Abstract

Tuberkulosis paru (TB) pada anak kian menjadi masalah kesehatan global yang masih terlupakan seiring dengan peningkatan proporsi TB di Indonesia. Penularan penyakit ini di populasi umum seringkali berdampak pada anak, terlebih ketika kontak TB terjadi di rumah tangga. Penelitian ini bertujuan untuk memperoleh faktor protektif sehingga anak tetap sehat meskipun memiliki kontak dengan penderita TB dewasa serumah. Penelitian ini dilakukan dengan pendekatan kasus kontrol pada 132 responden anak yang berasal dari delapan rumah sakit rujukan dan beberapa puskesmas di Provinsi Daerah Istimewa Yogyakarta. Penelitian dilakukan dalam periode Januari hingga Desember 2014 yang hasilnya dianalisis dengan uji bivariat (kai kuadrat) dan multivariat (regresi logistik ganda). Hasil penelitian menunjukkan bahwa rumah yang memenuhi syarat kesehatan, yakni ruang tidur yang sehat, serta paparan yang jarang diterima dari penderita TB dewasa mampu memproteksi anak agar tetap sehat meskipun tinggal serumah dengan penderita dewasa penyakit ini. Penelitian ini menyimpulkan bahwa lama tinggal bersama bukanlah faktor risiko penyakit TB pada anak.Hal ini karena meskipun lama tinggal bersama antara penderita TB dewasa dengan anak, namun apabila memiliki paparan yang jarang, hal ini pun tidak signifikan menjadikan anak untuk terkena TB. Pulmonary tuberculosis (TB) in children is a neglected global health problem, with an increasing proportion of TB cases in Indonesia. Children with TB are most often impacted by TB transmission in the population at large, especially adult TB that exists in the child’s household. This study aimed to find protective factors that can keep children healthy despite household adult TB contacts. This study reports on 132 respondents with a case-control study conducted at nine referred hospitals and several health centers based on medical records at Special Region of Yogyakarta Province. The study lasted from January to December 2014, while the data analysis was used by both of bivariate (chi-square) and multivariate (multiple logistic regression) analysis. The study found that healthy houses, especially those with healthy bedrooms and fewer exposures to adult TB sufferer, influenced by confounder variables, protected children from TB even though they were exposed to adult TB in their environment. Longer periods of living together is not a risk factor for children to contract TB when living with adult TB patients at home. However, this risk increases with frequent exposure among children to adult TB patients at home.

Abstract

Dalam banyak penerapannya, penggunaan sistem peresepan elektronik (computerized physician order entry, CPOE) dalam penulisan resep terbukti mampu menurunkan angka kesalahan peresepan obat (adverse drug event, ADE). Kesalahan pengobatan seperti kekeliruan penulisan nama obat, penentuan dosis, dan penggunaan jenis obat yang tepat untuk pasien dengan kondisi kesehatan tertentu di pelayanan kesehatan dapat ditekan. Namun, CPOE masih memiliki kelemahan seperti tambahan waktu untuk mengentri data pasien oleh para dokter ke dalam sistem. Untuk meng- evaluasi dampak persepsi pengguna tentang penerapan CPOE terhadap keselamatan pengobatan pasien, suatu penelitian kualitatif telah dilakukan di Gadjah Mada Medical Health Center yang telah mengadopsi sistem ini selama sepuluh tahun. Data dan informasi dikumpukan dengan wawancara mendalam terhadap sembilan dokter (tujuh dokter umum dan dua dokter spesialis) sebagai informen pengguna CPOE menggunakan pedoman wawancara, alat perekam suara, buku catat, dan daftar centang observasi. Hasil studi menunjukkan bahwa sepuluh tahun CPOE telah mampu meningkatkan efektivitas dan efisiensi pelayanan kesehatan, tetapi potensi ADE masih terjadi akibat gangguan komunikasi pasien-dokter karena perlunya waktu tambahan untuk entri data. Disimpulkan bahwa sistem CPOE belum mampu mendukung pengambilan keputusan untuk mencegah ADE dalam pengobatan pasien. In most of its applications, Computerized Physician Order Entry (CPOE) has been improved patient safety by reducing medication errors and subsequent adverse drug events (ADEs). Medication error such as elixir in writing, determination of dose, and correct drug type used for patient with certain health condition in health services can be reduced. But, the CPOE still has some weknesses such as additional time to entry the patient information by physician to the sistem. To evaluate the impact of end-user perception using CPOE sistem on patient safety medication, a qualitative research has been conducted at Gadjah Mada Medical Health Center which has adopted this sistem for ten years. Data and informations were collected by in-depth interviewing nine physicians (seven general docters and two specialists) as end-user informen using interview guide, tape recorder, note-book, and checklist observation tools. The results show that ten year CPOE implemention has improved the effectiveness and efficiency of medical health care, but potential adverse drug event (ADE) such as drug prescription errors still occur as a result of patient-physician communication distraction due to additional time for data entry. It is concluded that CPOE sistem has not been able to support decision to prevent the ADEs in patient medication.

