post-test values and increased family support The application of a family doctor approach is needed for holistic and comprehensive management of patients with moderate persistent Asthma to detect internal and external risk factors in order to support successful therapy and improve the patient's quality of life.
