Abstract

Background and Purpose: Breast cancer surgery may cause late effects of pain, range of motion loss, and activity limitation in the upper extremity (UE). The surgical complications was shoulder adhesive capsulitis (AC). AC is a condition of progressive loss of UE range of motion (ROM) and leading to limitations functional mobility. The purpose of this case report is to investigate the implementation of transcutaneous electrical nerve stimulation (TENS) combined with joint mobilization technique and hold relax technique for a patient with post mastectomy surgery and chemotherapy within a year with AC. Method: The patient was a 55-year-old female who was referred to physical therapy, Dr. Sardjito Central General Hospital. The patient had restricted mobility and pain in her left UE with a medical diagnosis of AC. A comprehensive physical treatments have done as well TENS, joint mobilization technique, and hold relax technique for twice a week. The outcomes were measured by goniometer, manual muscle testing (MMT), visual analog scale (VAS) and shoulder pain and disability index (SPADI). Result: The active ROM of shoulder increased significantly, decreased pain intensity (7/10 to 6/10) and increase muscle strength (3/5 to 4/5) and as well better functional performance (73.1% to 66.9%) in patient with AC affected by post mastectomy surgery. Conclusion: A good physiotherapy strategy for post mastectomy, clearly reduced impairments as muscle strength, joint mobility and functional activities of shoulder joint. As well this study could be a reference for clinicians to manage the patients with shoulder problem after breast cancer surgery.

Abstract

Introduction: Chronic Obstructive Pulmonary Disease(COPD) is a chronic lung disease accompanied by airflow limitation that is not fully reversible. Airflow obstruction is generally progressive and is associated with an abnormal inflammatory response of the lungs to noxious particles or gases. The most common problems include chronic cough, increased sputum and shortness of breath. This increase in sputum and shortness of breath can affect changes in the respiratory rate of COPD patients. Case Presentations: A 55-year-old male patient complained of shortness of breath and cough. The patient works as a textile factory worker. The patient was diagnosed with COPD by a Lung Specialist and referred for physiotherapy to improve breathing patterns so that the respiratory rate returns to normal. Management and Outcomes: Effect of Pursed Lip Breathing Exercise to determine the respiratory rate in patient with Discussions: After physiotherapy intervention and evaluation were carried out, it was found that there was a decrease in the respiratory rate of 24 times per minute and T1: 19 times per minute. This happens because the pursed lip breathing exercise mechanism itself is a technique that focuses on expiration by deliberately inhibiting the shape of the lips that make it difficult for air to escape, thus requiring the help of diaphragm and abdominal muscle contractions to expel the air so that the volume of air will increase. Conclusion: After the physiotherapy session, the results showed that there was a The effect of pursed lip breathing exercise on respiratory rate in COPD patients is to provide a decrease in respiratory rate.

Abstract

Introduction: A mitral regurgitation (MR) is the second most common single native valvular disease and accounts for 31.6% of valvular heart disease patients with degenerative etiology as much as 61.3%, rheumatism 14.2%, endocarditis 3.5%, inflammation 0.8%, congenital 4.8%, ischemic 7.3%, miscellaneous 8.1%.2 In patients who undergoing surgical intervention for severe MR, the most common etiologies being MVP (20%-70% of cases), ischemic MR (13%- 30% of cases), rheumatic disease (3%-40% of cases), and endocarditis (10%- 12% cases). Case presentation: A 44-year-old housewife patient complained of pain in the area around the operation of Dr. Moewardi’s hospital, Surakarta, especially when coughed and sneezed and body rotation involved movement in the chest area. Found muscle spasm in the area intercostals muscles, scaleni, levator scapula, and pectoralis major. On auscultation there was a clicking sound coming from a mechanical valve, no cardiac murmur was heard, percussion was resonant. laboratory test Fasting Blood Sugar 148 mg/dl, and other results look normal. Blood pressure was 108/46 mmHg, SPO2 was 98%, temperature was 36.8˚C, respiratory rate was 20 bpm, heart rate was 70 bpm. Vital examinations were performed before performing the strength test. Management and outcomes: Performed massage with efflurage technique to reduce spasm in the accessory muscles of respiration. The patient underwent a physiotherapy rehabilitation program using a 6 minutes walking test with telemetry. The exercise phase lasts about 20-60 minutes with the brisk walking exercise method. Borg Scale – RPE Category Scale was used to measure the perception of fatigue and dyspnea symptoms during exercise. Exercise is given 10 times in 2 weeks with one session of 60 minutes Disscussion: Physiotherapy technique as effleurage is an effective technique used to increase blood flow, reduce muscle tightness and relieve muscle tension. And walk brisk were able to reduce the fatique symptom of patient and increase the capability of tolerance activity. Although the aerobic capacity was not altering as well. Conclution: The regular exercises can significantly improve the quality of life in patient with post surgery mitral valve replacement.

