Abstract

Latar Belakang : Dislipidemia merupakan abnormalitas metabolisme lipid yang ditandai dengan peningkatan maupun penurunan fraksi lipid dalam serum. Berbagai penelitian terdahulu membuktikan bawang putih (Allium sativum L), kitosan dan yogurt sinbiotik pisang tanduk mempuyai efek memperbaiki profil lipid.Tujuan : Menganalisis perbandingan pengaruh pemberian ekstrak bawang putih, kitosan dan yogurt sinbiotik pisang tanduk terhadap profil lipid tikus Sprague-Dawley hiperkolesterolemia.Metode : Penelitian eksperimental dengan disain Post Test Control Group Design. Jumlah sampel 30 tikus Sprague-Dawley jantan, terbagi dalam 5 kelompok, terdiri dari kelompok kontrol normal (K1), kontrol hiperkolesterolemia (K2)dan kelompok perlakuan P1,P2,P3 yang masing-masing diberi ekstrak bawang putih, kitosan atau yogurt sinbiotik pisang tanduk. Analisis statistik yang digunakan adalah uji One Way Anova dan Post Hoc Bonferroni.Hasil : Rerata kadar kolesterol total K1,K2,P1,P2,P3 adalah 83,67 ± 3,68; 207,79 ± 5,36; 105,11 ± 3,03; 117,64 ± 4,17; 128,35 ± 3,85 mg/dL;. Rerata kadar trigliserida K1,K2,P1,P2,P3 adalah 73,68 ± 4,20;130,59 ± 3,03; 82,22 ± 1,94; 92,33 ± 3,21; 103,51 ± 3,11mg/dL;. Rerata kadar kolesterol HDL K1,K2,P1,P2,P3  adalah 89,06 ± 1,99; 25,23 ± 1,52; 74,13 ± 3,09; 62,42 ± 2,04; 56,63 ± 2,11 mg/dL;. Rerata kadar kolesterol LDL K1,K2,P1,P2,P3 adalah 24,57 ± 2,04; 77,02 ± 1,64; 35,46 ± 2,35; 45,15 ± 1,72; 56,18 ± 1,49 mg/dL.Kesimpulan : Terdapat perbedaan pengaruh pemberian ekstrak bawang putih ,kitosan dan yogurt sinbiotik pisang tanduk terhadap profil lipid tikus Sprague-Dawley  hiperkolesterolemia.Ekstrak bawang putih menunjukkan efek paling baik dalam memperbaiki profil lipid , diikuti oleh kitosan dan yogurt sinbiotik pisang tanduk.

Abstract

BACKGROUND: Inflammation occurs in chronic diseases such as type 2 diabetes mellitus (DM). Insulin resistance and inflammation in type 2 DM with obesity can increase interleukin (IL)-6, causing an increase in hepcidin synthesis in the liver. Increased inflammation can exacerbate the course of type 2 DM. This study aims to prove that there are differences in the levels of hepcidin and IL-6 between obese and non-obese type 2 DM.METHODS: This cross-sectional study was conducted on 61 patients with type 2 DM, consist of 22 male and 39 female with an age of more than 40 years. Type 2 DM subjects were obtained from a doctor's diagnosis and were divided into obese and non-obese groups based on body mass index (BMI). Hepcidin and IL-6 levels were examined using the Enzyme-Linked Immunosorbent Assay (ELISA) principle. The data were analyzed using an independent t-test and Mann-Whitney test.RESULTS: The mean level of hepcidin in the obese with type 2 DM group was 25.32±11.54 ng/mL, and non-obese was 11.94±5.31 ng/mL. The median level of IL-6 in the obese with type 2 DM group was 11.9 (5-61) pg/mL, and non-obese 4.8 (1.5-9.8) pg/mL. There was a significant difference in hepcidin and IL-6 levels between the obese and non-obese groups (p=0.000).CONCLUSION: Hepcidin and IL-6 levels in the obese group with type 2 DM were higher than non-obese group.KEYWORDS: type 2 diabetes mellitus, obesity, hepcidin, interleukin-6

