Abstract

Type 2 diabetes mellitus was a major non communicable disease with an increasing prevalence among the productive-age population. Multiple risk factors, including sociodemographic characteristics, lifestyle behaviors, clinical conditions, and family history, were known to contribute to its development. Employees of Regional Government Organizations (RGO) represented a productive age group with a potentially high risk of type 2 diabetes mellitus. This study aimed to analyze risk factors associated with type 2 diabetes mellitus. A quantitative cross sectional study was conducted from January to February 2023 involving 3,313 RGO employees. A total of 1,955 respondents were selected using accidental sampling based on the Lemeshow formula. Data were collected through questionnaire, physical examinations, and random blood glucose measurements. The questionnaire was not subjected to validity and reliability testing, as the data were derived from direct measurements and interview findings. Statistical analyses were performed using chi-square tests and multiple logistic regression. The results showed that body mass index, sex, family history of diabetes mellitus, blood pressure, and history of heart disease were significantly associated with type 2 diabetes mellitus (p<0.05). In contrast, smoking behavior, physical activity, fruit and vegetable consumption, and history of stroke were not significantly associated. Multivariate analysis indicated that family history of diabetes mellitus was the most dominant risk factor (OR = 4.882). It was concluded that family history of diabetes mellitus was the strongest determinant of type 2 diabetes mellitus among RGO employees, emphasizing the importance of early screening and risk factor management in the productive age population.

Abstract

Dengue Hemorrhagic Fever (DHF) remained a public health problem in Bontang City. From January to December 2023, a total of 457 DHF cases were reported among 188,101 residents, with an incidence rate of 242.95 per 100,000 population and two deaths (Case Fatality Rate = 0.44%). This study aimed to identify factors associated with the occurrence of Dengue Hemorrhagic Fever in the working area of Bontang Barat Public Health Center. An analytic observational study with a retrospective case control design was conducted. The study population consisted of all DHF cases reported from January to December 2023, totaling 128 cases. Using simple random sampling, 192 respondents were selected, comprising 96 cases and 96 controls. Cases were patients with dengue hemorrhagic fever (DHF) confirmed by healthcare professionals, while the control group consisted of neighbors of the cases who agreed to participate and were matched by sex and residential area (±100 m). Data were collected from DHF surveillance records, interviews, observations, and measurements of housing density. Data were analyzed using the Chi-square test. The results showed a significant association between 3M Plus practices, the presence of mosquito larvae (p < 0.001), and the frequency of cleaning water storage containers (p = 0.001) with the occurrence of DHF. In contrast, housing density (p = 0.721) and the habit of hanging worn clothes (p = 0.771) were not associated with DHF incidence. Conclusion the occurrence of DHF was mainly influenced by behavioral and environmental factors therefore, strengthening community based 3M Plus prevention practices was identified as a key strategy for DHF control.