Abstract

Postural balance is a crucial aspect of daily life that enables individuals to move with stability and safety. Children with Down Syndrome (DS) often experience balance disorders due to cognitive function limitations that affect their motor abilities. This study aims to explore the relationship between cognitive function and balance in children with DS. Cognitive impairments, including memory deficits, attention difficulties, and executive function challenges, can impact a child's ability to control posture and perform motor activities effectively. Additionally, difficulties in movement coordination and sensorimotor responses contribute to poor balance abilities. Understanding the connection between cognitive function and balance can facilitate the implementation of appropriate interventions, such as physical therapy and cognitive stimulation, to improve the quality of life of children with DS. A holistic approach is essential in supporting the development of children with DS to help them achieve greater independence. Parents and families play a vital role in actively participating in the growth and development of children with DS, ensuring that their cognitive function and balance are maintained and enhanced. Consequently, children with DS can become more independent and socially engaged, enabling them to interact with others and build healthy social relationships. Abstract

Pelemgadung Village is located in Sragen Regency, Central Java, where the majority of the population makes their living as farmers. According to the results of situation analysis and data collection, it was found that low back pain is one of the conditions that often occurs in the village. Low back pain is a musculoskeletal disorder that arises due to a static body position while working, resulting in pain in the lower back. The aim of this activity is to increase farmers’ understanding and knowledge of reducing lower back pain. The method used was education about low back pain using leaflets as a health promotion medium, as well as distributing pre- and post- questionnaires to assess farmers’ knowledge about low back pain. The results of the questionnaire show an increase in farmers’ understanding, which can be seen from the number of participants with the highest scores. During the pre-test, 24 people achieved the highest score, while in the post-test, the number increased to 35 people. Based on the education that has been carried out, it can be concluded that there has been an increase in farmers’ understanding of low back pain and how to overcome this condition.

Abstract

Osteoarthritis (OA) is characterized by pain that causes a decrease in knee joint range of motion, affecting a person's gait change and quality of life. Manipulation with Movement (MWM) is the most commonly used manual therapy technique in musculoskeletal disorders, especially in arthritis, to increase the range of movement (ROM).  The MWM technique with a sliding direction aims to minimize pain and increase ROM in patients with knee OA. Objective: To determined the effect of MWM on pain and range of movement (ROM) in knee joint OA patients. Methods: This research was an experimental study with a quasi-experimental approach without a control group design. The research group consisted of 18 female patients who used the MWM technique two times a week for four weeks. The pain and ROM were measured by NRS and goniometer, respectively. Results: There were significant effects of MWM to reduce pain and increase ROM of knee joint (p<0.05). The average ROM of knee flexion and knee extension were improved by 12.2 and 1.7 degrees, respectively. The average pain was decreased by 2.4 as well after being treated by MWM for four weeks. Conclusion: MWM exercises were trained for four weeks to reduce pain and increase knee range of motion in flexion and extension in OA patients. MWM is recommended as an approach exercise for patients with knee OA to be involved in a rehabilitation program.Keywords: mobilization with movement, pain, range of joint movement, knee osteoarthritis

Abstract

Plantar fasciitis is a common cause of heel pain that is often found in the population aged 40-60 years, especially women with prolonged standing activities and high body mass index. This case report discusses physiotherapy management in a 41-year-old female patient with plantar fasciitis dextra who underwent therapy at Panembahan Senopati Hospital. The physiotherapy interventions provided included ultrasound, gastrocnemius muscle massage, stretching (Towel Stretch), and exercises (Calf Raise and Towel Toe Curl) for three sessions in three weeks. Evaluation was performed using Wong Baker Scale, Manual Muscle Testing (MMT), Goniometer, and Foot and Ankle Disability Index (FADI). The results showed a decrease in tenderness and pain during movement, increased plantar fascia flexibility, and an increase in FADI score from 63.28% to 77.88%, indicating improved function. Physiotherapy interventions proved to be effective in managing plantar fasciitis symptoms conservatively and improving the patient's quality of life.