Abstract

BACKGROUND: Inflammation plays an important role in the atherosclerotic process. High-sensitivity C-reactive-protein (hs-CRP) is commonly used as inflammatory biomarker. It is well known that regular physical activity lowers hs-CRP levels, while prolonged exercise induces hs-CRP elevations. However, the relationship of training and exercise characteristics with hs-CRP levels remains not well elucidated. We evaluated baseline and post-exercise hs-CRP levels and its association with training and exercise characteristics.METHODS: Eighty-eight male endurance cyclists were involved. Demographic data, health condition and training characteristics were collected. Baseline and postexercise blood-samples were collected to determine hsCRP concentrations. A hs-CRP cut-off point of 3 mg/L was used. Blood-cell count and biochemical parameters were measured at baseline. Heart rate (HR) was measured during exercise.RESULTS: Cyclists performed 7.3 hours (interquartilerange (IQR) = 5.4-7.5) of endurance exercise at intensity of 81.8 % (IQR = 74.9-85.8). Cyclists with baseline hsCRP ≥ 3 mg/L reported higher body mass, body mass index (BMI), waist-circumference and total-cholesterol. An increase in hs-CRP was following endurance exercise. Cyclists with any elevation of hs-CRP reported a higher BMI, HR during exercise and exercise intensity. Binary logistic regression analysis showed BMI (OR = 1.24, 95% CI = 1.04-1.48) and cycling distance (OR = 0.22, 95% CI = 0.06-0.76) were associated with post-exercise hs-CRP elevations.CONCLUSION: Body mass, BMI, waist-circumference, total- and HDL-cholesterol are associated with baseline hsCRP, whereas BMI and cycling distance were associated with hs-CRP elevations. These findings suggest that anthropometry parameters and lipid levels attributed to baseline hs-CRP, while anthropometry parameters and cycling intensity attributed to post-exercise hs-CRP elevations.KEYWORDS: C-reactive-protein, exercise, endurancecycling, inflammation, acute-phase-response

Abstract

BACKGROUND: In tuberculosis infections, the immune system is weakened and cannot produce enough cytokines to against the infection. b-glucan is a potent immunomodulator that induces cytokine production in various bacterial infections. This study aimed to determine the effects of b-glucan on the production of interferon (IFN)-γ and interleukin (IL)-12 in peripheral blood mononuclear cells (PBMCs) induced by Mycobacterium tuberculosis DNA.METHODS: PBMCs were isolated from 11 healthy subjects. PBMCs were treated with/without 5 μg/mL b-glucan and M. tuberculosis rpoB wild-type or mutant DNA. The production of IFN-γ and IL-12 in the supernatant was performed with enzyme-linked immune-sorbent assay (ELISA).RESULTS: b-glucan increased significantly (p<0.05) IFN-γ of M. tuberculosis mutant DNA-induced PBMCs, M. tuberculosis wild-type DNA-induced PBMCs, and non-induced PBMCs. b-glucan also increased significantly (p<0.05) IL-12 of M. tuberculosis mutant DNA-induced PBMCs, M. tuberculosis wild-type DNA-induced PBMCs, and non-induced PBMCs. There were not any significant difference between male and female groups for IL-12 and IFN-γ in all treatment groups (p>0.05, ANOVA test).CONCLUSION: This in vitro study indicates that b-glucan increases the performance of PBMCs to produce IFN-γ and IL-12, with/without induction of M. tuberculosis wild-type/ mutant DNA.KEYWORDS: b-glucan, IFN-γ, IL-12, M. tuberculosis, rpoB

Abstract

ABSTRACT Background: An increase in fat accumulation in obesity has been suggested to link with an increase in inflammation. This inflammation may be associated with an elevated of Lipoprotein-Associated Phospholipase A2 (Lp-PLA2), Apolipoprotein B (Apo B), and Low-Density Lipoprotein (LDL), thereby associated with the risk of atherosclerosis.Objective: To investigate the differences between Lp-PLA2, Apo B, and LDL levels in obese and lean men.Methods: A cross-sectional study was conducted on 74 men (obese and lean) at the Faculty of Medicine, Diponegoro University, Indonesia, in 2020. The concentration of LDL was measured using the homogenous enzymatic colourimetric method, whereas the levels of Lp-PLA2 and Apo B were determined using the ELISA method. Data were analyzed using an Independent t-test, setting statistical significance at p <0.05.Results: This study showed that Lp-PLA2 levels were significantly different between obese and lean men (p = 0.039). Furthermore, LDL levels were also significantly different between obese and lean men (p = 0.002). However, we did not find any differences in Apo B between obese and lean men (p = 0.640).Conclusion: Lp-PLA2 and LDL levels were slightly higher in obese compared to lean men, but no difference of Apo B.

