Abstract

Lung cancer is a major health concern in Indonesia due to its increasing prevalence, late-stage diagnosis, younger population, and high mortality. Addressing this issue requires nationwide implementation of comprehensive lung cancer control, which includes risk reduction and prevention strategies, focusing on tobacco control and air pollution mitigation. Screening with low-dose computed tomography (LDCT) and early detection in symptomatic patients, along with TB screening programs and all non-communicable diseases, is strongly recommended to enhance early case findings, treatment effectiveness, and overall patient outcomes. A multidisciplinary team (MDT) approach is important to ensure accurate diagnosis and comprehensive care. Moreover, the integration of palliative care at the early stages of advanced lung cancer is vital, focusing on symptom management and enhancing the quality of life for patients. While national guidelines are available for the diagnosis and treatment of lung cancer, significant disparities in healthcare access remain across Indonesia. Thus, it is essential to improve universal health coverage and referral systems to guarantee equal access to lung cancer care for patients at all levels through advocacy and ease of access.

Abstract

Rokok merupakan gulungan tembakau seukuran jari kelingking yang dibungkus dengan daun nipah atau kertas. Asap rokok mengandung lebih dari 4.000 zat kimia, dengan 40 di antaranya diketahui sebagai penyebab kanker pada berbagai organ tubuh manusia. Sebanyak delapan dari sepuluh penelitian menunjukkan bahwa merokok dapat mengurangi fungsi paru, termasuk kapasitas vital paksa, volume ekspirasi paksa dalam satu detik, VEP1 / KVP, dan aliran ekspirasi paksa pada 25-75%. Studi literatur ini merupakan tinjauan naratif yang membahas berbagai literatur primer dengan desain penelitian observasional mengenai hubungan antara derajat merokok dan penurunan fungsi paru. Penulis melakukan pencarian literatur di enam basis data, yaitu PubMed NCBI, ProQuest, Google Scholar, Elsevier, ResearchGate, dan European Respiratory Journal. Dari 1.545 literatur yang ditemukan, hanya 15 literatur yang memenuhi kriteria inklusi. Sebelas dari lima belas literatur tersebut menyatakan adanya penurunan fungsi paru yang signifikan seiring dengan peningkatan derajat merokok, sementara tidak ditemukan perbedaan signifikan pada empat literatur lainnya. Kesimpulan dari studi ini adalah adanya hubungan antara derajat merokok dengan penurunan fungsi paru, dengan sebagian besar perokok mengalami penurunan fungsi paru yang signifikan. Beberapa faktor yang berhubungan dengan penurunan fungsi paru antara lain usia, jenis kelamin, aktivitas fisik, durasi merokok, serta jumlah batang rokok yang dihisap setiap harinya

Abstract

Latar Belakang: Wabah COVID-19 (Coronavirus Disease 19) menimbulkan dampak jangka panjang terhadap kesehatan mental. Studi menilai kesehatan mental akibat keparahan COVID-19 masih belum dijadikan sebagai prioritas utama penatalaksanaan COVID-19. Objektif: Penelitian ini bertujuan mengetahui hubungan antara tingkat keparahan COVID-19 dengan stres pasien COVID-19. Metode: Penelitian potong lintang ini dilakukan pada periode 1 Maret 2020 Sampai 31 Desember 2021 di Rumah Sakit Universitas Andalas. Penelitian menggunakan data rekam medis pasien terkonfirmasi COVID-19 yang telah dinilai tingkat stres menggunakan DASS-21 (Depression, Anxiety, Stress Scale 21). Sebanyak 55 responden di ambil dengan teknik pengambilan sampel acak sederhana yang memenuhi kriteria inklusi dan ekslusi. Hasil: Lebih dari setengah pasien COVID-19 (54,5%) memiliki tingkat keparahan penyakit sedang dan 56,3% mengalami tingkat stres sedang. Uji Chi–square menunjukkan hasil yang signifikan dengan nilai p sebesar 0,000. Kesimpulan: Terdapat hubungan yang signifikan secara statistik antara tingkat keparahan COVID-19 dengan tingkat stres pada pasien terkonfirmasi COVID-19. Oleh sebab itu, pendekatan psikologis diperlukan untuk tata laksana pasien COVID-19 untuk mengurangi pengaruh psikis terhadap keparahan penyakit.

