Abstract

The reseach aimed to know The Influence Of Completeness Form Informed Consent Surgical Cases Of Inpatients To SNARS Edition 1 Element Of Assesment HPK 5.2 At Rumah Sakit Umum Pindad Bandung. The research method used quantitative research with descriptive approach and data processing using SPSS version 16 program. Data collecting dissemination of the observation, interview, documentation, and review of the literature. Sampling teqnique used purposive sampling with a sample of 235 form surgical cases of inpatients. From the result of an intensive writers could formulate the subject matter that 1) it was still found filling informed consent Surgical Cases Of Inpatients from that’s not completed, there was 69 or 29% informed consent form filing uncompleteness 2) recording form informed consent of inpatient surgical cases in a hurry 3) lack of implementation in filling out informed consent form of inpatient surgical cases 4) lack of accuracy in filling out informed consent forms of inpatient surgical cases. The suggestion were given by :1) Giving punishment if informed consent form of inpatient surgical cases was reporting uncompletely 2) Increasing the complete informed consent form of inpatient surgical cases 3) Holding monitoring or evaluation by the hospital, 2) Coordination directly with the physician about the incompleteness from surgical cases of inpatients.

Abstract

This research aims to know the influence of medical record confidentiality protection against clinic accreditation standards 3.4. at Pratama Luhur Medica Centre Clinic Cimahi. The research Method used are quantitative research with descriptive approach. Data collection techniques used are by way of observation, questionnaires and studies of the literature. Based on a simple linear regression test results with the number of samples of n = 6 respondents and significant α = 0.1 confidence level of 90% with the influence of 99.2% of the variable and medical record confidentiality protection 0.8% is affected by other variables. The problems was found are : (1) Not yet available regulation and policies about the privacy of medical record, (2) File medical record storage shelves are still mixed with other things, (3) Storage space is still fused with other rooms such as the living room, (4) Not yet safeguards door to access medical record file storage space, (5) CCTV is not focus to medical record storage space (6) There are medical records are missing, (7) There are medical records are broken. As for the advice given to these problems : (1) Create policies and procedure standards operational related to medical record confidentiality, (2) Employee lockers provided, (3) Create a special room or given medical record delimiter between room, (4) Installing a finger print or access code on the entrance of storage space, (5) Addition of CCTV on entrance door and storage space, (6) Provision of tracer for mark to medical record that come out, (7) Create folders to minimize damaged medical records

Abstract

ABSTRACT This topic was choosen aimed to literature review medical information release and claims Covid-19 patient care costs in hospitasl. Type of review used with literature review methods. Data source used is a secondary data source obtained through Google and Google Scholar, choosed by inclusion criteria. The literature review is based on two journals about releasing medical information for patient financing. The results of the literature review in the two journals, there are similarities in the application of the procedure for the release of medical information, namely that there are still inconsistencies in: 1) The third party applicant does not include a power of attorney and incomplete filling of medical records, especially the medical resume form 2) The officer is not careful to re-examine the completeness filling out requests for medical information so that it can affect the confidentiality of medical records. Recommendations given include: 1) Further researchers can use a qualitative method with a descriptive approach, using triagulation data collection techniques between observation, interviews, and documentation that the information required is in accordance with the research 2) For further researchers, they must use indicators for the release of medical information, namely procedures for requesting medical information, requesting parties, medical information makers, forms, and delivery of medical information that the release of medical information can be known in accordance with the provisions as evaluation.

Abstract

This research aims to make design of system information design by quantitative analysis of medical records inpatient in Rumah Sakit Bhayangkara Kota Bengkulu.The method used in this research is qualitative research method which is a direct research method, such as data collection, field study and literature study, which is related to research problem, while software development using waterfall method with unified modeling language (UML) as design of software. Programming language used is PHP and database used is MySQL. Based on the research, various problems are found: quantitative analysis of medical records is not well computerized and reports have not been implemented properly. The suggestions given are: 1) the design of information system of quantitative analysis of medical record to optimize quantitative medical record analysis and recapitulation report of result of quantitative analysis, 2) maintain and develop information system of quantitative analysis of medical record and 3) follow up to doctors or an officer for completing an incomplete medical record form. Keywords: Design, system information, medical record, php and MySQL.

