Abstract

Abstract

Low birth weight babies (LBW) in agricultural and coastal areas have different health problems thus possibly having different care needs. Inappropriate promotion of child health, especially LBW, in both clinical and community service settings can increase morbidity and mortality rates. The purpose of this study was to identify the determinants of LBW babies care needs based on family perspectives in coastal and agricultural areas. This cross-sectional observational study was conducted in June 2023 using a questionnaire to identify the determinants of LBW’s babies care. The population of this study was children and mothers in coastal and agricultural areas. Multistage random cluster sampling with the G*Power application obtained 193 participants per region, resulting in a total sample of 386. Data were analyzed using difference in means between two regions. The results showed that agricultural regions had higher mean on LBW care, parental stress, and family empowerment. Coastal areas had higher mean scores on quality of life, maternal confidence, and food diversity practices. There was a significant difference between coastal and agricultural areas on the determinants of LBW’babies care between coastal and agricultural areas, especially on the variables of parental quality of life (p=0.000), parenting stress (p=0.029), maternal confidence (p-0.000) and family empowerment (p = 0.000). Optimizing health promotion by pediatric nurses in the scope of hospital and community settings can be achieved by empowering families and paying attention to parenting stress, maternal confidence, and parents' quality of life.

Abstract

Background: Quality of life (QoL) is a crucial indicator for evaluating the well being of patients undergoing hemodialysis, encompassing physical, psychological, social, and environmental dimensions. Accurate measurement of QoL serves as a fundamental basis for clinical decision making, care planning, and the development of patient centered policies. Purpose: To identify, synthesize, and analyze the instruments used to measure QoL in hemodialysis patients within the international literature. Method: The PRISMA-ScR approach, employing a systematic search across four major databases: ProQuest, BMC, PubMed, Wiley, and other sources. The search yielded seven articles that met the inclusion criteria and were subsequently analyzed thematically. Results: Seven primary instruments were identified, namely the KDQOL-SF™, KDQOL-36, QDIS-7, HARQ, WHOQOL-BREF, EQ-5D-5L, and the Caregiver Quality of Life Inventory. Each instrument demonstrated specific strengths related to construct design, contextual applicability, and target population, with adequate validity and reliability. However, variations in dimensional coverage, limited cross cultural validation, and a predominance of cross-sectional designs without longitudinal evaluation were observed. Conclusion: The findings highlight the importance of contextual adaptation, multi-cultural validation, and integration of QoL instruments into clinical evaluation systems. They also provide a foundation for developing more responsive and culturally relevant tools to improve care quality and patient well-being.

Abstract

The Maternal Mortality Rate (MMR) due to delayed decision-making remain a major issue, primarily because women and their husbands often do not take an active role as decision-makers. This study aimed to examine the characteristics and influence of various factors including level of knowledge, choice of referral health facility, considerations in hospital selection, family influence, main factors in referral decisions, belief or trust factors, and husband's psychological factors on referral decisions for high-risk pregnancies in Lumajang Regency. The respondents were husbands of pregnant women with a high-risk pregnancy (KSPR ≥ 6) during the period January-June 2023. This quantitative research employed a case-control design using probability sampling.  Cluster sampling was conducted across 25 Community Health Canters, followed by proportionate stratified random sampling, yielding a total sample of 300 participants. The Chi-Square test showed a significant relationship (p<0.05) between psychological factors (p=0.035) and referral decisions for high-risk pregnancies. Meanwhile, other variables including level of knowledge (p=0.893), choice of hospital type (p=0.155), consideration in selecting referral health facilities (p=0.969), family influence (p=0.394), main consideration for referral approval (p=0.901), and belief or trust factors (p=0.702) did not show a significant relationship with husband's referral decision. Binary logistic regression analysis revealed that husband without psychological issues were 2.397 times more likely to agree to referral for high-risk pregnancies compared to those with psychological problems.   Keywords: Husband's decision, referral, delivery, high risk pregnancy

Abstract

Adherence to therapy is an essential component of successful pulmonary tuberculosis treatment, affecting patient recovery, prevention of drug resistance, and public health outcomes. Understanding compliance factors can guide interventions to improve treatment success rates. This systematic review explored the factors influencing adherence to therapy among tuberculosis patients, synthesizing evidence from various studies to understand the problem comprehensively. A systematic literature search followed PRISMA guidelines, including primary databases and screening for relevant studies. Twenty-four studies that met the inclusion criteria were analyzed. Data extraction and quality assessment were carried out to ensure the robustness of the findings. There were 2,729 articles identified according to the criteria, and 24 articles were obtained that analyzed several factors affecting patient with tuberculosis (PTB) therapy adherence, which were classified into individual, socioeconomic, and healthcare system domains. Key individual factors include patient knowledge about TB, stigma, mental health challenges, and substance use. Socioeconomic barriers, such as financial constraints and limited social support, are particularly prominent. Health system factors, including accessibility, quality of provider-patient communication, and ease of care facilities, also significantly affect compliance. Addressing adherence to TB therapy requires a multi-faceted approach that combines patient education, socioeconomic support, and healthcare system improvement. Targeted interventions based on these findings are critical to improving treatment outcomes and reducing TB-related health disparities

Abstract

Nursing documentation is a critical element in ensuring patient safety, continuity of care, and professional accountability. However, the practice of documentation often encounters multiple challenges, including insufficient training, high workloads, limited resources, and the lack of clear operational guidelines. These barriers can impact the quality of care and patient safety, necessitating an in-depth exploration to understand and address them effectively. This scoping review aims to identify and analyze the factors hindering nursing care documentation, focusing on technical, individual, organizational, and contextual barriers across various healthcare settings. This review includes studies involving nurses and the factors affecting nursing care documentation. The concepts reviewed include barriers to documentation, such as individual, technical, organizational, and contextual obstacles. The context encompasses various healthcare settings, including hospitals, clinics, and community healthcare services across different countries. A comprehensive literature search was conducted using databases such as PubMed, ScienceDirect, Google Scholar, and ProQuest. Articles published between January 2020 and December 2024 were assessed using the JBI and MMAT critical appraisal tools. The selection process, based on PRISMA guidelines, identified 15 eligible articles for analysis. Key barriers to nursing documentation include inadequate training, low technological literacy, high workloads, infrastructure limitations, low motivation, and the absence of consistent documentation guidelines. These barriers affect the quality and accuracy of documentation, with implications for patient safety and service efficiency. Additionally, the findings highlight the need for improved supervision, auditing, and investments in documentation technology. Barriers to nursing care documentation require holistic interventions, including continuous training, technological infrastructure enhancement, and the implementation of clear operational guidelines. Further research is needed to explore effective solutions and their applications in diverse contexts.