Abstract

Background: Abnormal fat accumulation affects the compliance of the respiratory system which contributes to the process of normal lung expansion. Lung function can be evaluated through an examination using spirometry with the parameters assessed, namely FVC, FEV1, and FEV1/FVC. This study aims to determine the relationship between obesity and lungfunction in the spirometry examination of Andalas University Medical Education students. Methods: This study is an observational analytic study with a cross-sectional approach. The Number of samples in this study was 34 subjects. The sampling technique used wasconsecutive sampling from students of Medical Education at the Faculty of Medicine, AndalasUniversity, Class of 2018-2020. Evaluation of lung function was carried out using aspirometry examination with parameters assessed for FVC, FEV1, and FEV1/FVC while the evaluation of obesity was carried out using the BMI with a classification according toKemenkes RI. Data analysis was carried out using univariate and bivariate qualitative methods. Results: In this study, the majority of respondents were women (70.6%) with normal nutritional status (67.6%) followed by obesity (32.4%). The relationship between obesity and lung function was tested using fisher's exact test. The results showed that there was no relationship between obesity and lung function parameters FVC, FEV1, and FEV1/FVC on spirometry tests.

Abstract

Tumor karsinoid atipikal yang berasal dari mediastinum sangat jarang; hanya beberapa kasus yang telah dilaporkan dalam literatur. Tumor yang agresif secara klinis ini termasuk ke dalam kelompok tumor neuroendokrin. Tumor karsinoid/neuroendokrin timus adalah neoplasma epitel neuroendokrin yang berasal dari timus dengan gambaran inti derajat rendah; karsinoid tipikal memiliki <2 mitosis/2 mm2 dan tidak memiliki nekrosis, sedangkan karsinoid atipikal mempunyai karakteristik 2-10 mitosis/2 mm2 dan/atau fokus nekrosis. Kami melaporkan kasus seorang pria berusia 26 tahun dengan keluhan sesak napas yang semakin bertambah sejak dua minggu sebelum dirawat di rumah sakit. Pemeriksaan radiologis menunjukkan massa isodens yang tidak homogen di mediastinum anterior. Pasien didiagnosis dengan karsinoma timus Masaoka Koga stadium IVA. Secara mikroskopis, jaringan core biopsi menunjukkan pulau sel dengan inti bulat hingga oval dengan gambaran kromatin salt and pepper dan adanya fokus nekrosis. Jumlah mitosis 2–10 mitosis/2 mm2 terkonfirmasi dengan uji Ki-67. Pemeriksaan imunohistokimia menunjukkan sel tumor terpulas positif dengan sinaptofisin, kromogranin, dan CD117. Pemeriksaan histopatologi mengonfirmasi tumor karsinoid atipikal mediastinum primer. Meskipun prognosisnya buruk, penegakan diagnosis yang cepat, multidisciplinary team (MDT) serta pemberian terapi multimodal diketahui dapat memperpanjang harapan hidup.

Abstract

Over the past two years, the COVID-19 pandemic has infected many individuals worldwide. Indonesia is one of the countries affected. This virus primarily infects the respiratory system. Some survivors have reported experiencing persistent symptoms after recovering from COVID-19, with some experiencing such symptoms for months. These persistent symptoms can also be referred to as long COVID-19. The purpose of this study is to determine the pulmonary function profile in medical students and laboratory volunteers with and without a history of COVID- 19 at the Faculty of Medicine, Andalas University. This is an observational study with a cross-sectional approach using a total sampling technique. The sample size is 43 COVID-19 survivors and 53 without history of COVID-19 from medical students and laboratory volunteers at the Faculty of Medicine, Andalas University. It is concluded and recommended that the respondents in this study have an average pulmonary function (FEV1, FVC, and FEV1/FVC ratio) in the normal category, with males being the majority among COVID-19 survivors and females among non-COVID-19 survivors.

