Abstract

Background: Stroke is a brain disease from local and globalnervous function disorders. Strokes appear suddenly,progressively, and rapidly. Decreased balance results in a highrisk of falling and can be detected using the Timed Up and Gotest (TUG). Objective: This study aims to determine the validityand reliability of TUG in terms of intra-rater and inter-rater.This research was carried out at a Government hospital atBoyolali, Central Java, with a population of 36 chronic strokepatients. Methods: This study used an observational studywith methodological research. Results: The Cronbach's alphaof intra-rater and inter-rater reliability of the TUG were morethan 0.9, with both ICC were also more than 0.9. The Pearsonproduct-moment between tests 1 and 2 revealed asignificantly very high correlation (p<0.001 and r=0.986). TheTUG showed high validity and reliability for intra-rater andinter-rater for stroke patients to measure dynaimc balance instroke patients.

Abstract

Penyebab utama kematian dan merupakan masalah kesehatan di negara berpenghasilan rendah dan menengah adalah hipertensi. Penyebab hipertensi beragam salah satunya yaitu kurangnya aktivitas fisik. Olahraga pada penderita hipertensi dapat menjadi tindakan non farmakologis karena olahraga dapat memberikan efek penurunan tekanan darah yang berkelanjutan. Penulis melakukan pendekatan penyuluhan dan senam sebagai upaya promotif dan preventif guna mengontrol tekanan darah lansia dengan hipertensi. Penyuluhan dilakukan di posyandu lansia Kinasih di Kartasura yang diadakan selama satu hari. Sebelum penyuluhan dilakukan dilakukan pre test terlebih dahulu pada peserta penyuluhan dan dilakukan post test untuk melihat pemahaman peserta pada penyuluhan tersebut. Hasil menunjukkan bahwa terdapat peningkatan pemahaman dari 88,9% menjadi 96,42%. Mencegah atau membatasi respon hipertrofi terhadap hipertensi dapat menjaga fungsi jantung dan juga mencegah gagal jantung atau komplikasi lain. kesimpulan yang didapat adalah pemberian edukasi hipertensi dan aktivitas fisik dapat menjadi salah satu cara untuk menjaga tekanan darah normal. Aktivitas yang dapat dilakukan untuk penderita hipertensi salah satunya latihan aerobik. Jenis latihan aerobik yang dapat dilakukan salah satunya senam. Hal ini didukung dengan adanya penyampaian materi yang tepat dan media leaflet dan poster sehingga dapat dipahami oleh seluruh anggota komunitas

Abstract

Background: Pain in the lower back is a common case experienced by most people in the world and one of the global health problems and identified quite high prevalence among the productive age. Piriformis syndrome often cause of misdiagnosis of LBP and sciatica secondary to entrapment of the sciatic nerve in the piriformis muscle by the greater degree of sciatica. Piriformis Syndrome result the pain in the gluteal area, piriformis muscle spasm, decreases gluteal muscle strength and decreases functional activities. Physiotherapy management for Piriformis Syndrome can be done using stretching modalities. Research Objectives: to determine the effect of self stretching on the degree of pain due to piriformis syndrome at Teras Health Center, Boyolali Regency. Research Methods: This study used a case study. There were 5 people who met inclusion criteria. Pain was measured by VAS that confirmed after treatment. The demography data age, body mass, and bodymass index, gender, and grade of OA. Result: The pain showed decreased between pre-test and post-test after treatment. Conclusion: Self stretching was giving beneficial to reduce pain in individuals with piriformis syndrome.

Abstract

Background: Musculoskeletal disorders are a major problem and high prevalence among elderly, characterized by joint pain and associated with age. Based on the data for patients visited in physiotherapy department at the Indramayu General Hospital during January 2021, musculoskeletal disorders was the most common case. Methods: Medical records of patients as source of data that analyzed by high to low prevalence case from physiotherapy department, Indramayu General Hospital during January 2021 Results: The knee osteoarthritis was the highest number of musculoskeletal cases (50%). A quarter of the population suffers from low back pain, and lumbar radiculopathy had 12.5% of issues detected. Conclusion: the knee osteoarthritis was being the commonest musculoskeletal problem among elderly in Indonesia, especially in Indramayu General Hospital.

