Abstract

AbstractThe Clean and Healthy Behavior Program (CHBP) is a form of manifestation to increase knowledge, attitudes and behavior in order to be able to implement healthy ways of life in order to improve health. CHBP must start from the household order because a healthy household is an asset for future capital development. This study aimed to look at the problem and find a solution to the problem of the low coverage of CHBP in the working area of the Payakumbuh Health Office . This study was a qualitative study with in-depth interview method. Informants in this study were the Head of Family Health Department, Head of Health Promotion Department, Head of Payakumbuh Health Office, and Head of Padang Karambia Health Center. Data was presented and analyzed using Ishikawa diagrams for analysis of the causes of problems and effectiveness-efficiency methods for problem solving analysis. The results showed that the CHBP target had not been met because there was some problems in 5 aspects: man, money, machine, method, and environment. The priority of solving the problem was training the health promotion officers on effective counseling methods and empowering cadres in the community to conduct CHBP data collection.Keyword: Health Behavior, Cadre, Effectivity-EfficiencyAbstrakProgram Perilaku Hidup Bersih dan Sehat (PHBS) merupakan bentuk perwujudan untuk meningkatkan pengetahuan, sikap dan perilaku agar dapat menerapkan cara–cara hidup sehat dalam rangka meningkatkan kesehatan. PHBS harus dimulai dari tatanan rumah tangga karena rumah tangga yang sehat merupakan aset modal pembangunan di masa depan. Penelitian ini bertujuan untuk melihat permasalahan dan menemukan pemecahan masalah rendahnya cakupan PHBS di wilayah kerja Dinas Kesehatan Kota (DKK) Payakumbuh. Penelitian ini merupakan penelitian kualitatif dengan metode wawancara mendalam. Informan pada penelitian ini adalah Kepala  Bidang Kesehatan Keluarga, Kepala Sesi Promosi Kesehatan, Kepala Dinas Kesehatan Kota Payakumbuh, dan Kepala Puskesmas Padang Karambia. Data disajikan dan dianalisis menggunakan diagram ishikawa untuk analisis penyebab masalah dan metode efektivitas-efisiensi untuk analisis pemecahan masalah. Hasil penelitian menunjukkan bahwa target PHBS belum terpenuhi terdapat kendala dari segi man (manusia), money (dana), machine (mesin), methode (metode), dan environment (lingkungan). Prioritas pemecahan masalah adalah  melakukan pelatihan kepada petugas promkes tentang metode-metode penyuluhan yang efektif dan memberdayakan kader di masyarakat untuk melakukan pendataan PHBS. Kata Kunci: PHBS, Kader, Efektivitas-Efisiensi

Abstract

Rumah Sakit Umum Daerah Tebet, South Jakarta, has been using a computerized management information system for medical record services called the Hospital Information System (HIS) since 2017. The presence of the Medical Record Management Information System (SIMRM) at the Rumah Sakit Umum Daerah Tebet,  has brought many influences to services. Along with the implementation of SIMRM, it is necessary to assess whether the system is running properly and has been accepted by its users, in this case the medical record officer. The research method used The research method that the researcher uses is a quantitative descriptive method using the Technology Acceptance Model (TAM) to assess the perception of technology users as seen from 5 constructs, namely perceived ease of use, perceived usefulness. , perceptions of attitudes toward using, behavioral intention to use and actual usage by distributing questionnaires to 16 respondents consisting of 7 medical record unit officers and 9 registration unit officers. The results showed that the average user acceptance of SIMRM in South Jakarta Hospital as seen from the 5 TAM constructs was 75% received and 25% did not receive which means SIMRM was received  by the user. It is recommended for the hospital to conduct socialization and training in accordance with the needs of officers.

Abstract

Dengue Hemorrhagic Fever (DHF) is an infectious disease caused by the dengue virus (DEN) which is spread through mosquito bites. This research is a quantitative descriptive study using a non-random sampling method with a saturated sample technique. The study was conducted at the UKI General Hospital with a sample of 94 patients. The results showed that the age of the most patients was 17-25 years, 30 patients (32%), the most gender were male (60%), domiciled in East Jakarta (70%), the most occupations were self-employed (36%), months The highest incidence was in March 23 patients, the most felt symptom was fever (100%), the highest severity level based on the ICD-10 code was A97.0 94 patients (100%), the category of platelet count when it arrived was abnormal (93 %), while the category of the number of platelets at home is normal (10%), the category of the number of hematocrit is abnormal (43%), while the category of the number of hematocrit when going home is normal (51%), the ICD-10 code used is A91 , the longest length of hospitalization was <6 days 67 patients (71%). The condition when he returned home was healed (99%).

