Abstract

Premature birth is leading causes of neonatal mortality. According to the World Health Organization (WHO), global premature birth rates reached 9.9% in 2020, it’s rising compared to preterm birth rates in 2010. One of the risk factors for preterm birth is the mother's age, which pregnant adolescents are more at risk of giving birth to preterm babies due to physiological, psychological, and socioeconomic immaturity. This study is an analytical study with a retrospective cohort design to determine the relationship between adolescents pregnancy and preterm birth. This study was conducted in Cengkareng General Hospital West Jakarta using patient’s medical record data. The population in this study were all pregnant women who were admitted to the hospital with signs of labor in the period of 1st January 2022 until 31st December 2022. Sampling was carried out using simple random sampling technique with a total of 141 respondents in each group of respondents who experienced adolescent pregnancies (10-19 years) as the exposed groups and the group of respondents who experienced pregnancies in healthy reproductive age (20-34 years) as the non-exposed groups. Data were analyzed using cox regression test. The analysis revealed that adolescent pregnancy increased the risk of premature birth by 3.57 times (RR adj= 3,57; 95%CI: 2,06-6,20; p= 0,000) after controlling for maternal education, occupation, marital status, gestational hypertension, and anemia. Abstract

Stress during pregnancy can negatively impact the health of both the mother and the fetus, especially in the first trimester, which is a period of intense physical and emotional adaptation. This study aims to determine the prevalence and psychosocial factors associated with the occurrence of stress in first-trimester pregnant women in Makassar City. This study uses a cross-sectional design involving 256 first-trimester pregnant women from five public health centers in the suburban areas of Makassar City. Data were collected through interviews using a structured questionnaire. The Edinburgh Postnatal Depression Scale (EPDS10) instrument was used to measure stress levels. Analysis was conducted bivariately and multivariately using logistic regression. The prevalence of severe stress among pregnant women in the first trimester was 36.72%. Multivariate analysis revealed two factors significantly associated with severe stress: low family income (adjusted Prevalence Ratio (aPR) = 2.32; 95% CI: 1.15–4.66; p = 0.018) and poor interpersonal relationships with parents or in-laws (adjusted Prevalence Ratio (aPR) = 4.63; 95% CI: 1.53–14.02; p = 0.007). Socioeconomic factors and interpersonal relationships with parents/in-laws influence the high levels of stress experienced by first-trimester pregnant women. Early intervention that considers the family's economic aspects and social support from the surrounding environment is critical in preventing stress during pregnancy.

Abstract

Indonesia is among the five countries with the highest burden of stunting. Food insecurity reflecting the availability of food in the household is one of the indirect causes of undernutrition. This study examines the relationship between food insecurity and stunting in Indonesia. We used data from the 2021 Indonesian Nutrition Status Survey (SSGI) with a sample size of 82,777 under-five children selected using multistage random sampling. The relationship between food insecurity and stunting was calculated using cox regression to obtain crude and adjusted prevalence ratios (PR) considering strata and weights. Children from households with moderate food insecurity had a (PR unadj)  1.24 (1,18 – 1,31) times higher risk of stunting, and the risk rose in households with severe food insecurity (PR unadj) 1.39 (1,27 – 1,53). Subgroup analysis based on regional categories showed that the association between food insecurity and stunting was only significant in rural areas. Children in rural regions who experienced moderate food insecurity had a 1.09 (95% CI 1.02-1.16) times greater risk of stunting and an increased risk of 1.15 times (95% CI 1.03 – 1.28) in households with severe food insecurity. Interventions are needed to prevent stunting by improving household food security, particularly in rural areas. Further research is needed with better study designs to prove a causal relationship between food insecurity and stunting.

Abstract

The severity of COVID-19 is influenced by many factors, one of which is comorbid hypertension. The National Emergency Hospital Wisma Atlit Kemayoran is a special hospital and the only hospital that treats patients from severe to asymptomatic. This study aims to look at the association between comorbid hypertension and the severity of COVID-19. The design of this study was a case control conducted from October to November 2022. The data used was the secondary data from the medical records of patients at National Emergency Hospital Wisma Atlit Kemayoran in 2021. The case group totaled 288 research subjects who experienced COVID-19 with moderate and severe symptoms, while the control group totaled 302 research subjects who experienced COVID-19 with mild and asymptomatic symptoms. The results showed a statistically significant association between hypertension and the severity of COVID-19 with OR 3.40, 95% CI 1.88 to 6.34 and p-value <0.001. There is also a statistically significant association between comorbid hypertension and the severity of COVID-19 with Adjusted OR 3.33, 95% CI 1.79 to 6.13 and p-value <0.001 after being controlled by age, vaccination status, and education level. It is hoped to be an illustration that the management of hypertension is very important in order to prevent the worsening of the condition in COVID-19 patients.