Abstract

Human breathing involves a process that brings oxygen from the outside air to body tissues and expels carbon dioxide from the lungs. This mechanism is characterized by decreased inspiratory pressure, reduced minute ventilation, and increased tension in the respiratory muscles. So that breathing for humans is very important, in certain conditions, the respiratory tract can be disrupted and cause various symptoms, one of the respiratory disorders is pneumothorax. Pneumothorax is the presence of air in the pleural cavity. This can cause problems with ventilation, oxygen supply, or both. When these symptoms occur in the absence of lung trauma or obvious pathology, the patient is said to have primary spontaneous pneumothorax. If the pneumothorax is accompanied by complications or the presence of pulmonary disease, the prognosis of the disease will be worse and treatment will be more difficult. One of the actions in pneumothorax cases is physiotherapy rehabilitation. The purpose of this study is to find out whether physiotherapy intervention can reduce complaints in pneumothorax patients. . Patients are given exercise therapy in the form of pursed lip breathing, thoracic expansion exercise and gentle massage. From the therapy given as much as 3x therapy, the results were obtained in the form of decreased shortness of breath, increased thoracic expansion and increased functional activity.

Abstract

Anak difabel memiliki kebutuhan khusus akibat adanya gangguan dalam pertumbuhan dan perkembangan, baik secara fisik, sensorik, intelektual, maupun emosional. Salah satu gangguan yang umum adalah gangguan motorik, yang berdampak pada keterbatasan dalam melakukan aktivitas fisik sehari-hari. Penanganan melalui latihan motorik yang sesuai sangat penting untuk meningkatkan kekuatan otot, koordinasi, fleksibilitas, serta efikasi diri anak. Kegiatan pengabdian masyarakat ini dilaksanakan di YPAC Surakarta dengan tujuan meningkatkan pengetahuan guru olahraga dan orang tua mengenai pentingnya latihan motorik yang disesuaikan dengan klasifikasi gangguan anak. Metode kegiatan meliputi observasi, penyuluhan, pelatihan langsung, dan evaluasi menggunakan pre-test, post-test, serta monitoring keberlanjutan latihan di rumah. Hasil kegiatan menunjukkan bahwa pendekatan latihan yang individual dan terstruktur dapat meningkatkan pemahaman pendamping (guru dan orang tua), sekaligus membantu anak difabel mengembangkan potensi fisik dan psikologisnya. Kolaborasi antara pendidik, orang tua, serta tenaga profesional menjadi kunci dalam membentuk anak difabel yang berdaya saing dan berjiwa juara

Abstract

Anak dengan Down Syndrome (DS) menghadapi berbagai hambatan perkembangan, baik secara fisik, kognitif, sosial, maupun emosional, sehingga memerlukan layanan khusus. Salah satu masalah utama yang dialami adalah rendahnya keseimbangan akibat hipotonia, kelemahan otot, gangguan postural, serta keterbatasan sistem sensorimotor, yang berimplikasi pada risiko jatuh dan keterlambatan motorik. Selain itu, anak DS juga cenderung mengalami hambatan dalam sosialisasi yang berdampak pada rendahnya rasa percaya diri. Penelitian dan kegiatan pengabdian masyarakat di SLB D1 Surakarta ini dirancang untuk meningkatkan keseimbangan dan rasa percaya diri melalui intervensi menari di atas landasan dengan berbagai tekstur permukaan serta pemberian buku cerita interaktif. Metode yang digunakan meliputi baseline assessment, sosialisasi, latihan motorik, serta evaluasi melalui pre-test dan post-test terhadap pengetahuan guru pendamping dan orang tua. Hasil kegiatan menunjukkan adanya peningkatan signifikan pada pemahaman orang tua dan guru mengenai pentingnya keseimbangan dan rasa percaya diri, dengan capaian pengetahuan baik meningkat dari 57,9% menjadi 89,5%. Selain itu, anak menunjukkan keterlibatan aktif dalam latihan yang menyenangkan, yang diyakini dapat memperbaiki kontrol postural sekaligus memupuk rasa percaya diri melalui pengalaman positif. Dengan demikian, kegiatan menari dan membaca buku cerita bergambar dapat menjadi strategi komplementer yang efektif untuk mendukung tumbuh kembang anak DS secara holistik.