Abstract

Objective: This study aims to determine the relationship between BCG immunization status with mortality and severity of TB meningitis in children at RSUP Dr. M. Djamil Padang. Method: This study used a cross-sectional design in children with TB meningitis and used the Kolmogorov-Smirnov and Chi-Square statistical tests. Sampling using total sampling technique with a total sample of 58 people. Data obtained from the medical records of RSUP Dr. M. Djamil Padang in 2018 – 2021. Result: This study shows that most pediatric TB meningitis patients with the condition are women (53%), aged 0-4 years (48%), good nutritional status (40%), not in direct contact with TB patients (74%), received BCG immunization (55%), severe degree (59%), and alive (76%). There is a relationship between BCG immunization status and degree of severity (p=0.046) and mortality (p=0.047) of pediatric TB meningitis patients at RSUP Dr. M. Djamil Padang in 2018 – 2021. Conclusion: The conclusion of this study is that there is a relationship between BCG immunization status and the degree of severity and mortality of TB meningitis in children at RSUP Dr. M. Djamil Padang in 2018 – 2021.

Abstract

Introduction: Targeted therapy, particularly epidermal growth factor receptor-tyrosine kinase inhibitor (EGFR-TKI), is the first-line treatment for non-small cell lung cancer (NSCLC). However, drug resistance has grown in the last few decades. This study compared the progression time of lung cancer patients treated with first- and second-generation EGFR-TKI. Methods: Based on cytology and histological results, this cross-sectional study included 1,008 participants diagnosed with lung adenocarcinoma (LUAD) from 11 Indonesian Respiratory Centers. Every three months, the response to treatment was assessed using the Response Evaluation Criteria in Solid Tumours (RECIST) criteria in 1.1. Significant differences in the clinical features of the three TKI treatment groups were identified using logistic regression analysis, the median time to disease progression was estimated using the Kaplan-Meier technique, and independent prognostic factors related to the time to progression (TTP) were assessed using Cox proportional hazards regression. Results: This study examined 505 patients, the majority of whom were females (50.9%), never smoked (59.8%), diagnosed at an advanced stage (99.2%), and had an Eastern Cooperative Oncology Group (ECOG) scale of 0-1 (83.2%). Approximately 98.1% of patients were treated with afatinib (14.8%), erlotinib (18.6%), and gefitinib (66.1%) due to common mutations. The groups did not differ significantly (p>0.05). The median overall survival (OS) rate was 9 months. The time to LUAD progression in lung cancer was significantly impacted by poor performance (p=0.001). Conclusion: Epidermal growth factor receptor-tyrosine kinase inhibitor treatment can only prolong the TTP of LUAD by up to 9 months, and the performance scale when receiving the EGFR-TKI significantly affects the prognosis.

Abstract

Penelitian ini bertujuan untuk meneliti hubungan karakteristik dengan derajat Kejadian Ikutan Pasca Imunisasi (KIPI) COVID-19 pada tenaga kesehatan di Rumah Sakit Umum Se-Kota Padang. Metode penelitian ini adalah retrospektif cohort. Hasil penelitian menunjukkan bahwa kejadian terbanyak adalah reaksi lokal yaitu nyeri pada lokasi suntikan, yang terjadi pada 98% dosis pertama dan 95,3% dosis kedua. Reaksi sistemik terbanyak adalah kelelahan yang dialami oleh 49,8% pada dosis pertama dan 45,9% pada dosis kedua. Umur terbanyak yang mengalami KIPI berkisar 26-35 tahun, dan jenis kelamin yang paling banyak mengalami KIPI adalah perempuan. Tidak ada hubungan signifikansi antara karakteristik dengan klasifikasi KIPI COVID-19 pada tenaga kesehatan di rumah sakit umum se-Kota Padang. Penelitian ini menuturkan bahwa reaksi lokal adalah kejadian terbanyak dan tidak ada hubungan signifikansi antara karakteristik dengan derajat KIPI COVID-19.
