Abstract

Rumah sakit merupakan institusi pelayanan kesehatan yang melakukan pelayanan kesehatan yang menyediakan pelayanan rawat inap, rawat jalan, dan gawat darurat. Rumah sakit juga berfungsi memberikan pelayanan medik, pelayanan penunjang, pelayanan keperawatan, pelayanan rehabilitasi dan pencegahan penyakit. Instalasi rawat jalan adalah unit fungsional untuk berobat rawat jalan. Dalam rawat jalan akan ada pendaftaran pasien berobat rawat jalan dan pasien akan memiliki waktu tunggu dalam mendapatkan pelayanan. Waktu tunggu pasien adalah waktu yang digunakan pasien untuk mendapatkan pelayan kesehatan rawat jalan. Dalam penelitian ini menggunakan metode kualitatif dengan pendekatan deskriptif dengan menggunakan data primer dan melakukan wawancara serta observasi secara langsung. Hasil dari penelitian ini mendapatkan bahwa rata-rata pasien berkunjung adalah 86 orang perhari dengan rata-rata waktu pasien baru adalah 9 menit 31 detik dan pasien lama 15 menit 52 detik. Waktu ini dikategorikan lama karena rata-rata waktu pendaftaran rawat jalan adalah 10 menit. Dalam penelitian juga menunjukan beberapa kendala yang mengakibatkan waktu tunggu menjadi lama seperti penuhnya poli yang dituju, kurangnya petugas pendaftaran, kurangnya computer dan printer yang tersedia serta jaringan yang error dan tempat penyimpanan yang sudah padat. Saran dalam permasalahan ini bagi rumah sakit adalah melakukan evaluasi terhadap pelayanan terutama dalam waktu tunggu dan bagian penyimpanan dan saran kepada petugas adalah melakukan teamwork yang lebih merata dan membuat antrian yang lebih efektif seperti membuka loket tambahan dibagian pendaftaran rawat jalan.

Abstract

Technology that develops very rapidly brings many changes, especially in the development of medical records in hospitals that are increasing. Electronic medical records bring many benefits, one of which is improving the quality of services in hospitals. The medical records used at Hermina Arcamanik Hospital are mostly electronic-based, including reporting. However, problems were found in the medical record installation, namely, there were several reports that had not been managed optimally. This is because reporting still uses google spreadsheets which requires data to be moved manually one by one, so it takes a long time, and the quality of reporting becomes less accurate. A real example of this problem can be seen in the process of reporting patient Mortalitys. Meanwhile, accurate and systematic reporting of Mortality data is essential in supporting decision-making in hospitals. Therefore, the design of a patient Mortality reporting information system is urgently needed to make time efficient and improve the quality of reports. The method used in designing this system is extreme programming and web-based. The results of this information system design are able to shorten the time to make Mortality reports and can produce accurate data

Abstract

Puskesmas merupakan sarana pelayanan kesehatan yang menyelenggarakan upaya kesehatan warga serta upaya kesehatan perseorangan tingkatan awal dengan lebih mengutamakan upaya promosi kesehatan serta pencegahan penyakit di daerah kerjanya. Masalah yang terjadi dalam pencatatan kunjungan harian pasien rawat jalan dipuskesmas masih dikerjakan secara manual, sehingga tidak menutup kemungkinan akan terjadinya kekeliruan dalam pencatatan kunjungan harian pasien. Selain itu, dalam membuat laporan bulanan petugas menghitung satu-persatu jumlah kunjungan pasien pada hari itu secara manual sehingga membutuhkan waktu yang lama. Tujuan penelitian ini adalah merancang Program Sistem Informasi kunjungan harian pasien menggunakan Microsoft Visual Studio 2010 untuk mempermudah petugas dalam pencatatan dan pelaporanya setiap bulan. Metode penelitian ini menggunakan metode kualitatif dengan metode pengembangan perangkat lunak air terjun / Waterfall. Hasil akhir dari perancangan sistem ini menghasilkan Sistem Informasi kunjungan harian pasien menggunakan Microsoft Visual Studio 2010 yang diharapkan akan memudahkan dalam pencatatan dan pelapooran kunjungan harian pasien karena sudah terkomputerisasi.