Abstract

Background Blood transfusion remains the main therapy for anemia in β thalassemia major patients. However, frequent transfusions can cause oxidative stress in response to iron overload. Vitamin E is considered to be the best lipid-soluble exogenous antioxidant in humans. It can protect phospholipid membrane from peroxidarion. Erythrocyte osmotic fragility is a useful test to assess for the improvement of red blood cells in thalassemia patients after vitamin E supplementation.Objective To investigate the effect of vitamin E for improving erythrocyte osmotic fragility in β thalassemia major and for decreasing the need for frequent transfusions.Methods T his was a double blind placebo controlled randomized clinical trial on children aged 2-14 years with thalassemia major who received frequent blood transfusions. Fifty subjects were divided into 2 groups: group I with vitamin E supplementation and group II with placebo, as a control group, for a period of 1 month. Pre- and post-treatment data on erythrocyte osmotic fragility and hemoglobin level were analyzed with non-paired T-test.Results Improved erythrocyte osmotic fragility was found: in group I, pre-treatment 31.59 (SD 6.342)% to post-treatment 38.08 (SD 7.165)%, compared to the control group pre-treatment 34.40 (SD 6.985)% to post-treatment 29.26 (SD 9.011)% (P=0.0001). Comparison of the mean delta Hb level in group  I was 0.94 (SD 0.605) gr% and that of group II was - 0.23 (SD 1.199) gr% (P=0.0001).Conclusion Vitamin E supplementation improves erythrocyte fragility and Hb level in β thalassemia major pediatric patients.

Abstract

The H63D HFE mutation has been reported to be responsible for primary haemochromatosis. The allele frequency in Indonesianpopulation is about 2.8%. Co inheritance between H63D mutation and hemoglobin disorders such as Thalassemia may increase theseverity of iron overload. Nevertheless, the coinheritance of this mutation with HbE disorder is the most common hemoglobin disorderin Indonesia and the gene frequency have not been reported especially in Javanese ethnic. To identify the presence and the frequency ofH63D HFE mutation in HbE disorder among Javanese ethnic. A cross sectional study involved 24 Javanese individuals who consist of21 HbE heterozygotes (HbAE) and 3 HbE homozygotes (HbEE) subjects. The subjects were screened for H63D mutation by digestion ofPCR products with MbO I restriction endonuclease. The genotype frequency for wt/wt was 95.24% in HbAE, 100% in HbEE and for wt/H63D was 4.76% in HbAE. The allele frequency for H63D HFE mutation was 2.08% in total sample of HbE. The allele frequencies inHbAE and HbEE individual were 2.38% and 0%, respectively. H63D HFE mutation is found in 24 Javanese ethnic individual with HbEdisorder. However, the allele frequency of H63D HFE mutation is low and almost similar to the allele frequency of H63D HFE mutationin Indonesian population.

Abstract

Thalassemia major is one of the types of thalassemia that need a routine blood transfusion. If not treated immediately, the patient will only last for 1-8 months. Blood transfusions performed at least or more than 10 times causes iron overload. Excess levels of Fe ions in the body will be stored in the form of ferritin. If the ferritin level is high, cell damage will occur in the presence of a fat peroxidation reaction or Malondialdehyde (MDA). Cell damage can trigger proinflammation, which increases neutrophil counts and decreases lymphocyte counts. The Neutrophil-Lymphocyte Ratio (NLR), which measures the ratio between Absolute Neutrophil Count (ANC) divided by Absolute Lymphocyte Count (ALC) with a manual peripheral blood picture. This study aimed to determine the correlation of ferritin levels with MDA and NLR in iron overload. This study used an analytical observational design with a cross-sectional approach, with samples were thalassemia patients who received repeated blood transfusions at the General Hospital Dr. R Soetrasno, Rembang City and Regional General Hospital Dr. R Soedjati, Grobogan Purwodadi. Inclusion criteria were age 10-18 years, transfusion 10-20 times, normal body temperature. Exclusion criteria were Fe therapy orally, leukocytosis, chronic kidney disease. In the MDA levels, there was no significant difference where p=0.25 by Spearman test. In the NLR there was no significant difference where p=0.91 by Spearman test. There is no correlation between ferritin levels with MDA and NLR in iron overload.