Abstract

The medical record storage system is very important to do in health care institutions, because it can make it easier for medical records to be stored in storage racks, speed up finding or retrieval and return, and protect medical records from theft, physical, chemical, and biological damage. The purpose of this study was to find out how the implementation of medical record storage in the filing room at Karang Mulya Health Center, Bojongmangu District, Bekasi Regency. The research method uses descriptive qualitative analysis method with the main informant being the head of the Puskesmas, other informants in charge of the medical record room, and medical record staff. The results of the study show that there is no standard operating procedure for storing medical records specifically for internal health centers, the results of the analysis of the storage space capacity can accommodate 30,000 medical records, and currently there are 16,319 medical records. The storage rack is still sufficient if there is a new medical record. The problem factor is in the filing room, the preparation of medical records is still stacked not in a row, and the medical records used are not immediately returned to the shelves, there has been no retention since 2017. Suggestions for standard operating procedures for storage procedures are made for internal health centers. Evaluate and improve the placement of medical records according to the applicable standards in the filing room.

Abstract

Informed consent is the consent that a patient gives to a doctor to perform a medical action after the patient receives a complete and understood explanation of the medical action. The purpose of this study is to find out the completeness of filling informed consent of inpatients at Hospital X South Jakarta, the type of research used is quantitative descriptive with a population of 353 and a sample of 78 inpatient medical records in the period October-December 2020. The method of data collection is Observation and Interview, following the results of research that has been conducted at Hospital X South Jakarta on 78 medical records known percentage average value based on 4 components, namely completeness 82% and incomplete 18%. Filling of patient identification components averages the percentage of completeness (89%), completeness of filling important report components 90%, completeness of filling the author authentication component (73%), completeness of filling of good note components (63%). Factors causing the incompleteness of informed consent are, the absence of standards of special operational procedures governing the filling of informed consent, doctors and nurses are busy due to the number of patients so that it is not fully filled, and the number of patients who do not have or bring witnesses / families when going to approve medical actions, this makes incompleteness in informed consent. With this, it is expected that the creation of SPO and socialization about filling the completeness of informed consent to be improved again.

Abstract

Medical discharge is a summary of every action and treatment provided to the patient during hospitalization and must be signed by the DPJP doctor. The purpose of this study was to identify SOPs, determine the percentage of completeness of medical discharge and identify the factors causing the incompleteness. In this study, the method used is quantitative analysis method and data collection techniques are carried out through observation, quantitative analysis and interview guidelines, using the formula for estimating the proportion of the population of hospitalized patients in June and samples taken as many as 62 medical records. Based on the results of research and discussion, the completeness of the medical discharge at the Islamic Hospital of Jakarta Sukapura, the average percentage of completeness of the four components is 85.57% and the average incompleteness is 14.43%. Judging from the 4 components of quantitative analysis, the highest component of completeness analysis was found in the patient identification component 91.94% and the lowest was in the important note component 73.45%. Factors causing the incomplete filling of medical discharges are the absence of policies that regulate filling out medical discharges, the tight schedule of doctor's practice, and the lack of socialization of discipline in filling out medical discharges.

Abstract

The contents of the Medical Record are not only data on the treatment of sick patients, but also overall health data so that it is more accurately called Health Records. In general, the Health Record is an overview of the patient's health provided by the service provider/doctor to the patient to become the patient's health record. The purpose of this study was to determine the quality of good documentation in inpatients at Dharmais Cancer Hospital. The research design is cross sectional, namely research conducted at a certain time. Data sources: article searches conducted on Google Scholar to use articles that are in accordance with the research.research method Descriptiveis to describe directly the object under study using a quantitative approach. The results of the study obtained the number of completeness of medical records reached 89.13%. The sample obtained 92 medical record files, with the results of the Initial Medical Assessment Form getting a completeness score of 88.77%, CPPT Form 87.68%, Shift Handover 90.58%, and Consultation Sheets 89.49%. Medical Record Documentation still needs to be improved. Dharmais Cancer Hospital, the number of completeness of medical records needs to be increased so that the documentation of medical records is of higher quality.