Abstract

Background: Pain is one of the most commonly reported and prominent factors that are responsible for physical inactivity in patient with knee Osteoarthritis (OA). There are many measuring tools that can used in clinical practice, one of is which is the Numerical Rating Scale (NRS). The aim this study was analyzing the inter rater reliability of NRS in individuals with knee osteoarthritis. Metode: The study was planned in accordance with guidelines for reporting reliability studies. 10 participants enrolled in this study. Intra-class correlations, two-way random effects model, (ICC 2.1) with 95% confidence intervals, standard error of measurement and minimal detectable change. Result: The inter rater reliability of NRS was excellent (ICC=0.929 95% CI = 0.712 – 0.982). The correlation coefficient recorded has higher than 0.8. The average of ages 59,9 years old. Conclusion: This study had excellent inter rater reliability by excellent ICC, which means the NRS is acceptable measurement that can be used to evaluate pain.

Abstract

Introduction:Diabetic Peripheral Neuropathy (DPN) is one of the microvascular complications of diabetes, which occurs due to high levels of sugar in the blood, resulting in damage to a group of nerves. The purpose of this study is to determine the effectiveness of the giving of Transcutaneous Electrical Nerve Stimulation (TENS), Exercise Therapy, and Kinesio Tape in cases of Diabetic Peripheral Neuropathy. Case Presentation:The patient in the study was a 64-year-old woman with a diabetes history who complained of tingling, a slight numbness, and a burning sensation in both legs. The complaints will worsen at night. Management and Outcome:The subject got therapy in Transcutaneous Electrical Nerve Stimulation (TENS), Exercise Therapy, and Kinesio Tape as many as three times. The Paraesthesia was measured using the numeric pain rating scale (NPRS), lower extremity muscle strength was measured using 5STS, the static balance using MCTSIB, and the dynamic balance using TUG. Discussion:After being given therapy, It obtained results in the form of decreased Paraesthesia measured using the Numeric Pain Rating Scale (NPRS) silent pain T1: 10 to T3: 9, motion pain T1: 10 to T3: 9. Increased muscle strength in lower extremities and postural in T1: 18 to T3: 15. Increased static balance from T1:0.35 to T3:0.41. Improved dynamic balance from T1:14.90 to T3:14.45. Conclusion:Therapy in Transcutaneous Electrical Nerve Stimulation (TENS), Exercise Therapy, and Kinesio Tape effectively reduces Paraesthesia, increases lower extremity and postural muscle strength, improves static balance, and improves dynamic balance.

Abstract

Introduction : Rupture is the tearing of tissue caused by trauma. The anterior cruciate ligament (ACL) is one of the main ligaments of the knee which functions to prevent the tibia from shifting anteriorly from the femur and to control rotational movement of the knee. case presentation: A 29-year-old male patient with as a sports teacher came to physiotherapy complaining of pain and stiffness in the knee joint at Wongsonegoro Hospital, Semarang, Central Java. The patient did a surgery of anterior cruciate ligament on October 2020. The management of exercises set for phase 2 rehabilitation. Management and outcomes :The exercises rehabilitation program included static bicycles, forward lunges on bosu, side lunges on bosu, step ups on bosu, squads with theraband, forward step down, lateral step down with resistance bands for 1 month. Several physical examinations were carried out to examine pain of knee using VAS (Visual Analog Scale), muscle strength examination using Manual Muscle Testing (MMT) on the hamstring and quadriceps muscles, a goneometer used to measure joint range of motion for knee flexion and extension movements, and functional examination measured by Knee Injury and Osteoarthritis Outcome Score (KOOS). Discussion: exercises rehabilitation for phase 2 post surgery o anterior cruciate ligament (ACL) served to restore balance, increase muscle strength by stabilizing and strengthening during exercise for activation of neumuscular control. Conclusion : A month of exercises programs for three times a week were successfully decreasing pain, increasing muscle strength, range of joint motion and functional activity as well.

Abstract

Introduction: The slump test is a neurodynamic test to evaluate the mechanosensitivity of neuromeningeal structures within the vertebral canal. The straight leg raising (SLR) is a provocation test that evidences radicular irritation in the lumbosacral region by lower limb flexion and can be due to multiple causes. Active stretching effectively reduces pain and improves function in adults with LBP (first degree) in sub-acute and chronic phases. Methods: This study was a case report that observed twelve patients with chronic LBP divided into two groups. Group 1 received slum and SLR stretching, while in the group 2 did not treated with stretching, however only got the SWD and TENS. The pain intensity and functional ability were evaluated after treatment by VAS and Oswestry scale respectively. Objective: To determine the effectiveness of slump and SLR stretching for reducing pain and increasing ADL in patients with chronic myogenic LBP. Results: Fifty patients with a diagnosis of chronic myogenic LBP, 12 patients met the inclusion criteria devided into 2 groups. 11 of 12 patients reported decreased pain and increased functional activity. There was a decrease in pain 9 points and an increase in the functional activity of 12.3% in the group 1. While in the group 2, there was a decrease in pain as 3 points and an increase functional activity 2.33% as well. Conclusion: Slump and SLR stretching can reduce pain and increase functional activity in chronic myogenic LBP patients, although the intervention was only given twice and few samples. Further research needs to be done by increasing the number of samples and training doses.