Abstract

Anak Buah Kapal (ABK) kargo internasional merupakan kelompok pekerja dengan mobilitas tinggi yang rentan terhadap perilaku berisiko HIV. Penelitian mengenai determinan perilaku berisiko HIV pada ABK masih terbatas dan umumnya hanya menilai faktor perilaku secara parsial. Penelitian ini bertujuan menganalisis hubungan faktor individu, lingkungan, dan pelayanan kesehatan dengan perilaku berisiko HIV pada ABK kargo internasional di Muara Mahakam serta mengidentifikasi faktor yang paling dominan. Penelitian kuantitatif analitik dengan desain cross-sectional ini dilakukan pada 193 ABK kargo internasional di wilayah kerja Balai Kekarantinaan Kesehatan Kelas I Samarinda tahun 2025. Sampel diperoleh menggunakan teknik teknik purposive sampling. Data dikumpulkan melalui kuesioner terstruktur berbasis Health Belief Model (HBM) yang telah diuji validitas dan reliabilitas. Analisis data meliputi analisis univariat, uji Chi-square, dan regresi logistik berganda dengan metode Backward Likelihood Ratio pada tingkat signifikansi 5%. Analisis bivariat menunjukkan bahwa lama bekerja, lama berlayar, hubungan seksual dengan pasangan tidak tetap, jumlah pasangan seksual, riwayat pemeriksaan HIV, pengetahuan, persepsi hambatan, dan akses pelayanan kesehatan berhubungan dengan perilaku berisiko HIV (p<0,05). Analisis multivariat menunjukkan bahwa lama bekerja (Exp(B)=0,014; p=0,003), hubungan seksual dengan pasangan tidak tetap (Exp(B)=0,003; p<0,001), jumlah pasangan seksual (Exp(B)=0,032; p=0,005), dan persepsi hambatan (Exp(B)=9,004; p=0,004) berasosiasi secara independen dengan perilaku berisiko HIV. Model regresi memiliki kesesuaian yang baik berdasarkan uji Hosmer–Lemeshow (p=0,479) dengan nilai Nagelkerke R Square sebesar 0,888. Perilaku berisiko HIV pada ABK kargo internasional di Muara Mahakam berasosiasi dengan lama bekerja, hubungan seksual dengan pasangan tidak tetap, jumlah pasangan seksual, dan persepsi hambatan. Program pencegahan HIV perlu difokuskan pada pengurangan hambatan akses layanan kesehatan, penguatan edukasi berbasis Health Belief Model, serta penyediaan layanan konseling, tes HIV sukarela, dan kondom di lingkungan pelabuhan.

Abstract

Dengue Hemorrhagic Fever (DHF) remains a major public health issue in Indonesia. In 2023, a total of 114,720 DHF cases were reported nationwide, with an incidence rate (IR) of 41.4 per 100,000 population—still far above the national target. East Kalimantan is one of the provinces with the highest IR (147.33/100,000). Samarinda City is among the top three areas with the highest number of cases. One of the affected areas in Samarinda is Temindung Public Health Center, which recorded 83 DHF cases, with 52% occurring in children aged 10–14 years, indicating that this age group is the most vulnerable to DHF. This study aimed to analyze the risk factors associated with DHF incidence among children aged 10–14 years in the working area of Temindung Public Health Center. A quantitative approach was used with a case-control study design and total sampling of 81 respondents, consisting of 27 cases and 54 controls. Data analysis was conducted using the Chi-Square test and Odds Ratio (OR) calculation. The results showed significant associations maternal knowledge (p= 0.001; OR= 11.440), PSN behavior (p= 0.001; OR= 7.231), cadre support (p= 0.022; OR= 3.571), and health facilities availability (p= 0.034; OR= 9.100). Therefore, it’s recommended to involve students as mosquito larvae monitors, conduct thematic health education with PSN practice demonstrations, and optimize the distribution of facilities through Musrenbang and CSR-based partnerships.

Abstract

Dengue Hemorrhagic Fever (DHF) remains a major public health concern, particularly in tropical regions such as Indonesia, where it has persisted for over five decades. In 2023, Samarinda ranked third among ten regencies or cities in East Kalimantan with the highest number of DHF cases. One of the primary health centers consistently ranking among the top three with the highest DHF cases in Samarinda from 2021 to 2023 is Temindung Public Health Center. Environmental conditions and community behavior are believed to play significant roles in the transmission of this disease. This study aimed to identify factors associated with DHF incidence, including the condition of water storage containers (TPA), lighting, the presence of mosquito larvae, and Mosquito Nest Eradication (PSN) behavior. A cross-sectional design with a quantitative approach was used, involving 138 respondents selected through purposive sampling within the working area of Temindung Public Health Center. Data were collected using questionnaires, direct observation, and lighting measurements, and analyzed using the Chi-square test. The results showed significant associations between the availability of TPA covers (p = 0.037), household lighting (p = 0.017), PSN behavior (p = 0.000), and the presence of mosquito larvae (p = 0.000) with DHF incidence. These factors are interrelated in increasing the risk of DHF transmission in residential areas. Proper management of water storage, adequate household lighting, mosquito larvae control, and improved PSN behavior are crucial in reducing DHF cases. Active involvement of both the community and public health centers is essential in implementing continuous and sustainable preventive measures.