Abstract

Pelayanan yang dilandasi oleh kewaspadaan, kewaspadaan, kecepatan, dan daya tanggap dalam memberikan respon terhadap pasien ketika memerlukan bantuan guna mencapai kepuasan pasien disebut dengan daya tanggap. Kepuasan pasien tidak dapat dipisahkan dari sumber daya manusia yang disediakan oleh rumah sakit, dan perawat merupakan salah satu sumber daya manusia yang dimanfaatkan dalam pemberian pelayanan perawatan. Bukan hal yang aneh jika perawat berfungsi sebagai pengamat kepuasan pasien. Hal ini disebabkan oleh kenyataan bahwa pelayanan itu sendiri merupakan faktor paling signifikan dalam menentukan bagaimana pasien memandang suatu rumah sakit. Salah satu tujuan penelitian ini adalah untuk mengetahui dampak daya tanggap atau responsiveness perawat terhadap tingkat kepuasan yang dialami pasien. Tinjauan literatur adalah pendekatan penelitian yang digunakan dalam proses pembuatan esai ini. Anda dapat mencari makalah dengan menggunakan Google School dan sumber bibliografi lainnya. Temuan yang dikumpulkan menunjukkan bahwa daya tanggap atau responsiveness perawat mempunyai pengaruh yang signifikan terhadap tingkat kepuasan yang dialami pasien. Kesimpulannya, daya tanggap perawat dan tingkat kesenangan yang dialami pasien sangat erat hubungannya.

Abstract

Background: Quantitative analysis at Rumah Sakit TK II Dustira Cimahi is carried out by 2 (two) methods, namely manually using files and electronically through the E-Medrec application, where it is necessary to assess whether these activities have reached the standard and the aim is to find out whether there are obstacles or obstacles in their implementation, so that improvements can be made. Objective: To evaluate the implementation of the use of E-Medrec in the implementation of quantitative analysis of inpatient medical records at Rumah Sakit TK II Dustira Cimahi. Research Methods: The method used in this research is qualitative analysis with a descriptive approach using the TAM (Technology Acceptance Model) research model, researchers examine 3 aspects, namely aspects of usefulness (Perceived Usefulness), aspects of ease of use (Perceived Ease Of Use), and aspects of behavioral interest (Attitude Towards Behavior). Results: The results of data collection collected through questionnaires were processed and then converted into a percentage form to describe the results of the implementation of E-Medrec in the quantitative analysis of inpatient medical records. Conclusion: In the aspect of perceived usefulness, namely the perceived usefulness of E-Medrec, all respondents have good indicators. In the aspect of perceived ease of use, namely the perception of ease, all respondents have adequate indicators. In the aspect of attitude towards behavior, namely attitude / interest in using E-Medrec, all respondents have good indicators. The implementation of E-Medrec that has been running can be used as a consideration in planning the development and implementation of health information systems.

Abstract

This community service program aims to reports the result of community service concerning the implementation of diseases prevention through health education in Sapan to decrease morbidity.The method usage for the research is a quantitative method with a descriptive approach. Data collection techniques usage through observation, interviews, counseling and literature review related to the subject matter. The problem that were found from the research are : 1) The high rates of morbidity in Sapan Desa Sumbersari Kecamatan Ciparay Kabupaten Bandung, 2) The Lack of public knowledge about PHBS and public awareness will throw garbage in its place, 3) The unavailability of trash cans around the road. The suggestion given were: 1) The community needs to know and be aware of infectious and non-infectious disease and the application of PHBS, 2) Puskesmas needs to improve socialization and health education for the community, 3) The local goverment provides a trash cans on the roadside and garbage banks in each hamlet.

Abstract

The training and implementation of the medical record retention application at Klinik Pratama Prima Husada Bandung aim to enhance the efficiency of medical record document management in accordance with applicable regulations. The application is designed to support the selection, transfer, and disposal of medical record documents based on the specified retention period, thereby reducing the accumulation of physical documents and expediting information access. The training activities involved healthcare workers and administrative staff, covering topics such as the introduction of medical record retention concepts, application usage, and document management simulations. The implementation results showed that the application improved accuracy in categorizing documents into active and inactive groups and enhanced efficiency in retrieving required documents. Furthermore, the application ensured compliance with regulations related to medical record management. With the success of this training and implementation, Klinik Pratama Prima Husada Bandung is expected to improve the quality of medical record management, support digital transformation, and provide better-organized healthcare services to patients. Among the 20 participants, 5 (25%) were highly proficient, 5 (25%) were moderately proficient, and 10 (50%) were proficient.