Abstract

Penelitian dilakukan dengan tujuan untuk merancang sebuah Sistem Pelaporan Rawat Jalan dengan Microsoft Visual Studio 2010 di Puskesmas Pangalengan DTP. Pendekatan dalam penelitian ini menggunakan metode deskriptif kualitatif. Perolehan informasi dari penelitian ini melalui berbagai metode diantaranya riset, konsultasi dan studi literatur serta metode waterfall sebagai metode pengembangannya. Sistem ini dirancang dengan Data Flow Diagram (DFD) dan diimplementasikan dengan bahasa pemrograman Microsoft Visual Studio 2010 dan media penyimpanan menggunakan Microsoft Access. Hasil diharapkan, membantu petugas dalam melakukan statistik pasien dan penanganan pelaporan rawat jalan sehingga pelaporan menjadi lebih efektif dan efisien. Abstract

Aplikasi Electronic Medical Record (EMR) HINAI WEB di RS Hermina Arcamanik Kota Bandung berfungsi untuk mendukung pengelolaan data pasien dan pelaporan statistik. Namun, implementasinya masih menghadapi kendala dalam menyajikan data yang lengkap dan terintegrasi, sehingga berdampak pada efisiensi kerja tenaga medis serta akurasi laporan statistik rumah sakit. Penelitian ini bertujuan untuk menganalisis keterbatasan data statistik dalam EMR serta mengidentifikasi faktor penyebab ketidaktepatan data dalam sistem pelaporan. Penelitian ini menggunakan pendekatan kuantitatif dengan teknik survei, wawancara mendalam, observasi, dan dokumentasi. Responden terdiri dari petugas pelaporan data statistik rawat jalan dan rawat inap. Hasil penelitian menunjukkan bahwa minimnya fitur otomatisasi dalam EMR, ketidaksesuaian informasi, serta keterbatasan dalam pengelompokan data pasien menyebabkan tenaga medis harus melakukan banyak koreksi manual, termasuk mengambil data dari sumber lain seperti Google Drive. Untuk mengatasi masalah ini, diperlukan otomatisasi pemilahan data berdasarkan unit pelayanan, status pasien dan jenis jaminan kesehatan, integrasi dengan penyimpanan berbasis internet, serta penerapan sistem validasi otomatis. Dengan perbaikan ini, EMR diharapkan dapat lebih optimal dalam mendukung pengelolaan data statistik dan meningkatkan efektivitas layanan rumah sakit. Kata kunci: EMR, data statistik, pelaporan

Abstract

Evaluation of the implementation of Electronic Medical Records (EMDR) in the Hospital Management Information System (SIMRS) is crucial to ensure the effectiveness of health services. This study examines the implementation of EMR in SIMRS MORBIS at Hospital X. The purpose of the study was to evaluate the success of the implementation using the HOT-Fit model by analyzing the Human, Organizational, Technology, and Net Benefit aspects. The research method used a quantitative approach by distributing HOT-Fit -based questionnaires to SIMRS MORBIS users, and the data was analyzed descriptively. The results showed a successful implementation of the human component (average score of 3.95) showing high adoption and satisfaction, the organizational component (3.85) was supportive but internal communication needed to be improved, the technology component (3.58) had good information quality, but system quality needed attention and significant Net Benefit (3.83-3.91) was achieved in efficiency and service quality. It was concluded that the implementation of EMR in SIMRS MORBIS at Hospital X was generally successful and had a positive impact, but optimization of the system quality and internal communication of the organization was needed to improve overall performance.