Abstract

Acute Megakaryoblastic Leukemia (FAB AML M7) occurs in all age groups with two peaks in distribution. The one is in adults and theother in children 1 to 3 years of age especially in those with Down’s syndrome. The diagnosis of AML M7 requires more than 30% of thenucleated bone marrow cells being megakaryoblasts. The AML M7 was under diagnosed before the availability of monoclonal antibodies.The more common types of AML MO-M6 have to be excluded by morphological and cytochemical analysis whereas immunology is neededto exclude ALL. The megakaryocytic nature of the leukemia has to be proven by ultrastructural demonstration of platelet peroxidase or byimmunological demonstration of CD61, CD42, CD41 on the surface of the leukemic blasts. Megakaryocytic/megakaryoblastic leukemiasshow a wide morphologic spectrum. Cytoplasmic blebs and protrusions are the most prominent feature of many cases. The nuclei ofthese cells are round with more finely reticulated chromatin and with prominent nucleoli. The megakaryoblastic nature of these cells canbe suggested by morphology. Cytochemistry is of limited diagnostic value in megakaryoblastic leukemias. Usually it is used to excludethe more common types of leukemia. An eighteen months girl was admitted to hospital with anemia and hepatosplenomegaly. There isdismorphic - hypertelorism face and enlargement of neck lymph nodes. The laboratory examination found anemia, hyperleukocytosis with75 % blast cells. Morphologically the blast cells show prominent blebs and cytoplasmic budding resemble features of budding platelets.The cytochemistry staining for granulocyte and erythrocyte lineages were negative. The expressions of lymphoid and myeloid lineagesmarkers by immunoflowcytometry method were also negative. Cytogenetic examination was followed. Abstract

Latar belakang: Sindrom koroner akut (SKA) merupakan 80 % penyebab kematian akibat penyakit jantung. SKA disebabkan penurunan aliran darah ke jantung sehingga terjadi iskemik dari miokard. Neutrophil/lymphocyte ratio (NLR) merupakan pemeriksaan laboratorium yang akhir-akhir ini terbukti sebagai petanda untuk inflamasi sistemik. High density lipoprotein (HDL) merupakan lipoprotein yang berfungsi sebagai anti aterogenik dan  kardio proteksi. Studi sebelumnya menunjukkan terdapat hubungan NLR dengan keparahan SKA. Penelitian ini bertujuan untuk mengetahui hubunganNLR dan HDL pada sindrom koroner akut.Metode: Penelitian belah lintang pada penderita sindrom koroner akut di RS Dr. Kariadi Semarang periode Januari-Desember 2014. NLR diperoleh dengan membagi jumlah netrofil dengan jumlah limfosit. Kadar HDL serum ditetapkan dengan alat kimia klinik otomatik. Analisis statistik dengan Spearman Rank Correlation Test.Hasil: Lima puluh enam pasien dengan sindrom koroner akut (41 laki-laki dan15 perempuan) dengan usia antara 32-83 tahun. Nilai tengah NLR 3,55 (1,1-17,6) dan HDL 34 (16-71) mg/dL. Analisisstatistikmenunjukkanterdapathubungan yang bermaknaantaraNLR dan HDL (p = 0,001;  r =-0,44).Simpulan: Terdapat hubungan negatif sedang yang bermakna antara NLR dan HDL pada sindrom koroner akut. Hasil penelitian ini menunjukkan bahwa nilai NLR perlu diperhatikan pada pasien dengan HDL yang rendah. Kata kunci: Sindrom koroner akut, NLR, HDL
