Abstract

Abstract

Background: Diabetes Mellitus (DM) is characterized by hyperglycemia. Type 2 diabetes mellitus (T2DM) is a common type of DM. It gives microvascular and macrovascular complications in the body. The diabetic peripheral neuropathy (DPN) is declared as the highest complication of T2DM, about two third cases of DM. It impacts to high risk of falling. As well the duration of DM and body balance also contributed to balance stability. Poor of those factors increase the risk of falling. However, previous studies have not declared the dominant factor contributing to falling in individuals with T2DM. Purpose: The purpose of this study was to determine the dominant factors contributing in the risk factors of falling risk in individuals with T2DM. Methods: The patients confirmed T2DM by medical diagnosis and had 5 years above of T2DM. The glycemic level was set by taken blood sample from intravenous after fasted for 8 hours. The Michigan Neuropathy Screening Instrument (MNSI) was used to screen the DPN. The static balance was measured by the Berg Balance Scale (BBS), while dynamic balance was measured by Time Up and Go test. The risk of falling was confirmed by the cut score of the Timed Up and Go test. This study has done in DM community, Surakarta, Central Java, Indonesia. Results: The correlation test showed the duration of DM and DPN had correlation with balance. had a contribution to the risk of falling significantly (p-value <0.05). The Logistic Regression Model presented the duration of DM (OR : 1.008, p-value=0.001, 95% CI = 1.518 – 4.943) dan balance (OR = 0.317, p-value = 0.001, 955 CI = 1.518 – 4.943) influenced the risk of falling in individuals with T2DM. Conclusion: The longer suffer of DM and uncontrol glycemic level induced the development of DPN. The duration of suffering from DM must be accompanied by good lifestyle management to avoid development some complications of DM.

Abstract

Background: type 2 diabetes mellitus (T2DM) is characterized by hyperglycemia. Prolonged hyperglycemia is noted impacting microvascular and macrovascular complications. Diabetic peripheral neuropathy (DPN) is the highest microvascular complication in T2DM, increasing the risk of falling. Factors of duration of DM and balance have a strong influence on the risk of falling. However, the risk factors for falls in T2DM have not been established recently. Purpose: the purpose of this study was to determine the correlation between glycemic level, DPN and postural balance in patients with T2DM, as well as to determine the dominant factor of high risk of falling in patients with T2DM. Methods: The participant in this study confirmed having T2DM by medical diagnosis for 5 years and more. The glycemic level (FGL) was measured by fasting for 8 hours glycemic level, DPN was clarified by physical examination of MNSI and postural balance was assessed by mCTSIB, BBS and TUG tests. The risk for falling was taken from the categorical score of The Timed Up and Go test with a cut score of >13.5 secs as a high risk of falling. This research used a cross-sectional study conducted in DM community in Surakarta, Central Java, Indonesia. Results: There was a correlation between FGL and DPN, DPN and static balance, as well static balance and dynamic balance. The logistic regression revealed duration of DM as a dominant factor of high risk to fall (OR 1.376, p-value+0.023, 95% CI=1.044 – 1.814). Conclusion: prolonged duration of DM combines with hyperglycemia status impact to DPN and reduces balance performance in patients with T2DM.

Abstract

Introduction: Osteoarthritis (OA) is the most common joint inflammation that often occurs in the knee region and its prevalence increases with age. The prevalence of knee OA is the highest compared to other regions. Static balance is one of the problems in OA patients which is evaluated by the One leg stance test, but the reliability of this measuring instrument for patients with knee OA has not been reported in Indonesia. Research objective: To determine the reliability of the intra rater and inter rater one leg stance test as a measure of static balance in patients with knee osteoarthritis. Materials and methods: The research method used was an observational study with a methodological research approach. There were 58 respondents who signed informed consent and were involved in this study. Results/findings: Intra rater reliability showed a very high internal consistency in both right and left eye open positions (?>0.9) with ICC=0.8-0.9. Whereas OLS in closed eyes shows moderate to low internal consistency and moderate ICC. The results of the inter-rater reliability analysis showed very high internal consistency and ICC for the right and left leg open eyes. Whereas in closed eyes it has internal consistency and moderate to high ICC for the right and left. Conclusion: The ability to maintain static balance in the closed eye position is something that is not easy for patients with knee OA to do, possibly due to pain and also reduced function in the elderly, this is indicated by the low consistency value for OLS in the closed eye position.

Abstract

Introduction: Diabetes mellitus and hypertension are degenerative diseases with a high prevalence in the elderly population and commonly occur together as comorbidities, contributing to worsening metabolic and cardiovascular conditions. The coexistence of these diseases increases the risk of serious metabolic complications. Aerobic is a recommended non-pharmacological intervention because it helps control blood glucose levels, improves insulin sensitivity, and reduces blood pressure through mechanisms that enhance endothelial function and cardiorespiratory capacity. Objective: This study aims to analyze the relationship between aerobic activity levels and the status of diabetes mellitus and hypertension in elderly individuals diagnosed with type 2 diabetes mellitus in the community. Method: An analytical observational study with a cross-sectional design. Result and Discussion: The Mann-Whitney test showed a significant association between aerobic activity and diabetes mellitus status (p = 0.001), indicating that lower aerobic levels were associated with higher glucose levels. Aerobic activity was also significantly associated with hypertension status (p < 0.001), showing that lower aerobic increased the likelihood of elevated blood pressure. Conclusions: Increasing aerobic activity is an important non-pharmacological strategy to manage metabolic and cardiovascular risks in older adults with diabetes, and should be integrated into structured community health programs.

Abstract

Background: Knee osteoarthritis is a degenerative joint disease that often occurs in the geriatric population and causes pain, limited movement, and decreased function. In advanced conditions, management is carried out with Total Knee Replacement (TKR). However, post-surgery patients still experience functional limitations so that physiotherapy intervention is needed. Objective: To determine the effectiveness of static bicycle and quadriceps strengthening exercises on pain, range of motion, muscle strength, functional mobility, and quality of life in post-TKR patients. Methods: This study used a case study design in one patient with post-TKR dextra et causa bilateral knee osteoarthritis. The intervention was given for 3 weeks with a frequency of 2 times per week. Evaluation was carried out using the Numeric Rating Scale (NRS), goniometer, Manual Muscle Testing (MMT), Timed Up and Go Test (TUG), and the Knee Osteoarthritis Outcome Score (KOOS). Results: Results showed a decrease in motion pain from NRS 6 to 4 and stationary pain from 3 to 2. Knee range of motion increased from 75° to 90° of flexion and from -5° to 0° of extension. Muscle strength increased from MMT 3 to 4 in the quadriceps muscle. Functional mobility also improved, with TUG time decreasing from 22 seconds to 18 seconds. However, the KOOS score remained relatively unchanged at 38.84. Conclusion: Static bicycle and quadriceps strengthening exercises were effective in reducing pain, increasing ROM, muscle strength, and functional mobility in post-TKR patients. However, the improvement in quality of life as measured by the KOOS was not significant, requiring a longer rehabilitation period.