Abstract

Before the COVID-19 pandemic, the Bogor City Government regulated to cover the health financing claim during the Indonesian National Health Insurance (NHI) integration period due to the lower amount of health care claim per episode in regional hospitals compared to ones that NHI paid. This study aimed to address post-COVID-19 health financing at two hospitals in Bogor City, West Java Province, Indonesia. Descriptive analysis using the aggregate statistical summaries was taken to explore the medical care episodes of the data series at two hospitals for the last two years. Of the 890 checked medical records data, the deficit occurred in 197 (22.1%) medical care episodes, while five (0.6%) exceeded the hospitals' tariffs. The remaining 688 (77.3%) medical care episodes had suits with the Indonesian-Case Based Groups. Almost a quarter of medical care episodes in aggregate experienced a deficit in the two years before the pandemic. This study is the first to provide new insight into the discussion on medical care financing in a developing country's post-pandemic era in a newly implemented NHI system.

Abstract

Tuberculosis (TB) in Indonesia is still a health problem. The TB prevalence in Indonesia ranks second in the world after India in 2023. Regional factors, inadequate healthcare facilities, and limited resources (financial, human, and infrastructure) are challenges requiring innovation to help the government control TB. TB eradication efforts need to be made with a comprehensive and effective approach. One method used is to look at the vast territory of Indonesia, therefore, mapping the TB disease vulnerability is a highly recommended method. The use of a Geographic Information System is expected to help map the TB vulnerability areas in Indonesia. Given the existence of several epidemiological, socio-geographic, and environmental factors influencing TB, the question arises of how to map TB vulnerability areas in Indonesia. This study used a cross-sectional design, secondary data was collected from several sources, and a vulnerability analysis was performed by considering several socio-environmental epidemiological variables. Furthermore, after the analysis, the TB area vulnerability category would be obtained along with a map of TB vulnerability areas in Indonesia according to regional and district analysis units. This study produces a TB susceptibility index and map in Indonesia for the regions of Sumatra, Java-Bali, and other regions.

Abstract

Several health risks are associated with performing Hajj pilgrimage. Food poisoning is a major cause of diarrhea and vomiting during Hajj. Food hygiene is an effort to control aspects of food, people, places, and equipment that could cause disease or health problems. This study aimed to describe food hygiene and sanitation efforts implemented in 2023 at the Jakarta Pondok Gede Hajj Embarkation Dormitory in Jakarta, implemented in 2023 to safeguard foods for the Hajj pilgrims. Using a descriptive qualitative design with a document analysis approach, this study examined activities during the pre-embarkation and embarkation periods. Pre-embarkation efforts included inspecting the kitchen environment and coaching the catering team, while during the embarkation period, activities involved checking the hygiene of catering services, inspecting foods brought by pilgrims, and assessing the quality of food and drink. This study found that the dormitory kitchen did not meet the necessary facilities and infrastructure requirements, affecting food safety. Additionally, food handlers did not fully comply with hygiene and sanitation standards. This study emphasized the necessity of meeting hygiene requirements in the kitchen, food handling, equipment, and ingredients since the pre-embarkation period.