Abstract

Every hospital organizes a health insurance program, the implementation of the national health insurance program is operated by the Health Office, before the hospital collects payments to the Health Office, a complete inpatient claim requirement file is required, if the inpatient claim requirement file is incomplete it will hamper the process. the health office's claim to the hospital, causing a pending claim. The purpose of this study was to get an overview of the completeness of the claim file requirements for Covid-19 inpatients at Sumber Waras Hospital. The research was conducted using a quantitative descriptive method. The sample in this study was taken from the claim requirements file for Covid-19 inpatients at the Sumber Waras Hospital. Sampling using systematic random sampling. Data was collected using a checklist and interview guidelines submitted to Casmiex officers at Sumber Waras Hospital. Based on the results of a study of 87 files for claim requirements for Covid-19 inpatients, 75.90% were obtained. The factors causing the incompleteness of the Covid-19 inpatient claim file requirements are the Covid-19 inpatient claim requirement file provided by the service officer in hardcopy, the service officer does not provide all the files that exist at the patient's discharge date in that month, the occurrence of errors in inputting patient data and medical support officers do not directly enter the results of laboratory tests. Therefore, it is necessary to disseminate information to service personnel so that they can complete the claim requirements for inpatients in a timely manner.

Abstract

The hospital as a public health service institution is an important part that provides complete individual health services and provides inpatient, outpatient, and emergency services. Provision of medical records is a process of providing medical records starting from searching for medical records to sending medical records to the intended polyclinic for health services in accordance with health service standards. The purpose of this study was to get an overview of the timeliness of providing outpatient medical records at Fatmawati Hospital. In this study the authors decided to use a qualitative descriptive method which aims to describe the situation or field conditions regarding the occurrence of inaccuracies in the provision of outpatient medical records at Fatmawati Hospital. Based on Standard Operating Procedures, the provision of outpatient medical records at Fatmawati Hospital is <30 minutes (maximum 30 minutes), in this study it was found that the timely provision was 75.55%. With an average delivery time of 27,96 minutes. From this it is known that there is a gap related to the time delay in the process of providing medical records. Delays in the process of providing outpatient medical records are caused by factors including factors in the 5M management element which include man, machine, material, money, method. The staff's knowledge factor needs to be improved, the discipline of the supply officer is good but not optimal, the officer has not attended training on the timeliness of providing good medical records. The machine factor is the lack of number of outpatient medical record storage racks, and the lack of a trolley for the provision of medical records. The method factor is the need for periodic socialization of SOPs to remind officers' performance. The matherial factor requires trolly rejuvenation and good maintenance in order to function as it should. The money factor is that the budget should always be a priority that can be a support for the creation of good health services.

Abstract

Corona Virus Disease merupakan jenis virus yang dapat mengakibatkan infeksi pada saluran pernapasan atas (ISPA), yang menimbulkan gejala ringan hingga berat. Virus COVID-19 dapat mengakibatkan penyakit yang lebih serius misalnya pneumonia. Tanggal 2 Maret 2020 pertama kali kasus COVID-19 dilaporkan di Indonesia. Virus COVID-19 dengan cepat menyebar dan meningkat pesat di Indonesia dengan peningkatan jumlah yang terus bertambah. Penelitian ini bertujuan untuk mengkarakterisasi pasien meninggal akibat Coronavirus Disease (COVID-19) di RSUD Koja. Penelitian ini menggunakan analisis deskriptif kuantitatif. Populasi dalam penelitian ini adalah rekam medis pasien meninggal akibat COVID-19 bulan Juli 2020 sampai dengan Desember 2020. Dari hasil pengambilan sampel menggunakan rumus slovin didapatkan jumlah sampel sebanyak 81 rekam medis. Hasil penelitian menunjukkan bahwa kasus terkonfirmasi COVID-19 tertinggi terjadi pada kelompok usia ≥ 60 tahun dengan jumlah 44 %, berjenis kelamin laki-laki 61%, penyakit gagal ginjal 51%, diabetes mellitus 35%, Congestive heart failure 19%, hipertensi 16%. Kesimpulan: Sebagian besar karakteristik pasien meninggal karena kasus COVID-19 paling tinggi usia ≥ 60 tahun, laki-laki, riwayat komorbid chronic kidney disease (CKD), diabetes mellitus, penyakit kardiovaskular
