Abstract

Hypertension is a "silent killer". It is still one of the bigest challenges in Indonesia, where many cases are still found in primary health facilities. Hypertensive patients who require long-term therapy and adherence are very important factors to support the success of therapy. Based on the data from the Ministry of Health of the Republic of Indonesia (2018), 32.3% of hypertensive patients did not take medication regularly. Many factors influence medication adherence, such as level of knowledge, age, gender, working status, and length of illness. This study aimed to determine the effect of the level of knowledge and other factors on the level of medication adherence of hypertensive patients at the Dr. Mintohardjo Naval Hospital (RSAL). This research is a observasional study with a cross-sectional method and uses a questionnaire for data collection. Adherence level questionnaire uses MMAS-8. Analysis of the relationship between factors that influence the level of adherence uses logistic regression. The inclusion criteria of this study included hypertention outpatient aged ≤ 65 years old and are currently receiving antihypertensive therapy. The results of this study showed that out of 284 respondents, 92% of respondents had good knowledge regarding hypertension, while the level of adherence to taking medication was low at 47%. Factors that influence the level of patient adherence to taking medication in this study are gender (p-value = <0.001), education (p-value = <0.001), duration of hypertension (p-value = 0.033), and working status (p-value = <0.001). In contrast, the level of knowledge (p-value = 0.979) and age (p = 0.87) did not significantly influence the level of medication adherence. The Odds Ratio (OR) for working status was 5.73, OR for final education was 2.97, OR for the duration of hypertension was 2.24, and OR for gender was 0.34.

Abstract

Background: Hyperlipidemia is a condition that is characterized as an increase in total cholesterol and triglyceride levels in the blood. Lactoferrin is a protein that can serve as an antioxidant. This study aims to determine whether lactoferrin can reduce total cholesterol and triglyceride levels. Methods: This study used 24 Sprague–Dawley rat strains, which were divided into six groups: normal group; negative control; positive control; and dose groups 1, 2, and 3. The normal group was given standard feed, whereas the other group was given high cholesterol and fat. The positive control group and dose groups 1, 2, and 3 were given 1.5 mg/kg BW of simvastatin and 100, 200, and 400 mg/kg of BW lactoferrin, respectively. After 6 weeks, total cholesterol and triglyceride levels were measured. Results: This study showed that lactoferrin doses of 100, 200, and 400 mg/kg BW could significantly reduce total cholesterol and triglyceride levels (p < 0.05). Lactoferrin could also significantly reduce activated Kupffer cell and steatosis area in the liver (p < 0.05). Conclusions: Lactoferrin can reduce total cholesterol and triglyceride levels. Thus, further research is needed to address the existing bias and confirm that lactoferrin can reduce cholesterol and triglyceride levels.

Abstract

 Health-seeking behavior merupakan tindakan yang dilakukan oleh individu untuk mencari pengobatan dalam mengatasi masalah kesehatan yang dialami. Salah satu bentuk health-seeking behaviour adalah swamedikasi. Data Badan Pusat Statistik Indonesia menyatakan 84,34% penduduk Indonesia melakukan swamedikasi. Sebanyak 83,6% mahasiswa dormitory Universitas Katolik Indonesia Atma Jaya pernah melakukan swamedikasi, tetapi perilaku swamedikasinya belum diketahui, sehingga dilakukan penelitian untuk menganalisis hubungan pengetahuan dengan perilaku swamedikasi mahasiswa dormitory Universitas Katolik Indonesia Atma Jaya. Penelitian ini merupakan penelitian observasional analitik dengan pendekatan cross sectional. Penelitian dilakukan dari bulan Maret hingga Juni 2024. Analisis hubungan pengetahuan dan perilaku swamedikasi menggunakan uji chi square. Sebanyak 44,76 % memiliki tingkat pengetahuan yang cukup terkait swamedikasi dan 93,33% memiliki perilaku swamedikasi yang positif. Tingkat pengetahuan baik memiliki hubungan signifikan dengan perilaku swamedikasi (p-value = 0,019). Tingkat pengetahuan baik 1,2x lebih besar memiliki perilaku swamedikasi positif dibandingkan tingkat pengetahuan kurang. Pengetahuan memiliki hubungan yang signifikan dengan perilaku swamedikasi mahasiswa dormitory Universitas Katolik Indonesia Atma Jaya.