Abstract

This study aims to evaluate the variation in the use of International Classification of Diseases 10 (ICD-10) codes in the diagnosis recording of outpatient patients in the Integrated Management of Childhood Illness (IMCI) room of the Cinagara Public Health Center. This research employed a descriptive quantitative method with a cross-sectional approach. Data collection was carried out through observation of outpatient medical records from January to December 2024. The study sample included all medical records that met the inclusion criteria. The results showed variations in the use of ICD-10 codes, with a coding accuracy rate of 78.5%. The highest variations were found in the diagnosis of acute respiratory infections (ARI) and diarrhea. Further analysis indicated that these discrepancies could affect the accuracy of morbidity and mortality data, as well as the planning and evaluation of health programs. Factors influencing coding variations include the lack of health workers' understanding of ICD-10, workload, and the availability of appropriate coding guidelines. This study recommends periodic training on ICD-10 coding, the provision of comprehensive coding guidelines, and a routine audit system to improve the quality of diagnosis coding in the MTBS room of the Cinagara Public Health Center. These efforts are expected to improve the quality of health data and facilitate more appropriate decision-making. This research provides an important contribution to efforts to improve the quality of medical record data and health information systems at the Public Health Center level. By understanding the root of the problem and implementing the recommendations provided, it is hoped that the Cinagara Public Health Center can improve efficiency and effectiveness in health services.

Abstract

The implementation of the Electronic Medical Record (EMR) system at Hospital X is a strategic step to enhance the efficiency and quality of healthcare services. This study aims to evaluate data security aspects of the EMR system, focusing on access security, data integrity, and system availability. A descriptive qualitative method was used to explore patient data protection practices. The results indicate that the hospital has implemented role-based access control, audit trails, and user activity monitoring. However, several shortcomings were identified, such as the absence of a regular password change policy, lack of two-factor authentication, and infrequent system security testing. In terms of availability, the system is supported by 24-hour vendor services, although technical challenges such as network disruptions and hardware limitations remain. This study recommends strengthening security policies, conducting regular staff training, and improving technological infrastructure to ensure optimal patient data protection and service effectiveness.

Abstract

The accuracy of diagnosis and procedure coding in the INA-CBG’s system is a key factor in the smooth submission of BPJS claims at hospitals. This study aims to analyze the relationship between the accuracy of coding and the validity of BPJS claims in medical rehabilitation outpatients at Hospital X. The method used is mixed methods with a sequential explanatory approach. The quantitative stage analyzed 2,216 patient claim files for January 2025 using the Chi-square test. Next, in-depth interviews were conducted with those responsible for managing outpatient claims to explore the causes of coding inaccuracies. The results show that 96.2% of files were coded correctly, 3.8% were coded incorrectly, and all are pending claims. The Chi-square test shows a significant relationship between coding accuracy and claim eligibility (p = 0.000). Qualitative findings revealed major obstacles in the form of differences in interpretation of coding, limited training in coding, and suboptimal hospital information systems. Regular training, improved coordination between units, and development of an electronic recording system in line with medical rehabilitation service needs are recommended. This study is expected to contribute to improving the quality of medical documentation and the effectiveness of health care financing through the INA-CBG’s claims system.

Abstract

As health service providers, hospitals have been required to implement an electronic medical record (EMR) system. The goal of implementing an EMR is to improve the accuracy, efficiency, and security of patient medical data. Good medical records are also an indicator of hospital quality, supporting the accreditation process and service efficiency. The author's analysis is motivated by the need to understand the impact of EMR implementation at Oto Iskandar Di Nata General Hospital on the completeness of medical record data. The research method is a descriptive, quantitative analysis with a checklist form as the research instrument. The results showed that the highest percentage of completeness was in the important part of the report with an overall percentage of 89%, while the lowest percentage was in the author authentication section with an overall percentage of 66%. One of the reasons for incompleteness was due to system limitations. In conclusion, although the EMR system has begun to be implemented, the level of data completeness still does not meet the standards set by the Ministry of Health Regulation.