Abstract

Introduction: Leprosy is a skin disease caused by Mycobacterium leprae, which is an intracytoplasmic parasite on macrophages and Schwann cells. This bacterium attacks the peripheral nerves, especially the Schwann cells. Mycobacterium leprae was first discovered by a Norwegian named Gerhard Armauer Hansen. One of the consequences of peripheral nerve disorders in people with leprosy is drophand/wrist drop/finger drop caused by damage in radial nerve. Case Presentation: A male patient aged 29 years, at the Kelet’s Hospital, Jepara, Central Java, on January 11, 2021 in a bed rest phase, had a fever of 37.8 C. His diagnose was wrist drop affected by leprosy. The condition of the skin was dry and peeled and indicated wrist drop. Previously, the patient had treated at one of the health centers in Juwana city, Central Java, and received the 4th MDT, the haemoglobin was 7 and already received a blood transfusion. Thus, the patient was referred to Kelet’s Hospital to get better treatments and the doctor suggested to do physiotherapy treatment. Management and Outcome: The interventions used the virgin coconut oil (VCO) Oil, passive, active assisted and stretching for 3 sessions. The impairments were limitation movement for wrist flexion and extension. The evaluation carried out the Manual Muscle Testing (MMT) to measure the muscle strength of wrist flexion and extension and the xerosis problem value applied by Overall Dry Skin Score (ODSS). Discussion: After 3 sessions, the physical examination declaired muscle strength for wrist flexion and extension had the same pattern was 3 of 5 and xerosis problem had not decreased yet. The second treatment reported muscle strength was not increasing, however the skin condition was more moist. The third evaluation found a reduction of xerosis condition, as well as increased muscle strength for wrist flexion and extension were 4 of 5. Conclusion: A management of physiotherapy used VCO Oil and exercise therapy applied passive, active assisted and stretching in patients with wrist drop affected by leprosy, reported giving positive effects to xerosis condition and able increase muscle strength of flexion and extension of wrist joint.

Abstract

Introduction: Drop Foot is a gait disorder that drops the foot forward due to weakness, irritation or damage to peroneal nerve, and paralysis of the muscles in the back of the foot. This is usually a symptom of a bigger problem, not the disease itself. It is characterized by inability or impairment to raise the toes or lift the foot from ankle. Based on data quoted from the Agency for Healthcare Research and Quality (AHRQ), throughout 2009 in the United States there were 1.787 patients who experienced drop foot in hospitals. In the UK, there are 126.699 patients will experience unilateral and bilateral foot drop. Case Presentation: A 26-year-old man was involved in a traffic accident on 1 years ago when he worked in a mine located at East Kalimantan, Samarinda. One day he was going to work form home had a traffic accident with fracture arm and with a deep open wound at medial leg. He was unconscious for 3 days after the accident. After 4 months of being treated in Samarinda, the patient moved to Surabaya because he followed his wife worked. He was treated at UNAIR Hospital for 4 months. Thus, he moved to Klaten and stayed at his family’s place and started doing treatment at Klaten Islamic Hospital. Management and Outcome: Functional Electrical Stimulation (FES), Ankle and Foot Orthoses (AFO), Exercise can increase muscle strength and functional activity that measured by Manual Muscle Test (MMT) and Foot and Ankle Disability Index (FADI) Discussion: FES was able increase muscle strength. Its mechanism is based the depolarization of axons by an electric field. A bidirectional action potensial, activating the motor unit direct depolarization of muscle can used to stimulate the general peroneal nerve, activating the dorsiflexor muscles of the foot during the swing phase of the gait. The AFO improved subtalar joint motion, while also providing stability anterior-posterior and medial-lateral. AFO has beneficial effects in term of improving functional mobility, gait quality and decreased fall rates in these subjects. Therapeutic exercise was a systematic and planned performance intended body movement, posture, or physical activity to provide patients/clients with the means to fix or prevent impairment, repairing, restoring or improving physical function, prevent or reduce health-related risk factors, optimize your health status, fitness, or overall sense of wellbeing. Conclusion: The treatment of Functional Electrical Stimulation (FES), Exercise, and Ankle Foot Orthoses (AFO) in this case is to be useful for patients with foot drop cases caused by nerve damage after a traffic accident, increase muscle strength and increase functional activity.