Abstract

A simple, cost-effective, and validated reversed-phase high-performance liquid chromatography (RP-HPLC) method is crucial for accurately monitoring trimethoprim and sulfamethoxazole therapy. However, conventional HPLC methods often rely on buffered mobile phases and additives, which can lead to column blockage and shorten column lifespan. To address this issue, we developed and validated a reversed-phase HPLC method with a photodiode array (PDA) detector, using a buffer-free and additive-free mobile phase with gradient elution. Sample preparation was performed using organic solvent-based deproteinization to minimize matrix effects. Chromatographic separation was performed on a ZORBAX Eclipse Plus C18 column (250 × 4.6 mm, 5 µm) using a mobile phase of acetic acid (pH 2.3), methanol, and acetonitrile at a flow rate of 1 mL/min. Detection was carried out at 270 nm with a total run time of 17 minutes. The analytical method exhibited linearity, precision, and accuracy, with adequate sensitivity for the analytes of interest. Stability testing yielded favorable results under various storage conditions, including the autosampler, freeze-thaw cycles, and long-term storage. However, the trimethoprim stock solution was only stable for two days and must be freshly prepared. Full validation was conducted in accordance with FDA and ICH bioanalytical guidelines, confirming its suitability for clinical application.

Abstract

Telepharmacy offers a promising solution to overcome the geographical limitations of traditional pharmacies. However, concerns regarding the quality of services delivered through telepharmacy highlight the need for a comprehensive evaluation of patient satisfaction. This study aimed to compare patient satisfaction between telepharmacy and conventional pharmacy services and to identify areas for improvement. A cross-sectional study was conducted in March and April 2024, involving a sample of 136 customers from the Atma Jaya Teaching Pharmacy in North Jakarta. Patient satisfaction was measured using a validated questionnaire, and key satisfaction dimensions were identified through Importance-Performance Analysis (IPA). The analysis revealed that both telepharmacy and conventional pharmacy services demonstrated strengths in the dimensions of Assurance and Responsiveness, which are critical to customer satisfaction (quadrant II : keep up the good work). Meanwhile, Reliability and Empathy were perceived as less important in both service models (quadrant III : low priority). The Tangible dimension, however, was identified as an area requiring improvement (quadrant I : concentrate here). These findings suggest that telepharmacy can provide patient satisfaction comparable to conventional pharmacies, particularly in the dimensions of Assurance and Responsiveness. However, the Tangible dimension remains a key area for improvement.

Abstract

Elevated d-dimer levels due to venous thromboembolism and hypercoagulability in covid-19 patients play a role in disease severity and poor clinical outcomes. This study aims to compare the effectiveness of heparin and enoxaparin, two recommended anticoagulants, in reducing d-dimer levels and hospital stay duration in hospitalized covid-19 patients. This retrospective cohort study involved 106 hospitalized covid-19 patients with abnormal d-dimer levels treated between March 2020 and March 2023. The assessment included changes in d-dimer levels and hospitalization duration. Statistical analyses were performed using stata software. Patients receiving enoxaparin were 5.913 times more likely to experience a reduction in d-dimer levels compared to those receiving heparin (p=0.021). While smoking status (or=0.034; p=0.001) and the number of comorbidities (or=11.247; p=0.018) were notable factors influencing d-dimer reduction, enoxaparin administration was linked to significantly longer hospitalization (p=0.024). The severity of covid-19 also had a substantial impact on hospital stays, with severe (β = 5.757; p= 0.004) and critical (β = 9.147; p 0.001) cases requiring extended care. Thus, enoxaparin demonstrates greater efficacy in reducing d-dimer levels in hospitalized covid-19 patients, though at the cost of prolonged hospital stays.

Abstract

This community service activity aimed to enhance the knowledge and awareness of students at Bunda Hati Kudus (BHK) Junior High School on helminthiasis prevention. The program was implemented through education on clean and healthy living behaviors (PHBS) and training on proper handwashing based on World Health Organization (WHO) standards. Program effectiveness was assessed using pre- and post-test evaluations. Education was provided to 51 junior high school students of class XI BHK through lectures, discussions, question and answer sessions, case studies, and workshops. Knowledge evaluation was conducted using a questionnaire completed by participants before and after the activity. Data analysis was performed descriptively to determine an overall picture of pre- and post-test data, Fisher's Exact test to identify differences in the proportion of correct answers for each pre- and post-test question, and the Wilcoxon signed-rank test to determine differences in medians between pre- and post-test data overall. Descriptive analysis shows that the proportion of students with a good level of knowledge increased from 72.00% to 96.08%. A significant increase in correct answers was observed in questions 1, 5, 6, and 10 (p<0.05), and overall there was an increase in the median pre- and post-test scores from 8 to 10 (p<0.001). This educational activity successfully increased students' knowledge about the prevention of worm infections and proper handwashing practices. Recommended follow-up actions include extending the duration of the activity, developing more varied educational materials, and implementing a follow-up program to ensure the sustainable application of healthy living behaviors among students.