Abstract

Penerapan Rekam Medis Elektronik (RME) di Rumah Sakit X bertujuan untuk meningkatkan efisiensi kerja, akurasi data pasien, serta kualitas pelayanan kesehatan di unit rawat inap. Penelitian bertujuan untuk mengevaluasi penerapan RME dari perspektif pengguna menggunakan pendekatan Technology Acceptance Model (TAM) berdasarkan tiga aspek utama yaitu kebermanfaatan (usefulness), kemudahan penggunaan (ease of use), dan minat perilaku pengguna (behavioral intention to use). Metode yang digunakan dalam penelitian ini adalah observasi dan wawancara terhadap tenaga medis yang menggunakan sistem RME di unit rawat inap. Hasil evaluasi menunjukkan bahwa RME berhasil meningkatkan efisiensi kerja tenaga medis dengan mempercepat akses data pasien secara real-time dan mengurangi ketergantungan pada berkas fisik. Selain itu, RME juga meningkatkan akurasi pencatatan data medis, mengurangi kesalahan yang sering terjadi pada sistem manual. Kemudahan penggunaan RME didukung oleh antarmuka yang user-friendly dan adanya pelatihan yang memadai bagi tenaga medis, meskipun beberapa kendala teknis seperti konektivitas internet dan kebutuhan pelatihan tambahan masih perlu diperhatikan. Secara keseluruhan, pengguna menunjukkan niat yang kuat untuk terus menggunakan RME, yang dipengaruhi oleh manfaat yang dirasakan serta kemudahan dalam pengoperasiannya. Penelitian ini menyimpulkan bahwa penerapan RME di Rumah Sakit X, khususnya di unit rawat inap, berkontribusi positif terhadap efisiensi, akurasi data, dan kualitas pelayanan kesehatan, dengan potensi untuk terus berkembang melalui peningkatan infrastruktur teknologi dan pelatihan berkelanjutan.

Abstract

Untuk mendapatkan kinerja instalasi rekam medis yang berkualitas maka dalam penyelenggaraan rekam medis harus dilakukan dengan benar dan tepat, termasuk pada pengembalian dan pendistribusian rekam medis. Standar pengembalian rekam medis di Rumah Sakit Umum Pusat Nasional dr. Cipto Mangunkusumo adalah kurang dari 1x24 jam dan pendistribusian rekam medis adalah kurang dari 10 menit. Keterlambatan dalam pengembalian rekam medis dan pada pendistribusian rekam medis akan mengganggu pelayanan medis yang akan diberikan kepada pasien saat kontrol ulang. Tujuan penelitian ini adalah untuk mendapatkan gambaran mengenai pengembalian rekam medis rawat jalan dan kecepatan pendistribusian rekam medis ke poliklinik di Rumah Sakit Umum Pusat Nasional dr. Cipto Mangunkusumo. Metode penelitian yang digunakan adalah metode deskriptif. Hasil penelitian pada pengembalian rekam medis yang tepat waktu sebesar 83% dan yang tidak tepat waktu sebesar 17%. Rata – rata pendistribusian rekam medis ke nurse station adalah 13 menit. Berdasarkan hasil penelitian tersebut diketahui bahwa pengembalian rekam medis rawat jalan dan pendistribusian rekam medis belum berjalan dengan baik. Keterlambatan pada pengembalian rekam medis dikarenakan tidak ada pengecekan terhadap rekam medis yang dipinjam dengan yang dikembalikan oleh perawat poliklinik dan petugas casemix. Keterlambatan pada pendistribusian rekam medis karena pengembalian rekam medis yang lebih dari 1x24 jam. Agar pada proses rekam medis bisa berjalan dengan baik maka disarankan adanya pengawasan serta pengontrolan terhadap pengembalian rekam medis.
